Abstract

Objectives : To analyze the correlations of elderly fitness exercise with fitness measured by 6-minute walk test (6-MWT) and balance measured by Berg Balance Scale (BBS) of geriatrics in nursing home.Methods: Cross-­ sectional observational study of 51 elderly who lived in nursing home that conducted in December 2009 to January 2010.Attendance of subject doing elderly fitness exercise was rated by 2 months previously recorded to one month after trial. We recorded the weekly frequency of doing the exercise for 12 weeks (the 3 months). At the observation, fitness was measured by 6-MWT, balance was measured by BBS.Results: A positive and an significant correlation between number of exercise in 12 weeks of observation to 6-MWT was analyzed by Pearson correlation 0.418, p=0.002, and also positive and significant correlation to BBS was found 0.353, p=0.011. There were also positive and significant correlation between exercise frequency to 6-­MWT which analyzed by Spearman’s correlation 0.408, p=0.003 andto BBS as well Spearman’s correlation 0.404, p=0.003.Conclusions:The Elderly fitness exercise improves geriatric fitness that measured by 6-MWT and also improves balance measured by BBS in nursing home. The frequent exercise perfomed also correlates to elderly fitness which measure by 6-MWT and to balance that measured by BBS.Keywords:Fitness,balance, eld erly, 6 minute walk test, berg balance scale, ecercise, geriatric fitness exercise

Abstract

Objectives : To observe the effect of ergo-­cycle exercise on lung functional capacity and physical fitness level of COPD patients, to observe relationship between lung functional capacity and physical fitness level of COPD patients before and after regular ergo-cycle exercise.Methods : Quasi-­experimental research of twenty eight subjects with mild and moderate COPD who came to Medical Rehabilitation Instalation. They conducted ergo-­cycle exercise for 30 minutes 3 times a week, by one day interval, for 4 weeks. This research was a pre and post test design.Results :There was significant increase on some lung function parameters such as % FEV1, % FVC, but it didn’t happen with FEV1/FVC ratio (p<0.001; p<0.001; p = 0.3). The result of measurement 6-MWD indicated a significant increase score (p<0.001).There was a significant change on severity of COPD before and after ergo-­cycle exercise for 4 weeks. Mild COPD as 8 subjects at the initial study became 15 subjects after the exercise, and moderate COPD as 20 subjects became 13 subjects after the exercise (p=0.002). Relationship of lung functional capacity with 6-MWD indicated significant correlation on % FEV1 and % FVC, but not on % FEV1/ %FVC ratio.Conclusions : Thirty minutes ergo-­cycle exercise for 3 times a week within 4 weeks is respectively able to increase lung functional capacity and physical fitness level of mild and moderate COPD patients. Abstract

ABSTRACT Introduction: Cardiac surgery has been improved patient’s outcome with cardiac valve anomaly. There was 111 cardiac valve replacement surgery performed in Kariadi General Hospital Semarang in 2018.Postoperative pulmonary complication (PPC) is the most common complication in this procedure compared to cardiac complication which are thought caused by the disruption of normal respiratory function as a result from surgical and anesthetic procedure. Additional preoperative threshold inspiratory muscle training (Threshold IMT) has been considered as an ef fective intervention to reduce PPC. Methods: This is a quasi experimental study with main reason to know the role of Threshold IMT on PPC incidences. Subjects in the intervention group were given routine conventional rehabilitationexercises according to Clinical Practice Guide (PPK) with additional of Threshold IMT, which applied based on research protocols, while control group did convention al rehabilitation exercises only. Results: 18 subjects were divided into intervention group (n=9), and control group (n=9), PPC incidences (Intervention group n=2, control group n=7) were analyzed statistically using Chi-squared test andshowed significant differences (Fisher exact test p=0.02 with α=0.05). Conclusion: Additional of Threshold IMT preoperative may reduce the incidence of PPC on heart valve replacement surgery.Keywords: Postoperative pulmonary complication (PPC), Threshold inspiratory muscle training (Threshold IMT)

Abstract

Abstract: Physical exercise in patients with heart failure can decrease the level of proinflammatory biomarkers andincrease functional capacity. Circuit training is one of the most advantageous exercise models because it improves cardiorespiratory fitness and muscle strength.This study aimed to investigate the effect of circuit training on TNF alphaandsix minutes walk test in patients with chronic heart failure in RSUP Dr. Kariadi. Twenty-six stable chronic heart failure with reduced ejection fraction patients were randomized into exercise group that received circuit training in the rehabilitation center of Kariadi Hospital for a month and control group. TNF- α levels as a inflammatory biomarker and distance of six minute walk test as a functional capacity parameter were taken before and after the exercise period.Nine-teensubjects completed the study without any significant side effects.There was no significant difference in TNF-α levels before and after treatment between treatment groups and control groups (p=0,513). The treatment group with circuit training showed a greater increase in distance of six minute walk test than the control group (p=0,034).It was concluded that circuit training in patients with chronic heart failure can increase distance of six minute walk test, but no changes in TNF alpha levels. Keywords: Circuit training, TNF-a, six minute walk test

Abstract

Introduction: Indonesian Health Ministry had a strategy to make the primary care, including rhabilitation program by continuum of care. The purpose of this study was to determine factors related to the levelof knowledge, practice, and referral for continuum care cases that required rehabilitation services in Primary Health Care Doctors.Methods: The study design was cross-sectional. Participants were 23 Primary Health Care Doctors in Wonosobo District. Data collection was conducted in December 2017. The independent variablesconsist of gender, length of work, employment status, while the dependent variables were the level of knowledge, practice and referral cases of maternal, infants and toddlers, adolescents, adults and elderly.The measuring instrument was a questionnaire. Data were tested with chi-square, significance level <0.05.Results: Gender has correlation with the level of knowledge in maternal cases (PR = 0.121, 95% CI = 0.017-0.087, p = 0.026). Gender has correlation to the level of practice in maternal case (PR = 0.121,95% CI = 0.017-0.867, p = 0.026). Employment status has correlation to the referral rate of elderly cases(PR = 10.500, 95% CI = 1.015-108.577, p = 0.029).Conclusion: Gender has correlation with the level of knowledge and practice of maternal cases.
