Abstract

The implementation of a hospital management information system at the 'Aisyiyah Padang public hospital opens opportunities for academics to conduct research and experiments. The amount of data stored in the information system database can be used as an alternative for decision making by hospital management, including medical record data for patients who go to the hospital. Existing medical record data can be summarized and presented visually in the form of a dashboard according to the needs of the hospital. Dashboard can provide an overview to management to determine future policies or as a decision support system. This study aims to create a dashboard that can be used as a decision support system. This type of research is descriptive with the subject of medical record data in the database. From the research that has been done, it is found that the number of outpatient patients in 2021 is 23,635 patients. Of this number, the 3 insurers for the most patients were the Badan Penyelenggara Jaminan Sosial (BPJS) with 19,834 patients, Umum with 3,551 patients and Smile Train with 69 patients. For the 3 doctors who treat the most patients, Dr. Lisa Oktamuva, Sp.PD as many as 3284 patients, dr. Yanasta Moendanoe, Sp.OG with 3112 patients, and drg. Harfindo Nismal, Sp.BM as many as 2351 patients. From the research that has been done, it can be concluded that the designed dashboard is still simple and incomplete. From the dashboard it was also found that the person who is responsible for the most patients is BPJS and the doctor who serves the most patients is Dr. Lisa Oktamuva, Sp.PD.)

Abstract

Siti Rahmah Islamic Hospital, Padang is a type A private hospital located in Padang in preparation for implementing electronic medical records and also improving the quality of human resources and health services. This study aims to determine the preparation for implementing electronic medical records at RSI Siti Rahmah Padang. The research used was qualitative using interviews, the method of taking the subject of informants using purposive sampling with several informants 5 people and data collection using analysis techniques by interviewing informants. The results of the research that has been carried out to prepare for the implementation of electronic medical records at RSI Siti Rahmah Padang have been running from the start of the enactment of the PMK Law 24 of 2022 by increasing human resources through training on electronic medical records and providing facilities and infrastructure to support the successful implementation of medical records. electronic medical from the aspect of technology, and information technology (IT). From this research, it can be concluded that preparations for the implementation of electronic medical records have started with human resources increasing insight into medical record officers and medical staff in other healthcare facilities.

Abstract

The government's efforts in achieving the target of exclusive breastfeeding have been stated in Government Regulation no. 33 of 2012. In Padang city, Perwako Regulation Number 7 of 2015 on Provision of Space for Breastfeeding and or Expressing Breastmilk has been issued. This is a form of efforts to accelerate the achievement of the target coverage of exclusive breastfeeding. This study aims to analyze the implementation of the exclusive breastfeeding policy in achieving the target in the city of Padang. In this study, an analytical study will be carried out on the achievement of exclusive breastfeeding coverage and the implementation of PP No. 33/2012 and Perwako in Padang City. Achievements from the exclusive breastfeeding program coverage in the city of Padang, obtained an average of 79.40%. However, this figure cannot be generalized because there is still an equity gap that occurs in several working areas of Puskesmas in Padang City. In the last three years (2017- 2019) the achievements in several health centers have experienced significant fluctuations and decreases, including at Ulak Karang Puskesmas where in 2019 it was 54.5%. The same thing happened at Puskesmas Alai, and Puskesmas Lubuk Buaya experienced a sharp decline from 2017 to 2019 from 77.22% to 52%. Policy implementation has not been carried out properly. So it is necessary to carry out evaluation and follow-up related to the achievements over the last few years (2017-2019), increase cooperation and coordination between the government, health agencies, health workers, and community leaders, and affirmation in determining administrative and written sanctions to support efforts to accelerate the achievement of breastfeeding targets.

Abstract

According to Permenkes No. 24 of 2022, health facilities must organize Electronic Medical Records no later than December 31, 2023. At the moment the health center is still in the preparatory stage. This study aims to assess the readiness of health professionals to adopt RME and the factors that influence its implementation in health centers. The research design is quantitative with a cross-sectional study approach. The data were collected using a questionnaire with a sample of 47 health professionals. Sampling uses the total-sample technique. Data analysis using chi-square statistical tests. Based on the results of the study there was no significant relationship between age, education, work experience, and profession in RME implementation (Pvalue > 0.05). However, productive age factors are best prepared to apply RME (100%). The level of education does not guarantee the readiness of officers to implement RME, nor does work experience determine the readiness of officers with a service life of 2-5 years unprepared by 95.5%. Regarding the profession, only 20% of medical and health information recorder (PMIK) officers are declared ready. It is expected that there will be socialization and training in the use of RME for healthcare professionals to be optimally organized.

Abstract

The completeness of medical records is an important factor that affects the quality of data and coding accuracy. Based on the initial survey at the 'Aisyiyah General Hospital, found from 20 medical records still 75% completeness of filling in the incoming and outgoing summary sheets, the absence of anamnesis and diagnosis results, and 60% completeness of the doctor's item recording component. The purpose of this study was to find out the relationship between the completeness of medical record documents with the accuracy of the ICD-10 and ICD-9 coding. This study used descriptive and inferential quantitative methods with a population of 638 documents, and the number of samples of 84 files uses simple random techniques. The frequency of filling in the medical record document is incomplete 89.3%, the duration of the ICD-10 code is 26.2% and the ICD-9 CM was 32.2%. The analysis results showed that there was no significant relationship between the completeness of medical record documents and the accuracy of the ICD-10 and ICD-9 coding (P-Value > 0.05). Complete medical records tend to have more accurate coding, while incomplete medical records often cause errors in coding. The importance of maintaining the completeness of medical records to increase coding accuracy has a positive impact on the management of health data, clinical decision-making, and health insurance claims. Efforts to increase the completeness of medical records, such as training in medical staff and effective electronic system implementation, are recommended to ensure better coding quality.
