Critical Medical and Surgical Nursing Journal (CMSNJ) (p-ISSN: 2355-1992), (e-ISSN: 2656-470X) was established at the end of 2012 under Universitas Airlangga. The CMSNJ was prepared in response to the development of nursing/health science and technology to contribute to the development of sustainable quality nursing/health resources. CMSNJ is published two times per year every April and October. Articles published in CMSNJ is written in English since Vol. 10 No. 1 April 2021. Abstract

ABSTRACTIntroduction: HIV/AIDS sufferers must consume antiretrovirals every day and control routinely each month to take ARVs in the hospital. The condition make patients having a risk of lost to follow up attitude. The purpose of this study was to determine the factors related to the attitude of lost to follow-up on people living with hiv/aids with arv therapy at rsud dr. soetomo surabaya. Method: This study used descriptive correlation design with a cross-sectional approach. The population were 135 patients and 100 patients were required as research participant with consequtive sampling. Independent variables were perceived susceptibility, perceived severity, perceived barrier to action, perceived benefits of action, cues to action, and self efficacy. The dependent variable was lost to follow up behavior. Data were obtained by questionnaire and analyzed with Spearman's Rho. Result: There was a relationship between perceived susceptability (p=0.002), perceived severity (p=0.025), perceived barrier to action (p=0.022) and cues to action (p=0.011) with lost to follow-up behavior. There was no correlation between perceived benefit of action (p=0.196) and self efficacy (p=0.071) with lost to follow-up behavior. Discussion: Knowledge and awareness regarding the importance taking antiretroviral drugs regularly and control routinely needs to be increased for patients with antiretroviral drugs. Further researchers are advised to conduct in-depth research (qualitative research) related to the causes of lost to follow-up behaviour thus.

Abstract

Introduction: Five-finger hypnosis is a relaxation technique that can provide a relaxing and calming effect by recalling pleasant experiences that have been experienced so that they can have a positive effect on the vital-sign of respondents with fractures. The purpose of this study was to determine the effect of the influence of five-finger hypnosis auditory on vital signs: blood pressure, pulse frequency, respiratory frequency, and pain in clients with limb fractures. Method: This research uses quasi-experiment with post test only non-equivalent control group design. The total population is 57 patients and 54 respondents were obtained with consecutive sampling. The independent variable is a five-finger hypnosis auditory intervention and the dependent variable is systolic blood pressure, diastolic, frequency of frequency, respiratory frequency, and pain. Data were obtained from the Numeric Rating Scale (NRS) and observation sheets. Then the data were analyzed by the Mann-Whitney U Test. Result: There were differences between the treatment and control groups on systolic (p=0.002), diastolic (p=0.003), pulse frequency (p=0.003), respiratory frequency (p=0.000) and pain (p=0.001). Discussion: So the hypnosis auditory as a relaxation technique can have a positive influence on the respondent's vital sign which includes systolic blood pressure, diastolic, pulse frequency, respiratory frequency, and pain. It is expected that nurses can apply this therapy as a non-pharmacological therapy to patients with extremity fractures and other researchers can apply to patients with all types of fractures as well as the development of cultural cultures.

Abstract

Introduction: Nurse is the spearhead of hospital services at disaster, which is also links to the Integrated Emergency Response System (SPGDT). This study aims to describe self efficacy of instrument nurse during the assignment period. Method: The study used a qualitative design with descriptive phenomenology approach. The study population was instrument nurse assigned to the earthquake in West Nusa Tenggara (NTB), the study sample was obtained by snowball sampling, participants were 16 instrument nurse. Method of collecting data using in-depth interview. Data collection tools used are voice recorder, interview guideline, and field note. Data is processed by thematic analysis according to Colaizzi. Results: Nurses' self efficacy can be seen from a positive assessment of their capabilities as evidenced by expressing understanding, forming a team, and establishing an emergency response team. In addition, nurse have high motivation to do assignments, and nurse show a positive attitude when appointed to work in NTB. Discussion: Knowledge, attitudes, and motivation of nurses, will be build a nurse self efficacy. Nurses who have high self efficacy, are confident in their abilities in carrying out their duties, will be able to carry out their duties well despite the demands and high workload.

Abstract

Introduction: Triage is the process of sorting patients according to their severity. However, the phenomena that occur in the emergency room of some hospitals are different, triage is not carried out according to the standards. This is due to many factors, one of which is the triage model used. The purpose of this study was to look at the relationship between the use of Standard Operations Procedure for admitting new patients using ATS triage which was modified with the speed of response of nurses in the Emergency Installation. Method: This study used a correlative analytical design with a case study approach. The total population is 31 people and a sample of 28 people was obtained through inclusion and exclusion criteria. The independent variable in this study was the use of ATS (Australasian Triage Scale) and the dependent variable in this study is the nurse's Response Time. Data obtained by means of sample observation and analyzed using the Spearman correlation test rho. Results: There was no association between the use of ATS (Australasian Triage Scale) modification and the response time of nurses in the morning shift (p = 0.720), day shift (p = 0.866) and night shift (p = 0.173).Discussion: From the results of the study there was no relationship between the use of ATS (Australasian Triage Scale) and the response time of nurses in the morning, afternoon and night shift. The use of ATS modification and the response time of nurses that are appropriate and fast are found in morning service. This is caused by several factors both from the nurses themselves and from the hospital. Most nurses do not get training in Triage and this affects nurses' skills in sorting patients.

