Abstract

Stroke merupakan penyebab kematian dan disabilitas tertinggi di Indonesia. Stroke juga penyebab disabilitas jangka panjang tertinggi di dunia. Stroke Iskemik mencakup 85% dari seluruh jenis stroke. Stroke iskemik disebabkan oleh adanya sumbatan pembuluh darah akibat tromboemboli atau penyempitan akibat aterosklerosis. Stroke iskemik merupakan penyakit yang dapat disembuhkan. Pengobatan dengan trombolitik intravena (IV-tPA) dan trombektomi mekanik dapat memperbaiki luaran pasien stroke iskemik akut bila dilakukan pada periode emas, yaitu kurang dari 4,5 jam sejak onset gejala untuk IV-tPA dan <6-24 jam untuk trombektomi mekanik. Akan tetapi, hanya sebagian kecil pasien yang dapat memenuhi periode tersebut. Di Indonesia, keterlambatan ini disebabkan oleh kurangnya pengetahuan masyarakat mengenai gejala stroke, masalah transportasi, dan sistem penanganan stroke yang belum terintegrasi. Setiap satu menit penanganan stroke yang terlambat, terjadi kematian 1,9 juta neuron di otak. Sistem yang baik dalam menangani stroke dapat meningkatkan luaran pasien serta menurunkan lama rawat dan biaya perawatan pasien ke depannya. Salah satu pendekatan penanganan stroke adalah dengan sistem 8D yang terdiri dari detection, dispatch, delivery, door, data, decision, drug/device, dan disposition. Sistem ini membantu penanganan stroke secara multidisiplin dan komprehensif dengan pembagian tugas yang jelas. Sehingga pasien mendapat penanganan yang tepat dan diharapkan memiliki luaran yang baik.

Abstract

Introduction: High myopia correction with clear lens extraction, with or without IOL implantation, remains controversial and associated with a high risk of postoperative complications, especially retinal detachment. However, advanced technology development in the cataract surgery field had resulted in excellent surgical outcomes with a very low complication rate. In this article, we will present an outcome of clear lens phacoemulsification for high myopia correction in four eyes from two patients.Case presentation: Two women with high myopia undergone clear lens phacoemulsification with supracapsular tilt and tumble technique. IOL calculation using SRK-T formula. Both patients had 20/20 postoperative visual acuity in both eyes, which remains until one year postoperative. No retinal abnormalities found in both patients.Conclusion: Clear lens phacoemulsification for high myopia correction is an effective procedure and can prove excellent visual outcome for a long-term period. It can be considered as an alternative method for high myopia management.

Abstract

Introduction: Type 2 diabetes mellitus is caused by decreased tissue sensitivity to insulin. The prevalence of diabetes in the world has almost doubled since 1980, from 4.7% to 8.5% in adult population. Early diagnosis and treatment aimed at normalizing glycemic control are very important. The objective of this study was to evaluate and compare glycemic control of metformin and glimepiride in monotherapy of type 2 diabetes mellitus patients at Islamic Jemursari Hospital Surabaya. Method: This was a retrospective observational study using secondary data (medical record), include glycemic control (RPG) before and two months after receiving therapy of outpatients’ type 2 diabetes mellitus with metformin or glimepiride therapy in 2018. 96 samples were found that fit the inclusion criteria. The data were analyzed by Mann-Whitney test. Result: Most patients were female, aged 50-69 years old, and dosage of metformin therapy 1500 mg/day or glimepiride therapy 2 mg/day. There was no significant difference (p>0.05) of glycemic control (RPG) of metformin compared to glimepiride therapies in type 2 diabetes mellitus patients at Islamic Jemursari Hospital Surabaya in 2018. Conclusion: Metformin and glimepiride were not significantly different in glycemic control (RPG). There were patients with RPG >200 mg/dl after two months of metformin or glimepiride therapy.

Abstract

Introduction: Intelligence affects someone in thinking and solving problems. Frisson sensation increase dopamine hormone production, activity and blood flow to the brain. Aim: To prove the association between frisson sensations on music stimulus and intelligence performance change. Methods: A quasy experimental study with one group pre- and post-test design was done to female medical students of Diponegoro University. A total of 30 participants were selected by purposive sampling technique. The stimulation was by listening to mp3 of 5 researcher-selected songs that last for 15 minutes in 3 consecutive days. The frisson measured by using a stopwatch. Intelligence performance before and after the frisson sensation was measured using Ravens Progressive Matrices, so that the difference could be calculated. The collected data was then analyzed using Wilcoxon test and simple linear regression. Result: The pre-test score had a median of 50.50 (30-56) and a post-test score of 54 (33-59), with a significant increase in RPM scores (p <0.001). The duration of frisson sensation had positive effect on RPM scores differences (p <0.001). Association between the duration and the score improvement could be formulated as: y = 0.047 + 2,711x. Conclusion: The frisson sensation on music stimulus can improve intelligence performance. The frisson that lasts longer will have more significant improvement on intelligence performance

