Abstract

Neonatal asphyxia is a condition in which a newborn does not immediately breathe spontaneously and regularly after birth. Neonatal asphyxia is one of the causes of infant mortality in the world which ranks second. The purpose of this study was to analyze, discuss, and interpret the factors that cause neonatal asphyxia. This study used a qualitative descriptive design with a phenomenological strategy using in-depth interviews, observation, and document review. Informants in this study amounted to 25 people with techniques purposive sampling. Processing uses data reduction, data presentation, drawing conclusions and verification. Data analysis using technical triangulation and source triangulation. The results showed that most of the causes of neonatal asphyxia in Pariaman Hospital were mostly caused by LBW and prematurity, human resources, infrastructure still needed quality improvement, improvement, and additions, especially for cases of severe asphyxia. SPO, RKK, and funds are good enough. In terms of process, there must be a resuscitation team and supervision of the implementation of the SPO. Cases of mild to moderate neonatal asphyxia can be handled properly, but if there are cases of severe asphyxia, a referral needs to be made because of incomplete infrastructure. Pariaman Hospital is not ready to anticipate the occurrence of neonatal asphyxia due to lack of training and quantity of health workers,standbywhen there is a risk of birth asphyxia neonaturum For this, it is hoped that the hospital canupgradeHuman resources, infrastructure, and being able to form a neonatal resuscitation team if a baby is born at risk of neonatal asphyxia.

Abstract

Vitamin D is essential to immune system function and influences many dermatological diseases, such as psoriasis and atopic dermatitis, a fat-soluble steroid hormone derivative sourced from food intake and synthesized in the skin through exposure to sunlight. Vitamin D deficiency is recognized as a global problem, with high cases in South and Southeast Asia. Acne vulgaris is a chronic inflammatory disease of the skin, specifically the pilosebaceous unit. Inflammation is a critical factor in the development of acne vulgaris. IL-17 is a proinflammatory cytokine that plays a central role in acne vulgaris inflammation. This study aims to obtain information regarding serum 25(OH)D deficiency for IL -17 acne vulgaris. The data collection method was through collecting data from articles related to serum 25-(OH)D deficiency against IL-17 acne vulgaris. The inclusion criteria involved original articles, case-control studies, and review papers on 25(OH)D, IL-17, and acne vulgaris. Vitamin D deficiency can lead to cosmogenesis and inflammatory exacerbations that characterize the acne vulgaris nodulocystic phenotype. The immune response in acne vulgaris is stimulated by the innate immune system via toll-like receptor 2 (TLR2) and adaptive immunity via Th1 lymphocytes. There is evidence that vitamin D inhibits TLR2 expression in monocytes and reduces proinflammatory cytokine production. The most common biomarker in inflammation is IL-17 which was significantly higher in acne vulgaris patients with vitamin D deficiency than in patients with normal vitamin D levels.

Abstract

The COVID-19 pandemic had an impact on the implementation of health programs, especially immunization services. This is certainly a risk for the occurrence of PD3I Extraordinary Events. The coverage of basic immunization in Padang City was so far from the national basic immunization target that has been set were 95%, coverage of BCG immunization (74.6%), DPT-HB-HIB immunization (55.5%), polio immunization (49%) and MR immunization (46%). Health behavior, especially basic immunization, can be influenced by several determinants. This study aims to determine the determinants of basic immunization visits for infants during the COVID-19 pandemic in the West Rawang Health Center, Padang City working area. This research was a quantitative approach with a cross-sectional design. This research was conducted from January to November 2022. The total sample was 206 mothers with children aged 12-28 months in the Rawang Barat Health Center working area. The sampling technique used proportional random sampling. Data were collected through questionnaires and univariate, bivariate, and multivariate analyses were performed. The results of the study on the determinants of the infant’s basic immunization visits during the COVID-19 pandemic in the Rawang Barat Health Center working area, namely mother's knowledge (p-value<0.001), socioeconomic (p-value 0.444), family support (p-value<0.001), healthcare professionals support (p-value<0.001). The dominant factor related was the mother's knowledge (OR 24,301). The conclusion of this study that there was a significant correlation between maternal knowledge, family support, and health worker support but socioeconomic factors had no significant correlation with basic immunization visits for infants during the COVID-19 pandemic, and the most dominant factor related was the mother's knowledge

