Abstract

Infeksi HIV sering dikaitkan dengan gangguan jantung. Namun demikian, keterlibatan jantung pada populasi pasien HIV sering kurang didiagnosis atau dikaitkan dengan salah pada proses penyakit non-jantung lainnya. Efusi perikardial (PE) tidak jarang ditemukan pada pasien dengan HIV / AIDS yang dilaporkan mempengaruhi hampir 5% pasien HIV. Telah dibuktikan bahwa tamponade jantung, keadaan darurat hemodinamik yang serius sebagai akibat efusi perikardial, tidak perlu dikaitkan dengan efusi besar tetapi dengan cepatnya akumulasi PE dan kurangnya kepatuhan kompensasi dalam ruang perikardial. Selain itu, tingkat keparahan PE pada pasien HIV / AIDS telah terbukti berkorelasi dengan prognosis buruk sebelum meluasnya penggunaan terapi antiretroviral (ART), meskipun demikian, implikasi prognostik PE tidak dapat ditentukan karena HAART telah mengubah riwayat alami PE. .Kata kunci : Efusi perikardial; Tamponade Jantung; HIV; AIDS.

Abstract

BNP secreted by left ventricle as response to wall stress in patient with heart failure. Elevated concentration of NT-pro-BNP correlate with severity of heart failure across all stages of the condition and left ventricle ejection fraction in patient. Several clinical trials have demonstrated that neurohormonal modulation on the RAAS decreases NT-proBNP level and results in favorable outcomes. One of the drug used for blocked RAAS system is ACE inhibitor, decrease of NT-proBNP level show response to therapy include therapy with ACE inhibitors. To analize changes in the levels serum NT-proBNP levels after ace inhibitor therapy in patients with heart failure and monitoring creatinine serum. This study was a observational, prospective, non-randomized trial involving patient age 21-75 years, with NYHA class II-III HF, using ACE inhibitor therapy plus other therapy maximum 3 months before study without ARB or beta blocker. We compared serum NT-pro-BNP and creatinin serum parameters before and after two months treatment with ACE inhibitor. This study conducted in cardiovascular ambulatory patient dr. Soetomo hospital Surabaya. Between August-November 2015, 13 patient (38-63 years, 6 woman, 7 men) include in this study. The mean baseline level of NT-proBNP is 2166.92±1236.73 pg/ml, and creatinin serum 1.023±0.601 mg/dL. The NT-pro-BNP were significantly decreased after two months of treatment with ACE inhibitors 1508.23±651 pg/mL (p=0.025), there were no significant differences creatinin serum between two groups 0.951±0.0365 mg/dL (p=0.111). The results demonstrated the benefits of ACE inhibitor on the neurohormonal profile in patients with HF. If necessary we could measure NT-proBNP level to support prognosis data and monitoring effectivity therapy especially ACE inhibitor which had antiremodelling effect towards patients with HF.

Abstract

Heart failure (HF) is the final common stage of many diseases of the heart. NT-proBNP levels are increased in HF and correlate well with ventricular wall stress and severity of HF. Combination therapy with Bisoprolol and ACE-inhibitor decreases NT-proBNP level in patient with HF. The use of Bisoprolol as a combination with ACE-inhibitor is still dominate in outpatient setting at Dr. Soetomo teaching hospital. The objective of this study is to analyze NT-proBNP level changes as an indicator in cardiac function after combination therapy with Bisoprolol and ACE-inhibitor in patient with HF.Methods: This study was prospective, observational and conducted in outpatient setting. Consecutive patients who meet the inclusion criteria of the study were included. Blood samples were taken at pre and 2 months post combination therapy with Bisoprolol and ACE-inhibitor, then NT-proBNP level was measured with IMMULITE®. There were 14 patients enrolled in this study (7 males, 7 females). The result showed that NT-proBNP 2 months post combination therapy with Bisoprolol and ACE-inhibitor is significantly decreased than baseline with mean baseline of NT-proBNP level is 4191.43 ± 4367.277 pg/ml to 2786.79 ± 2485.199 pg/ml (p=0.025). From a total 14 patients, 9 patients had NT-proBNP decreases >20% (20.1% – 56.4%) and 3 patients had NT-proBNP decreases <20% (1.8%, 6.6%, and 12.4%). There were 2 patients with NT-proBNP increases >40% (43.4% and 40.4%). In conclusion, there was a significant decreases in NT-proBNP level after 2 months combination therapy with Bisoprolol and ACE-inhibitor in patient with HF.

