Abstract

Objective: To demonstrate usefulness of Intra Prostatic Protrusion (IPP) as Predictive Factor for Acute Urinary Retention (AUR) in Benign Prostate Hyperplasia (BPH) Patients. Material & methods: Cross sectional retrospective study of case records from January to July 2012 of BPH patients at Saiful Anwar General Hospital (SAGH) Malang was carried out. The data were collected from the Medical Record Division in SAGH Malang. We noted age, prostate volume, IPP, IPSS and urinary retention status. The patients were classified by IPP degree < 5 mm (group A), 5-10 mm (group B), 10 -15mm (group C), > 15mm (group D) and we compared all parameters that we noticed. Results: Eighty patients, mean age was 66.32 years were enrolled. Transabdominal ultrasound determined the mean IPP was 13.5 mm, and prostatic volume 95 cc. IPP values were distributed as follows: group A 10 (12.5%), group B 25 (31.25%), group C 24 (30%), group D 21 (26.25%), with AUR incidence in group A 20%, group B 36%, group C 79%, and group D 81%. The IPP showed a significant correlation with urinary retention (r = 0.8, p < 0.05, OR = 15) and IPSS (r = 0.6, p < 0.05). Conclusion: IPP can be used as predictive factor for the incidence of acute urinary retention in BPH Patients.Keywords: Intra Prostatic Protrusion, Acute Urinary Retention, Benign Prostate Hyperplasia.

Abstract

ABSTRACTBenign prostatic hyperplasia (BPH) is one of the most common diseases affecting the elderly, and Transurethral Resection of the Prostate (TURP) is a gold standard surgical procedure in BPH patients. Although the TURP rate is high, the publication of TURP profile data in Indonesia is still limited. This study aimed to determine the characteristics of BPH patients who underwent TURP at Saiful Anwar General Hospital. This study is a descriptive study by collecting data of BPH patients who underwent TURP from January 2015 to August 2017. As many as 162 patients with BPH underwent TURP. TURP is primarily performed in patients aged 61-70 years (39.5%). Recurrent urinary retention was the most common indication for the procedure (54.9%) followed by bladder stone (21%), pharmacological therapy failure (10.5%), inguinal hernia (8%), severe LUTS (3.7%), and renal insufficiency (1.9%). As many as 94 patients (58%) have a prostate volume higher than 50 ml. Urinary retention is the most common complaint among BPH patients who underwent TURP, and recurrent urinary retention is the most common TURP indicator.

Abstract

The acute scrotum is a common urological emergency and has a broad differential diagnosis. There is no clear consensus on a particular algorithm to follow in the diagnosis of testicular torsion. Testicular Workup for Ischemia and Suspected Torsion (TWIST) Score, a risk scoring system based on signs and symptoms, can be invaluable in managing patients with the acute scrotum, specifically testicular torsion. This study aimed to evaluate the effectiveness of the TWIST scoring system for differential diagnosis of acute scrotal patients. The study was conducted using secondary data, medical records from 111 male patients diagnosed with acute scrotum in Saiful Anwar General Hospital Malang between January 2015 and December 2018. Data about the onset of pain, first contact, history, physical examination findings, any adjunct test performed, and intra-operative findings were collected. The effectiveness of the TWIST score in diagnosing testicular torsion was determined by calculating the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV). Of the 111 patients, 43 patients (38.7%) had testicular torsion. The mean age was 17.49 (6.69) years. A total of 39 patients were classified as high risk as per the TWIST Score with cut point 5 had 83.7% sensitivity, 95.6% specificity, 92.3% positive predictive value (PPV), and 90.3% negative predictive value (NPV). The area under the curve was 0.93. The use of the TWIST Score in clinical practice can accurately determine high-risk patients who can proceed straight to the operating theatre without the need for an ultrasound.

Abstract

Migration of the intrauterine device (IUD) into the bladder has been a rare case. There were reported 31 cases of IUD migration into the bladder until 2006. Although IUD migration is asymptomatic, it should be removed to prevent complications such as pelvic abscess, bladder or intestinal rupture, and adhesion. A 52-year-old woman came to the urology clinic with pyuria since the previous 3 months. She had a history of IUD insertion in 1982; and two months later, she got pregnant. Since 2015, she has suffered from dysuria but has never been treated and has worsened in the past 3 months. On physical examination, tenderness was found in the suprapubic region. The results of urinalysis showed pyuria and hematuria. The ultrasonography findings, there were large bladder stones. An abdominal x-ray revealed the presence of a bladder stone with the IUD tail. Vesicolithotomy was performed and the IUD was found attached to an 11x7 cm bladder stone. The patient had a good postoperative condition without any special complications. The IUD in the bladder is a medium for forming secondary bladder stones. Most cases of IUD migration are caused by a lack of evaluation after the installation procedure. This case suggested that the physician should be more careful in carrying out the installation procedure. It is necessary to evaluate the location of the IUD after installation to prevent further patient complications.

Abstract

Background: Nocturia is a common urinary system disease. Objective: This study aimed to investigate the causes of nocturia in women Indonesian inhabitants aged≥ 40 years. Methods: A stratified sample strategy was used to conduct a randomized cross-sectional study on 562 residents under the age of 40 in Malang City, East Java, Indonesia. A questionnaire was completed, which included socioeconomic demographics, lifestyle characteristics, and clinical history. Each night, nocturia was defined as at least two voids. The chi-squared test was used to determine proportional differences between age and gender groups. Multivariate logistic regression analysis was used to assess gender-related factors. This was determined that P0.05 was statistically significant. Results: Data on 562 people aged 61.60 + 9.81 years eligible for statistical analysis at the end, comprising 185 (32.92%) men and 377 (67.08%) women. Overall nocturia prevalence was 31.8% (179/562). It rose significantly with age (P<0.001) and reached >48% in those above the age of 70. In both men and women, nocturia was linked with diabetes, hypertension, cardiovascular disease, and the overactive bladder symptom score (OABSS) (P0.05). There was no link discovered between nocturia and education, profession, civil status, BMI, female birth history, or the International Prostate Symptom Score. Conclusion: Nocturia is linked to aging, cardiovascular disease, hypertension, OABSS, and diabetes in Indonesians over the age of 40.

