Abstract

Introduction: Bronchial carcinoids are slow-growing tumors that are usually asymptomatic until the later stages and mimic most benign conditions clinically. This study presented pulmonary carcinoids (PCs) in terms of diagnosis and management in a tertiary care center. Case: The mean age of presentation was 42.5 years old. Right lung involvement was more common, and the frequency was higher in men. In all patients, second to fourth generations of airways were the site of involvement. The most common symptom was a dry cough. Bronchoscopic biopsy complemented by immune histochemistry was the main diagnostic modality. The majority of patients (60%) had a typical PC. Stage I or III of the disease was the most common presentation. Nine lobectomies and two pneumonectomies were performed on 11 of 15 patients. These patients are performing well to date. Four patients were given only chemotherapy, of which one has survived. The longest follow-up period was 26 months. Conclusion: A high degree of clinical suspicion and familiarity with carcinoids is essential for early identification and a positive clinical outcome, especially in typical carcinoids (TC), as there are no obvious risk factors and various clinico-radiological presentations. In this study of 15 cases, most of the PCs were right-sided and more common in males. The use of a biopsy, either by a guided bronchoscopy or by computed tomography (CT), was confirmatory, and surgical removal was the treatment of choice. Medical therapy may be considered for select patients with carcinoid syndrome or terminal disease.

Abstract

Introduction: Chronic obstructive pulmonary disease (COPD) is a prevalent respiratory disease that has a significant global impact. Unfortunately, current treatments and prevention remain insufficient. Nigella sativa (NS) has been traditionally used as an anti-inflammatory, antioxidant, and immunomodulatory agent, which can be helpful in the treatment of COPD, through its essential component thymoquinone. This review examined the potential benefits of NS for COPD patients as a possible treatment option. Methods: The literature investigation was conducted by searching relevant articles on Google Scholar and NCBI using the keywords "black seed", "black cumin”, "Nigella sativa", "thymoquinone", "carvacrol", and "COPD". The results from five identified studies were then summarized. Results: This review reveals that NS offers benefits to individuals with COPD by reducing inflammation and oxidative stress, thereby leading to improved pulmonary function tests (PFTs). However, it is important to note that the majority of experiments have been conducted on animals, with only one involving humans, and these experiments have mostly been short-term. Additionally, since different studies have used various preparations of NS, determining the recommended dose is challenging. Unfortunately, the research does not provide any insights into the potential long-term effects of these treatments or the possible adverse effects of the medications used. Conclusion: Studies on the benefits of NS as an anti-inflammatory and antioxidant for COPD adjuvant therapy show promising results, but more research is needed to confirm the findings with larger and more well-designed studies to determine the appropriate doses and long-term effects.

Abstract

Introduction: Asthmatics, especially uncontrolled asthmatics, must be vigilant during the COVID-19 pandemic to avoid major respiratory complications. This study aimed to describe these characteristics in emergency unit patients with asthma and COVID-19 at Haji General Hospital, Surabaya, an East Java referral hospital. Methods: The study gathered medical information on 160 asthma patients from March to December 2021. The collected data were related to socioeconomics, allergy history, family allergy history, and medications. Results: 160 confirmed cases of asthma occurred during COVID-19 at Haji General Hospital, Surabaya. Most adult asthma sufferers were women (59.2%), high school graduates (31.25%), housewives (21.2%), had payment method with Social Health Insurance Administration Body (BPJS) insurance (73.8 %), had a history of drug allergies (11.5%), had no family history of allergies (50.8%), and used albuterol control drugs (10.7%). Meanwhile, the majority of children with asthma were boys (53.3%), did not attend school or did not finish elementary school (33.3%), did not work (33.3%), had payment method with BPJS (73.3%), had a history of allergies to seafood and cow's milk respectively (10%), had no family history of allergies (40%), and used albuterol control drugs (16.7%). Conclusion: The proportion of asthma in the emergency unit was dominated by women over 18 years old with a high school education. Most patients were housewives and used BPJS. Many of them had a history of allergies to medications (not asthma medication), and there was no family history of allergies. Commonly used medications were albuterol to control asthma.

