Abstract

Ditinjau kembali berat badan lahir, nilai Apgar, cara persalinan dan kematian perinatal pada 186 bayi kembar yang lahir di Rumah Sakit Universitas Gadjah Mada, Yogyakarta, tahun 1977 sampai dengan tahun 1981,Mereka terdiri atas 80 bayi pertama dan 106 bayi kedua. Tigapuluh sembilan (21%) bayi lahir kurang bulan, 112 (60,2%) dengan berat badan lahir rendah dan 78 (42%) termasuk sebagai bayi kecil untuk masa kehamilan.Bayi kedua mempunyai nilai Apgar yang lebih rendah dibanding dengan bayi pertama. Demikian Pula presentasi kematian perinatal dan cara persalinan abnormal pada bayi kedua lebih banyak dibanding dengan bayi pertama.Key Words: twin infants - Apgar score - neonatal mortality - abnormal labour - Lubchenco curve

Abstract

The process of aging of an individual has begun since his existence in the womb. Old age forms the last stage of the process.In this aging process changes take place continuously affecting the morphology and function of the individual, his systems, and organs down to the ultrastructure and biochemical reactions, probably due to "intrinsic mutation"; some of those changes are universal and some are specifically affecting certain organ, tissue or cells.In the aging process the heart is not free from these change characteristics which affect the structure of the heart muscle and the vascular wall, and also their biochemistry. All these changes have their impact on the cardiac function.In old age differences are found in the heart from its previous stages of the aging process, which among others are:1. fragmentation of some myofibrils down to complete degeneration with its specific characteristics,2. decrease of the number of mitochondria: some mitochondria show signs of degeneration, 3. heart perfusion decreases down to 80%,4, degeneration of the coronary vessels.5. a change of noradrenalin metabolism so that in the heart muscle it remains 75%,6. degenerative changes of the cardiac valves.All these changes tend to decrease the function of the heart.Key Words: aging process - "intrinsic mutation" - aging heart - myocardium degeneration - senescence

Abstract

Genetic counselling is a relatively new medical activity. There.are some conditions that usually need genetic counselling.•The classification of genetic disorders, their mode of inheritance and the method of establishing the diagnosis must be well understood before giving the counselling. The mode of inheritance of genic disorders and some difficulties in the diagnosis need special attention. Chromosomic disorders and embryopathies have been shortly explained ,as well.Finally, a short explanation of prenatal diagnosis, carrier detection and some methods of treating genetics disorders have been proposed so that genetic counselling can be given effectively.Key Words: genetic counselling - genetic disorders - genic disorders - prenatal diagnosis - carrier detection.

Abstract

This article describes the process and mechanism of phagocytosis, especially their immunological aspects. Phagocytosis is one of the bodys second line of defence against foreign particles, e. g. infectious and non-infectious material. The macrophage system, mononuclear and polymorphonuclear cells are responsible for phagocitizing and processing those particles, moreover they serve the processed material to the specific immunocompetent system.In addition CO the normal mechanism of phagocytosis, this article describes some disturbances which interfere with each step of phagocytosis as chemotaxis, opsonization and intracellular killing; this inadequacy results as a certain disease.Key Words: mononuclear phagocyte system - non-specific immune response - inflammation - receptor of the phagocyte cell - opsonin

Abstract

An anaphylactic shock survey was carried out in Yogyakarta, Indonesia, during November December 1981. Physicians whith private practice were asked to complete the questionnaires given by the author. These questionnaires were collected without physicians identities.One hundred out of 135 respondents have had 270 patients suffering from anaphylactic shock accident. Two hundred and sixty one (96.7%) out of 270 patients had good recovery and 9 patients (3.3%) were dead. Preconditioning disturbances which made the therapy of these shocks difficult occurred in 27 patients (10 %).The drugs which caused these shocks were penicilline (27.8%). streptomycin (20%), penicilline streptomycin combination (14%), and others (38.2%). It was revealed that the route of administration of those drugs were intramuscular injection (97.4%), intravenous injection (1.1%), subcutaneous injection (1.1%). and oral administration (0.4%).Initial treatment of these shocks done by the physicians were intramuscular epinephrine (53%), intramuscular corticosteroid (23.7%), intramuscular antihistamine (15.9%), and intramuscular el-feud (1.5%). Two patients (0.7%) were immediately referred to the hospital without initial treatment given by the physicians. The rest (5.2%) had nonspecific treatment.Key Words: anaphylactic shock - allergen - penicillin - hypersensitivety - initial drug therapy
