Abstract

Introduction: The vast majority of renal injuries occurred by blunt mechanisms with fall and motor vehicle crashes. Massage therapy is widely used as traditional medicine and has been viewed as a safe treatment tool without major complications or side effects. Renal rupture due to traditional massage therapy is a rare complication. Moreover, it occurs even less commonly considering the anatomical location of the kidneys, where they are well-protected. The purpose of this article is to present a case of a patient with an acute ruptured kidney due to traditional massage therapy four hours before admission to the hospital. Case description: A 52-years-old man came to the emergency department with right upper and lower abdominal pain. The pain began in the flank region and spread to the right lower abdomen. He visited a traditional massage therapist and received massage therapy in the abdominal and back region with no history of other trauma or other activities before admission. The hemodynamic status was unstable. Physical examination revealed tenderness in the right upper and lower abdominal region and bulge area at the right flank region. We performed an emergency laparotomy due to hemodynamic instability. Right retroperitoneal exploration revealed active bleeding from a ruptured right kidney. Thus right nephrectomy was performed. Post-operative non-contrast CT scan showed ureteral obstruction due to the presence of right distal ureteral stone Conclusion: Kidneys with anatomical abnormalities such as hydronephrosis and blunt abdominal trauma may increase pressure in the hydronephrotic renal pelvis and cause a rupture. We suggest that surgeons should be cautious of the possibility of renal rupture in blunt trauma due to the change of normal morphological structure of the kidney and attenuation of the renal parenchyma.

Abstract

Introduction: An aneurysmal bone cyst (ABC) is benign but has a progressive nature that can cause severe problems if it occurs close to joints such as the knee. The ultimate goal of treatment is to remove the tumor, maintain limb function, and prevent a recurrence. Local therapy is still better than tumor resection. This study aims to describe a rare case about distal femur ABC treated with two stages of surgery in the pandemic COVID-19 by excision-skeletal traction and endoprosthesis replacement. Case description: A 31-year-old woman presented with a lump on the right knee. The patient has complained of limping. Open biopsy was performed, and the patient was confirmed to have ABC on the distal of her right femur. The patient had a leg length discrepancy of 8 cm. We decided to do skeletal traction first. One week after the skeletal traction, the patient undergoes endoprosthesis replacement. Conclusion: The management of wide excision of the tumor requires special postoperative attention. This is because aggressive measures can reduce the recurrence rate of tumors, but large reconstructions are also needed to maintain joint function.

Abstract

Diabetes mellitus (DM) is a metabolic disease characterized by hyperglycemia that occurs due to insulin disorders, insulin action, or a combination of both. Based on data from the 2014 IDF, Indonesia ranks 5th in the world of patients with DM. Diabetic foot is one of the chronic complications of DM that is often encountered. Early vascular assessment in the legs of a diabetic patient is recommended to reduce mortality and morbidity. The vascular examination begins with a capillary refilling examination, palpation of the lower extremity arteries, and a further ankle-brachial index assessment. The vascular approach aims to assess the vascularization of the diabetic foot is still good or has experienced occlusion or obstruction due to inadequate blood vessels. Diabetic foot care is based on the classification recommended by the International Working Group On Diabetic Foot. Management of diabetic foot involves a variety of multidisciplinary, one of them with a vascular approach. The vascular approach also aims to determine the appropriate therapy for the diabetic foot. Abstract

Introduction: A vesicocutaneous fistula (VCF) represents an abnormal association between the bladder and the skin's surface. VCF mostly present as a complication of the diseases and treatments. Maceration, difficulty in treating the infection, moisture-associated skin damage, malodor, inconveniences, and physical disability were the sign and symptoms of VCF besides the visible urine leakage. A vesicocutaneous fistula should be treated adequately when recognized due to it may give bothersome complaints, social effects, and a possibility of recurrence to the patients. Proper surgical interventions are imperative because VCF leads to life-threatening complications such as cancer and sepsis. In this case, we report a case of vesicocutaneous fistula following inguinoscrotal hernia surgical repair in a 63-years-old man in order to provide an overview of the management of patients in similar cases. Case Description A 63-years-old man with a history of strangulated hernia and underwent exploratory laparotomy and ileostomy two weeks ago. Intermittent small amounts of urine discharge from the left groin was developed eleven days after the operation. The vital sign was normal, with a physical examination revealed a wet postoperative wound at the left groin. Mild tenderness over the suprapubic area. The hematology tests were normal. Cystography with contrast revealed the contrast appears to fill the bladder, and there is neither extravasation of contrast from the bladder to the distal end of the fistulous tract. The result was confirmed as the multiple diverticles within vesica urinaria. Incision of VCF was performed, and the bladder was closed in two layers with 3-0 Chromic for the mucosa and running polyglycolic acid (vicryl) 2-0 for the muscular. Conclusion: Laparotomy should be performed with care, and the surgeon must be aware of possible organ trauma. In our patient VCF was treated by suturing the fistula after drainage.