Abstract

Premature rupture of membranes is one of many obstetric problems that is dangerous for a mother in maternity. It is one of the top five reasons behind child mortality in the world and also one of the cause of premature labor. Premature rupture of membranes could be caused by anemia through many pathological mechanism. This research was conducted to determine the prevalence of premature rupture of membranes in pregnancy of anemic mothers in RSUD Kota Tangerang on 2018 – 2020. This research is descriptive research with cross-sectional design. Data retrieval was carried out in RSUD Kota Tangerang throughout January – May 2022 with medical records instrument. Samples were retrieved using non-random sampling method. Based on the research results, a total of 75 samples of pregnant women with anemia was collected. The result showed a total of 38 samples (50.7%) pregnant women with anemia experienced premature rupture of membrane. In addition, as many as 37 samples (49.3%) of pregnant women with a history of anemia who did not experience premature rupture of membranes. It can be concluded the prevalence of premature rupture of membranes in pregnant anemic woman is more than those who did not experience premature rupture of membranes in RSUD Kota Tangerang.

Abstract

Diarrhea is the second leading cause of mortality in children under the age of five. In order to reduce the incidence of diarrhea cases in the working area of the Sindang Jaya Public Health Center. The research problem was analyzed using Blum's Paradigm. Priority problem employed non-scoring technique method (Delphi). Fishbone diagram was used to determine the root cause of the problem. Data were obtained through mini survey, pre-test, and post-test. The intervention was carried out through diarrhea counseling, demonstrations on how to make a solution of sugar and salt, and how to wash hands correctly. Monitoring was carried out using the PDCA cycle and systems approach. Therefore lifestyle was the main problem that contributes to the high cases of diarrhea. The result of the root cause was known to be a lack of public knowledge about diarrhea. The findings of the intervention increased knowledge about diarrhea, with the average post-test score of 87.33. Participants' knowledge of how to make a solution aimed to increase sugar and salt on how to wash hands properly. The problem had caused the increase in cases of diarrhea. After the community diagnosis was conducted, an increase in the knowledge of the participants who took part in the counseling was obtained. The average value of post-test counseling is above the target value. Participants could re-explain about how to make a solution of sugar and salt, and can practice how to wash hands in 6 steps correctly

Abstract

Diabetes Mellitus (DM) is a metabolic disease caused by abnormalities in the secretion of insulin, and the action of insulin, or both of them. In 2019, The International Diabetes Federation organization estimates 463 million people in the world who suffer from DM and Southeast Asia ranked third at 11.3%. While Indonesia is ranked 7th in Southeast Asia with 10.7 million patients. The prevalence of DM increased from 6% in 2013 to 8.5% in 2018. Based on Riskesdas 2018, DM disease in Banten province is amount of 1.62% (23,262). In 2021, there were 146 cases at Legok Public Health Center continuing to increase. Controlled blood sugar and risk factors for Diabetes Mellitus were conducted in Mr. F so that the complications could be prevented. Family medicine approaches with the Mandala of Health paradigm in a 49-year-old man was used for the complaints of wounds in both legs, itching since 2 years ago, headaches, fatigue, and tingling in the legs suffering from DM since November 2021. Based on the anamnesis of physical and supporting examination, the patient was diagnosed with uncontrolled DM accompanied by peripheral neuropathy, central obesity, and neurodermatitis potential diabetic ulcer. The results of the intervention showed a decrease in blood sugar, weight loss of 2 kg, complaints of itching, tingling, and wounds on both legs improved in Mr. F. Risk factors for uncontrolled blood sugar in Mr. F involved lack of knowledge, unhealthy lifestyle, lack of physical activity, non–compliance with medication and, doing control to the Health Center, financial limitations, as well as lack of support from Mr. F's family.

