Abstract

Background: Due to the outbreak of Covid-19, medical students were suffering from poor mental health, leading to depression, anxiety, and stress. Research to better understand mental health is essential because of the high prevalence of poor mental health among students during the pandemic.Objective: Therefore, the study aims to determine the mental health status of UNIMAS medical students during the Covid-19 pandemic. In this study, the predictive validity of the Depression, Anxiety, and Stress Scale (DASS-21) for the presence of a depressive disorder and its associated coping factors were investigated among medical students.Methods: A cross-sectional study was conducted among medical students from the University Malaysia Sarawak's Faculty of Medicine and Health Sciences. The DASS-21 was administered to 411 respondents, and data were analyzed using Man-Whitney and Chi-Square tests.Results: The result showed that most medical students had normal levels of depression (75.7%), anxiety (74.2%), and stress (92.5%).   The highest coping method used was rest and sleep (95.1%), followed by singing and listening to music (91.5%).Conclusion: Most UNIMAS medical students were in good mental health during the COVID-19 pandemic. They tend to have a better-coping mechanism before the stressful condition deteriorates.

Abstract

Background: Post-COVID is a multifactorial disease that describes the residual effects of acute COVID-19 infection that are continuous or relapsing and in remission. The study found that 87% of people who recovered and were discharged from the hospital showed persistence of at least one symptom even within 60 days. COVID-19 patients who have bipolar disorder require therapeutic adjustments to avoid specific drug interactions between psychotropic drugs and those used in COVID-19 protocols. Objective: To determine the treatment of post-COVID patients with bipolar affective disorder.Methods: Collect and analyze research articles on Update Therapy for Post-COVID Syndrome Patients with Bipolar Affective Disorder. These articles were obtained by searching using Google Scholar, PubMed/NCBI, and SAGE Journal. Results: The use of combination drugs between antipsychotics and antidepressants with hydroxychloroquine/azithromycin is not recommended because it has side effects that can induce psychiatric symptoms. COVID-19 patients with bipolar disorder (BD) who do not respond to pharmacotherapy may receive electroconvulsive therapy (ECT). Non-pharmacological therapies such as counseling and the use of telepsychiatry are effective in treating mental health and overcoming adverse psychological effects.Conclusion: COVID-19 patients with bipolar affective disorder require unique therapy to avoid drug interactions and pharma-cotherapy. Non-pharmacological efforts include counseling and telepsychiatry to cope with emotional distress and mental health challenges. However, challenges remain in providing care for these patients.

Abstract

Background: Uterine leiomyomata, often known as fibroids, are frequently accompanied by symptoms that are common to a variety of different pelvic disorders. When considering treatment choices, it is critical to correctly identify myomata and rule out other disorders, including malignancies. In this case report, the author emphasizes the difficulty in establishing a preoperative diagnosis and the actions taken during the operation.Case report: Female, 46 years old, has three children who live with two children. She was complaining of left abdominal pain and penetrating to the waist. There were no complaints of defecation or urination. A mass in the lower abdomen, solid springy, and half of the symphysis center tend to the right with limited mobility. The preoperative diagnosis is pedunculated uterine myoma. It was decided to do a Supravaginal Hysterectomy and Bisalphingo-oforectomy. The pelvic cavity still looks bulging, and exploration was carried out to find uterine myomas of the intrasacrouterina ligament. Then, it was decided to do a Myomectomy.Conclusion:Pedunculated uterine myoma intrasacrouterina ligament performed Supravaginal Hysterectomy and Myomectomy

Abstract

Background:  Pleuropulmonary amebiasis is the most common complication of amoebic liver abscess, occurring in 15% of patients with amoebic liver disease and in 1% of patients with amoebic dysentery. Most commonly occurs by direct extension of a right superior lobe hepatic abscess through the diaphragm to the right lower lobe of the lung with a mortality rate of 5-16%. Primary pleural amebiasis without liver involvement is rare.Case Presentation: Reported an 18-year-old male patient with pleural amebiasis from the results of parasitological examination of pleural fluid found Entamoeba histolytica. History, physical examination and support showed normal liver function. The patient has a history of poor sanitation with a dissertation of unhygienic habits. Patients without liver problems remain at risk for amoebiasis pleuraConclusion: Pleural amebiasis without liver involvement was a rare case. Further investigation was needed in patients with pleural amoebiasis to other organs, such as the liver and digestive tract. Diagnose pleural amebiasis should be the main focus in determining the etiology. Entamoeba histolytica may be involved in pleural inflammation and cause effusion.

Abstract

Background: Hands are the most common medium for spreading disease. Red betel leaf has antibacterial bioactive compounds such as flavonoids, alkaloids, polyphenols, tannins, and essential oils. The most common way to clean hands is to use hand sanitizer.Objective: To determine the antibacterial potential of red betel leaf extract (Piper crocatum Ruiz & Pav.) against the growth of Staphylococcus aureus, Streptococcus pyogenes, and Salmonella typhi as an active ingredient in hand sanitizers. Methods: Each bacterium, namely Staphylococus aureus, Streptococcus pyogenes and Salmonella typhi were divided into 6 groups, PEG 5% as a negative control, clindamycin for Staphylococus aureus and Streptococcus pyogenes and chloramphenicol for Salmonella typhi as a positive control, 70% alcohol as a comparison for hand sanitizer, extract 20%, 40%, and 80% as treatment groups. The obstacle zone is measured using a ruler.Results: Zones of inhibition against Staphylococus aureus, Streptococcus pyogenes, and Salmonella typhi were formed at concentrations of 20%, 40%, and 80%. Kruskal Wallis statistical analysis showed significant results with p=0.000.Conclusion: Red betel leaf extract has statistically significant antibacterial activity against Staphylococcus aureus, Streptococcus pyogenes and Salmonella typhi. The extract concentration of 40% is the optimal concentration as a raw material for hand sanitizer.

