Abstract

Abstract: Maternal mortality in Semarang District was in the 2th ranking (highest) of 35 districts/cities in Central Java Province by 2015. The purpose of this study was to analyze maternal mortality based on maternal characteristics in Semarang City. This research used descriptive analysis, derived from secondary data that taken from Maternal Verbal Autopsy data on maternal death at Semarang City Health Office. The sampling of this study were all death maternal cases whose died during pregnancy, delivery and postpartum in District Semarang as many as 35 people in 2015. From the analysis showed that maternal mortality in Semarang City based on mothers were aged 20-35 years (68.6%), level education in senior high school (54.3%), in working mothers (51.4%), at risk spacing births (52.4%), risk parity (51.4%), place of delivery were 89.2% in hospital, delivery by cesarian section (67.9%). Beside that, most of mortality cases occurred in postpartum period (80%), with the highest caused preeclampsia/ eclampsia (37%) and 71.4% of mothers also do Antenatal Care more than 4 times during pregnancy. There are Most of mothers have a history of heart disease (22.8%), 39.3% preeclampsia/ eclampsia as a delivery complication and 74% mothers with postpartum complication. This study recommends the need for early recognition of mothers about antenatal care and danger during pregnancy, childbirth and postpartum period and increased alertness of health workers to complications of pregnancy, complication of delivery and complication of postpartum especially in mothers with a history of illness.

Abstract

Population growth results in a larger health budget. A health budget is a funding plan created to promote public health. The central government is the source of the health budget through the Deconcentration Fund which comes from the State Budget. This study aims to determine the utilization of the Public Health Efforts budget at Puskesmas X in Medan City. The type of research used is qualitative research with a case study approach. Data analysis techniques using descriptive analysis. Data collection was carried out using observation techniques, documentation and in-depth interviews with informants who manage the health budget. The research informants were the head of the puskesmas, the treasurer, the head of the program implementer, the head of the administrative subdivision and the person in charge of the Community Health Efforts program. Selection of informants using purposive sampling, namely the sample is determined by certain considerations. The results of the study revealed that the utilization of the Community Health Efforts budget of Puskesmas X Medan City has been running in accordance with the provisions. Target accuracy is families, groups and communities help achieve better health development goals.

Abstract

Pengelolaan Dana BOK untuk mendukung kegiatan puskesmas harus dilakukan dengan efisiensi, efektivitas, dan secara independen, karena hal ini menjadi dasar evaluasi kinerja program puskesmas. Penelitian ini bertujuan untuk menganalisis Pemanfaatan Dana BOK dalam Peningkatan Pelayanan UKM Esensial di Puskesmas Medan Johor. Penelitian ini menggunakan metode kualitatif dengan pendekatan deskriptif, data diperoleh dari data primer yaitu melalui wawancara mendalam dan data sekunder yaitu mengumpulkan data dari studi dokumen di Puskesmas Medan Johor. Informan dalam penelitian ini adalah kepala puskesmas, pengelola BOK, dan penanggungjawab UKM. Hasil penelitian ini mengungkapkan bahwa terdapat peningkatan  terhadap program UKM yang didanai oleh BOK setiap tahunnya serta kucuran dana yang sudah terealisasi tapi masih belum optimal. Pada level input SDMK secara umum terlihat sudah baik. Pada tingkat proses, dari RUK sampai RUKK sudah terlaksana dengan baik, namun di PKP masih ada capaian yang belum tercapai. Pada tingkat Output capaian kinerja meningkat dalam pelaksanaan program UKM. Dalam menjalankan pelayanan UKM Puskesmas Medan Johor mengalami beberapa hambatan yaitu dari sisi gizi masyarakat, Imunisasi anak, pemberian makan gratis untuk balita, serta cek kesehatan. Dimana hambatan-hambatan tersebut dipengaruhi oleh pola pikir, kurangnya pengetahuan, dan kurangnya kesadaran masyarakat tentang kesehatan.

Abstract

Quality health services are very important to maintain the welfare of the community. The role of Puskesmas has significance in providing health services according to the needs of residents in an area. This research focuses on the Technical Implementation Unit (UPT) of Simalingkar Health Center in Medan. The performance of employees in this unit is a major factor in achieving health service standards. This study used a qualitative approach with a descriptive approach, involving participants from the puskesmas and patients who had received services at the Simalingkar Health Center. Data was collected through in-depth interviews with stakeholders. The results of research at the Simalingkar Puskesmas as a whole, both from the puskesmas and patients, that the performance of employees at the Simalingkar Puskesmas UPT is generally considered good. However, several aspects need to be improved at the Simalingkar Health Center. Such as improving the quality of patient care, communication training among fellow employees, and further evaluation of certain health programs. This recommendation can be used as a basis to continue to improve the quality of health services at the Simalingkar Health Center. It can be the basis for continuing to improve the quality of health services at Puskesmas Simalingkar, focusing on aspects that can have a positive impact on the community. Periodic evaluation of employee performance, patient involvement as the main stakeholder, and implementation of concrete improvements are expected to improve the effectiveness and efficiency of health services at the Simalingkar Health Center.

