Abstract

Sejak terjadi pandemi COVID-19 di seluruh dunia, banyak pembatasan hampir ke semua layanan rutin termasuk pelayanan kesehatan ibu dan anak. Seperti ibu hamil menjadi enggan ke puskesmas atau fasilitas pelayanan kesehatan lainnya karena takut tertular, adanya anjuran menunda pemeriksaan kehamilan, serta ketidaksiapan layanan tenaga kesehatan dan sarana prasarana termasuk alat pelindung diri, Situasi ini telah mengubah cara dan gaya hidup ibu hamil, karena tidak menutup kemungkinan muncul masalah fisiologis dan patologis yang dapat mengganggu proses kehamilan bagi ibu dan janin.Dampak negatif yang dialami terutama adalah kesehatan mental seperti ketakutan, depresi, kecemasan dan perasaan tidak aman. Ibu hamil perlu memperoleh pengalihan fikiran ke hal-hal yang positif untuk mengurangi masalah psikologis yang dialaminya. Yoga merupakan salah satu teknik yang dapat menghasilkan energi positif dalam diri ibu, memberikan suasana nyaman, dapat menghilangkan stress dan perasaan cemas pada ibu hamil serta meningkatkan rasa percaya diri dalam persiapan persalinan.

Abstract

Third stage active management and anticipation of primary postpartum haemorrhage is one of the basic competencies of midwives. During the period of 2013 - August 2015 there are 25 cases of maternal deaths in North Tapanuli, with the biggest cause factor is bleeding. This study aims to analyze the performance of midwives in the implementation of third stage active management and anticipation of postpartum bleeding in North Tapanuli Regency in 2015. This study is a quantitative study with cross sectional approach. Sampling was done by purposive sampling method that is midwife who helped normal delivery and anticipate primary postpartum haemorrhage in September-October 2015 (32 people). Data were collected through observations and a valid and reliable questionnaires. Analysis of data used univariate, bivariate analysis with Pearson product moment correlation and multivariate analysis with multiple linear regression. The results showed that midwives' performance was mostly in poor category (62,50%). Logistic regression test showed that education variables had the greatest and significant effect on midwife performance, followed by knowledge, motivation and experience. Age, employee status, workload, and coworkers have no effect on midwife performance with p-value> 0,05. Form of regression equation obtained performance = 6,476 +1,038 X1 + 0,500 X2 + 0,369 X3 +0,324 X4 + e. R Square value of 0.995 so that obtained coefficient of determination (KD) is 99.5% which means that independent variables have a contribution influence of 99.5% to Y and 0.5% other influenced by other factors not included in this study. This study concludes that education, motivation, experience and knowledge have a significant influence in the management of active management at the third stage and anticipation of postpartum hemorrhage

Abstract

Background: High-risk pregnancies can cause more complications for the mother and fetus during pregnancy, childbirth, and the puerperium compared to normal pregnancies. High-risk factors during pregnancy can increase worry and stress, which can influence labor and delivery and are risk factors for postpartum depression. During pregnancy, stress can increase the risk of having a premature, low-birth-weight baby. Hypnoprenatal is an effective communication method to incorporate positive suggestions into the subconscious mind of pregnant women to reduce anxiety and stress during pregnancy. Objectives: to determine the effect of hypnoprenatal intervention on anxiety and stress in high-risk pregnant women in the North Tapanuli District. Methods: This research method used a pre-experimental method with a one group pre-test post-test design. Samples were taken by the purposive sampling method, which, based on the inclusion criteria, obtained as many as 36 high-risk pregnant women who experience anxiety and stress. Hypnoprenatal intervention was carried out for 3 days, twice a day, and then observations and interviews were carried out using the DAS 42 questionnaire. Data analysis used a paired t-test. The results showed that the characteristics of the respondents based on the age of the majority were >35 years (44.7%), secondary education (33.3%), farmer's work (52.8%), primigravidas (17%), high-risk pregnancies (72.7%), and low risk (27.8%).Results : The results of the statistical tests showed differences and decreased levels of anxiety and stress in pregnant women at risk before and after the hypnoprenatal intervention, with a p-value of 0.000 (p<0.05). Conclusions: hypnoprenatal has a significant effect on the level of anxiety and stress among pregnant women at risk. It is recommended that pregnant women use hypnoprenatal techniques to help reduce anxiety and stress during pregnancy, which affects the mother's psychological condition.