Abstract

Introduction: Children admitted to the Pediatric Intensive Care Unit (PICU) are a vulnerable population with a high degree of morbidity and mortality. Laboratory tests such as full blood count and infection parameters were additional tests that help clinicians assess the severity of pediatric patients’ illnesses. Besides that, the PELOD-2 score, a scoring system, also helped evaluate the degree of a critical condition in children. However, the components of PELOD-2 are quite complicated and unattainable in the limited-resource healthcare facilities. We expect that the simple laboratory examinations could be an alternative to the PELOD-2 score when the score is not accessible. This study evaluates the correlation between leukocyte count, Neutrophil-Lymphocyte Ratio, CRP, and PELOD-2 score. Methods: This is a comparative retrospective study in 75 pediatric patients admitted in PICU RSCM from January 2019 to August 2020 and passed the ethical review. The exclusion criteria for this research was a patient with incomplete data. To analyze the variables independently, we used Spearman analysis. Meanwhile, to evaluate the relationship between the independent variables and PELOD-2 simultaneously, we used logistic regression. Results: Based on the Spearman analysis, this study found a significant correlation between Leukocyte count (0,059; CI:90%) and PELOD-2 score, as well as Procalcitonin (0,076; CI: 90%) and PELOD-2. The logistic regression also showed that Leukocyte count and Procalcitonin significantly correlated with the PELOD-2 score.Conclusion: Leukocyte count and Procalcitonin can be an alternative to the PELOD-2 score when healthcare facilities cannot fulfil the PELOD-2 scoring system’s components.

Abstract

Intoduction: Acute and chronic exposure to toluene at high doses is known to affect all organs of the body including the spermatogenesis process. In the industrial sector, the use of toluene as a solvent is still widely used, up to 10 million tons per year. The control over health problems that may occur is carried out by applying work exposure threshold values. This research aims to explore the effect of toluene exposure at the threshold value range on spermatogenesis.Method: This research used laboratory experiment on 30 male Wistar rats which were divided into five groups of different exposure levels, namely 12.5 parts per million (ppm], 25 ppm, 50 ppm, 100 ppm, and no exposure (control). Exposure was given for 4 hours daily over 14 days through a hood with measured release in the glass cage. The toluene exposure markers observed were Malondialdehyde (MDA) in the blood tissue and testicles using the Thiobarbituric Acid Reactive Substances (TBARS) method. The effect on the spermatogenicity process was assessed by counting the spermatogonia A cells of male Wistar rats with Periodic Acid Schiff (PAS) staining and is calculated by the Abercrombie formula. Analysis of the correlation between the level of exposure and its effect on the increase in malondialdehyde, and spermatogenesis was carried out using the Spearman correlation analysis.Result: There was a moderately positive correlation between levels of toluene exposure and plasma MDA levels (r = 0.42; p = 0.025). Meanwhile, on [the issue of] the quantity of spermatogonia cells, a high level of negative correlation with exposure levels was obtained (r = -0.68; p = 0.001).Conclusion: Toluene exposure in male Wistar rats within the range of threshold values influenced the increase in plasma MDA levels and decreased the Spermatogenia A cells. However, toluene exposure did not affect the testicular MDA levels of male Wistar rats.

Abstract

Introduction: The method to improve learning achievement is peer tutoring method. Students can grow and increase their interest in learning, so that the learning materials in the tutorial can be easily understood. By increasing student interest in learning, student learning success will be achieved. The problem raised in this study is how the peer tutoring method influences the results of tutorial activeness.Research Objectives: Knowing the participation of peer tutoring on the value of the activity of the “Blok Masalah Pada Anak” tutorial on the students of Faculty of Medicine UII Force 2015.Methods: The experiments were quasi experimental research. This research design uses posttest only with control design. A total of 40 research subjects came from 2 different groups. The ratio of the control group to the treatment group was 1: 1. The control group was a group that was not given peer tutoring and the treatment group was a group given peer tutoring. Data analysis using Independent T TestResults: There was no difference in scores of scenario 6 activation score in the control group and treatment group with p value 0.704 and 95% CI interval between -9.79 to 4.79 (past 0)Conclusion: There is no relationship of peer tutoring with the score of the activity of tutorial block Problems In Children at the Faculty of Medicine, Universitas Islam Indonesia batch 2015.

Abstract

Introduction: Studies showed that the condition of joint hypermobility is quite common in children. In general, joint hypermobility asses through the Beighton score method. The objective of this research was to investigate the relationship between beighton score and walking age in children.Method: This research was conducted at a kindergarten in Surabaya using an observational analytic research type and a cross-sectional design. Data collected through interviews and physical examinations. A joint hypermobility examination performed using the Beighton score criteria. Meanwhile data was analyzed using the crosstabulation and Spearman test.Result: It was found that 84% of children had ≥4/9 score on the Beighton score and there were 92% with positive Beighton score in the knee joint. In this study, as many as 20 children experienced the first independent walks over 18 months when 18 of them who has Beighton score ≥4 points. However, there is no correlation found on the Beighton score and the walking age [sig. (2-tailed) = 0.053].Conclusion: Children who have ≥4 points of Beighton score are mostly found in this study. The relationship between the Beighton score and walking age was not statistically significant.

Abstract

Pandemi COVID-19 dan Produktifitas Pandemi COVID-19 telah berdampak merugikan masyarakat, tidak hanya kesakitan dan kematian tetapi juga pada berbagai bidang sosial dan ekonomi. Efek pandemi dengan berbagai situasi yang berhubungan seperti pembatasan kegiatan, pembatasan perdagangan, pembatasan pergerakan, dan penguncian daerah menyebabkan terbatasnya kegiatan berbagai sektor produksi. Pada bulletin Word Food Program COVID-19: Economic and Food Security Implications (4th Edition)1 merilis data tentang situasi Indonesia setelah 9 bulan mengalami masa pandemi. Dari data yang disampaikan didapatkan bahwa Product Domestic Bruto (PDB) Indonesia terus menyusut selama kuartal kedua berturut-turut karena pandemi, dengan tingkat inflasi tahunan meningkat menjadi 1,6% pada November 2020. Harapan optimis dengan gambaran secara berangsur adanya pertumbuhan positif di semua sektor ekonomi.