Abstract

Introduction: N95 masks are very popular for daily use and can actively filter microparticles such as viruses. However, there are concerns that prolonged wearing of the mask may result in exhaled CO2 accumulation between the face and the mask, which may cause increased end-tidal carbon dioxide (EtCO2) levels and physiological changes. This study aimed to determine EtCO2 levels in students wearing the N95 mask. Methods: This was an experimental study with a case-control design. The subjects were divided into a treatment and a control group and were subjected to five measurements in 120 minutes. The data were analyzed using the independent samples T-test (normally distributed data) and the Mann-Whitney U test (not normally distributed data). Results: Mann-Whitney test at minute 0 showed that p = 0.010, independent sample T-test at minute 30 showed that the control group p = 0.016, while in the treatment group p = 0.019. Independent sample T-test at minute 60 for the control group p = 0.034, while the treatment group p = 0.037. Independent sample T-test at minute 90 for the control group and treatment p = 0.001, and independent sample T-test showed that at minute 120 in the control and treatment groups p = 0.000. Conclusion: The EtCO2 measurements at each of the five time points showed a statistical difference (p < 0.05). However, medical students who wore an N95 mask did not suffer adverse effects because the EtCO2 values in the control group were higher than in the treatment group.

Abstract

Introduction: Asthma is a global problem, with around 100-150 million people estimated to suffer from this disease. Based on data from the Centers for Disease Control and Prevention (CDC), around 8.4% of people in the United States suffer from asthma. Asthma requires holistic treatment, one of which is family medicine. However, there is a lack of data or research discussing this matter. Hence, this case report aims to provide an overview of family medicine in asthma, especially in children. Case: This study is a case series report involving four pediatric patients with asthma. Data collection was performed by conducting home visits and hetero-anamnesis. Many things affect asthma treatment apart from pharmacology, including family medicine. In this case series, the role of the family proved significant in the patient's healing and daily functioning. Conclusion: Family medicine is a type of holistic management critical in various diseases, especially asthma. Support from the patient's family and environment has an essential role in the success of asthma therapy in children.

Abstract

Nowadays, around 50% of the world's population lives in cities and urban areas and is exposed to increasingly higher levels of air pollutants including gaseous pollutants and particulate matter (PM). The latter is a key indicator of air pollution transported into the air by a variety of natural processes and human activities. The particles' size has been directly related to their potential impact on health problems. Particles of concern include small particles with diameters of 2.5 to 10 μm (coarse particles) and smaller than 2.5 μm (fine particles). Exposure to air pollutants over a long period of time not only decompensates pre-existing diseases but also leads to pulmonary and respiratory health problems such as asthma, chronic obstructive pulmonary disease (COPD), and lung cancer even in rural areas. A thorough analysis has to be provided to address the implications for policymakers. Hence, more stringent strategies can be implemented to control air pollution and prevent its health effects.

Abstract

Introduction: Pulmonary aspergilloma is a disease frequently found in immunocompromised patients. In Indonesia, around 18% of diabetes mellitus (DM) patients suffer from chronic pulmonary aspergilloma. However, data on type 1 DM (T1DM) are still limited. Case: We presented the case of a 22-year-old male admitted to the emergency room due to hemoptysis of approximately 200 ml within six hours before admission, nonspecific chest pain during cough, fever, night sweats, and weight loss of 3 kg over the last months. The patient had a history of T1DM and no prior history of respiratory diseases. Physical examination showed tachycardia, tachypnoea, subfebrile, normal body mass index, and rhonchi on the left thorax. Routine laboratory tests revealed increased blood sugar level (503 mg/dL) and HbA1c 16.4%. Chest X-ray and high-resolution computed tomography (HRCT) showed a cavity with an opaque lesion, crescent sign, consolidation, and reticulonodular infiltrate on the left lung. Rapid molecular tests, tuberculosis (TB), and serum galactomannan (GM) were negative. The patient was diagnosed with pulmonary aspergilloma, T1DM, and clinical TB. He was treated with fluconazole, insulin, and a fixed-dose combination of anti-TB. The patient refused the surgery procedure. Immediately, the hemoptysis stopped, blood glucose level was normal, and concomitant with weight gain. Conclusion: Due to innate and acquired immunity impairment, T1DM is a risk factor for pulmonary infections, including TB and pulmonary aspergilloma. Chest HRCT may help diagnose fungal balls. Though the patient refused to undergo surgery, administering antifungal, anti-TB drugs, and glucose control as initial treatment presented a good prognosis in the patient.