Abstract

NT-proBNP is an inactive fragment of BNP secreted by stretched ventricle as response to wall stress in patients with heart failure. As a specific cardiac marker, elevated NT-proBNP correlates well with heart failure severity. The principle of heart failure therapy is modulation on neurohormonal activation. ARB can modulate neurohormon on RAA system, that result in decreasing NT-proBNP level and favorable outcomes. Reduction in NT-proBNP more than biologic variability (> 25%) shows a therapy response.This study was to analyze change of NT-proBNP after ARB therapy in ambulatory HF patients. This observational prospective study was carried from September to December 2015. Blood sampling was performed on patients who meet the inclusion criteria of the study at first visit and after 2 months therapy. NT-proBNP was measured by IMMULITE® as primary parameter and creatinin as secondary parameter. There are 14 patients met the inclusion criteria of the study (11 males and 3 females). ARB therapy used in patients were Valsartan (64%), Telmisartan (22%) and Candesartan (14%). After 2 months ARB therapy, a decrease in level of NT-proBNP with initial median 3092.5 (216 – 32112) pg/ml to 2135.5 (350 – 16172) pg/ml respectively were statistically significant (p=0.003). And the secondary parameter creatinin serum convert to eGFR shows a change in eGFR with initial median 73.33 (37.05 – 266.68) ml/minute to 81.04 (39.31 – 167.02) ml/minute respectively were statistically not significant (p=0.657). There were 7 patients (50%) have a decrease > 25%. In this study, we found that ARB therapy can change NT-proBNP level significantly after 2 months therapy.

Abstract

Background : Assessment of left ventricular function in patients with acute heart failure is important for prognostication, determination of treatment plan, for decisions related to expensive device therapies and for assessing response to treatment. Echocardiography is the "gold standard" of assessing left ventricular function, and in establishing a diagnosis of heart failure. Left ventricular Global Longitudinal Strain (GLS) assessed through Speckle Tracking Echocardiography (STE) is currently considered sensitive in assessing left ventricular motion, by analyzing multidimensional myocardial deformation . s ST2 used to be correlated with stretch miocard and inhibid ligand of IL-33 wich also inhibit the cardioprotectve effect. Objective : To prove the positive correlation between GLS-left ventricle with soluble ST2. Methods : This is a correlational study with cosecutive sampling technique. Thirty subjects participate in this reseach and each subject underwent echocardiography and GLS-left ventricle and soluble ST2 blood examination. The correlation between GLS-left ventricle and soluble ST2 were evaluated using Spearman correlation test. Results : There is a strong, significant, positive correlation between the GLS-left ventricle with soluble ST2 (r = 0.99 and p = 0.0001). Conclusion : There is a strong, significant, positive correlation between the GLS- left ventricle with soluble ST2. Abstract

Abstract Background: Hypertension is associated with diastolic dysfunction and impaired exercise capacity. LA has a substantial role to provides an optimal left ventricle diastolic filling. In this study, we aimed to assess the left atrial strain through STE as a predictor of functional capacity in the hypertensive population. Methods: A total of 43 hypertensive women (mean age 50 ± 5.2 y.o; BMI 28.5± 4.1 Kg/m2) consecutively enrolled in this study. Patients with LVEF <50% or significant valvular pathology excluded. Two-dimensional STE performed to asses LA strain parameters, including PALS , PACS, and conduit strain. Functional capacity assessed by treadmill stress test using Bruce protocol. Result: Mean PALS, PACS, and conduit strain in this study was lower than reference normal value 25.8%< 39%; 12.4% <17%; 13.5% <23%, respectively). All of the subjects had a preserved systolic function (mean LVEF 72.5 ± 7.6%) and normal resting LA pressure (mean PCWP 11.96 ± 2.09). Left atrial strain parameters value in the low-fair functional capacity group was not significantly different to average-good functional capacity group (p>0.05). Left atrial strain parameters value was not significantly correlated to exercise duration and achieved METS (P>0.05). Conclusion: LA strain value in this study was below the standard reference limit. LA strain cannot predict functional capacity in the hypertensive population with preserved ejection fraction and normal resting LA pressure.