Abstract

Introduction: People with diabetes mellitus were at 15% risk of developing diabetic foot ulcers. Diabetic foot ulcers can be prevented if people with DM have the knowledge, positive attitude and thus able to perform the diabetic foot care. The aim was to determine the effect of health education on knowledge, attitudes and preventionof diabetic foot ulcers of people with diabetes mellitus. Method: quasi-experiment research design pretest-posttest control group design. Sample was DM patients who visited the Bangkalan City Health Center, recruited by purposive sampling technique with sample size of 35 people in the treatment group and 35 people in the control group. Independent variable was the health education and the dependent variables were knowledge, attitudes and of diabetic foot ulcers. Data were collected with questionnaires andthen analyzed by Wilcoxon Sign Rank Test and Mann-Whitney Test.Result: The results showed that there was an effect of health education on knowledge (p=0.000), attitudes (p=0.000) and preventive actions (p=0.000) of diabetic foot ulcers.Conclussion: Health education was important as an effort to persuade DM patients to take charge on their knowledge and attitude foot care by firstly enhancing.

Abstract

ABSTRAKPendahuluan: permasalahan yang sering terjadi pada klien yang menjalani hemodialisis (HD) adalah terjadinya  penambahan berat badan atau Interdialytic Weight Gain (IDWG) karena  ketidak patuhan klien dalam pembatasan cairan. Untuk itu diterapkan Self help Group (SHG) dan Acceptance and Commitment Therapy (ACT).Tujuan penelitian adalah mengetahui perbedaan efektifitas pemberian Self Help Group dengan Acceptance and Commitment Therapy terhadap penurunan IDWG klirn yang menjalani HD.Metode: desain penelitian quasy experimental the pretest-posttest one control group disgn. Jumlah responden 57 orang dengan menggunakan purposive sampling. Variabel independen adalah SHG dan ACT, sedangkan variabel dependen IDWG. Untuk variabel independen menggunakana instrumen Satuan Acara Kegiatan,  sedangkan untuk variabel dependen menggunakan instrumen timbangan BB dan lembar observasi IDGW. Uji statistik untuk SHG dan pembanding  menggunakan Wilcoxon Signed­_Rank Test dengan tingkat kemaknaan p≤0,005, untuk ACT menggunakan  Paired T Test dengan tingkat kemaknaan p≤0,005 dan untuk perbandingan antara  SHG, ACT dan Pembanding menggunakan Mann Withney dengan tingkat kemaknaan p ≤ 0,005.Hasil: Wilcoxon Signed Rank Test  kelompok SHG p=0,002, hasil Paired T Test kelompok ACT p= 0,000, hasil Wilcoxon Signed Rank Test   kelompok pembanding p= 0,224. Hasil statistik Mann Whithney antara SHG- ACT didapatkan nilai p= 0,838, uji Mann Whithney SHG-pembanding p= 0,044 uji Mann Whithney ACT-pembanding p=0,015. Hasil uji statistik ini menunjukan bahwa antara SHG dan ACT keduanya efektif terhadap penurunan IDWG namun pemberian ACT lebih efektif terhadap IDWG dengan p=0,015.Diskusi: Penerapan SHG dan ACT efektif terhadap penurunan IDWG pada klien yang menjalani HD, sehingga diharapkan SHG dan ACT dapat diterapkan untuk mengantisipasi IDWG yang berlebihan pada klien yang menjalani HD. Kata Kunci: Self Help Group, Acceptance and Commitment Therapy, Penyakit Ginjal Kronis, Interdialytic Weight Gain , Hemodialisis        ABSTRACTIntroduction: Problem  that often occurs in hemodialysis clients is the occurrence of Interdialytic Weight Gain (IDWG) due to client’s nonadherence in fluid restrictions. For this reason, Self Help Group (SHG) and Acceptance and Commitment Therapy (ACT) are applied. The purpose of the study is to determine the differences in the effectiveness of SHG with ACT towards the decrease of IDWGs in clients hemodialysis Method: the study used the quasi-experimental method with the pretest-posttest one control group design. The number of respondents is 57 people who were determined by using purposive sampling. The independent variables are SHG and ACT, while IDWG is the dependent variable. The instrument used for independent variables is activity event unit, while weight scale and IDGW observation sheets were used for the dependent variable. Statistical test using Wilcoxon Signed_Rank Test with significance level p≤ 0.005 to compare between SHG and the comparator, Paired T Test with significance level p≤ 0.005 for ACT, and Mann Withney with significance level p ≤ 0.005 to compare between SHG, ACT, and the comparator.Result: Wilcoxon Signed Rank Test of SHG’s group obtained p = 0.002, Paired T Test of ACT’s group obtained p = 0.000, Wilcoxon Signed Rank Test of comparator’s group obtained p = 0.224. Statistical test using Mann Whithney between SHG-ACT obtained p = 0.838, Mann Whithney test between SHG-comparator obtained p = 0.044, while Mann Whithney test between ACT-comparator obtained p = 0.015. The results of these statistical tests show that both SHG and ACT are effective against the decline of IDWG, but ACT is more effective than SHG with p= 0.015Discussion: the application of SHG and ACT is effective against the decrease of IDWG in clients Hemodialysis, therefore it is expected that both SHG dan ACT can be applied to anticipate excessive IDWG in clients who undergo HD.

