Abstract

The problem often found in patients wearing a complete denture with a flat ridge condition is the lack of retention and stabili-ty, which interferes with masticatory function. These conditions can be overcome not only in the moulding process, but also in determining the occlusion concept during tooth preparation. To overcome this problem, the concept of lingualised occlusion is a solution and is expected to improve retention, stability and better support in flat ridge cases. This article reports the concept of lingualised occlusion in denture preparation in the case of a patient with mandibular posterior flat ridge. A 62-year-old woman came to the Department of Prosthodontics of RSGM Unhas with complaints of difficulty chewing food due to the loss of all her teeth. The patient had used a denture but now felt loose and wanted to make a new denture so she could chew well. It was concluded that the concept of occlusion in patients with flat ridge uses the concept of lingualised occlusion which can im-prove retention, denture stability, and patient mastication efficiency.

Abstract

Complications often occur in mandibular complete dentures, so to overcome retention and stability problems, implants are used as overdenture implant support. Attachment systems that connect implants and overdentures vary widely, one of which is a magnetic system. This article reports the use of magnetic retention on an implant-supported overdenture as rehabilitation of the total edentulous in the mandible. A 53-year-old man came to the Department of Prosthodontics Dental Hospital Hasanuddin University with complaints of difficulty chewing food due to the loss of all RA and RB teeth, the patient wanted to make a den-ture that was not easily separated so that it could function properly while eating. Implants were placed on both sides of the RB as overdenture support, implant osseointegration was assessed and followed by the installation of magnetic attachments on the implant and GTL inner surfaces. The 2-IOD results improved the patient's oral function, provided good stability and retention for the denture and improved mastication performance. It was concluded that the use of two implants in RB coupled with a magnetic attachment system can improve retention and stability in overdenture denture use.

Abstract

Epulis fissuratum (EF) is a reactive tissue response to sustained mechanical stress generated by an ill-fitting denture. This title is important as additional information about EF and its management for dentists and dental specialists. This article demonstrates the treatment of EF by excision and vestibuloplasty. A 61-year-old woman came to a Dental and Oral Hospital with com-plaints of enlargement of the lower left gum felt since about 1 year ago. Initially the size was small like rice but over time it enlarged to what it is now. There was a history of the patient using dentures since about 10 years ago which were installed by a dentist. On clinical examination, a lump was found in the lingual vestibule region 45-36 with a size of 5x2x0.5 cm, pedun-culated, soft consistency and smooth texture. The lesion was diagnosed as EF and was treated with excision and vestibulo-plasty which is the treatment of choice to optimise prosthetic integration by increasing the residual alveolar height. It was con-cluded that ill-fitting greatly increases the prevalence of mucosal lesions, especially EF. Surgical excision and vestibuloplast-ty can be as one of the treatment of choice.

Abstract

This case report describes a single visit endodontic of teeth with pulpitis irreversible asymptomatic. A 23-year-old male came to the dental hospital with a complaints of caries and pain on his left molar mandibular. Positive on cold thermal stimuli, nega-tive on percussion and palpation. Radiographs show caries has reached the pulp chamber, and a diagnosis of pulpitis irreversi-ble asymptomatic was obtained. Single visit endodontic with single length technique using Protaper Next file was conducted, so there were no subjective and objective complaints after 1 week control. It is concluded that single visit endodontic of tooth with pulpitis irreversible asymptomatic cases gives good and efficient results.

Abstract

Epulis is a tumour-like enlargement of the gingiva originating from the periodontal connective tissue, generally caused by chronic irritation, usually growing on the anterior teeth, reducing aesthetics, confidence and lip function so that treatment is ne-cessary. Various epulis treatment techniques with advantages and disadvantages of each treatment technique. This article com-pares the management of fibromatous epulis using a scalpel combined with periodontal flap surgery and electrosurgery through the presentation of several cases. Clinical examination showed no residual epulis, marginal gingiva at the epulis area was slight-ly reddish and the healing process went well in cases 1 and 2 compared to case 3. Pathological examination results showed that the tissue was covered by epidermis with pseudoepithelomatous hyperplasia, showed no malignancy and the diagnosis was fibromatous epulis. It was concluded that the management of fibromatous epulis showed good results with surgical technique using OFD combined scalpel compared with electrosurgery at 1 week control.

Abstract

Gingival enlargement is a condition of increased gingival size that can cause aesthetic and dental hygiene problems. One of the etiologies of gingival enlargement is systemic factors caused by the consumption of drugs from the calcium channel blocker class. Amlodipine taken by hypertensive patients can trigger gingival enlargement. Gingivectomy is performed to remove gi-ngival pockets and inflammation so as to obtain physiological, functional and aesthetic gingiva. A 47-year-old woman came to Hasanuddin University Dental Hospital, Makassar with complaints of discomfort and lack of confidence with the condition of the maxillary and mandibular front gums since six months earlier. The patient had been taking hypertension medication, amlodipine 10 mg once daily since 2 years ago. After disinfection of the surgical area, anaesthesia with lidocaine 2% nor epi-nephrine, pocket marking using pocket marker base, incision using Krickland knife, gingival removal using Orban knife and scaling and root planing, periodontal pack was used. It was concluded that hypertensive patients who take amlodipine can trig-ger gingival enlargement.

Abstract

The maxillary labial frenulum is often attached to the centre of the upper lip and between the two upper front teeth. Frenectomy is one of the periodontal plastic surgeries to remove high frenulum attachment. This article presents the management of cen-tral diastema correction with labial frenectomy to support successful orthodontic treatment and improve aesthetics. A 25-year-old female wanted to straighten her teeth, found an enlarged central diastema due to a labial frenulum that reached the gingival margin and incisor papillae; there was a high maxillary labial frenulum on the interdental papillae and surrounding tissues. Frenectomy was performed to remove the predisposing factor of periodontal disease due to high frenulum attachment and aimed to correct the central diastema along with orthodontic treatment. The 2-month postoperative control showed that the central diastema had been corrected. It was concluded that frenectomy is one of the periodontal plastic surgeries to remove the highly attached frenulum to achieve a pleasing aesthetic.
