Abstract

Objective: To compare the efficacy and safety between chemotherapy and radiotherapy as the main therapy of Extranodal Marginal Zone Lymphoma (EMZL). Method: Literature searching was conducted using PubMed, ScienceDirect, Google Scholar, ProQuest, and SpringerLink. All studies that met the inclusion and exclusion criteria were categorized based on the level of evidence. The data of demographic of the patients, staging of disease, type of treatments, and outcomes of this review including the number of local control rate, disease-free survival rate, overall survival rate, dosage, adverse drug reaction, complication and recurrence and/or relapses were also reported. Result: From ten article, both radiotherapy and chemotherapy as the first-line treatment have high complete remission rate. Local control (complete remission) of chemotherapy group ranged from 56,25%-93,9%, while in radiotherapy group ranged from 70%-100%. Overal survival rate ranged from 92%-100% for chemotherapy group and 90,4%-100% for radiotherapy group. The most complication of the radiotherapy group were cataract formation, while the chemotherapy group showed systemic complications (hematologic or non-hematologic). The most relapse cases were shown in radiotherapy group. Conclusion: Radiotherapy and chemotherapy showed high local control rates and survival outcomes especially in the early stages of extranodal marginal zone lymphoma (EMZL). Radiotherapy had a relatively higher incidence of ophthalmic complications that could interfere with patient’s quality of life. Therefore chemotherapy could be considered especially in younger patients.

Abstract

Introduction: Adie’s tonic pupil is a neuro ophthalmological disorder characterized by dilated pupil, which is unresponsive to light. It is caused by damage to postganglionic fibers of the parasympathetic innervation of the eye. Recent events of the coronavirus disease 2019 (COVID-19) pandemic had suggested that severe acute respiratory syndrome coronavirus-2 infection (SARS-CoV-2) had neurotropic and neuroinvasive capabilities. Purpose: To report a rare case of unilateral Adie’s tonic pupil after COVID-19. Methods: A single case report study. Case Report: A 31-year-old woman presented with a glare on the right eye 3 months ago. No other symptom was reported. No prior history of using any eye drops and trauma. Further investigation revealed a history of asymptomatic SARS CoV-2 infection. Her best-corrected visual acuity of both eyes was 1.0, but we observed an impairment of accommodation on the right eye. A fixed dilated right pupil, 8 mm in diameter, was observed without any constriction on near reflex. Slit lamp and funduscopy examination were within normal limits on both eyes. Pilocarpine 0.1% test was positive on the right eye. Neurological examinations and brain imaging was within normal limits. The patient was diagnosed with Adie’s tonic pupil. She was treated with pilocarpine 0.1% once daily. After 2 months of follow up, her complaint of glare was diminished but no improvement of near acuity was seen on the right eye. Conclusions: This case report was one of the first few cases of Adie’s tonic pupil possibly associated with COVID-19.

Abstract

Background: Steven-Johnson syndrome is one of life-threatening skin abnormality. SJS complication involve multiorgan disfunction such as mucosa, ocular, respiration, and digestive tract. Around 30- 50% of SJS patient experience chronic ocular sequel, which concludes progressive symblepharon, margo palpebral keratinization, trichiasis, entropion, corneal pannus, dry eye syndrome, and persistent epithelial cornea defect. Scleral contact lenses has beneficial in managing SJS patient, on both acute and chronic phases. Methods: A 36-year-old female patient with SJS, presented for eye examination due to unpleasant sensation on both of their eyes and unsatisfactory quality of her vision. She presented high myopia, margo palpebral keratinization with trichiasis and distichiasis, symblepharon and hyperemic conjunctiva, wide corneal scar, and ocular neovascularization. A diagnostic trial set was used in the fitting process, and she was assessed according to standardized fitting methodology. Visual acuity, corneal topography, biometry, and ocular aberrations were evaluated. The follow-up period was 12 months. Results: The best corrected visual acuity was 20/60 with correction lens S – 1.00 D for the left eye (LE) and 20/25 cannot be corrected with correction lens for the right eye (RE). After corneal contact lens fitting, visual acuity was improved to 20/20 and 20/25 for the RE and LE, respectively. The patient wore these contact lenses an average 8 hours a day. The patient’s dry eye complaint was significantly decrease with more comfortable ocular sensation Conclusion: The present case report describes how the patient had scleral contact lens fitted successfully for management of SJS ocular complication. They provided optimal visual quality and restore the patient’s comfort.

