Abstract

Hypertension is called the "silent killer" which is a risk factor for heart disease, stroke and kidney failure. High blood pressure that can not protect properly will increase the mortality and morbidity. Elderly patients aged ≥ 60 years, most of them already have hypertension and even have experienced the risk factors caused by hypertension. The goal of therapy is not to exacerbate organ damage and reduce mortality and improve patient quality so that evaluation is necessary, especially for the selection of antihypertensive drugs. This study aims to meet the suitability of the types of antihypertensive drugs given to elderly patients at the General Hospital of Imelda Pekerja Indonesia Medan with the standard of hypertension treatment. Research data collection was carried out in a retrospective manner in the period January-April 2020. Data were analyzed using the SPSS 21, JNC VIII (2016) method. There were 9 patients (28.1%), the use of antihypertensive drugs in elderly patients who did not comply with the standard recommendations for hypertension therapy and 23 patients (71.9%) who used antihypertensive drugs in accordance with JNC VIII standards.

Abstract

Hypertension is a degenerative diseases that can take long treatments or even a lifetime. Hypertensive treatment costs tend to be as high as medical costs and other administrations in hospitals, so a CMA analysis is needed to getmoreeconomical and effective treatment cost. The study was intended to learn the minimum costs of the use of drugs from the CCB combination of ARB. The study is a descriptive observational study of retrospective nature. Studies show that of 60 patients there are 24 male patients (40%) and 36 female patients (60%). The average age of most patients between 51-60 years, with the most hospitalization is 1-5 days. The most widely used antihypertension medication is candesartan 8 mg combination of amlodipin 10 mg (42%) and candesartan 16 mg combination of amlodipin 10 mg (25%). Studies have concluded that antihypertension drugs that have a minimum costs (CMA) is candesartan 16 mg combination of amlodipin 10 mg at Rp.3.870.065.-. Where the average costs paid by patients daily for antihypertensive drugs are Rp.4.952.-, treatment costs Rp.167.819.-, doctor costs Rp. 69.619.- and room administration Rp.310.476.-. So the total daily costs incurred by hypertensive patients at RSU Imelda Pekerja Indonesia Medan amounts to Rp.552.866.-.

Abstract

Penyakit Hipertensi merupakan penyumbang utama resiko kematian terbesar. Terapi pengobatan pasien hipertensi memerlukan waktu yang lama bahkan sampai dengan seumur hidup. Terapi antihipertensi dapat menggunakan pengobatan tunggal maupun kombinasi agar tercapainya tekanan darah yang normal. Bentuk penelitian ini merupakan deskriptif yang bersifat retrospektif dengan menggunakan total sampel data rekam medik sebanyak 60 pasien dari periode bulan Januari – Desember 2020 bertempat di RSU Imelda Pekerja Indonesia Medan. Analisis data yang diperoleh dikalikan dengan 100% sehingga didapat suatu nilai dalam bentuk persentase. Hasil penelitian karakteristik pasien berdasarkan kelompok usia terbesar adalah 51-60 tahun sebanyak 26 pasien (43,3%) sedangkan kelompok usia terkecil adalah 81-90 tahun sebanyak 1 pasien (1,7%). Terhadap karakteristik pasien berdasarkan jenis kelamin menunjukkan kelompok pasien terbesar adalah perempuan sebanyak 36 pasien (60%) sedangkan laki-laki sebanyak 24 pasien (40%). Karakteristik berdasarkan tekanan darah menunjukkan persentase terbesar pada tekanan darah derajat 2 sebanyak 30 pasien (50%). Terapi antihipertensi golongan obat kombinasi CCB + ARB yang diberikan adalah Amlodipin + Candersartan dan Amlodipin + Valsartan. Dimana jumlah pasien hipertensi yang paling banyak diberikan obat hipertensi kombinasi adalah Amlodipin + Candersartan yaitu 77% dibandingkan Amlodipin + Valsartan yaitu 23%. Persentase kombinasi amlodipin 10 mg dan candersartan 8 mg merupakan terapi pengobatan antihipertensi terbanyak yang diberikan kepada pasien yaitu 54% dan Persentase kombinasi amlodipin 10 mg dan Vandersartan 8 mg merupakan terapi pengobatan antihipertensi terbanyak yang diberikan kepada pasien yaitu 50%. Kerasionalan penggunaan obat hasil evaluasi terhadap JNC8 sudah sesuai meliputi kriteria tepat indikasi, tepat pasien dan tepat dosis dimana masing-masing kriteria memiliki nilai 100%.

Abstract

Comorbid diseases can be found in HIV/AIDS pateients. Comorbid are comorbid diseases that are not related to the diagnosis of the main disease or the patients condition. The purpose of this study was todetermine the pattern of therapy comorbid diseases (comorbid) in HIV/AIDS patients in the inpatient room at Rumah Sakit Umum Imelda Pekerja Indonesia Medan. The sample of this study amounted to 34 people. This type of research is a descriptive observation study where the date are taken retrospectively and analyzed by descriptive statistical methods. The result showed that majority of patients in Rumah Sakit Umum Imelda Pekerja Indonesia Medan is men as many 27 people (79,4%). The most common comorbid diseases foun in HIV/AIDS patients is hyponatremiaas many 20 people (25%), hypokalemia as many 12 people (15%), and dyspepsia as many as 9 people (11,25%). The most common comorbid treatment patterns given to HIV/AIDS patients are antibiotic abd analgesic. Where the antibiotic is ceftriaxone (52.9%) and the analgesic is ketorolac (61.8%), In addition, there is the provision of diarrhea medicine using Diaform (55.9%), Stomach medicine using Antacids (52.9%), suplements using Channa (44,1%) and sedatives using Alprazolam (52.9%). In addition to treatment therapy, HIV/AIDS patients also recived blood transfusion as many as 5 people.

Abstract

Syrup nutraceutical product has been made from the fresh extract of Tiger Nails fruit (Citrus Medica L). This research is a laboratory experimental. Fresh extracts were divided into FI (10%), FII (20%) and FIII (40%). The formulations were compounds of extracts, sugars and waters. Organoleptic’s test results were color is yellow, orange’s aroma, sweet taste and thick textures. Hedonic’s test showed the color of Formulations were not like by panelists (60%) and aroma test of FII showed panelists like it (60%). The texture test of formulations showed the panelists like it (70%) and the taste test of FI showed the panelists were very like it (70%). The pH values were ranged from 4-5. The fastest dissolving time test showed from FI (8 seconds). The highest viscosity test was showed by FII (6,08 poise). The determination of vitamin C levels using UV-Vis Spectrophotometry showed FIII the highest levels of vitamin C (0.828%). But all antioxidants activity had low activity. FIII has the highest IC50 value (454172,74 ppm). Anova test value showed 0,1505>0.05 that means the antioxidant activity were not significantly different. Tukey test showed the comparison of antioxidant properties between the formulations groups showed a significance value of 0,135 > 0.05.