Abstract

Background: Blunt violence on the head often occurs in the community, which often causes fatal conditions and even death. Death due to blunt force on the head can be caused by intracranial bleeding or destruction of brain tissue. Case description: A case of suspected murder was reported due to blunt violence on the victim's head with the initials RA, 16 years old, a student at an official school. On forensic medical examination found signs of blunt violence in the form of bruises on the forehead and face, and the results of an autopsy found the destruction of the bones forming the face, fracture of the skull with the release of brain tissue. In the police investigation, the suspect admitted to committing violence with a kick on the head of the victim using shoes without other tools. There was a mismatch between the strength of the violence recognized by the suspect and the autopsy findings so that the forensic examination focused on seeing whether blunt violence that occurred on the victim could result in death or not. Conclusion: It is necessary to deepen police checks on suspects about how violence was carried out, how the power of violence was carried out, and what were the motives for violence. During the trial, the panel of judges stated that there was a possibility that the perpetrators would use hard objects to attack the victim.

Abstract

Lung cancer is a malignancy of the lung organs consisting of primary lung cancer originating from lung epithelial cells and metastatic lung cancer originating from other organ cancers that spread. In lung cancer, the vast of recent studies investigated the role of molecular alterations in patients. Molecular characterizations of lung cancer give further information in order to better diagnosis, prognostic, and therapeutic implications in patients. c-ROS oncogene 1 (ROS 1) is naturally expressed by healthy and normal cells arranged in human chromosomes. The role of ROS1 in its normal development is not fully understood. Point of view in oncology ROS1 expressed normally is referred to as inactive ROS1. Mutated ROS1 due to fusion with a specific gene produces tyrosine kinase receptors on the surface of the cell that has the advantage of sending signals into the cells to induce growth factors and proliferation of ROS1. Although the prevalence of ROS1 rearrangement was low, the parallel with Non-Small Cell of Lung Cancer (NSCLC), the most dominant lung cancer, indicating the number of ROS1 mutation patients also increased. The challenge of diagnosing ROS1 mutation by Immunohistochemical (IHC) ROS1 is only a screening method and confirmed by fluorescence in situ hybridization (FISH) examination. The appropriate therapy regimen for positive ROS1 mutation will increase the survival rate in lung cancer patients.

Abstract

Background: Various efforts have been made to accelerate the wound healing process in white rats, one of which is using Chromolaena odorata. The leaves contain tannins, phenols, flavonoids, saponins, and steroids, which accelerate the wound healing process. The purpose of this study was to determine the effect of Chromolaena odorata leaf jelly extract on the wound healing process in the inflammatory and proliferative phases in white rats microscopically (Rattus norvegicus).  Methods: A total of 27 healthy adult male white rats aged 3-4 months old were given open wounds on the back measuring 2x2 cm. The rats were then divided into four groups, a group administered with jelly without Chromolaena odorata leaf jelly extract (P0) and groups administered with Chromolaena odorata leaf jelly extract with concentrations of 10% (P1) and 30% (P2), respectively. Chromolaena odorata leaf jelly extracts were given daily by rubbing them on the wound surfaces. Skin biopsies were collected and evaluated histopathologically on days 3, 7, and 14 post-treatment. The histological assessment of wound healing was based on epithelialization and collagen density using Nagaoka criteria. The data were analyzed by analysis of variance (ANOVA) and continued with the Duncan test. Results: The Duncan test results showed that the mean score of the degree of epithelialization and collagen density of wound tissue on days 7 and 14 was significantly higher in the P2 group compared to the P0 and P1 treatments, while the P1 group was not significantly different from P0.  Conclusions: In conclusion, the administration of Chromolaena odorata leaf jelly extract could accelerate the degree of epithelialization and collagen density in the wound healing process of the inflammatory and proliferative phases of white rats (Rattus novergicus).

