Abstract

Menurut data WHO tahun 2000, Indonesia menempati urutan ke-4 terbesar dalam jumlah pasien diabetes melitus di dunia. Data angka kematian dan resiko komplikasi diabetes melitus menunjukkan pentingnya pemberian terapi dan pencapaian keberhasilan dari terapi diabetes melitus. Dalam hal ini peran farmasis sangat dibutuhkan untuk menjamin bahwa pengobatan yang diterima pasien adalah pengobatan yang rasional, dengan cara mengidentifikasi dan mencegah terjadinya masalah-masalah yang berkaitan dengan terapi obat (Drug Therapy Problems). Tujuan penelitian ini adalah mengidentifikasi dan mengelompokkan kejadian Drug Therapy Problems (DTPs) dalam rangka meningkatkan therapeutic outcome pasien. Data resep pasien dan rekam medik dikumpulkan pada periode September-November 2009 secara retrospektif. Dari penelitian terhadap 63 pasien diabetes melitus tipe 2 rawat jalan, terdapat 41 pasien (65,08%) mengalami DTPs, dengan rincian 2 pasien (4,88%) membutuhkan terapi obat tambahan, 6 pasien (16,63%) menerima terapi obat yang tidak diperlukan, 5 pasien (12,20%) menerima dosis terlalu rendah, 16 pasien (39,02%) mengalami reaksi obat merugikan, 1 pasien (2,44%) menerima dosis terlalu tinggi, dan 11 pasien (26,83%) dengan interaksi obat. Reaksi obat merugikan merupakan DTPs yang paling sering muncul pada kedua kelompok pasien; 38,71% kejadian pada pasien dewasa dan 40% pada pasien geriatri. Analisis DTPs ini menunjukkan bahwa peran farmasis penting dalam mencapai keberhasilan terapi dan mencegah terjadinya reaksi obat merugikan.Kata kunci: drug therapy problems, diabetes melitusAccording to the WHO in 2000, Indonesia is in fourth position based on numbers of peoples suffer from diabetes mellitus. Numbers of death and complication risk shows how important therapy and progressing achievement from the diabetes therapy. Therefore, pharmacist is definitely needed to guarantee that the medication on the patient is rational, by identifying and avoiding DTPs. The aim of this research was to identify and classify DTPs incidences in order to increase patient's therapeutic outcome. This retrospective study was conducted by collection of the patient's prescription and medical record within September-November 2009 period. The result to 63 patients of diabetes mellitus type 2, showed that 41 patients (65.08%) had DTPs, 2 patients (4.88%) need additional drug therapy, 6 patients (16.63%) showed unnecessary drug therapy, 5 patients (12.20%) received too low dose, 16 patients (39.02%) showed adverse drug reactions, 1 patient (2.44%) received too high dose, and 11 patients (26.83%) with drug interactions. Adverse drug reaction is the most occasional DTPs event in both group of patients;38.71% in adults and 40% in geriatric patients.Keywords: drug therapy problems, diabetes mellitus

Abstract

Diabetes mellitus type 2 patients in RW 02 and RW 03 Sukajadi Bandung, increased 50% during the last 5 years. Many factors lead to an increase in the number of DM patients including lifestyle (uncontrolled food and low activity), genetic and community knowledge about low DM disease. So the purpose of community service is to know the influence of the application of healthy foods in lowering blood glucose levels and the application of foot exercises in reducing peripheral neuropathy The method of implementation begins with an interview to determine the general description of the subject, followed by blood glucose examination on days 0, 3, 7 and 14 after being given 2x daily healthy meals for 7 days, leg and educational gymnastics. Results of field observations, based on the long suffering DM most of 1-10 years even there is 1 person who just knew that she was suffering DM. Decrease in blood glucose level at 50%, after subjects are given healthy food with calories between 1500-1800 kcal .. Of 80% of patients with DM who experience peripheral neuropathy, decreased morbidity after doing leg exercises. Conclusion: healthy food prescriptions with regard to calorie intake can lower blood glucose while, regular exercise for leg exercises can reduce perifer neuropathy symptoms.

Abstract

Abstrak Misoprostol sebagai off-label sering digunakan dalam tindakan persalinan baik untuk induksi persalinan maupun untuk pencegahan perdarahan pasca persalinan. Perdarahan pasca persalinan merupakan peyebab terbanyak pada kasus kematian akibat persalinan. Penelitian ini bertujuan untuk mengidentifikasi ketepatan penggunaan obat misoprostol sebagai off-label berdasarkan kriteria penggunaan medis dan ketepatan dosis yang diberikan kepada pasien. Penelitian ini merupakan penelitian observasional data rekam medis pasien yang dilakukan secara retrospektif di Klinik Utama Al-Islam Bandung. Penelitian dilakukan terhadap 87 data rekam medik pasien selama periode bulan Januari-Desember 2016. Hasil menunjukkan bahwa 100% penggunaan misoprostol sebagai off-label sesuai dengan kriteria penggunaan medis dan tepat dosis. Terdapat 64% dari total pasien yang diberikan misoprostol dengan total dosis sebesar 800 μg. Kata kunci: Misoprostol, Off-Label, Persalinan  Abstract As off-label misoprostol is the most used in labor either for labor induction or for prevention of postpartum hemorrhage. Postnatal bleeding is the most common cause of death due to childbirth. The purpose of this research is to identify the accuracy of misoprostol used as off-label based on the criteria of medication and the dose accuration for the patient. An observational retrospective research on medical record data of patients at Al-Islam Main Clinic Bandung. During 2016 had 87 patient's data from January to December was obtained. The results showed that 100% of misoprostol is used as off-label following the criteria of medication and dose accuration. Therefore 64% of total patients had given misoprostol for 800 μg each. Keywords: Labor, Misoprostol, Off-Label
