Abstract

Background: The main challenge of the health financing mechanism in Indonesia is the allocation of health spending which is still dominated by the private sector, whereas the largest proportion comes from out of pocket payments. The system are a significant barrier in accessing health services. Many individuals with chronis diseases postpone the search for medical services because of high health care cost. The consequences of the delay is the loss of opportunities to overcome chronic illness. This research was aimed to analyze the relationship between health financing mechanism and mortality rate (GDR and NDR) in Santa Maria Pekanbaru Hospital. Methods: This cross-sectional study involves GDR and NDR of patients using out of pocket payment and those using health insurance in the year between 2014 - 2017. Data analysis was performed by independent samples t-Test (significance level p < 0,05).Conclusion: The health service outcome in patients with out of pocket payment is lower than health insurance patients. The out of pocket payment financing mechanism is one of the major problems in the transition to Universal Health Covered. It is recommended that our government must increase the health budget which is at least in accordance with the rule of law and improve the allocation of public sector health funds at least 2/3 of the total health budget to reduce the proportion of out of pocket to total health expenditure. Health care providers should do efficiency in all areas to reduce the cost of health services. Patient who do not have health insurance will have to rearrenge the allocation of their household expense to pay premium insurance and implement the healthy life habits.

Abstract

Background: The progress of the hospital industry now requires effective management to improve the quality and quality of service appropriate to the standard goals. Preparation of this business strategic plan should involve decision-maker in hospitals and existing organizational resources efficiently and effectively by taking into account current conditions. This study aims to provide a comprehensive review of the process of preparing a hospital business strategic plan so that an alternative strategy can be obtained for the next 5 years. Methods: This study was used operational research, qualitative method with in-depth interviews and hospital performance reports analysis. External and internal environment factors included finance, customer, demographics, geography, policy, marketing, information system, human resources, physical facility and organization were used as evaluated variables. SWOT analysis and Balanced Scorecard framework were used to identify critical success management factors. The Consensus Decision Making Group (CDMG) matrix were used to determined factor evaluation matrix as a final score. Results: Based on an analysis of external and internal environmental factors, alternative strategies that are suitable for current hospital conditions are market penetration and service product development. In order to realize this strategy there are several things that must be of particular concern, namely the limited resources that are available which include the availability of human resources and financial support and there is no available integrated hospital information system. Conclusion: The business strategy plan recommended for developing RSD Kolonel Abundjani Bangko’s services for the next 5 years is market penetration and service product development. In order to realize this strategic plan requires a strong commitment from all human resources in the hospital and related stakeholders.

Abstract

Background: Breastmilk provides nutrition that newborn and baby need during early stage of their lives. Breastmilk production can be increased by giving the mother oxytocin or prolactin enhancement that called galactogogue during breastfeeding. Giving galactogogue to mother bring side effect such as nausea, vomit, and insomnia. The example of galactogogue are domperidone and metoclopramide. Domperidone and metoclopramide not only useful to increase the breastmilk production as galactogogue but also can be used as antiemetic to reduce the effect in the galactogogue. This study aims to examine the effect of antiemetic for galactogogue during breastfeeding. Methods: This study was a systematic review that used PRISMA guidelines. Data obtained from Electronic databases ProQuest and Google Scholar that published between 2015 and 2018. By using keywords such as effect, antiemetic, galactogogue to find the relevant journal. Results: The search found 58 studies, of which 7 studies were relevant to analyze in this study. Majority of these studies used Randomized Control Trial (RCT) and 1 study used case study. Analysis of the studies found out that domperidone and metoclopramide can be used as antiemetic for galactogogue. Domperidone was not enter the blood circulation and did not interfere the breastmilk production

Abstract

Background: Examines properly implementation of Electronic Medical Records (EMR) which associated with workflows, policies and health services in enhancing patient safety. The electronic medical records have benefit to the patients due to the efficiency in the healthcare process. Therefore, the aim of this study was to examine the literatures on patients safety reporting based on electronic system review and assess the implementation. Methods: Systematic review with Prisma at the literature published between 2010 – 2019 using three large database: Proquest, Oxford Academic Journals, and Scopus. The eligible articles in this review should show an effect of patient’ safety, and product quality in hospital in correlation on using EMR. It must include some effect on medication error, prescribing error, error in the use of EMR that potentially endanger patient safety. Results:The most important function of EMR implementation is to improve patient safety in hospital and to reduce the cost. EMR reduce excess cost of Hospital Acquired Condition (HAC) by 16%, reduce death due to HAC by 34%. Doctor and nurses believe that the quality of patient data would be better by using EMR as their daily routines. Conclusion: EMR can improve patient safety due to some skills in technology. Thus, it would not harm to the patients safety. However, the implementation of EMR requires the human skill resources in using technologies, computer and programs.

