Abstract

Dermatophytosis is a disease cause by the colonization of dermatophyte fungi that attack  keratin-containing tissues such as the stratum corneum of the skin, hair and nails in humans. The prevalence of dermatophytosis in Asia reaches 35,6%.  In Indonesia, dermatophytosis is 52% of all dermatomycoses, which is most are tinea cruris and tinea corporis.  The study was Case Report study.  Primary data is obtained through history taking  and physical examination by making home visit, filling out family folders, and filling in patient files.  Assesment is carried based on the initial holistic diagnosis, process, and end of the visit quantitatively and qualitatively.  Lack of patient knowledge about the disease and poor personal hygiene of patient are the main internal risk factors causing patient to have gray patch type of Tinea capitis.  The intervention that was given is education and counseling  of the patient and patient family about matters that must be modified and the prevention about the disease.  The intervention divided into patient center, family focus and community oriented.  After holistic intervention with educational and counseling methods, there are decrease in clinical symptoms and behavioral changes was obtained by maintaining personal hygiene and the environment

Abstract

COVID-19 is a respiratory infection caused by the coronavirus. The main target of this virus attacks the mucosa of the upper respiratory tract and angiotensin-converting enzyme (ACE2) which is the receptor for viral entry. COVID-19 has many clinical manifestations. The clinical manifestations that appear in COVID-19 patients are similar to other respiratory viral infections, such as high fever, dry cough. However, over time there are many other manifestations such as fever, dry cough, diarrhea, shortness of breath (dyspnea), ageusia, anosmia, and the onset of skin manifestations. This article discusses skin manifestations in COVID-19 patients based on literature studies. The research method used in this journal is literature review. The author is looking for literature related to the topics discussed in this journal. From the research results, the skin can appear within 3 days before being diagnosed up to 13 days after being diagnosed with COVID-19. In the case of positive Covid-19 asymptomatic patients, clinical manifestations will appear on the 14th day and can be used as indicators of diagnosis. The most common skin manifestation that appears in COVID-19 patients is a maculopapular rash. Other lesions that can occur in COVID-19 patients, especially in young adults to the elderly, are urticaria, erythema nodosum, vesicular rash, petechiae / purpuric rash.

Abstract

Trauma is another word injury that can injure. Trauma is another word injury that can injure. Injury to the bones results in fractures (fractures) and dislocations. Fracture is a condition of bone discontinuity. Fractures are divided according to the contact of the outside world, which includes closed and open fractures. The classic symptoms of fracture are a history of trauma, pain and swelling in the fractured bone, eformity (angulation, rotation, discrepancy), tenderness, crepitus, impaired musculoskeletal function due to pain, broken bone continuity and neurovascular disorders. Fracture is one of the causes of trauma to the blood vessels (vascular). Vascular trauma should be suspected in the case of trauma occurring in areas anatomically traversed by large blood vessels. This case report is a man aged 23 years, having difficulty moving his right and left hands, and right lower leg since ± 12 hours of SMRS. Post traffic accident patients ± 10 hours SMRS. From the examination of the localis status of the right brachii region, it was obtained swelling, hematoma ± 5x4 cm, vulnus laceratum ± 2x0.5 cm; there is tenderness, wick pain and increased pain; Pain-limited ROM. The status of the left brachii localist was obtained by swelling, hematoma in the left brachii region ± 6x3 cm, vulnus laceratum ± 2 cm, pulsation A. Radialis not palpable, cold acral from 1/3 distal to the brachial region to the manus, hypoaesthesias and anesthesia present, limited ROM. Localis status of the femoral region until the right cruris is rotated, vulnus laceratum ± 30 cm, pain-limited ROM. In emergency ward, we treated the patient with oxygen administration through no re-breathing mask with 10 ml of oxygen per-minute, infusion with crystaloid solution, wound irrigation,  broad spectrum antibiotic, analgetic and fracture stabilisation with posterior slab. The patient underwent urgent surgery to treat his fracture and neurovascular trauma.