Abstract

Introduction: Diabetes Mellitus is a disease that occurs because the body is unable to produce insulin or the body cannot use insulin effectively. The purpose of this study was to find out the effect of Progressive Muscle Relaxation (PMR) on stress and blood glucose decrease in patients who have diabetes mellitus type 2 disease.Method: The design of this study is quasi-experimental study. The population of this study were the patients who have type 2 of diabetes mellitus disease at Puskesmas Kedungdoro and Puskesmas Medokan Ayu. There would be 70 respondents chosen based on inclusion and exclusion criteria. The researcher chose the purpose sampling technique in choosing the participants in this research. The independent variable in this study was Progressive Muscle Relaxation (PMR). The dependent variable in this study were stress and blood glucose decrease. The data in this study were collected using questionnaires and measurement of pre-prandial blood glucose level. Data were analyzed using analysis of Wilcoxon and paired t-test for pre-post measurements, then for the inter groups the researcher used Mann-Whitney analysis.Results: The results showed that there was an effect of PMR on stress (p = 0.001) and in decreasing blood glucose (p = 0.021).Discussion: PMR is a relaxation focused on the patient’s attention in distinguishing the group of muscle when they relaxed than while in contraction condition. Thus, the patients are expected to be able to manage the body condition in managing stress and blood sugar decrease. For health officers, they are expected to use the PMR as the combination in reducing stress and blood glucose to improve the quality of life of the patients who have diabetes type 2 disease.

Abstract

Pendahuluan: Diabetes Mellitus (DM) dengan Ulkus Kaki Diabetes (UKD) dapat berakibat pada gangguan psikologis. Terapi Hiperbarik Oksigen (HBO) dapat mempercepat penyembuhan luka sehingga psikologis pasien dapat membaik. Penelitian ini bertujuan menjelaskan pengaruh hiperbarik oksigen terhadap psikologis penderita UKD. Metode: Penelitian ini menggunakan desain quasi experimental dengan pendekatan non-equivalent control group design. Sampel sebanyak 70 pasien pasien DM dengan UKD. Variabel indepeden adalah terapi HBO dan dependen adalah psikologis penderita UKD. Data dikumpulkan melalui kuesioner WHOQOL-BREF. Data dianalisis dengan Wilcoxon signed rank test dan Mann Whitney U test. Hasil: Intervensi HBO meningkatkan psikologis pasien UKD (p=0,012) dan terdapat perbedaan yang signifikan antara kelompok kontrol dan perlakuan yang signifikan (0,040). Diskusi: Pasien DM dengan UKD yang diberi HBO menunjukkan perbaikkan psikologis yang terlihat dari pasien lebih sehat, semangat, dan terdapat perbaikan UKD. HBO dapat dijadikan sebagai pilihan pengobatan dalam meningkatkan respon psikologis penderita UKD.

Abstract

Introduction. Kesadaran pasien DM tipe 2 yang kurang dan lamanya menderita merupakan faktor yang menghambat penderita DM tipe 2 sehingga tidak menjalankan self-management dengan baik. Tujuan dari penelitian ini adalah untuk mengetahui hubungan kecerdasan spiritual dan lama menderita dengan self-management pada penderita Diabetes mellitus (DM) tipe 2. Methods. Rancangan penelitian menggunakan cross sectional. Populasinya adalah pasien diabetes melitus tipe 2. Total sampel adalah 167 responden, diambil sesuai kriteria inklusi dan ekslusi. Teknik sampling yang digunakan adalah purposive sampling. Variabel independen adalah kecerdasan spiritual dan lama menderita. Variabel dependen adalah self-management. Data dikumpulkan dengan menggunakan kuisioner Kecerdasan Spiritual dan The Summary of Diabetes Self Care Activities (SDSCA), dan dianalisis menggunakan analisis spearman rho. Results.Hasil menunjukan bahwa ada hubungan kecerdasan spiritual dengan self-management (p=0,000) dan tidak ada hubungan lama menderita dengan self-management diet (p=0,879). Discussion. kecerdasan spiritual yang tinggi dapat membuat penderita DM tipe 2 memiliki self-management yang baik. lama menderita DM belum tentu membuat penderita DM memiliki self-management baik. selama penderita DM tipe 2 memiliki kesadaran untuk mejalankan self-management dengan baik, komplikasi dapat dicegah