Abstract

The need and development of health service personnel as an integral part of health development is directed to meet the needs of the number and type of qualified health workers in providing all kinds of health service needs. This study aims to determine the needs of health workers both quantity and quality at the Simalingkar Health Center in Medan City.  This research includes Descriptive Research with a qualitative approach, to find and obtain information about the analysis of energy needs at the Simalingkar Health Center in Medan City. The data and information obtained are then compared with the capabilities currently possessed with those expected for planning managers in the preparation of labor analysis of each work unit associated with the description and specifications of work at the Simalingkar Health Center in Medan City. The types of data used in this study are primary data and secondary data. Primary data are obtained through interviews and observations, while secondary data are obtained through tracing official records (documentation). The results showed that the preparation of health worker planning has been carried out so that the analysis of energy needs both in quantity and quality has been well planned. The number of health workers for the implementation of health services is quite sufficient at the Simalingkar Health Center in Medan City. Where health service personnel at Puskesmas have met the standards, or by the Minister of Health Regulation Number 75 of 2014. That way, the needs and development of health workers are well implemented.

Abstract

Patient satisfaction is one of the things that must be considered in health services and is an important index or parameter. Patient satisfaction is the result of assessment and consideration of health services in terms of facilities, staff performance, and other aspects provided by patients by comparing expectations with the reality of health services received in a service system. This research aims to determine the description of patient satisfaction with the performance of health workers at the Pancur Batu District Health Center. This research uses a quantitative approach with descriptive methods which aims to express patient satisfaction with the performance of health workers at the District Health Center. Pancur Batu. Based on the research results, a picture of patient satisfaction with the performance of health workers was obtained from several assessments, namely Emphaty, with 30 respondents (100%) feeling satisfied. Furthermore, regarding Assurance, 29 respondents (97%) were satisfied. Furthermore, Tangibles (direct evidence) as many as 28 respondents (93%) were satisfied. Responsiveness with 27 respondents (90%) and Responsiveness with 26 respondents (87%) were satisfied. This research concludes that the majority of patients at the Pancur Batu District Health Center are very satisfied with the performance of the health service employees.

Abstract

One of the problems of health development that occurs in the villages of Deli Serdang Regency is the high level of community diversity, which results in the complexity of the problems that arise. To complete development, especially in Tuntungan II Village, larsger funds are needed. Based on the information above, the researcher is interested in conducting further research on "Analysis of the Management of Village Fund Allocation (ADD) on Health Programs in Tuntungan II Village", which was conducted using qualitative research methods. The aspect analyzed was the allocation of village fund management on welfare programs. The results show that the two main sources of village income are the Village Development Movement program funds from the APBD of Deli Serdang Regency and village funds provided starting in 2021 from the APBN. The village fund policy process in Tuntungan II Village is strongly influenced by the development planning meeting (Musrenbang), the result of which is the RAPBDes document. The increase in village funds each year shows the government's commitment to development, as outlined in the third Nawacita. Village development is influenced by the village head, village officials, political factors, and kinship; without professionalism, the process can be hampered. Using village budgets for health development becomes more difficult as there are limitations in basic management of health workers.

