Abstract

Background: Leptospirosis is a zoonosis that is still a problem in Indonesia. The kidneys are often the target organs in leptospirosis. The outer membrane of Leptospira Sp contains antigenic components and toxins that stimulate the inflammatory process in the kidneys. The neutrophil-to-lymphocyte ratio (NLR) is used as a marker of the inflammatory process, while urea creatinine is a sign of kidney damage. Objective: To prove the correlation between NLR and serum urea and creatinine levels in leptospirosis patients. Methods: This study was carried out retrospectively on the medical records of patients with confirmed leptospirosis by Microscopic Agglutination Test (MAT) at Dr. Kariadi Semarang between January 2020 and July 2022. NLR examination was obtained by dividing the number of neutrophils by the number of lymphocytes in the leukocyte count, urea, and creatinine levels from the patient's medical record. Statistical analysis using the Spearman correlation test, with a significance level of p <0.05. Results: From 33 samples, the median NLR was 10.75 (2.19-47,5), the median urea level was 131.0 mg/dL (15–467 mg/dL) creatinine 2.0 mg/dL (0.7-11.9 mg/dL). The correlation between NLR with urea (p=0.001 and r=0.561) and creatinine (p=0.003 and r=0.501). Increased NLR, urea, and creatinine were found in leptospirosis patients. Conclusion: There is a moderate positive correlation between NLR and urea and creatinine. An increase in NLR is a sign of an inflammatory process from leptospirosis. An increase in NLR correlates with an increase in urea and creatinine

Abstract

Background: The implementation of the National Health Insurance System in Indonesia has not only increased access to medical services but also raised various dilemmas among doctors. Objective: This study aims to analyse various ethical and professional dilemmas among physicians in the National Health Insurance System (NHIS).Methods: This is a cross-sectional descriptive study with a purposive sampling method. A questionnaire was distributed online between June to December 2019 to physicians practicing in Indonesia.Results: Of the 87 doctors who participated in this study, 60.92% were general practitioners and 39.08% were specialists. The inability to maintain professional independence (63.41%) is the most common ethical dilemma faced by doctors with 65.38% of them experienced in advanced level health facilities. Meanwhile, the limitation of adequate medical care in an emergency (25.45%) is the most dilemmatic situation experienced in terms of professionalism with 85.71% occurring in advanced level health facilities.Conclusion: Physicians in Indonesia were deal with various dilemmas while practicing in the NHIS. The high rate of dissatisfaction and the various ethical problems and professional disciplines require a review of the health system to improve the quality of services and maintain the professionalism of doctors.

Abstract

Background:Stunting is a condition of failure to thrive due to chronic malnutrition that occurs in toddlers. Posyandu is an intervention facility for a specific nutrition program carried out by the Ministry of Health. The role of posyandu cadres is very important in efforts to reduce the prevalence of stunting. The Government of Malang Regency implements a cash incentive program for all posyandu cadres to improve their performance.Objective:This study aims to compare the effect of giving cash incentives on the performance of cadres of Pagelaran Health Center and Gedangan Health Center, Malang Regency.Methods:This study used an observational analytic design using a proportional cluster random sampling technique on 125 Pagelaran Health Center cadres and 125 Gedangan Health Center cadres. The research results were analyzed and interpreted using the SPSS Windows release version 2.1 computer program.Results:The results of this study prove that giving incentives to posyandu cadres in the Pagelaran Health Center work area has no effect on cadre performance with a significance value of 0.98 and giving incentives to posyandu cadres in the Gedangan Health Center work area has no effect on cadre performance with a significance value of 0.657.Conclusion:It can be concluded that the provision of incentives has no direct effect on the performance of posyandu cadres in the work area of Pagelaran Health Center and Gedangan Health Center.
