<![CDATA[Abstract The outpatient medical record service at the UPTD Puskesmas Gunungsitoli Idanoi District from October 1st, 2021 to December 31st, 2021 was 1.452 medical records with 2 (0,1%) incompleteness in identification, 456 (31,4%) important reports, 718 (49,4%) authentication and 413 (28,4%) recording. These factors can affect the legal dimensions of medical records because it concerns the issue of law for certainty. This study aims to determine the correlation between identification review, important report review, authentication review, and recording review of the legal dimensions of medical records at the UPTD Puskesmas Gunungsitoli Idanoi District. The design of this study is qualitative analytics with a cross-sectional research design. The research sample totaled 339 outpatients with medical records. The statistical test used the chi-square. From March 14th, 2022 to June 30th, 2022, from 339 outpatient medical records studied, obtained incompleteness in the identification components totaled of 2 (0,6%), important reports totaled of 3 (0.9%), authentication totaled of 2 (0,6%) and recording amounted to 21 (6,2%) The conclusion of this study shows a correlation between the completeness of filling out the outpatient medical records to the legal dimensions of medical records at the UPTD Puskesmas Gunungsitoli Idanoi District. The identification with value of p=0,012, important reports with p-value = 0,000, authentication with p-value = 0,000 and recording with p-value = 0,000.Keywords:Completeness, Incompletenes, Medical Records, Legal Dimensions Abstrak Pelayanan rekam medis rawat jalan di UPTD Puskesmas Kecamatan Gunungsitoli Idanoi dari tanggal 1 Oktober hingga 31 Desember 2021 adalah 1.452 rekam medis dengan 2 (0,1%) identifikasi tidak lengkap, 456 (31,4%) laporan penting, 718 (49,4%) autentifikasi dan 413 (28,4%) pencatatan. Hal ini dapat mempengaruhi dimensi hukum rekam medis karena menyangkut masalah adanya jaminan kepastian hukum. Penelitian ini bertujuan untuk mengetahui hubungan antara review identifikasi, review laporan penting, review autentifikasi, review pencatatan terhadap dimensi hukum rekam medis di UPTD PuskesmasKecamatan Gunungsitoli Idanoi. Rancangan penelitian ini adalahanalitik kuantitatif dengan desain penelitian cross sectional. Sampeldalam penelitian ini berjumlah 339 rekam medis rawat jalan. Uji statistikyang digunakan adalah uji chi-square. Hasil penelitian pada 14 Maret2022 sampai 30 Juni 2022 dari 339 rekam medis rawat jalan yang ditelitididapatkan ketidaklengkapan pada komponen identifikasi berjumlah 2(0,6%), laporan penting berjumlah 3 (0,9%), autentifikasi berjumlah 2(0,6%) dan pencatatan berjumlah 21 (6,2%). Kesimpulan dari penelitianini menunjukkan bahwa ada hubungan antara kelengkapan pengisianrekam medis rawat jalan terhadap dimensi hukum rekam medis di UPTDPuskesmas Kecamatan Gunungsitoli Idanoi yaitu identifikasi dengannilai p = 0,012, laporan penting dengan nilai p = 0,000, autentifikasidengan nilai p = 0,000 dan pencatatan dengan nilai p = 0,000.
