Abstract

Peningkatan prevalensi diabetes akan meningkatkan biaya pengobatan. BPJS diberlakukan di Indonesia sejak tahun 2014 dan sistem klaimnya menggunakan tarif INA CBGs. Rumah sakit harus dapat melakukan penghematan sehingga tarif INA CBGs menutupi seluruh biaya pasien. Salah satu penghematan adalah dengan menggunakan obat yang ada dalam formularium nasional atau obat generik terpilih. Tapi masih banyak masyarakat yang menganggap obat generik adalah obat murah dan tidak berkualitas. Penelitian ini bertujuan untuk menganalisis perbedaan pengobatan Diabetes Melitus pada pasien BPJS dan pasien umum dilihat dari lama rawat, penurunan kadar gula darah dan biaya terapi. Pengambilan data dilakukan secara retrospektif di tiga rumah sakit. Data dianalisa terhadap 50 pasien umum dan 129 pasien BPJS dengan SPSS-22 menggunakan uji T dan Mann-Whitney. Hasil penelitian menunjukkan  tidak ada perbedaan antara pasien BPJS dan pasien umum dalam hal lama pasien dirawat (p=0,219), penurunan kadar gula darah sewaktu (p=0,509) dan ada perbedaan yang signifikan dari biaya rata-rata terapi obat pada pasien BPJS  dan pasien umum (p=0,030). Hasil penelitian tidak ada perbedaan penurunan kadar gula darah dan lama pasien di rawat di rumah sakit pada pasien BPJS dan pasien umum, sehingga dapat disimpulkan kualitas pelayanan pasien BPJS dan pasien umum adalah sama. Perbedaan terdapat pada biaya pengobatan, dimana biaya pengobatan pasien BPJS lebih murah dibandingkan dengan pasien umum sehingga pasien BPJS lebih diuntungkan dari pasien umum.

Abstract

Sauropus androgynus (L.) Merr. or katuk is one of the medicinal plants broadly used in Indonesia as it has active compounds that can, among others, stimulate reproductive hormones. This study was aimed to determine which active fraction of S. androgynus leaves that have the potential to improve the sperm quality of male rats based on three parameters, namely sperm count, viability, and motility. It employed fractionation using the liquid-liquid technique with n-hexane, ethyl acetate, and water solvents to obtain the fractions. Twenty-four mature Sprague-Dawley male rats were divided into four equal groups: the normal group (untreated group) received 0.5% Na-CMC suspense, and the three fraction groups were given the n-hexane, ethyl acetate, and water fraction p.o., respectively, at the dose of 11.85 mg.Kg-1 BW daily for seven days. Sperm count, viability, and motility were measured on Day 8 (after treatment) from the sperm samples collected at the cauda epididymis of the sacrificed test rats. The results showed that compared with the normal group, the n-hexane and ethyl acetate fractions significantly increased the three parameters (p<0.05). Therefore, the S. androgynus leaf fractions have the potential as a natural material that can increase the fertility of male rats.

Abstract

The objectives of this study were to know (1) Patients’ characteristics (2) The most etiology of leucorrhoea (3) Association between clinical manifestations or genital symp-toms with etiology of leucorrhoea (4) Therapy management of leucorrhoea by obstet-ric-gynecologist and venereologist (5) Factors influenced the treatment of leucorrhoea (6) Compliance with hospital therapeutic guidelines. The study was cross sectional and retrospective. A total of 437 patients hospitalized from January 2006-December 2007 were included. The results showed that leucorrhoea was found in 17,6% of patients at sexually transmitted disease clinic and 82,4% of patients at obstetric-gynecology clinic. The majority of patients were in productive age, married, and housewife, with most of genital symptoms were pruritus and curd-like vaginal dis-charge. The most of etiology leucorrhoea in this study was candidiasis. Statistically, there were association between genital symptoms with candidiasis and bacterial vaginosis (p<0,05). The specific genital symptoms of candidiasis were pruritus and curd-like vaginal discharge, whereas for bacterial vaginosis were homogeneous and increased vaginal discharge. There were different treatments of vaginal discharge be-tween obstetric-gynecologist and venereologist. For candidiasis, the obstetric-gyne-cologist preferred to use fluconazole, and metronidazole+nystatin (Flagistatin®); whereas the venereologist used clotrimazole and itraconazole. For bacterial vaginosis, obstet-ric-gynecologist used clindamycin and metronidazole+nystatin (Flagistatin®), while venereologist preferred to use metronidazole. For trichomoniasis there was no different treatment between obstetric-gynecologist and venereologist. In pregnancy, antibiot-ics used to treat leucorrhoea were clindamycin, fluconazole, metronidazole+nystatin (Flagistatin®), metronidazole, and nystatin. Prescribing compliance with the hospi-tal therapeutics guidelines were 37,8%. The type of antibiotics used were azitromycin, clindamycin, clotrimazole, doxycycline, fluconazole, itraconazole, ketoconazole, and metronidazole. Statistics analysis by Logistic regression (Cl 95%) showed that factors influenced the treatment of leucorrhoea included genital symptoms (OR = 0,975), risk factors (OR = 0,917), etiology (OR = 1,103), and comorbid diseases (OR = 1,387).

Abstract

Decreased of libido is illustrated with disinterest in sexual activity caused by erectile dysfunction, impotence, and infertility. It can be treated by aphrodisiac agents. Katuk or Sauropus androgynus (L). Merr has long been used as a medicinal plant. The aim of this research was to evaluate which fraction of katuk leaf ethanol extract that had the aphrodisiac activity with parameters of climbing, introduction and the weight of testicular and seminal vesicle of male rat. Sprague-Dawley male rats as animals model divided into five groups: the normal control group, the positive control group (X-gra®), the n-hexane, ethyl acetate and water fraction groups in which each fraction group given a dose of 11.85 mg/kg body weight. The number of climbing and introductions were calculated on 0, 1st, 3th, and 5th day. The data were tested statistically with one-way ANOVA test followed by Tukey test. The weight of testicular and seminal vesicle of male rat were observed on the 15th day. Previously, rats were anesthetized using ketamine and then performed surgery. The results showed that the n-hexane fraction (11.85 mg/kg body weight) increased libido with the average number of climbing was 16.5 times and the average number of introductions was 27.75 times. It was also able to increase the weight of testicular and seminal vesicle of male rat compare to positive control (X-gra® 51.37 mg/kg body weight). The compounds contained in n-hexane fraction are terpenoids and steroids.