Abstract

The circumcision clamp has been the primary alternative to the traditional surgical circumcision procedure since its invention in 1934. The circumcision clamp has the following advantages such as improved wound healing, shorter operation time, and no stitch removal pain. However, the clamp technique has a disadvantage where the operator cannot visualize the glans penis. Many studies have already reported the incidence of trauma or psychiatric disorders related to penile amputation, but the case of circumcision clamp-related penile amputation in Indonesia had never been reported. An eight-year-old boy was taken to the emergency department with post-circumcision bleeding. The patient had been circumcised using an Anastomat® circumcision device in the clinic, two hours prior and then referred to the emergency department. We perform penile exploration, and a defect in the ventral part of the glans penis was found, with the source of bleeding coming from the glans penis, and the urethra remained intact. Penile degloving with urethral spatulation and catheterization with a 10 Fr 2-way foley catheter was installed, continued with penile reconstruction and skin closure. The operation lasted for about sixty minutes. After six months of follow-up, the patient could urinate normally and spontaneously without any clinical complaint. Even though circumcision using a circumcision clamp is considered a safe procedure and preferable to surgical circumcision nowadays, it is necessary to emphasize that the use of a circumcision clamp still has a risk rate for penile amputation.

Abstract

Introduction. Benign Prostatic Hyperplasia (BPH) is a medical condition where there is an excessive growth of smooth muscle and epithelial cells in the prostate gland. BPH is typically seen in older men and is non-cancerous. It is estimated that around 50% of men aged 60 or above worldwide are affected by BPH. Some research has looked into the factors that may increase the risk of BPH, and one theory suggests a possible connection between metabolic syndrome and the development of BPH. This study aims to investigate the potential relationship between metabolic syndrome and the development of BPH.Methods. This study is an analytical observational research conducted at Saiful Anwar Hospital in Malang from 2015 to 2020, employing a case-control research design. The data used for this study are secondary data extracted from medical records. The data analysis involved the application of both Chi-Square and Logistic Regression methods.Results. The study gathered data from 90 patients, and the analysis revealed that the highest number of BPH patients fell into the category of those with hypertension, totaling 34 individuals (75.56%). Among the factors considered, including fasting blood glucose, body mass index, and metabolic syndrome, the chi-square test indicated that body mass index and blood pressure were significantly associated with BPH, both yielding p-values of 0.046; OR (95%Cl) = 2.473 (1.006 – 6.075). Furthermore, in the logistic regression test, it was determined that fasting blood glucose, blood pressure, and body mass index categories had a significant impact on BPH.Conclusions. This research illustrates that there is an association between type 2 diabetes, hypertension, and obesity with the occurrence of BPH. However, there is no statistically significant relationship between the components of metabolic syndrome and the occurrence of BPH.

Abstract

Objective: This research wanted to figure out the characteristics and determinant factors of short-term complications of TURP performed by urology resident in educational hospital. Material & Methods: This study was an analytical study using secondary data from electronic medical records of the total sample of 502 BPH patients undergoing TURP performed by urology resident January from 2015 until June 2022, and evaluated short-term complications, Clavien Dindo, and level of urology resident. Inclusion were BPH patients undergoing TURP by urology resident, while the criteria of exclusion were incomplete data of patient’s demographic and records of clinical data. Results were displayed descriptively in distribution tables and analyzed using logistic regression with STATA 17. Results: Short-term complications of TURP were found in 10 patients (1.99%), i.e., bleeding (0.79%), clot retention (0.6%), and urinary retention (0.6%). The commonest groups were age at 61-70 (0.79%), Clavien Dindo I (0.79%), urology resident level at 5th semester (0.99%), prostate volume >50cc (0.99%) and complaint of urinary retention (0.99%). There were no significant correlations between patient age, level of urology resident, prostate volume, and chief complaint compared to rate of complication (p>0.25). Conclusion: TURP remained surgery of choice for BPH despite several short-term complications performed by different level of urology resident and there were no significant correlations between level of urology resident and rate of complication. Abstract

Objective: Comparing two cases of the management of bladder calculi caused by Intra Uterine Device (IUD) migration with other similar cases worldwide. Case(s) Presentation: A forty-eight-year-old woman diagnosed by Abdominal Non-contrast Computed Tomography (NCCT), already done IUD extraction through the bladder approach also hysteroscopy to evaluate the position from the womb side, and continued with the stone removal by vesicolithotripsy. The second case revealed a bladder stone with IUD tail presence based on Kidney Ureter Bladder (KUB), performed vesicolithotomy, and IUD was found attached to the stone. Both patients’ conditions improved after surgeries. IUD migration into the bladder causes perforation, stone formation, and Urinary Tract Infection (UTI). NCCT, KUB, and Ultrasonography can be used as imaging modalities for initial screening when IUD migration is suspected. Discussion: The managements consist of open cystolithotomy, transurethral grasping forceps, or minimally invasive laparoscopy. Open and laparoscopic surgery should be considered for the IUD removal with partial penetration due to vesicouterine fistula and high-risk stone removal, especially for the large stone with IUD embedded inside. Conclusion: Minimally invasive methods are efficient, safe, and frequently preferred for the bladder stone caused by IUD migration. Keywords: Bladder stone, IUD, migration.