Abstract

AbstrakPenelitian ini bertujuan untuk mengetahui: 1) Ada tidaknya Pengaruh Latihan Footwork Terhadap kelincahan Pada Pemain BuluTangkis Banjarbaru. Metode yang digunakan dalam penelitian ini adalah metode pra-eksperimental dengan teknik menggunakan rancangan pretest dan posstest dalam satu kelompok, dengan memberikan perlakuan latihan berupa shadow 6 arah mata angin. Disusun dengan mengkuti  pedoman dan prinsip-prinsip latihan  dengan memadukan beberapa pendapat ilmuan kepelatihan. Populasi dalam penelitian ini adalah pemain bulutangkis Banjarbaru yang berjumlah 21 orang. sedangkan sampel berjumlah 8 orang dengan teknik pra-eksperimental tes dan pengukuran. Kesimpulan penelitian: Ada Pengaruh Latihan Footwork Terhadap kelincahan Pada Pemain BuluTangkis Banjarbaru Kata Kunci: Pengaruh, Footwork, Agility, Pemain BuluTangkis

Abstract

Pelaksanaan protokol covid-19, pengecekan suhu tubuh secara manual menggunakan thermo gun ataupun thermal scanner dapat menyebabkan antrian yang panjang serta kerumunan. Selain itu pengecekan suhu tubuh dengan cara ini dapat membahayakan petugas karena masih ada kemungkinan salah satu orang yang dicek suhu tubuhnya membawa covid-19. Prototipe pintu otomatis menggunakan sensor temperatur merupakan sebuah inovasi dimana pintu tersebut memiliki sensor suhu non kontak MLX90164 yang dapat melakukan pembacaan suhu kepada setiap orang yang ingin memasuki ruang kuliah. Alat tersebut dapat membaca suhu tubuh seseorang yang berada di depan sensor dengan jarak 1 cm. Alat ini juga dilengkapi dengan bilik sterilisasi dibagian depan pintu sehingga dapat menjaga setiap orang yang ingin memasuki ruangan tetap bersih dan steril. Dengan sedikit modifikasi, pintu otomatis menggunakan sensor temperatur ini dapat diterapkan pada setiap ruang kuliah di instansi penyelenggara pendidikan yang ada di seluruh Indonesia untuk melaksanaan protokol kesehatan covid19 dengan lebih efektif. Kata Kunci: Protokol Kesehatan Covid-19, Sensor MLX90614, Pintu Otomatis, Bilik Sterilisasi

Abstract

Penelitian ini menggunakan desain analitik, rancangan cross sectional dalam bentukunivariat dan bivariat (uji chi square). Sampel berjumlah 35orang. Dengan variabel independenadalah faktor-faktor yang mempengaruhi status gizi pada balita usia 1-4 tahun. Dan variabeldependen adalah status gizi pada balita usia 1-4 tahun.Hasil penelitian menemukan tidak ada hubungan pola makan (p = 0,042) dan riwayatpendidikan ibu (p = 0,011) dengan status gizi balita usia 1-4 tahun. Ada hubungan riwayat penyakitinfeksi (p = 0,013), pengetahuan ibu tentang gizi (p = 0,030), sikap ibu (p = 0,012), jumlah anggotakeluarga (p = 0,007), pemanfaatan pelayanan kesehatan (p = 0,039) dan pendapatan keluarga (p =0,004) dengan status gizi balita usia 1-4 tahun di wilayah kerja Puskesmas Rajabasa Indah kotaBandar Lampung tahun 2014. Disarankan bagi masyarakat untuk lebih mengetahui faktor penyebabmasalah gizi pada balita, sehingga diharapkan dapat meningkatkan pencegahan terjadinya gizi burukpada balita, dengan mengikuti penyuluhan gizi yang dilakukan pihak puskesmas dan mengikutikonseling gizi, bagi tenaga kesehatan khususnya di bidang Kesehatan Ibu dan Anak (KIA) agar dapatmeningkatkan pelayanan kesehatan pada balita dan menjalin kerjasama dengan sektor lain dalammempromosikan pentingnya status gizi pada balita

Abstract

Good physical fitness is needed to ensure the continuity of education, especially during the Covid-19 pandemic which requires students to do online learning. This study aims to analyze the relationship between sleep quality and physical activity with fitness levels in college students during the Covid-19 pandemic. This study used a descriptive-analytic study method with a cross-sectional design using 228 respondents. Sleep quality was measured using the Pittsburg Sleep Quality Index (PSQI) and physical fitness level were measured using a questionnaire. The data were analyzed using the SPSS Chi-Square statistical test. The results showed good sleep quality in 82 people (36%) and 146 people (64%). The level of physical fitness of respondents is very fit 15 people (6.6%), fit 46 people (20.2%), quite fit 119 people (52.2%) and less fit and not fit 17.1% and 3 respectively,9%. The results of the statistical test of the relationship between sleep quality and physical fitness level obtained p-value = 0.019, which means that there is a significant relationship. There is a significant relationship between sleep quality and student fitness levels during the Covid-19 pandemic.
