Abstract

Bakcground: Leprosy continues to remain public health problem particularly in developing countries. Treatment is the most important aspect in leprosy control, however as the long duration of treatment compliance is critical point to success of therapy.Objective: Determine how big the problem of treatment compliance og leprosy patients in Kabupaten Blora and what factors influencing the compliance of those patients. A case control study was conducted in 22 health centers involving 126 leprosy patients. Patients who took irregularly Multi Drug Therappy (MDT) and heve been Released From Treatment (RFT) were considered as the case. Whereas those who regularly took MDT were considered as the control. Various variables observed from the patients, treatment and health providers were analysed using bivariate as well as multivariate statistical methods.Results: Showed that by using bivariate analysis dominant factors of irregular treatment of the patient to MDT in Kabupaten Blora were low social economy (odds ratio/OR:2,69), low knowledge on leprosy (OR:2,89), side effect of the drugs (OR:6,22), leprosy reaction (OR:3,43), disabilies (OR:4,2), no health education (OR: 13,12) and no home visit of provider (OR: 2,79). However, by multivariate log regression only 2 variables i.e. no health education and side effect of drugs were significantly influencing the irregularity to treatment. Nosignificant correlation between sex, education, detection methods, distance of health services, leprosy type, supervison and patient perception and the irregular treatment were found.Conclusion: That no health education and side effect of drugs are significant factors of irregularity to treatment in leprosy. Abstract

Background: Community Participation in weighing children under five years old in Posyandu is important to be examined as it has an important impact toward disease prevention and promotion as well as in improvement of human resource quality and decreasing infant and maternal death through community's nutrition status improvement. Previous study showed that out of 39 children under five years old who were rarely come to Posyandu, there were 13 children who were malnourished and 1 person who had poor nutrition.Objective: This research was aimed to investigate and describe community's participation of Kampung Naga in weighing children under five years old in Posyandu from sociological perspective.Methods: This research was using qualitative design. Data was collected with focus group discussion, interview, and observation. Interview was done toward mothers of the children, kuncen, punduh, tukang jampe, head of community, village's officers, cadre, midwife and head of Primary Health Care Center. Data validity was implemented with source and method triangulation, and reliability was done with data auditing. Data analysis was carried out using content analysis.Results:The result of this research showed that community's participation of Kampung Naga in weighing of children under five years old was influenced by internal and external factors.The internal factors consist of community's value and believe, social interaction and stratification. On the other hand, the external factors of community's participation were cadre's perception, posyandu process, and social support.Conclusion: Kampung Naga Community's participation in weighing of children under five years old had strong relationship with their norms, custom, value, believe and their culture. Abstract

Background: Maternal mortality depends much on health service quality especially pregnancy care and delivery attendant. Therefore, the availability of professional attendant at delivery is really needed and midwife is one of the options. In reality, delivery service by birth attendants is still of interest to the community. To close this gap, there should be collaboration between midwives and birth attendants, known as companion delivery.Objectives: To find out the effect of companion delivery on the consumers' satisfaction of delivery service.Methods: This was static group comparisson. One community health center served as the treatment group (companion delivery) and another community health center as the control group (birth attendants). The respondents of the treatment and control groups were 83 mothers with newborn babies. Samples were taken proportionally. The analysis used were chi-square test, t-test and the multiple-regression analysis using SPSS-10 program, with 5% significance level.Results: There was significant difference between consumers' satisfaction of companion delivery and consumers' satisfaction of delivery by birth attendants

Abstract

Background: Mental retardation is one of disabilities that can be implied in all families without any limitation of race, education and economic background. Mental retardation happened among Indonesia children is fairly high: in Surabaya, it predicted 1%-3%. Like any normally children, children with mental retardation have probability occurred in accidental, the probability could be greater because of cognitive limitation and its sensory and motorist development.Objective:The research is tried to find out risk factor of accidental occurred in mental retardation children.The risk factors will be beneficial as an effort to prevent and control.Methods: The research is done in irregular school C1 in Surabaya for 40 students with medium level of mental retardation (IQ between 30-55). Independent variables are children characteristics, row of birth, family protection, and time period of school. The dependent variable is accidental occurred in mental retardatioh. Data collection is done to respondents, which are parents of children. Descriptive analysis and measuring risk factors is implemented by calculating odds ratio (OR).Results: The research indicated that 40% children ever occurred in any accidental. Greater frequency of accidental is failing or slippery when running (50%). Falling or slippery from chair (18,75%); falling or slippery from bed (18,75%). Highest risk factor happened in the absence family protection (OR = 4,2) and did not happened on first children in row of birth (OR = 2,0).Conclusion: To prevent any accidental occurred, children with mental retardation should be frequency under control, either by special guardian or by family members. Construction of household or furniture must be adjusted in order to ease children in using them. The cleaning of bathroom and other places can prevent any accidental occurred in home. Due to the research is preliminary, it is suggested to do similar research in widen population and more complete variable such as adjustment of household utilities and high risk factors but not implies in population. Abstract