Abstract

Empathy is the ability to participate in feeling or imagining other people's feelings and this is one of the competencies that medical graduates must possess in order to create good doctor-patient relationships. A good doctor-patient relationships can improve their compliance and satisfaction with the services provided, so that it will provide better clinical results. However, it was found that during medical education, the level of student’s empathy could change due to various factors, one of them is level of study. The purpose of this study was to find out the mean empathy score between early and end clinical clerkship student, and to find out whether there were significant differences between the levels of study and the level of empathy of the clerkship students of the Medical Faculty of Lampung University. The research method used was an observational study with a cross sectional analytic approach at Abdoel Moeloek Hospital in February 2019. The research instrument used was The Jefferson's Scale of Physician Empathy - Student Version (JSPE - SV). Before processing the data, a normality test was carried out using the Kolmogorov Smirnov test. Then the data is processed using univariate tests and unpaired t-test as the bivariate tests. The results obtained are a mean empathy score of 100.09 which is included in the medium category. Based on the results of the unpaired t-test, it was found that there were statistically significant differences in the level of empathy among clerkship medical students in Medical Faculty of Lampung University.

Abstract

Clinical reasoning is a complex set ideas of diagnosis decision making process that involves those experiences, skill, and reflective thought. Medical faculty students undergoing pre-clinical and clinical students should improve their clinical reasoning as early as possible, so it can produce good quality of medical students. This study aims to determine the difference of clinical reasoning score between pre-clinical and clinical medical students of Lampung University. This was a descriptive analytic study using cross sectional study. Sampling using non-probability sampling method with the type of consecutive sampling and measuring instruments in the form of script concordance test. Data analysis was done by independent T test. The research conducted on 124 respondents with independent test result shows the score of preclinical student with mean 4,927 ± 1.3727 and score of clinical students with mean 5,416 ± 1,6020 with p = 0,041. There are a significant difference clinical reasoning score between pre-clinical and clinical medical students of Lampung University. This study has a significant difference of clinical reasoning score between pre-clinical and clinical medical students of Lampung University. Clinical reasoning in students can be improved by increasing cognition and metacognition through learning self-regulation.

Abstract

Chronic kidney failure has become a very important issue in recent years due to its increasing frequency and the high costs associated with it is increasingly being seen as a major public health problem associated with premature death with important social and economic implications. In addition to early diagnosis and detection, knowledge related to the causes of hemodialysis patient mortality in Indonesia in 2018 can be used as scientific evidence in planning, program implementation, program evaluation, and determining the priority scale of the hemodialysis patient mortality prevention program in Indonesia, which is the aim of this study. . This type of research is literature review research. The target population is all hemodialysis patients in Indonesia. The affordable population in this study were all hemodialysis patients recorded in the 2018 Indonesian Renal Registry (IRR) data set. The sample of this study was all hemodialysis patients (total sampling) recorded in the 2018 Indonesian Renal Registry data set. Incidence of 3 months of mortality in new HD patients amounted to 31.7%. Age> 60 years, hemoglobin <8 g / dl, serum albumin <3.5 g / dl, ECG abnormalities, and femoral access were significant predictors of mortality in the first 3 months of HD. The comorbid conditions associated with the mortality of CKD patients undergoing hemodialysis are age, coronary heart disease, congestive heart failure, stroke, sepsis, tuberculosis, and pneumonia.

