Abstract

Background: The application of patient safety goals is an effort made by hospitals to improve the quality of health services. Handover activities that are not accompanied by effective communication can cause errors that can harm the patient. Recommendations from WHO require nurses to improve communication patterns, especially when carrying out handovers, namely by using SBAR communication which consists of Situation, Background, Assessment and Recommendation. Based on the data summary of reported incidents that occurred in the hospital as many as 20 incident cases in the past year, including cases of KTD, KNC, KTC and KPCS. From these incidents, there were 6 cases related to miscommunication during handovers. Some nurses still do not apply the SBAR communication method when carrying out handovers, resulting in cutting/lagging of the information provided and resulting in incidents. Objective:to determine the reltionshipbetween SBAR Communication during handover and the implementation of patient safety at Bakti Timah Karimun Hospital. Methods: This research is quantitative with a cross-sectional approach. The population of this study were all inpatient nurses at Bakti Timah Karimun Hospital with total sampling, which consisted of 63 respondents. Results: showed that 41 respondents (65%) used SBAR communication during handovers in the good category and implemented patient safety. Conclusion:This means that the implementation of SBAR communication during the handover is good but not optimal because there are still 35% who have not implemented it. Based on the chi square test, there is a relationship between SBAR communication during handover and the application of patient safety (p=0.000 <0.05). Suggestion:, it is expected that nurses can carry out SBAR communication during handovers effectively so that they can implement patient safety optimallyKeywords : Newbron, Skin wrap, hypothermia.

Abstract

Hyperbilirubinemia is one of the most frequent clinical phenomena in newborns and the most common cause of hospitalization in newborns. In Indonesia, the incidence of hyperbilirubinemia in newborns is 25-50% in neonates. The incidence of hyperbilirubinemia in the Tanjungpinang City Regional Hospital from January 1 2022 to April 30 2023 was 35.6%. This study aims to determine the factors associated with the incidence of hyperbilirubinemia in infants in the Perinatology room at Tanjungpinang City Regional Hospital. The type of research used is a quantitative analytical method using a cross-sectional research design with a retrospective approach. The population of this study was all 202 babies treated in the Perinatology room and the total sample was 72 medical records of babies who had hyperbilirubinemia. The data collection method uses a data checklist sheet from medical records, then the data is analyzed univariately and bivariately with the Chi-square statistical test. This research hopes that nursing services, especially in the perinatology room, will further improve the quality of service, especially in providing comprehensive nursing care for hyperbilirubinemia babies.

Abstract

Neonates are a transition period from intrauterine to extrauterine life that live with limitations according to the gradual maturity of body organ function. During this period, neonates can be at high risk of experiencing health problems, one of which is low birth weight (LBW). Phenomenon in the Perinatology Room at Tanjungpinang City Regional Hospital, by observing 3 mothers who had babies with low birth weight using the kangaroo method, it was found that 1 person experienced weight gain after kangaroo treatment for 5 days. Meanwhile, 2 other people did not experience an increase. This is due to the mother's or family's lack of knowledge regarding kangaroo method care (PMK). In 2021, neonatal deaths in the Riau Islands Province were still dominated by LBW causes at 42% (102 cases) and data from the perinatology room at the Tanjungpinang City Regional Hospital in 2022 and January-March 2023 saw 46 babies born with LBW. Pre-experimental research method. The research design uses a one group pre test-posttest design approach without control group. The sample numbered 30. It was concluded that there was an influence of the kangaroo method of care on increasing body weight in LBW babies. Suggestions for hospitals for this therapy can be considered as the first treatment for LBW newborns to increase the baby's weight.

Abstract

Tuberculosis is an infectious disease caused by the bacterium Mycobacterium tuberculosis. India is ranked 3rd with the highest number of TB sufferers in the world after India and China. Globally, it is estimated that 9.9 million people suffer from TB in 2020. The total number of TB cases that have been discovered and treated (Treatment Coverage) in the Riau Islands Province in 2021 is 5,055 cases. This study aims to determine the relationship between family support and treatment compliance in Tuberculosis patients at the Pulmonary Polyclinic, Tanjungpinang City Hospital. This research design is descriptive quantitative with a cross sectional approach. A sample of 46 respondents was selected using the Purposive Sampling sampling technique. Data were processed using the Chi-Square test. Univariate data was obtained for the variable "High" Family Support of 87% and "Low" Family Support of 13%. For the "high" medication compliance variable it was 80.4% and "low" medication compliance was 19.6%. Bivariate analysis results show that there is a relationship between family support and treatment compliance with a p value of 0.000 (< 0.005). There is a relationship between family support and treatment compliance in Tuberculosis patients at the Pulmonary Polyclinic at Tanjungpinang City Hospital. In this research, nursing services can provide counseling or psychosocial support to families of Tuberculosis patients who still have a low level of compliance.

Abstract

Phlebitis is vein inflammation caused by chemical or mechanical irritation. The achievement of Phlebitis rates at Bakti Timah Karimun Hospital in the last three months from October to December 2022 exceeds the tolerable standard. to find out the factors that influence the occurrence of Phlebitis. Analytical survey design with a cross sectional approach and sampling technique using purposive sampling with a sample size of 30 patients. Research data was collected through observation methods. The research results were analyzed univariately and bivariately with chi square. The factors that influence the occurrence of phlebitis are age > 60 years (13.3%), female gender (13.3%), comorbidities (10%), duration of infusion > 3 days (10%), location of installation infusion in the cephalic vein (10%) and hypertonic infusion fluid (10%). The results of the bivariate analysis showed a significant relationship between the 6 factors assessed (age, gender, comorbidities, duration of infusion, location of infusion and type of fluid used), namely with a p value <0.05. All the factors assessed show the potential to cause phlebitis and there is a significant relationship between the factors assessed and the incidence of phlebitis. It is hoped that if factors are found that have the potential to cause phlebitis, phlebitis prevention can be implemented in accordance with the SOP in the hospital.

Abstract

Unsmooth milk production is the main problem causing the failure of the breastfeeding process. One way to increase milk production for post partum mothers is to support the nutritional needs of mothers, one of which is by consuming soy milk made from soy beans. This study aims to determine the effec tof soymilk (Glycine Max) on breast milk production in post partum mothers at Bakti Timah Hospital Karimun Riau Archipelgo. This researchis a pre-experimental study with a one-group pretest-posttest design. The population in this study were post partum mothers at Bakti Timah Hospital Karimun Riau Archipelago in the last one month as many as 102 people. The sampling technique in this study used a sampling technique carried out using purposive sampling of 34 respondents. The research instrument used a breastmilk production questionnaire, namely the Lactation Assessment Tool (LATCH) which consisted of 5 points. Data analysis was performed univariately and bivariately using the Wilcoxon test. The results showed that before consuming soy milk the average milk production was 3.35 and after being given soy milk the average milk production increased to 4.03. The results of the Wilcoxon test found a p-value of 0.001 (p <0.05) so it can be concluded that there is an effect of soy milk (Glycine Max) on increasing milk production in post partum mothers. It is recommended for the community, especially post partum mothers, to be able to use soy milk as an alternative to addressing problems with increasing breast milk production.
