Abstract

Breast feeding has been intensively campained throughout the country in the last decade, which is thought to be responsible for the decline of the incidence of gastroenteritis. One of the most important advantages of breast feeding is the immunologic properties of breast milk. The importance of breast milk in the protection of many infections of the newly born infants, who are naturally immunologically deficient, is well-established. Breast milk contains humoral as well as cellular immunity components, each component has its elements although it is possible to describe the role of each components, it is generally thought that the protective properties of human milk against infections in neonates and young babies are the results of interactions of many factors rather than the effect of solely each factor. The role of humoral (immunoglobulins) and cellular (macrophages, lymphocytes, and other cellular elements) immunity of human milk, especially its protective properties, is discussed in this article.

Abstract

A longitudinal study on acute inspiratory infections was conducted from April 1988 until June 1990, in Cikutra, an urban community in the municipality of Bandung, Indonesia. The study consisted of. 3 parts: a presurvey, a cross sectional study, and a one-year prospective study. All children aged less than five years in Cikutra were included in the presurvey. A simple questionnaire was used for collecting data. In the cross sectional study 500 children were selected by stratified random sampling. Field investigators visited the children's homes and interviewed mothers using a standardized questionnaire. For the prospective study 269 children of less than 48 months of age were enrolled, and followed for one year. The prevalence of all ARI was 57-58%, mild-moderate ARl 55-56% , and severe ARJ 5%. On average the children suffered from 6.7 episodes of ARl per child per year, with a mean duration of episode of 5.3 days. Several factors showed significant relationship with the prevalence, incidence, severity of duration of ARI.

Abstract

The subjects of the study consisted of 164 sick newborn infants (97 boys and 67 girls) who had no signs or symptoms of infections. The intravenous fluid drip (at scalp vein or saphenous vein) was put on soon after taking blood culture. Removal of venous catheters was indicated when the patients condition. No longer needed their use or there was sign of local infection. This study showed that the lower the birth weight or the gestational age, the higher the incidence of infection. The most prominent nosocomial infection in this study was phlebitis (16.5%) followed by infiltration (14.6%), and bacteremia (7 ,9%). The predominant microorganism was E. coli (59.7%). The overall incidence of nosocomial infection was 44.5% and the mortality was 24.7%.

Abstract

The purpose of this prospective cohort study was to evaluate the breastfeeding promotion program m Dr. C1pto Mangunkusumo general hospital which we started m 1991. The study was done from July 1992 until March 1993. During that time. We could follow 249 mother-infant pairs every month for 4 months. Results: 1. Babies who received only breast milk during hospital stay did not lose more weight if compared to babies who got some formula. 2. Not a single baby lost weight more ~10% during hospital stay· Prelacteal feeding delayed the adequacy of breastmilk; 4. Though prelacteal feeding once or twice by spoon did not interfere with full breast feeding at the age of 4 months, yet mothers whose babies were given prelacteal feeding started to give supplementary food earlier; 5. Family income places a role in the decision to give early food supplement.

Abstract

The risk factors of low birthweight infants were assessed in a retrospective study covering 3607 singleton livebirth infants at Manado Hospital from January until December 1993. The analysis confirmed that patterns of risk birthweight hypertension in pregnancy (P<0.01), maternal education (P<0.01), maternal age (P<0.05), and parity (P<0.01), marital status (P<0.01), history of abortion (P<0.05), and parity (P<0.01). Anemia in pregnancy was also associated with birthweight in low birth weight (P<0.05). Asymetric intrauterine growth retardation (Ponderal Index below 2.32) was found both in premature and term infants.

Abstract

Between December 1993 and December 1994, 46 patients with acute leukemia treated with vincristine were evaluated for the possibilities of peripheral neuropathy. Of the 46 patients, 39 patients had acute lymphoblastic leukemia, and 7 had acute non-lymphoblastic leukemia. All patients had received vincristine; 29 (63%) of the 46 patients had it for 5 - 9 weeks, and 17 (37%) had it for 4 weeks or less. rn 10 (21.7%) patients peripheral neuropathy was detected clinically, and in 35 patients (76%) the neuropathy was detected by electrodiagnostic examination. No evidence of neuropathy was detected in 11 patients. The electrodiagnostic examination was more sensitive than the clinical examination. Pe1ipheral neuropathy, either detected clinically or by means of electromyography, occurred mostly in patients with the dosage of vincristine of 5-20 mg, and the duration of treatment of 5-9 weeks.

Abstract

We report a case of 12 years old girl with acquired torticollis caused by juvenile rheumatoid arthritis. The main complaint are the head inclined to the right and the pain of right neck. We established the working diagnosis based on the history and typical clinical pictures. After having physiotherapy and drug therapy with acetosal, the patient become beter with the head and neck is in normal condition again. The importance is to treat cases of tortocollis as soon as possible, before there is a contractire of the muscles of the neck.