Abstract

Preeclampsia is the leading cause of morbidity and mortality in mothers during perinatal period, accounted for 70.000 mother deaths and 500.000 fetal death reported each year. Adolescent have not been set as the risk factor in guidelines to develop preeclampsia due to inconsistencies in literature findings. This study is a cross-sectional study which aims to find the incidence rate of preeclampsia and its complications in adolescent pregnancy located in Ciawi Regional Hospital during 2014-2016. This study involved patients aged 15-20 years old with complication of hypertension in pregnancy. This study found that 102 pregnant mothers with hypertension complication and 69 of them (67,6%) diagnosed as preeclampsia. Neonatal complication found in this study were 14,5% low-birth weight, 2,9% high-birth weight, and 26,1% IUFD. Maternal complication found were 24,6% preterm delivery, 2,9% post-term delivery, two patients (2%) with anemia, and one patient (1%) with uterine inertia, uterine rupture, and decompensated cordis. More than half of the sample in this study population were diagnosed with preeclampsia followed with various maternal and neonatal complications.

Abstract

Progressive multifocal leukoencephalopathy (PML) is a neurological condition caused by the JC virus. PML is a demyelinating disease preferentially affecting the central nervous system. JC virus is a DNA virus of the polyomaviridae family. The virus remains latent in most immunocompetent hosts and rarely presents pathologically. In immunosuppressed hosts, however, a combination of poor cellular response of the host along with reactivation of the virus secondary to recombination of genes results in active disease. This case report described a 42 years old male patient with a HIV, PML, Anemia. The patient complaints that the body feels weak, loss of consciousness and difficult to communicate with. Physical examination, laboratory examination and MRI study findings was support the diagnosis of PML in HIV patients. This case report described a 42 years old male patient with a HIV, PML, Anemia. Abstract

Hypertension is a condition in which systolic blood pressure is ≥ 140 mmHg and/or diastolic blood pressure is ≥ 90 mmHg after repeated measurements in a clinic or health care facility. Based on the results of RISKESDAS in 2018, Banten Province has a prevalence of hypertension amount 29.4%. Based on data at the Kresek Health Center in July-December 2021 which mentioned that hypertension was the disease with the most cases in the working area of the Kresek Primary Health Centerv(Puskesmas). In order to assess the problem, this study used the family medicine approach and the Mandala of Health as the problem assessment to determine the factors that cause uncontrolled hypertension. A 66-year-old woman came to Kresek Puskesmas with hypertension since 6 years ago. The patient has complaints of pain all over the head. She claimed routinely take drugs prescribed by the Puskesmas, but not routinely control blood pressure. Thus, blood pressure is still not controlled. After the intervention, the patient's complaints became reduced and her blood pressure decreased from 160/100 to 140/90. In addition, she obtained a family understanding of hypertension, factors that cause uncontrolled hypertension, as well as complications that can occur due to hypertension. The discovery of factors caused uncontrolled hypertension in patients. It is expected that she can improve her lifestyle to be better so that complications from hypertension do not occur.

Abstract

Wounds are a form of tissue damage to the skin caused by directly contact with a heat source, the result of medical action, or changes in physiological conditions. Wounds cause disturbances in the function and structure of the body's anatomy. The body has a physiological response to injury, which is in the form of a wound healing process. This type of research is a case study conducted on Mr. MI, male patient and 38 years old. Techniques for analyzing and handling cases were employed as a family medicine approach. The case diagnosis was a deep dermal/full thickness burn (degree 2-3) with a 28% TBSA e.c flame which was classified as a severe degree burn. The patient experienced primary and secondary trauma management. Fluid resuscitation was administered using the Parkland formula. Wound care was performed by tangential excision, followed by covering the STSG. The patient went home with an improved condition after 3 days of treatment. His prognosis is generally good, but with the risk of functional limitations in the burn area.
