Abstract

Chronic Kidney Failure is a progressive and irreversible end-stage kidney disease. Data obtained from medical records on the number of patients with CRF at dr. Chasbullah Abdulmajid Bekasi City from January to March 2023 reached 122 patients. The aim of the study was to obtain an overview of the implementation of nursing care in patients who experience hypervolemia with CRF at dr. Chasbullah Abdulmajid, Bekasi City. The research design used is descriptive qualitative. Participants in this case were 2 patients with the same medical diagnosis and sex. Nursing care was carried out for 3 days. After the assessment was carried out, the results showed edema in the extremities, abnormal respiration, red itchy spots, decreased hematocrit hemoglobin in laboratory tests, increased creatinine urea, and abdominal ultrasound. Based on the study, 5 nursing diagnoses were obtained with the same 4 diagnoses, namely hypervolemia associated with decreased renal filtration, ineffective peripheral perfusion associated with decreased hemoglobin concentration, risk of impaired skin integrity associated with uremia and anxiety associated with less exposure to information related to hemodialysis treatment. After carrying out 3x24-hour nursing actions, namely monitoring intake output, the diagnoses of Mr. R and Mr. C have not been resolved. There was a gap between the two patients, in Tn. R there were problems with nursing ineffective breathing patterns as evidenced by increased respiration. At Mr. C, acute pain is evidenced by a pain scale of 6. Suggestions for further researchers are to collect and validate complete data so as to get maximum results.

Abstract

Schizophrenia is a condition where individuals experience difficulties with their perceptions of life, difficulties in establishing relationships with other people, and difficulties in determining attitudes for themselves. One of the negative symptoms of schizophrenia is withdrawing from the environment. The subjects in this case study used 2 clients of social isolation in the Eagle room of RSJ Dr. Suharto Heerdjan Jakarta. The aim is to obtain an overview of nursing care for mental clients with social isolation in paranoid schizophrenia. The design of this study uses a qualitative approach with descriptive study analysis. The results of research conducted on client I (Mr. S) and client II (Mr. F), in this study both found the same medical diagnosis, namely paranoid schizophrenia. After conducting an assessment of the two clients, they found the same nursing problems, including social isolation, sensory perception disorders, hallucinations: hearing loss, low self-esteem, ineffective family coping, ineffective therapy regimens, risk of violent behavior, and only self-care deficits that only exist in client I (Mr. S). Then carry out the implementation strategy 1 to build a trusting relationship, educate clients about the advantages and disadvantages of socializing, and train clients to get to know 1 person. Implementation strategy 2 trains to get acquainted with 2 people. Implementation strategy 3 teaches getting to know 3 or more people. Implementation strategy 4 teaches socializing in groups. Conclusion; Nursing care for Mr. S & Mr. Suggestions for families to include clients in activities and involve clients in family discussions.

Abstract

Hypertension is known as a cardiovascular disease where sufferers have blood pressure above normal. This disease is often called the silent killer because there are no symptoms and unknowingly the sufferer experiences complications in other organs. At the Cita Sehat Pratama Clinic, East Jakarta, in 2019, hypertension was included in the 10 most common diseases. Data from the World Health Organization states that 50% -70% of patients do not comply with prescribed antihypertensive drugs. This study used an analytic method with a cross-sectional approach. The number of samples taken was 160 people and carried out from August to September 2020. The sampling technique used the Purposive Sampling method. Bivariate results showed a significant relationship between the knowledge of hypertensive patients (p-value = 0.0001) and adherence to taking hypertension medication, with OR = 6.919 and attitudes of hypertensive patients (p-value = 0.001) with adherence to taking hypertension medication, with OR = 17,647. Based on the research results, it is suggested to the clinic to make a sustainable hypertension patient management program in an effort to treat and improve the quality of life of hypertensive patients