Abstract

Introduction: Cataract is the major cause of reversible blindness and visual impairment in developing countries. It has been assumed that there is poor access to information related to the cause, prevention, and treatment of cataracts in developing countries. There is also a paucity of data on knowledge, attitude, and behavior toward cataracts in Semarang. Objective: To determine the Knowledge, Attitudes, and Behavior Regarding Cataracts of The Elderly around the Primary Care Center Semarang Method: A community-based cross-sectional study was conducted among 40 participants in November 2019. The study participants were selected by a simple random sampling method. Data were collected with a structured questionnaire by using face-to-face interviews. The data were entered into Microsoft Excel 2016 and analyzed using Statistical Package for Social Sciences (SPSS) version 25. The level of statistical significance was set at a p-value less than 0.05. The Socio-demographic factor as Gender, Age, Education, Occupation, and Income set as independent variable in this study. Result: There were 40 respondents, based on questionnaire data 52,50% of respondents had good knowledge, 85% had a good attitude, and 60% had poor behavior. The Education and Knowledge (p=0,001), Education and Behavior (p=0,004), Knowledge and Behavior (0.57749p>0,5, Gender, Education and Income (p>0,05), Education and Income (p>0,05)had positive significant on the variables. Conclusion: A Good approach strategy is needed to achieve to increase public Knowledge, Attitude, and Behavior like doing health promotion periodically and improving health care performance, and facilitate their availability in primary care.

Abstract

Introduction: Intraoperative floppy iris syndrome (IFIS) remains a challenge that increases the risk of complications in patients undergoing cataract surgery who use ?1-adrenergic receptor antagonists. To date, no definite consensus on a preventive strategy for IFIS is available. The aim of this review is to assess various pharmacological managements to prevent IFIS in high-risk patients. Methods: This review was based on Preferred Reporting Items for Systematic Reviews and Meta- Analyses guidelines. A systematic search using PubMed, Science Direct, Cochrane Library, and WorldCat database was performed. Quality of each study was evaluated using Cochrane Risk of Bias 2.0 (RoB 2.0), Risk of Bias in Non-randomized Studies of Interventions (ROBINS-I), or Newcastle- Ottawa Scale (NOS). Results: The search identified 1589 articles of which 7 met the eligibility criteria. Experimental and observational studies between 2010 and 2018 were included. Pharmacological managements included in this review are administered in varying routes. Phenylephrine, lidocaine, a combination of lidocaine and epinephrine are given intracamerally. Other pharmacological managements included are sub-tenon injection of lidocaine, topical atropine, a combination of topical atropine with intracameral epinephrine, combined irrigation solution of phenylephrine and ketorolac, and mydriatic cocktail- soaked wick sponges. Conclusion: Various pharmacological managements for IFIS prophylaxis have shown promising potential. However, studies that evaluate the efficacy of each agent and comparison between these strategies are still limited. Further research is needed to determine the best prophylaxis strategy to reduce the incidence of IFIS.

Abstract

Introduction: Non-arteritic ischemic optic neuropathy (NAION) is the second most common optic neuropathy in adults. Since circulatory insufficiency is presumed to have a role in pathogenesis of NAION, optical coherence tomography angiography (OCTA) is suggested as a tool in assessing NAION patients, while optical coherence tomography (OCT) can detect structural changes. This case report aims to describe the congruity of visual field defect with OCT and OCTA result in a case of bilateral NAION, highlighting the role of both OCT and OCTA in NAION. Case Report: A 52-years-old male came due to painless, progressive blurry vision in both eyes, especially in lower visual field, for the past 7 months. Humphrey visual field examination revealed inferior visual field defects in both eyes. OCTA showed reduced retinal perfusion in the superior part bilaterally. OCT revealed ganglion cell loss in the superior part of his right eye and almost all parts of his left eye. Retinal nerve fiber layer (RNFL) thinning was found in the superior part bilaterally. Discussion: In this case, the congruity of visual field defect, reduced perfusion, ganglion cell-inner plexiform layer (GCIPL), and RNFL thinning portrayed the connection of hypoperfusion as the presumed underlying mechanism of NAION, neuron loss as the result of the hypoperfusion, and the visual field loss as the presenting symptom in NAION. Conclusion: This finding demonstrates the role of OCT-A and OCT in diagnosing and monitoring progression in patients with NAION.OCT-A is a useful tool to evaluate microvascular changes, while OCT can be used to evaluate RNFL and GCIPL thinning.

