Abstract

Introduction: Nasopharyngeal carcinoma (NPC) is one of the most common malignancies in Indonesia with over 13.000 new cases reported yearly, especially in the male. The TNF-? serum is reported as a new biomarker to predict bone invasion, post-therapy distant metastasis, and poor life sustainability in NPC. Objective: To identify the expression of TNF-? in regards to the clinical stage of NPC. Material and methods: This is an analytic study with 126 NPC samples of patients in Haji Adam Malik General Hospital. Results: NPC mostly found in age group 41-60 years (57.1%), male (71.4%),non-keratinizing SCC (79.4%), and stage IV group (54.8%). In immunohistochemistry evaluation, most of TNF-? overexpressed in non-keratinizing SCC(68.0%), T3-T4 (50.8%), N2-N3 (62.7%) and clinical stage III-IV (69.9%). Spearman's test for categorical correlation yield a p-value of <0,001. Conclusion: There is a significant correlation between TNF-? expression and the stage of NPC.

Abstract

Introduction: Stroke is the main cause of mortality and morbidity in Indonesia. Dysarthria is a symptom that can be seen from stroke. These symptoms occur in 15% of stroke events. Dysarthria is a motor speech disorder. Atypical lacunar stroke (ALS) is a stroke subtype that rarely occurs in one in 14 lacunar strokes or occurs in 6.8% of lacunar strokes, where isolated dysarthria or dysarthria facial paresis is the most common form of presentation. Method: This study is an observational analytic study with a case-control type of research with the aim of knowing the relationship between CT scan images of lacunar infarcts and dysarthria in ischemic stroke patients. The population in this study were all medical records of stroke patients who went to the Department of Neurology at the Haji Adam Malik General Hospital (RSUPHAM) Medan for a period of one year from January 1, 2019 to December 31, 2019 who underwent a head CT scan for 18 patients. Results: The subjects on the CT scan results of lacunar infarction with dysarthria 18 people (72%), and 7 people (28%) on the CT scan results there is no lacunar infarction. The same number and percentage were also found in the non-chivalrous control group. The test results obtained p value = 1,000 and odd ratio = 1.00, in other words there is no significant relationship between lacunar infarct and dysarthria. Conclusion: No significant relationship was found between the incidence of lacunar infarction and the incidence of dysarthria. The population with the most lacunar infarcts is men, hypertension, obesity, hypercholesterolemia, smoking and diabetes.

Abstract

Introduction: Coronary Artery Disease (CAD) is the one of manifestation for cardiovascular disease and a cause of death and disability. Abdominal aortic calcification (AAC) has been shown to be strongly associated with atherosclerosis and CAD events. This study to investigated the association of risk factors for CAD with the incidence of AAC on abdominal CT Scan. Method: This study was an observational analytic study with a cross-sectional design on 105 patients who underwent abdominal CT scans with/without intravenous contrast at Radiology Department of  H. Adam Malik General Hospital. AAC score was identified calcification on the abdominal aorta (from 1 cm above the origin of the celiac trunk to 1 cm below the iliac bifurcation) use the Agatston method. Analyzing the association CAD risk factors such as gender, age, diabetes mellitus, hypertension, smoking, and alcohol and AAC score. Results: The results of the analysis using the Chi Square comparative test showed that there was a significant relationship between age (p=0.000), hypertension (p=0.017), diabetes (p=0.006), in othe words there is no significant relationship between gender (p = 0.613), smoking (p=0.22) and alcohol (p=0.28) with AAC score. Conclusion: There is a significant relationship between age, hypertension and diabetes with the AAC score.

Abstract

Introduction: CT is widely used to evaluate patients with aneurysm or aortic dissection. Therefore, it is very important to assess accurate aortic size in detecting aneurysm and dissection. The thoracic aortic diameter varies depending on the measurement level. Several demographic factors affect the thoracic aorta diameter such as age and gender. Risk factors for cardiovascular disease such as hypertension also have a relationship with the diameter of the thoracic aorta. To determine the difference of thoracic aorta diameter based on age, sex, and history of hypertension in patients with contrast-enhanced CT chest. Method: A case-control study in 140 patients with contrast-enhanced CT chest without signs or symptoms of car¬diovascular diseases. Thoracic aortic diameters were measured at seven predetermined levels on CT images. Analysis of data was performed with regard to age, sex, and hypertension. Results: The mean diameters of the thoracic aorta at the level of the aortic valve sinus is 3.06 ± 0.39 cm, ascending aorta 2.90 ± 0.39 cm, proximal to innominate artery 2.73 ± 0.39 cm, transverse aortic arch 2.53 ± 0.38, distal transverse aortic arch 2.37 ± 0.35, aortic isthmus 2.17 ± 0.33, and descending aorta at the level of diaphragm 1.91 ± 0.33 cm. There is a significant difference in all thoracic aortic diameters based on age and gender on statistical analysis (p<0.001). There is a significant difference in all thoracic aortic diameters between subjects with history of hypertension and without history of hypertension, except at the level of transverse aortic arch (p<0.051). Conclusion: There is a difference in the mean thoracic aortic diameter at each level of measurement based on age, sex, and history of hypertension in patients with contrast-enhanced CT chest.