Abstract

Background: Fibroadenoma mammae is the most common type of benign breast tumor in women. Fibroadenoma mammae can occur on various ages, there is no much data recorded about anatomical pathology especially regarding the distribution of fibroadenoma mammae growth pattern in Siaga Medika Purbalingga Hospital. Study in Indonesia that focus on this topic also remain scarce.Objective: To analyze comparison between the growth pattern in fibroadenoma mammae patients with age range 11-50 years.Method: This cross-sectional study with data collected restrospectively based on medical records and histopatological examination in Siaga Medika Purbalingga Hospital. Statistical analysis was done using JASP 0.14.1.0.Results: The majority of patients with fibroadenoma mammae were in the age range 21-30 years (48.9%) with pericanalicular pattern (66%). The four groups had different two growth pattern (p <0.001). Odds ratio showed 3.4 that means patient with age ≤30 years has chance 3.4x to diagnose with pericanalicular fibroadenoma mammae than patient with age >30 years.Conclusion: Incidence of pericanalicular and intracanalicular fibroadenoma mammae growth pattern in Siaga Medika Purbalingga Hospital patients can be affected by age.

Abstract

Background: Pneumonia is an inflammation of the lung parenchyma. Pneumonia became the first most lung health problem in West Lombok. Smoking is one of the main factors in CAP. This study aims to determine the relationship between smoking status and severity of inpatient CAP patients.Methods: Case control study design was used with medical record measurement tools. A total of 70 samples were taken using a purposive sampling technique for community pneumonia patients (CAP) at Patut Patuh Patju Regional General Hospital Gerung, West Lombok in November 2021-October 2022. Calculation of the severity of CAP patients used the PSI score. Results: The results obtained a p-value of 0.000 (p-value <0.05) for smoker status with a degree of severity of CAP, and a p-value of 0.064 (p-value> 0.05) for a degree of smoking with a degree of severity of CAP. The estimated risk (OR) for smoking status with the severity of CAP is 19.33.Conclusions: There is a significant relationship between smoking status, but not with smoking degree, to severity of CAP at Patut Patuh Patju Gerung Regional General Hospital, West Lombok.

Abstract

Background:  Ovarian Germ Cell Tumors originate from primitive germinal cells and can be either malignant or benign. MRKH syndrome is characterized by congenital hypoplasia of the uterus and upper vagina and can occur due to disrupted fusion of the Mullerian ducts. Diagnosis of ovarian tumors in MRKH patients is difficult but can be characterized by abdominal pain and distended.Objective: The aim of this case report is to explain the diagnostic methods and interventions performed in patients with ovarian tumors and MRKH syndrome.Case Presentation: A 25-year-old female came to the hospital with complaints of a lower abdominal lump three months ago. It was followed by severe pain, weight loss, shortness of breath, and yellowish vaginal discharge. Physical examination showed anemia, obesity, and a vagina size of 7cm with a probe. Chest X-ray showed a mass in the mediastinum and pleural effusion, and USG showed suspicion of an ovarian cyst and uterine agenesis.Conclusion: Ovarian tumor with MRKH syndrome is a rare case. Diagnosis was based on the patient’s history, clinical findings, radiologic examination, and confirmed with laparotomy and histopathology. Regular examinations are recommended to prevent and identify genital tract problems and pelvic diseases in women, especially adolescents.

Abstract

Background: Antibiotics are a treatment used to treat bacterial infections. However, the many antibiotic-resistant bacteria caused researchers to use plants as an alternative treatment, one of which is garlic (Allium sativum L.).Objective: Previous research stated that the diameter of the inhibition zone by garlic had a more excellent value than the antibiotics used. Hence, the researchers were interested in conducting an antibacterial test of the ethyl acetate fraction of garlic against the growth of Staphylococcus aureus and Escherichia coli.Methods: This study was an experimental laboratory using the posttest-only control group design method in the Kruskal Wallis test data analysis.Results: the results of the inhibition analysis with p-value < 0.05 in each test group for both Staphylococcus aureus and Escherichia coli. With the Mann-Whitney test, the results obtained a p-value <0.005 for each bacteria and test group. The most significant value, namely p= 0.002. This result shows significant differences between the test groups for each bacterium.Conclusion: There is an antibacterial effect of the ethyl acetate fraction of garlic (Allium sativum L.) at concentrations of 25%, 50%, and 100% against the growth of Staphylococcus aureus and Escherichia coli

Abstract

Background:  Adherence to taking medication is a prerequisite for hypertension treatment to be more effective and most likely to cure patients in controlling hypertension so that complications and death do not occurObjective: The objective of this study is to discover the dominant factors of elderly hypertension patients' conformity to medication at the Jatinegara District Health CenterMethods: It is a quantitative study with Cross Sectional approach. It involved 200 respondents, as the sample was taken using a quota sampling technique.Results: Univariate results showed that 25% of elderly hypertensive patients did not adhere to medication. In bivariate analysis, it was found that family support, self-efficacy, motivation, and health workers' roles were related (p-value <0.05) to adherence to hypertension medication consumption. Meanwhile, no relation (p-value 0.05) existed between age, knowledge, attitude, access to healthcare facilities, and JKN participation variables. The dominant variable observed from the multivariate result was the interaction of attitudes and self-efficacy (OR: 23,943).Conclusion:Based on multivariate analysis, it was found that the influential factors were self-efficacy and attitudes.