Abstract

Motivasi karyawan dan kepuasan kerja merupakan aspek penting dalam pengelolaan sumber daya manusia. Para profesional layanan kesehatan harus melakukan yang terbaik sebagai pemimpin layanan kesehatan. Oleh karena itu, sangat penting untuk memenuhi kebutuhan tenaga kesehatan dan memastikan kepuasan terhadap pekerjaannya. Tujuan penelitian ini adalah untuk menganalisis motivasi dan kepuasan kerja tenaga medis di Puskesmas Padang Bulan serta mengidentifikasi faktor utama penyebab motivasi dan ketidakpuasan karyawan. Penelitian ini memakai metode pendekatan kualitatif deskriptif dikarenakan tujuan penelitian ini merupakan untuk mendeskripsikan ataupun mendapatkan data ataupun informasi. Penelitian ini menggunakan teknik purposive, untuk memperoleh data yang diperlukan, maka teknik yang digunakan dalam penelitian ini menggunakan observasi dan wawancara mendalam. Berdasarkan dari hasil dan pembahasan penelitian motivasi karyawan berada dalam kondisi baik, hal ini disebabkan faktor- faktor yang menjadi motivasi kerja terkait gaji yang sesuai, lingkungan, dan fasilitas yang sudah terpenuhi sehingga meningkatkan motivasi kinerja karyawan di puskesmas Padang Bulan. Sedangkan kepuasan kerja karyawan terdapat pengaruh positif dan signifikan yang disebabkan adanya rasa keharmonisan terhadap sesama karyawan, adanya pengawasan serta adanya peluang kemajuan bagi karyawan yang berkinerja tinggi. Faktor terpenting memotivasi petugas layanan kesehatan adalah kepuasan kebutuhan mereka akan gaji yang memadai, lingkungan kerja yang memadai, dan kondisi kerja yang memadai. Faktor-faktor yang berhubungan dengan kepuasan kerja antara lain hubungan kerja yang harmonis antara karyawan dan timnya, pengawasan yang suportif dan konstruktif dari atasan, serta peluang kemajuan bagi karyawan yang berkinerja tinggi.

Abstract

Dana kapitasi Jaminan Kesehatan Nasional (JKN) dikelola dan dimanfaatkan oleh puskemas sesuai dengan ketentuan Peraturan Menteri Kesehatan Nomor 6 Tahun 2022 tentang Penggunaan Jasa Pelayanan Kesehatan dan Dukungan Biaya Operasional Pelayanan Kesehatan dalam Pemanfaatan Dana Kapitasi Jaminan Kesehatan Nasional pada Fasilitas Kesehatan Tingkat Pertama Milik Pemerintah Daerah. Puskesmas adalah organisasi nirlaba atau nirlaba BPJS Kesehatan memberikan insentif JKN, yang dikenal sebagai kapitasi JKN, kepada Puskesmas sebagai penyedia layanan kesehatan. Dana Insentif JKN dibayar setiap bulan tanpa memperhitungkan berapa banyak Pasien Peserta JKN yang berobat atau jenis pelayanan kesehatan yang diberikan kepada mereka. Artinya, Puskesmas menerima bantuan tunai dari JKN setiap bulan. Puskesmas Tanjung Anom adalah salah satu Puskesmas yang ada di Pancur Batu, Kabupaten Deli Serdang. Penelitian ini menggunakan pendekatan metode penelitian deskriptif kualitatif. Data primer digunakan, yang berarti data yang diperoleh langsung dari sumbernya melalui observasi langsung dan teknik penunjang adalah wawancara dengan informan yaitu kepala puskesmas, bendahara jaminan kesehatan nasional dan tata usaha puskesmas. Penelitian ini bertujuan untuk mengetahui gambaran pengelolaan dana kapitasi JKN di PuskesmasTanjung Anom. Setelah dilakukan penelitian, kami menemukan bahwa elemen perencanaan sudah optimal. Artinya, perencanaan sesuai dengan anggaran yang ada dan memenuhi usulan kebutuhan puskesmas

Abstract

Masalah gizi buruk dan kurang gizi pada balita masih menjadi tantangan besar di Kota Medan. Tujuan penelitian ini bertujuan untuk menganalisis proses perencanaan dan penganggaran program PMT pada balita di Dinas Kesehatan Kota Medan agar program dapat berjalan lebih efektif dan efisien. Metode penelitian ini menggunakan pendekatan kualitatif deskriptif dengan metode wawancara mendalam terhadap 4 informan yaitu pengelola gizi, kepala bidang kesehatan masyarakat, kepala seksi kesehatan keluarga&gizi dan kasubag perencanaan di Dinas Kesehatan Kota Medan. Hasil penelitian ini adalah perencanaan yang komprehensif dengan melibatkan berbagai pihak menjadi kunci keberhasilan program ini. Namun, keterbatasan anggaran dan kendala lain perlu diatasi. Proses penganggaran masih memiliki kekurangan dalam pemenuhan kebutuhan di lapangan. Kesimpulan penelitian adalah proses perencanaan dan penganggaran program PMT di Dinas Kesehatan Kota Medan telah dilakukan dengan cukup baik, namun masih menghadapi beberapa tantangan terutama terkait keterbatasan anggaran. Perbaikan dalam hal penganggaran, pemanfaatan teknologi informasi, serta peningkatan partisipasi masyarakat perlu dilakukan untuk meningkatkan efektivitas program
