Abstract

The clinical impression of association between hypertension and diabetes mellitus has not been consistently confirmed by epidemiological studies. The variety of methodological problems in this area of research includes the choice of appropriate patients and control group, the measurement techniques used, and consideration of many possible confounding variables.Hypertension in diabetes mellitus patients contributes substantially to the increased cardiovascular morbidity and mortality, and should be actively treated as accurately as diabetes mellitus itself. The first therapeuticstep is reduction of overweight, saltintake, and stopsmoking; the omission of interfering drugs and adequate education. Prescription of diuretic drugs or alternatively of calcium antagonists, vasodilators of hydralazin or prazosin. The adds of minoxidil or cap topril to the previousdrugsshouldonlybe taken after evaluation of the overall situation. Abstract

A 6-year old boy suffering from Poland anomaly has been reported. The patient showed a complete picture of this malformation which consisted of aplastic greater pectoral muscle and breast on the left side of the chest, as well as oligodactily of the left hand.Other anomalies found were ocular and oral anomalies. The ocular anomalies consisted of hypertelorism, bilateral esotropias due to bilateral abducent palsy, and bilateral optic nerve hypoplasia. Oral anomalies consist of the inability to close the mouth properly and the inability to smile may be due to the defect of oral muscles.Key Words: Poland anomaly - chest anomaly - oligodactily - ocular anomalies - oral anomalies

Abstract

A randomized controlled study on the insertion of Norplant without prophylactic antibiotics had been carried out. The aim of the study was to compare the risk of complication happening shortly after insertion. A total of 500 acceptors completed their follow-up, 244 belonged to the antibiotic group (tetracycline), while 256 belonged to the control group. They come from suburban areas stuTounding the city of Yogyakarta.Insertion was carried out outside the hospital during the Safari Mass Campaign, with hospital standard asepsis. Follow-up was carried out one week after insertion to detect the presence of such complication or side effects as pain, inflammation, abscess and sense of itching. Data were processed with version 2.0 SPSS program using Nikon PC Computer. t-test, chi-square test and relative risk were statistical analysis used in the study.Results showed that both groups were comparable in. terms of age, body weight, parity, duration of insertion and level of education. The overall incidence of complication in the non-an tibiotic group was 19,9% compared to 24.2% in the antibiotic group (RRO.82, 95% CL 0.57– L19) . More detailed complications such as pain, inflammation, abscess and sense of itching show neither clinical nor statistical significant differences. The risk of complication in all, cases was influenced neither by duration of insertion nor by acceptors level of education.The study concluded that the insertion of Norplant without prophylactic antibiotics was as safe as with antibiotics.Key Words: Norplant - contraceptive mass campaign - antibiotics - obstetrics - contraceptive complication

Abstract

Gravesdisease is the second common endocrine disease found in the clinic, characterized by hyperthyroidism, ophthalmopathy, diffuse struma with/without dermopathy.CoMmon treatments for hyperthyroidism of Graves disease are anti thyroid drugs, subtotal thyroidectomyorradioiodine (ml). However, reports in the medical literature present widely varying and often conflicting with regard to cure, relapse, frequency aside effects, cost, and compliance. No single form of therapy is acceptable to or suitable for all patients.The choice of treatment should be based upon several factors, such as the age of the patients, the size of the thyroid gland, existence of complications, and other factors.Key Words: Graves disease - hyperthyroidism - antithyroid drugs - thyroidectomy - radioiodine