Abstract

Schizophrenia is a group of psychotic disorders with typical distortions of thought patterns, sometimes having the feeling that they are being controlled by forces outside themselves, sometimes strange delusions, perceptual disturbances, abnormal affect that is integrated with real or actual situations, and autism (Zahnia & Wulan Sumekar, 2016). On January 31, 2023, at Dr. Mental Hospital. Suharto Heerdjan Jakarta in the knee room obtained data on 35 hallucinatory patients. Hallucinations are a symptom that often appears in people with mental disorders and are closely related to early psychosis due to trauma in childhood. Hallucinations usually arise due to a change in the orientation of reality, the patient feels a stimulus that is not actually there. General goals in carrying out nursing care for patients who experience sensory perception hallucinations: hearing at the Dr. Suharto Heerdjan Jakarta. In this scientific paper, the author uses a qualitative descriptive research method with a case study approach with interventions in nursing care problems in patients with hallucinatory sensory perception disorders: hearing d. In this study, 5 diagnoses were found that were the same in Mr. A and Mr. H is hallucination sensory perception disorder, social isolation, low self-esteem, ineffective therapy regimen, and risk of violent behavior. And 1 different diagnosis, namely Mr. A is experiencing family coping, while Mr. H occurs ineffective individual coping. The next stage is carrying out implementation strategy 1 identifying hallucinations and teaching how to rebuke them, implementation strategy 2 teaching controlling hallucinations by conversing, and implementation strategy 3 controlling hallucinations by carrying out activities. There was no discrepancy between the theory and the results of the study of the two patients.

Abstract

Based on data from mid-July 2021, there were 286 active cases of COVID-19 in Papanggo Village. This study aims to find out the relationship of knowledge and media exposure to COVID-19 prevention behavior in adolescents in Papanggo Village in North Jakarta in 2021. This research was conducted by quantitative methods with a cross-sectional study design. The research was conducted in July - August 2021 with Google Forms. The study sample included 83 teenagers living on RT 003/03 & and RT 006/03, which numbered 83 people taken by accidental sampling method. The analysis in this study used the chi-square test. Univariate results showed that as many as 59% of respondents had good COVID-19 prevention behavior, then it was found that 92.8% of respondents had good knowledge about COVID-19, in the media exposure variable as many as 54.2% of respondents were positively exposed to COVID-19 information media. The results of this study also found that there is a significant relationship between knowledge and COVID-19 prevention behavior (p-value = 0.028), and there is also a relationship between media exposure and COVID-19 prevention behavior (p-value = 0.001). Based on the results of the study, adolescents are advised to maintain knowledge and keep abreast of COVID-19 development information.

Abstract

Risiko perilaku kekerasan merupakan salah satu respon marah diekspresikan dengan melakukan ancaman, mencederai diri sendiri maupun orang lain. Skizofrenia menimbulkan perubahan persepsi, emosi, dan tingkah laku yang mengarah ke perilaku kekerasan yang dapat berbahaya dengan diri sendiri maupun orang lain disekitarnya. Berdasarkan hasil pencarian dari Our World in Data 2019 terdapat 829.735 skizofrenia di Indonesia, sedangkan pada kasus risiko perilaku kekerasan di ruang Nuri terdapat 60 kasus. Tujuan umum pelaksanaan asuhan keperawatan pada Tn.I dan Tn.E dengan risiko perilaku kekerasan pada skizofrenia diruang Nuri Rumah Sakit Jiwa Dr. Soeharto Heerdjan Jakarta. Desain penelitian yang digunakan pada karya tulis ilmiah ini adalah kualitatif dan deskriptif. Hasil pengkajian dilakukan pada 2 pasien yaitu Tn.I dan Tn.E ditemukan persamaan diagnosa keperawatan yaitu: Risiko perilaku kekerasan, Gangguan sensori persepsi: halusinasi, isolasi sosial, harga diri rendah, regiment terapi in efektif. Perbedaan diagnosa antara 2 pasien yaitu koping keluarga in efektif dan koping individu in efektif. Dan yang lebih diperhatikan yaitu Tn.E. Tahapan selanjutnya yaitu perencanaan dilakukan tindakan strategi pelaksanaan 1,2, dan 3 tetapi ada pengulangan strategi pelaksanaan ditahap 2 pada Tn.I dan Tn.E. Pada tahap evaluasi dari diagnosa keperawatan yang dimiliki oleh kedua pasien yaitu belum teratasi, tetapi kedua pasien sudah memiliki perkembangan tahap mengontrol emosi sesuai dengan tujuan dan kriteria hasil yang sudah ditetapkan. Pada saran, semoga dapat digunakan sebagai buku dan jurnal keperawatan jiwa di perpustakaan.