Background : The prevalence of stunting in Indonesian elementary school children is still high. It has been reported that micronutrient deficiency was associated with growth retardation. However, the effect of micronutrient supplementation on growth remains unclear.Objective : The-study was conducted to examine the effect of iron supplementation in combination with Zn and Vitamin A on growth nutritional status of stunted elementary school children.Method : The study was a randomized double blind placebo-controlled trial conducted in Bantul Regency in 2002. Stunted children aged 11-12 years were randomly assigned into four treatment groups : Iron group (n=38), Zinc + Iron group (n=41), iron + Vitamin A group (n=39), and Iron + Zinc + Vitamin A group (n=39). This study was done in August 2002-September 2003. Weight and height of each child were measured every month. Hb levels, Zinc plasma and Serum Retinol were measured at the start and the end of the study. Nutrient intakes of each child were collected by using multiple 24 hour recall.Result : The improvement in height for age Z-Score (HAZ) of children who received iron and vitamin A supplementation was higher than that of children who received Fe only (p=0.026). There was no significant difference in the improvement of height for age Z-score between children who received zinc + iron, or zinc + viaminvitamin A + iron and those who received Fe only (p>0.05). The effect of vitamin A and iron supplementation on HAZ the height for age z-score was modified by hemoglobin level;. T the effect of iron and vitamin A supplementation on the improvement of HAZ appeared mainly in children with initial hemoglobin level < 12.0 g/ There was no significant difference in the improvement of weight for age z-score and weight for height z-score between treatment groups.Conclusion :Iron and Vitamin A supplementation significantly improved Height for Age Z-Score (HAZ) of stunted elementary school children. Abstract

Background: Among cause of death identified in Medical Forensic Department of Sardjito Hospital was due to poison. Poisoning can occur as an accident, muder or suicide.Methods: A retrospective study et repertum during four year periode (1998-2001) in Medical Forensic Department of Sardjito Hospital was carried out to obtain incidence of poisoning and type of poison used.Results: The study showed that in this periode mote than 65% of poisoning case can be identified, and more than 77% cause of deaths can be known.Conclusion: The rinds of materials which make poisoning were alcohol (28,5%, cyanide (17,1%) and insectitisides (14,3%). The most cost of death in poisoning cases was asfixia (77,1). Keywords: forensic autopsy, visum et repertum, poison pattern

Abstract

Background: It is approximately that the prevalence of schizophrenia in Indonesia is 1% out of total population. Within the total population of 205 million, the number of schizophrenia cases in Indonesia is 2 million.This number should be considered as the burden of disease impact on the whole family and its environment, not only the patient itself.Objectives: To know the demographic characteristics of schizophrenia among Puri Nirmala Mental Hospital in-patients, to assess the efficacy of therapy on decreasing positive and negative symptoms manifested by Schizophrenia patients; and to assess the contribution of demographic characteristics and treatments toward decreasing symptoms.Methods: A hospital based retrospective study cohort was used as the design of this study. Samples of this study were 750 Puri Nirmala Mental Hospital in-patients from January 2000 to December 2002. Samples were chosen based on schizophrenia diagnosis established by Psychiatrists based on PPDGJ III. Outcome variable is the absence of negative and positive symptoms, whereas study factors are demographic and treatments variables. Descriptive analysis, paired t-test and logistic regression were used for data analysis.Results: Subjects mostly males (63.3%), living in Yogyakarta province (69,5%), age 21-30 years old (42,3%), high school educations (50,7%), and jobless (68,9%). There was a significantly statistics difference of the positive and negative symptoms before and after treatment manifested among subjects. Jobless, less visit, and both treatments (pharmacotherapy and pharmacotherapy added with electrical convulsion therapy (ECT) were associated with the decreasing of positive and negative symptoms.Conclussion: Treatments with pharmacotherapy and pharmacotherapy added with ECT are effective for decreasing positive and negative symptoms manifested of Schizophrenia patients.