Abstract

Introduction: Coats disease is a retinal vascular disorder that may lead to progressive exudative retinal detachment. The standard regiment of treatment is focal therapy. However, its effectiveness may decrease when more than two quadrants of the retina exhibit vascular abnormalities. Thus, recent studies tried to use anti vascular endothelial growth factor (anti-VEGF) as therapy for the vascular problems. Aims: Comparing the efficacy between anti-VEGF injection and standard therapy regiment such as focal laser therapy. Methods: Literature searching was conducted through PubMed, ScienceDirect, Google Scholar, ProQuest, and SpringerLink. Search terms included "Coats’ Disease" and mesh terms of “Anti- VEGF". The efficacy is assessed based on Best-Corrected Visual Acuity (BCVA) and improvement in fundus manifestations. Results: All of the studies that conducted primary intravitreal anti-VEGF treatment were followed by necessary ablative treatment. These studies, such as by Yang Q and Zheng XX, et al. showed remarkable improvements in both visual acuity and anatomical outcomes. Laser therapy also gained satisfactory result in upgrading clinical stages, even though some complications including subretinal fibrosis, cataract, and vitreous hemorrhage were reported. A comparative study by Ray R, et al. noted that group undergoing anti-VEGF might require longer treatment sessions despite possible benefits. Conclusion: Anti-VEGF agents can be used as neoadjuvant in standard therapy. It did not reduce the time for full treatment, but the resolution of disease was seen in the most severe cases treated with combination therapy.

Abstract

Introduction: The objective of this study is to determine whether there is a relationship between intraocular pressure (IOP), diabetes mellitus, and hypertension with visual acuity in senile cataract patients after phacoemulsification surgery. Methods: A retrospective cross-sectional study of senile cataract patients who underwent phacoemulsification surgery from 2019-2020 at Christian University of Indonesia General Hospital. Data were taken from medical records using purposive sampling, consist of visual acuity before and after phacoemulsification, visual acuity during 3 months of follow up, IOP before and after phacoemulsification, and the presence of systemic comorbidities such as hypertension and diabetes mellitus. Data were analyzed statistically with SPSS. Result: 122 eyes from 83 patients mostly had severe visual impairment before and a day after phacoemulsification, and improved to normal vision after a month and 3 months following phacoemulsification. The mean IOP was 18,36 mmHg and 21,33 mmHg before and after phacoemulsification. There were 78 patients who had systemic comorbidities such as hypertension and diabetes mellitus. There was no significant correlation between IOP and hypertension with visual acuity after phacoemulsification (p>0,005). Meanwhile, significant correlation was found between diabetes mellitus with visual acuity a day and a month after phacoemulsification (p<0,005). Conclusion: Diabetes mellitus has a significant relationship with visual acuity a day and a month after phacoemulsification surgery.

Abstract

Introduction: Due to difficulties in attending direct patient care and to limit the spread of COVID-19, the Indonesian ministry of health encourages hospitals to establish telemedicine services. This study aimed to report the initial practice of real-time teleophthalmology during the COVID-19 pandemic in Indonesia.  Methods: This retrospective descriptive-analytical study collected data from patients who had teleophthalmology consultations within 3 months (April 27 – July 27, 2020). All consecutive patients were included in this study, except those with incomplete records. Patients’ demographics, the main reason for the consultation, working diagnosis, prescribed medication, management plan, and satisfaction survey were collected for the analysis.  Result: There were 251 video consultations from 206 patients with 4 patients (1.9%) excluded due to incomplete medical records. There were 52 (25.7%) new patients. The median consultations were 2 (1-8) each day. The median age was 43.5 (from <1 year to 95 years). The main reasons for consultation were dry eye-related symptoms and red eye. The three most common primary working diagnoses were dry eye syndrome, conjunctivitis, and stye. Medication was prescribed for 149 patients (73.8 %) and 43 patients (21.3 %) were advised to have immediate direct consultation. The satisfaction survey revealed a 21.3% response rate, of which 41.9% and 58.1% felt very satisfied and satisfied respectively. Conclusion: Real-time teleophthalmology consultation seemed to be well-accepted by our population in spite of its early adoption with a high satisfactory rate.