Abstract

Introduction Preoperative oral glucose administration is part of the ERAS protocol, which is still not implemented worldwide. This study aims to determine the effect of preoperative oral glucose administration based on the ERAS protocol on the patient's quality of life and insulin resistance. Method This study is a quantitative quasi-experimental study (post-test with control group) involving patients undergoing surgery using general anesthesia at dr. Zainoel Abidin Banda Aceh from January 2021 to April 2021. The intervention was carried out on patients who were about to undergo surgery divided into two groups: the group receiving and the group not receiving 50 grams of oral glucose 2 hours before surgery according to the ERAS protocol. The data obtained were processed using Chi Square statistical test, Mann- Whitney, and unpaired T-test to assess patients' quality of life and insulin resistance. Result A total of 48 patients (24 treatment group and 24 control group patients) were involved in the study. There were significant mean differences in all quality of life indicators between the group receiving and the group not receiving oral glucose with a significance value of p<0.05. There is a relationship between oral glucose administration and prevention of insulin resistance with a significance value of p<0.05. Conclusion: There is a relationship between preoperative oral glucose administration on quality of life and postoperative insulin resistance according to the ERAS protocol.

Abstract

Background: SARS-CoV-2 (Severe Acute Respiratory Syndrome Coronavirus 2) is a virus with a positive chain of RNA (RiboNucleic Acid) This viral infection spread rapidly with subsequent human-to-human transmission and increase in the number of cases in other countries around the world. Aceh is one of the areas affected by COVID-19 in Indonesia. Despite the need for fast and precise diagnostics, facilities capable of carrying out RT-PCR are still limited in some places. This study aims to determine the accuracy of the Sardjito score in establishing the diagnosis of Covid 19 in surgical patients.. Methods: Consecutive sampling is included in the non-probability sampling technique where all subjects who come sequentially will be used as research samples until the required number of samples is met. The data obtained will be presented using tables and curves. For the analysis of the data results, the Spearman-Pearson correlation test was carried out, and for the correlation between the Sardjito Score and the PCR swab results, it was continued by analyzing the sensitivity and specificity values. Results: Based on the cross tabulation, it shows that the majority of patients' Sardjito scores are moderate probability. Research subjects with moderate probability and negative PCR results were 83 people (91.2%). The specificity of the Sardjito score is 87% compared to the PCR results. In addition, the positive predictive value of 62%. While the negative predictive value of 96%. Conclusions: This Sardjito Score can be used as a tool for COVID-19 diagnostic tests.

Abstract

ABSTRACT Introduction: Coronavirus disease-19 (Covid-19) pandemic has brought a dramatic change in health care worldwide, including urologic care. Patients requiring non-covid care are encouraged to avoid hospital visits and postpone elective surgeries to reduce the risk of transmission and anticipate a surge in Covid-19 patients. This study aims to describe Covid-19-related changes in urologic care in tertiary hospitals in Banda Aceh, Indonesia.  Methods: This is a descriptive study in dr. Zainoel Abidin Regional General Hospital, Banda Aceh. The number of outpatients’ visits and urologic surgeries performed in March – October 2020 were recorded and compared to the numbers within the same period in 2019. The most common surgery performed during a pandemic is determined.  Results: The number of outpatient visits in March – October 2020 was 2,595 visits, while in 2019 was 3,886. There was a 54.29% decrease in March-May 2020 and a 17.76% decrease in June – October 2020. The number of surgeries carried out in March – October 2020 was 381, while in 2019 was 264. The most common surgeries performed during the pandemic is stones removal (n = 119) and reconstructive surgeries (n = 104).  Conclusion: There were decreases in both number of outpatients’ visits and surgeries performed during the Covid-19 pandemic. The most common urologic surgery carried out during the pandemic were stones removal and reconstructive surgeries.