Abstract

In December 2019, an outbreak of viral pneumonia caused by a novel coronavirus (since named SARS-CoV-2) was identified in Wuhan, China. Within few months, this disease known as coronavirus disease 2019 (COVID-19) spread worldwide, causing a global health emergency. The classic routes for transmission of SARS-CoV-2 are airborne droplets and surface contamination. The clinical presentation of COVID-19 ranges from asymptomatic infection to severe respiratory failure, with fever, fatigue, and cough occurring in most cases. The pathophysiology of SARS-CoV-2 infection is complex, endotoxin and tumor necrosis factor (TNF-α) trigger the production of interleukin (IL)-6 and IL-8, which is followed by a cytokine storm. Moreover, SARS-CoV-2 binds to angiotensin-converting enzyme-2 (ACE2) receptors, which are widely distributed not only in the lung alveolar epithelial cells and nasopharyngeal and oral mucosa but also in the endothelium and vascular smooth muscle cells, in the brain, in the gut, and in peripheral organs such as the liver and kidneys. This suggests that the clinical spectrum of COVID-19 is not limited to local pneumonia, but rather represents a multisystem illness with involvement of different organs and potential for systemic complications. Abstract

Social media is one of the online media that often used by the wider community to convey information to others and play an important role in the education process. Social media also provides the concept of space to interact and share knowledge by providing group facilities for students who join and discuss various topics, collaborate and use educational applications to manage learning activities. Medicine is one of the fields that has gained an important foundation through the application of one concept such as e-learning. To formulate learning with an effective e- learning system, collaboration must occur between educational institutions, instructors and technology experts. Although the use of the internet as a learning medium is very potential, not all learning materials can be replaced through social networks. In the learning process, the use of social networking is more stimulating cognitive abilities of medical students, but the skills and motor skills to conduct physical examination, history taking, practicum, and affective assessment of students should still be done face-to-face learning.

Abstract

Musculoskeletal chronic pain is characterized by pain and loss of function in the joints, tendons, ligaments, bones, and or muscles lasting for greater than equal to three months. There are several diseases caused by rheumatic disorders that have been identified, such as osteoarthritis, gout, rheumatoid arthritis, fibromyalgia. However, until now, chronic rheumatic disorders have become a world problem, namely “fibromyalgia”. Fibromyalgia is a syndrome characterized by generalized chronic musculoskeletal pain, hyperalgesia in specific points and psychosomatic symptoms, such as fatigue, sleep disturbances, anxiety, depression, cognitive dysfunction, headache, and gastrointestinal disorders. The prevalence of fibromyalgia in the world is higher in women than in men, which is between 2,4% to 6,8% with the ratio between women and men is 2:1. The etiology of fibromyalgia is idiopathic, but it is suspected that psychosocial factors play a role in the development of fibromyalgia. Currently, the diagnose of fibromyalgia can only be made by a comprehensive clinical examination according to the ACR (American College of Rheumatology) criteria. Therapy of fibromyalgia requires an optimal approach to the success of treatment. Therapies that can be done in patients with fibromyalgia include pharmacological therapy, complement therapy, cognitive behavioural therapy, psychotherapy and physical therapy.

Abstract

Diabetes Mellitus (DM) is a global public health problems, in 2014 the prevalence of DM sufferers reached 9% of the world's total population and 90% of them are DM type 2. The mortality rate for DM disease in 2012 worldwide reached 1.5 million. In 2013, the International Diabetes Federation (IDF) estimated there were 382 million people with DM in the world. Family approach in the management of diabetes mellitus helps identify factors that influence both clinical, personal and psychosocial family. This study is intended as the application of family doctor services to patients by identifying risk factors, clinical problems, and patient management based on a patient's problem-solving framework with a patient centered and family approach. This is a case report study. Primary data were collected by history, physical examination, examinations and home visits to assess the physical environment. A 60 years old woman diagnosed with type 2 diabetes mellitus with lacking knowledge of the disease, curative treatment behaviour, poor diet, rarely exercising and low physical activity and low family knowledge about diabetes mellitus. After the intervention, the patient has shown clinical improvement and changes in behavior towards the patient's disease. In the quantitative assessment, there is an increase in all aspects assessed which are knowledge, diet and physical activity. Management of diabetes mellitus patients holistically and comprehensively, patient center, family approach with diabetes mellitus treatment is needed in order to prevent acute and advanced complications.