Abstract

Peningkatan konsumsi pangan olahan diantaranya mi instan yang mengandung natrium berlebih dapat menjadi salah satu kontributor yang signifikan terhadap penyakit tidak menular. Kebijakan label informasi nilai gizi merupakan upaya untuk menurunkan prevalensi penyakit tidak menular akibat konsumsi gula, garam, dan lemak berlebih. Faktor yang berkontribusi dengan penggunaan label informasi nilai gizi yaitu pengetahuan gizi, kemampuan membaca label. Frekuensi konsumsi mi instan dilihat untuk mengetahui hubungan penggunaan informasi nilai gizi dengan frekuensi konsumsi mi instan per minggu. Tujuan penelitian ini bertujuan untuk mengetahui hubungan antara pengetahuan gizi, kemampuan membaca label informasi nilai gizi, dengan penggunaan label informasi nilai gizi pada konsumen di Jakarta dan sekitarnya (Bogor, Depok, Tangerang, dan Bekasi). Juga untuk mengetahui hubungan penggunaan label informasi nilai gizi dengan frekuensi konsumsi mi instan. Metode: penelitian kuantitatif dengan pendekatan cross-sectional. Penelitian dilakukan melalui survei daring menggunakan aplikasi Google Form. Hasil penelitian menunjukkan jumlah responden sebanyak 64 orang terdiri dari 16 orang laki-laki dan 48 orang perempuan. Responden yang mengonsumsi mi instan kurang dari 2 kali per minggu sebanyak 82,8%. Responden yang menggunakan label informasi nilai gizi sebanyak 37,5%. 41,4% responden dengan pengetahuan cukup menggunakan label informasi nilai gizi. 44,0% responden dengan kemampuan baik menggunakan label informasi nilai gizi. 79,2% responden yang menggunakan label informasi nilai gizi mengonsumsi mi instan dengan frekuensi <2 kali per minggu. Dari hasil penelitian disimpulkan pengetahuan gizi, kemampuan membaca label informasi nilai gizi dapat mempengaruhi penggunaan label informasi nilai gizi. Konsumen yang menggunakan label informasi nilai gizi lebih banyak yang mengonsumsi mi instan <2 kali per minggu. Penggunaan label informasi nilai gizi dapat menurunkan konsumsi pangan yang tidak sehat.

Abstract

One of important things in managing Coronavirus Disease 2019 (COVID-19) pandemic is managing medical waste production. Medical waste management start from waste sort until waste manage by licensed parties, both of patients’ waste and health workers’ waste. Sentra Medika Hospital Cikarang is one of the referral hospitals that handles COVID-19. This research is a descriptive analytic study. Researchers analyze hazardous and toxic waste management before and during a pandemic by conducting interviews with the nursing unit and using secondary data. Secondary data was obtained from the hospital environmental health unit. There is no significant differences medical waste before and during pandemic. This happens because there are decreasing of patients amount which come to hospital. Besides, health services waste is patients waste such as food or drink waste and personal protective equipment such as hazmat, masks, gloves and others. The weight of waste does produce is lighter. Medical waste at Sentra Medika Hospital has been managed in accordance with regulations.

Abstract

Pelayanan operasi merupakan salah satu pelayanan kesehatan di rumah rumah sakit yang dapat menggambarkan mutu rumah sakit. RSUP Sanglah, Denpasar merupakan rumah sakit di tingkat provinisi yang memiliki waktu tunggu operasi yang cukup panjang. Adapun waktu tunggu tindakan operasi yang Panjang pada umumnya terkait dengan factor-faktor seperti ketersediaan sarana dan prasarana, sumber daya manusia, serta lamanya persetujuan dari pasien dan keluarga terhadap tindakan operasi. Perlu dilakukan penelitian untuk mengetahui factor yang secara siginifkan menjadi dasar dalam penentuan kebijakan di level RSUP Sanglah, khususnya di Instalasi Rawat Darurat, untuk meningkatkan pelayanan operasi. pendekatan observasional dan desain analisis potong lintang. Penelitian ini melibatkan 54 responden pada penelitian kuantitatif dan 7 informan pada penelitian kualitatif. Bulan Desember 2017-Februari 2018 didapatkan terjadi bed block sebanyak 38,9 % dan tidak terjadi bedblock 61,1%. Persetujuan operasi didapatkan data persetujuan operasi ≥ 1 jam sebanyak 17 responden (31,5%) dan persetujuan operasi < 1 jam sebanyak 37 68,5%. Alat dan sarana didapatkan tidak lengkap 5,6% dan lengkap 94,4%. Tindakan operasi ≥ 5 jam dikategorikan delay sebanyak 33,3%. Tindakan operasi < 5 jam dikategorikan tidak delay sebanyak 66,7%. Faktor yang paling berpengaruh terhadap keterlambatan operasi adalah persetujuan operasi. Persetujuan operasi menyebabkan waktu persiapan operasi sampai pengamprahan kamar operasi menjadi lebih lama. Terdapat hubungan yang signifikan dari bed block, persetujuan operasi serta alat dan sarana terhadap keterlambatan operasi. Temuan dari secara kuantitatif tersebut didukung pula dengan temuan secara kualitatif. Hasil penelitian dapat dijadikan pedoman dalam penyusunan strategi peningkatan kualitas pelayanan kamar operasi di RSUP Sanglah

Abstract

The national TB control program target is TB elimination at 2035 and TB eradication at 2050. Hospitals must establish a DOTS Team who have responsibility to implementing the TB control program and must be record and report any disease cases. TB case reporting uses the Integrated Tuberculosis Information System (SITT) since 2020. Integrated Tuberculosis Information System has become the Tuberculosis Information System (SITB). Syarif Hidayatullah Hospital is one of the 3 hospitals that have not completed the report. This research is a descriptive analytic study with a qualitative approach. The data collecting data include documents review, observation, and in-depth interviews with key informants. The results showed that recording and reporting did not run optimally because 1) the lack of human resources such as nurses as the spearhead of manual record, 2) there was no funding for the TB DOTS program including for recording and reporting, 3) Hospital Information System was not integrated into all service units causing difficulties in data collection TB patients, 4) the internal network did not working properly because there is no socialize about TB recording and reporting flowchart. Increased management commitment in implementing the TB DOTS program so that recording and reporting can working properly.