Abstract

Introduction: Kidneys have filtration and excretion functions; the glomerular filtration rate (GFR) is considered the best indicator of kidney function and may indicate abnormalities. Kidney volume can be an early indicator of kidney disorders before there is a decrease in kidney function. Changes in kidney volume may indicate structural and functional changes that indicate a disorder or disease progression. Radiological examination such as a CT-Scan has an important role in measuring the size and kidney volume and assessing kidney morphological changes for the diagnosis and staging of kidney disease as well as planning therapy. Objective: This study aims to find a correlation between the estimated kidney volume with the ellipsoid method through CT scanning and estimated kidney function by glomerular filtration rate in patients with normal kidneys. Method: An analytical study used a case-series design; all subjects were patients who had normal CT-Scan results and normal kidney function at the Haji Adam Malik Hospital in Medan in 2020–2021. Used the consecutive sampling technique and had 90 total samples. Results: There is a moderately positive correlation (r = 0,410-0,499) between volume and kidney function. A moderately negative correlation (r = -0,196 to -0,282) between kidney volume and age. A moderately positive correlation (r = 0,456-0,548) between kidney volume and body weight. There is a moderately negative correlation (r = -0.495) between renal function and age. A moderately positive correlation (r = 0,567) between renal function and body weight. Conclusion: there is a correlation between kidney volume and kidney function, the correlation between kidney volume and body weight, the correlation between kidney function and body weight.

Abstract

Background: Fatty liver is a broad term for a condition in which there is an increase triglyceride level in hepatocyte. In Indonesia there is not much research about this condition. Fatty liver is a major health problem in West countries, and is slowly increasing in Asian countries. For many individual with fatty liver, have no specific complaint or can have no symptoms at all. This aim of this study to analyzed the relationship between fatty liver images on abdominal CT scans with dyslipidemia. Methods: This study is an observational analytic study with a case – control type of research with the aim of knowing the relationship between CT scan images of fatty liver and dsylipidemia. The data in this study were all medical records who went to the Radiology Department for abdominal CT scan at the Haji Adam Malik General Hospital for a period of one year from January 1st, 2020 to December 31st 2020. Results: The subjects on the abdominal CT scan results with fatty liver and high triglyceride is 5 peoples out of 12 (41.7%). The test results obtained p value = 0.,688 (p>0,05), in other words there is no significant relationship between fatty liver and high triglyceride level. Conclusion: No significant relationship was found between the incidence of fatty liver and the incidence of dyslipidemia.

Abstract

Introduction: Enlargement of the ventricles caused by disturbances in the production, flow, and absorption of cerebrospinal fluid is known as hydrocephalus.Radiological examinations such as computed tomography (CT) play an important role in establishing the diagnosis of hydrocephalus. Assessment of ventricular enlargement in most cases is done subjectively and based on radiological experience. Objectively, ventricular size can be assessed by linear ratios on CT. Evan's index (EI) is the simplest and most reliable method for assessing ventricular size. Age and sex have an influence on the size of the ventricles. The aim of this study was to determine the average Evan's Index and its relationship with individual characteristics (age and sex) in patients who underwent head CT scans at H. Adam Malik General Hospital, Medan. Method: This study used a case series design to determine the relationship between individual characteristics (age and gender) with the Evan's Index value and to obtain an average normal Evan's Index value in patients who underwent head CT scans at the Haji Adam Malik General Hospital in Medan. Results: : The highest sample age was in the 19-30 year age group, namely 39 people (35%). In this study, there were more female subjects, namely 51% compared to males (49%). Mean + standard deviation and median (min-max) in Evan’s Index data were 0,26 + 0,03 and 0,25 (0,2-0,36). There was a significant correlation between age and the Evan's Index (p <0.001) with a moderate degree of correlation (0.442). There was no significant relationship between gender and Evan's Index (p=0,121; ηp2=0,022). There was no significant difference between education levels and the average Evan's Index (p=0.434). Conclusion: There is a relationship between the increase in the age group of the participants and the increase in the EI value which is linear and correlative. There was no difference in EI values between gender and level of formal education regardless of the age group of the individuals analyzed.

Abstract

Introduction: Cerebral atrophy is a condition associated with brain volume reduction and is a common manifestation of aging. Age is an important factor in the incidence of cerebral atrophy. This research aims to know the relationship between age and the incidence of cerebral atrophy in patients at H. Adam Malik General Hospital Medan. Methods: This study is an observational analytic study with a cross-sectional design conducted at the Haji Adam Malik General Hospital Medan by collecting head CT scan data from the electronic medical records of 91 patients with the period January 1st 2021 to December 31st 2022. The research time will be carried out from October 2022 to December 2023. Results: Cerebral atrophy began to be found in the age group of 20-39 years, the largest gender was male (56%), the highest level of education in patients with cerebral atrophy was elementary school / equivalent (66.7%) and D1 to D3 (66.7%), and from the area of residence, patients who experienced cerebral atrophy mostly lived in urban areas (58.7%). The age of onset of cerebral atrophy was found to be 20 years old. The types of cerebral atrophy found in the study subjects were cortical atrophy as many as 49 people (53.8%) and global atrophy as many as 10 people (11%). There was no significant relationship between age and the incidence of cerebral atrophy in patients at H. Adam Malik General Hospital (p> 0.05), but there was a significant relationship between age group and the incidence of cerebral atrophy in patients at H. Adam Malik General Hospital (p < 0.05). Conclusion: There is no significant association between age and the incidence of cerebral atrophy in patients at Adam Malik General Hospital. Additional research is needed to gain a comprehensive understanding of cerebral atrophy and its relationship with other risk factors.