Abstract

Introduction: Coronavirus is a viral pathogen that can be transmitted through respiratory droplets, leading to severe infectious diseases. This study aimed to ascertain the influence of the COVID-19 pandemic on birth weight during pregnancy. Methods: This study used secondary data and a retrospective case-control approach for observational analysis. The case and control groups each had 153 samples. The case group was further separated into symptomatic and asymptomatic categories. Statistical analyses were performed to determine how COVID-19 infection in pregnant women affected birth weight. Fisher's exact test was used to evaluate data for the subgroups of asymptomatic and symptomatic individuals, while the Mann-Whitney test was used to compare the infection's impact. Results: Most participants were 21–35 years old (78.2%). The findings showed that 85% of pregnant COVID-19 patients were in their third trimester. Mothers diagnosed with COVID-19 had infants with birth weights of >4,000 grams (3.3%), 2,500–4,000 grams (43.1%), and <2,500 grams (53.6%). Birth weight distribution in women unaffected by COVID-19 was >4,000 grams (14.4%), 2,500–4,000 grams (67.3%), and <2,500 grams (18.3%). The Mann-Whitney statistical test was performed using SPSS. It yielded a p-value of 0.001, less than the significance level of 0.05, indicating a correlation between confirmed COVID-19 infection in pregnant women and birth weight. Conclusion: This study found that the presence of COVID-19 infection during pregnancy significantly affects infants' birth weight.

Abstract

Introduction: Depression affects 45.19% of pulmonary tuberculosis (TB) patients' adherence to treatment, resulting in increased morbidity and death, medication resistance, and continued disease transmission. This study aimed to examine the relationship between omega-6/omega-3 (êž·-6/êž·-3) polyunsaturated fatty acids (PUFA) intake ratio and depression score in patients with pulmonary TB at Persahabatan National Respiratory Referral Hospital, Jakarta. Methods: This was a cross-sectional study of 99 subjects with pulmonary TB. Data were collected using the Semi-Quantitative Food Frequency Questionnaire (SQ-FFQ), anthropometric measurements, and the Beck Depression Inventory-II (BDI-II). Results: Correlation analysis used the Spearman test and revealed the ratio of êž·-6/êž·-3 PUFAs intake of 7.78 ± 1.13, the median value of depression score was 9 (10-36). There was no correlation between êž·-6/êž·-3 PUFA intake and depression score (r = 0.063; p = 0.534). Conclusion: There was no correlation between êž·-6/êž·-3 PUFAs intake ratio and depression scores in pulmonary TB patients. This is the first study to examine the correlation between êž·-6/êž·-3 PUFA intake ratio and depression score in pulmonary TB patients.

Abstract

Introduction: Vapor is considered a safer alternative to tobacco cigarettes because the high nicotine content is less. However, vapor still contains substances that are classified as toxic to humans. Short-term exposure to vapors from liquids can induce an inflammatory response in the lungs and cause oxidative stress. This study aimed to determine the effects of e-cigarette vapor smoke on pulmonary alveoli in Rattus norvegicus lungs. Methods: This was an experimental study on 32 adult male Rattus norvegicus rats. They were divided into two groups exposed to nicotine-containing vapor smoke two times a day for one and three months, respectively, and one group as control. After vapor exposure, the lung tissues of the rats were taken and then subjected to histopathological examination under a microscope. Results: After exposure for one month, epithelial and endothelial cells degenerated, characterized by a decrease in collagen and elastin fibers in the extracellular matrix. For three months, there were changes, the alveolar membrane had no nucleus, the surrounding endothelial cells were not visible due to damage to the extracellular matrix, the alveolar lumen had widened, causing edema in the lumen of the alveoli, and the alveoli wall was destroyed. Therefore, the connection between the alveoli was stretched. Conclusion: This study found that short-term exposure to nicotine vapor causes damage to the alveoli membrane.
