Abstract

Background: Depression is highly prevalent in primary care settings and depressed patients of all ages are seen by their Primary Care Provider (PCP) for treatment (Datto et al., 2003).  In order to adequately treat depression, PCPs must follow the clinical guidelines for follow-up.Purpose: The purpose of this project was to implement a standardized follow-up plan for patients aged 12 and older with a positive screen for depression using the Patient Health Questionnaire (PHQ-9) screening tool. Methods: The setting was a rural clinic in Broken Bow, Nebraska.  The sample included 89 patients seen between September 1 -November 30, 2020 who met the criteria of a PHQ-9 score of 5 or greater compared with 47 patients seen between September 1 -November 30, 2019. Quantitative data was analyzed using descriptive statistics, frequency and means and compared between before and after implementation of this quality improvement project. Results: In 2019, 11.3 % of patients reported depression. That more than doubled to 25.6 % in 2020.  Of the depressed patients in 2019, 36.2% had mild depression, while the number almost doubled to 60.7% in 2020. The 2020 PHQ-9 results were reviewed from most severe down to mild depression.  Of the 35 patients with the highest severity, 89% had at least one measure completed representing an adequate follow-up plan.  Referral rates improved from 12.8% in 2019 to 14.6% in 2020.  Medication was the most common treatment in both years with 57.4% in 2019 and 27% in 2020.  Completed follow-ups improved from 45.5% in 2019 to 73% in 2020; with 1 nurse phone call follow-up and one CSSR-S completed. Conclusion: It is unknow the affects from the 2020 pandemic.  However, and increase in mild depression shows a need to know how to treat sub-threshold depression. The use of the template as a guide led to improvement on all measures. However, some measures were used only sparingly and use may be improved with increased acceptance of the guide.

Abstract

Background: A mother feels complete when she fulfills her role in protecting her child from harm.  If a child is diagnosed with a terminal illness and reaches End of Life (EOL), the mother’s life is affected negatively.  The mother is affected mentally, psychologically, and physically because her child is dying.Purpose: Explore the recent studies that focused on experiences of mothers caring for a child receiving End of Life Care (EOLC) at home or a healthcare facility. Methods: An electronic search was done in multiple databases, including the University of Wisconsin-Milwaukee Library, PsycINFO, PubMed, CINAHL, and Academic Search Complete. The included studies were published within the past five years, studies about EOL/ EOLC, hospice care, pediatric care, quantitative, qualitative, and mixed methods studies. Thematic synthesis following PRISMA guidelines.  Results: Four themes emerged from the thematic synthesis.  The main themes included decision making challenges, communication with healthcare professionals, parents’ support system, and grief. Parents were included in most studies. However, the number of mothers exceeded the number of fathers.  Some researchers mentioned that there was no difference between their participants (most participants were Caucasian).  Conclusion: There is limited information available about children and their family members’ experiences at EOL/ EOLC in Saudi Arabia as has been studied in other countries.  Based on this systematic review, more studies are needed related to children at EOL in Saudi Arabia.

The application of the Professional Nursing Practice Method (MPKP) can illustrate the efforts of various countries to improve the quality of nursing care and the working environment of nurses in hospitals.Purpose: The purpose of this study was to determine the application of MPKP in hospitals. Methods: This research design uses a literature review, which is a series of studies relating to the method of collecting library data, or research whose research objects are explored through various library information (books, encyclopedias, scientific journals, newspapers, magazines, and documents) from several journals that have been researched by previous researchers who have been screened with inclusion and exclusion criteria using several databases namely google shoolar, lib UI, and SINTA which will be used as a literature review in this study. There are 155 journals according to keywords which are screened into 10 journals according to the theme and analyzed which examines the application of the Professional Nursing Practice Method (MPKP).  Results: The results showed that the implementation of MPKP was in the good category and was more dominant, namely 40% in the nursing care system. In the application of the maximum Professional Nursing Practice Method (MPKP) and good categories, nurses have to make professional values, professional relationships, management approaches, nursing care systems, and reward compensation systems according to nursing standards by looking at performance and determining the appropriate MPKP model.  Conclusion: The better the implementation of the Professional Nursing Practice Method (MPKP) is carried out, the better the services provided by the hospital to patients

Abstract

Background: Indonesian Population Demographic Survey (IDHS) in 2017 stated that the Maternal Mortality Rate (MMR) in Indonesia was 309/100,000 live births. According to Ristica (2017) the high maternal mortality rate in Indonesia is caused by the unpreparedness of the mother in facing childbirth. Maternal unpreparedness is usually seen among primigravida who struggle in preparing for childbirth as they have yet experience in delivering. Geniofam in Muthoharoh explained (2018) the factors that influence mother's readiness including knowledge, education, socio-culture and economy.Purpose: This study aimed to determine the mother social characteristic (age, education, occupation) that affect the readiness of primigravida in facing childbirthMethods: This type of research is analytical research with an observational approach. The sample in this study involved 53 primigravida on the third trimester of pregnancy. Data collection was carried out by filling out questionnaires on the readiness of mothers to face childbirth by respondents. Results: The results of statistical tests showed that age, education, or occupation did not affect mother's readiness in dealing with childbirth with p value > 0.05.Conclussions:  mother’s social characteristics do not affect the readiness of primigravida in facing childbirth.

Activities in Chronic Disease Management Program (Prolanis) include medical or educational consultations, home visits, reminders, club activities.Purpose: This study aimed to analyze the application of the prolanis program based on the caring theory by reducing blood pressure in hypertensive patients. Methods: The design of this research is pre-experimental with cross sectional study approach. The population in this study were all patients with hypertension who took part in management program activities in the public health center (PHC), a number of 40 people, with the sampling technique used was accidental sampling and the total sample was 28 people. The data collection method used a questionnaire. The data analysis of this research is to use the Paired t test. Results: The result of this study is that there is a relationship between the application of a chronis disease management programs based on caring theory with blood pressure reduction with an average systolic blood pressure before the intervention of 142?26.15 and after the intervention of 138?1.21 with p= 0.000 which means that the module intervention can decrease systolic and diastolic blood pressure. While for diastolic blood pressure, the results of the study showed that the mean blood pressure before the intervention was 85?10.36 and after the intervention was 85?7.61 with p= 0.000.Conclusion: It can be concluded that there is a possitive effect between giving prolanis module based on caring theory with systolic and diastolic blood pressure
