Abstract

There are a lot of companies want to transform theirselves to be a “World Class Manufacturer”. Therefore, the World Class Maintenance (WCM) concept which refers to the best practice for maintenance system is widely adopted by several companies. WCM is a structured dan systematic concept of maintenance system. It has 5 main pillars which are EARM (Equipment Availability, Reliability, and Availability), planning and scheduling, maintenance management, material management, and contracted services. Those five pillars must be well applied in the company in order to achieve a high WCM score when being audited by auditor. PT X is one of the company who has applied WCM concept in its  maintenance system. The company has been assessed on WCM periodically and it has the pre-determined target. The standard assessment which has been determined is used to evaluate the WCM concept in PT X. Such improvements related to WCM has been done in order to be well applied in maintenance system and achieve a high score according to the target. WCM score from audit simulation is 4.17. It shows that the improvements which has been done give an impact on WCM score enhancement.

Abstract

PT. X is a company which produces a variety of glassware product. PT. X wants to develop Hazard Identification Risk Assessment and Risk Control (HIRARC) in order to fulfill the requirement of OHSAS 18001:2007 application. The area covers in raw materials warehouse, production, and finished good warehouse.The preparation of HIRARC is divided into 3 stages which are hazard identification, risk assessment, and risk control. Hazard identification is performed based on interviews with the workers, the observations in the factory, and the historical data of accidents at PT. X. The result of the risk assessment shows 34% of activities is categorized as low risk, 58% of activities is categorized as moderate risk , 8% of activities is categorized as high risk and there is no activity in the category of extreme risk. Risk control will be performed on activities with moderate risk and high risk category. Based on the result, there are 4 potential hazards with high risk category, which are the sharpness of the material in the production area, shortness of breath due to workers are not wearing APD, shortness of breath due to the pungent smell from additives during the casting process, and the hook of the basket is fragile in the mixing process. Risk Control is undertaken by giving APD, safety sign, punishment, equipment maintenance, and make a special area for walking.

Abstract

PT. X is one of the leading furniture and also only exporting its products. One of the desire of the company is “To be one of the market leaders of furniture industry”, while the vision of PT. X is ” Producing high quality with reliable design”.This factory is currently running the system of health and safety work  in the factory area. However, the improvement is still need to be done by minimizing potential risks with HIRARC(Hazard Indentification, Risk Assessment and Risk Control). The result shows  47% of activities are considered as low risk, 36% of activities are considered as moderate risk, and 17% of activities are considred as high risk. There are 4 activities categorised as high risk that must be considered which are sawdust that is inhaled by an operator, member of the body missing, member of the body and exposed side off wood that isn’t neat.

Abstract

PT. Charoen Pokphand Indonesia is a fodder company which is located in Surabaya – Mojokerto Street 26 Km, Krian City. This company consists of 4 areas which are feed mill area, raw material area, silo area, and finished good area. The company was in the preparatory stage SMK3 certification .Companies now want to evaluate the system of K3 with HIRARC that is one of the documents supporting SMK3 certification certification. If the company applies HIRARC, the dangerous activities in the company can be detected and the solution can be made immediately. As a result, the number of the accident in the workplace can be minimized. HIRARC method consists of 3 things which are hazard identification, risk assessment, and risk control. The data which is processed with HIRARC is got from the interview, observation, and company’s data. The result of health and safety evaluation with HIRARC method through PT Charoen Pokphand Indonesia is that there are still many dangerous activities; for example, an activity which has extremely high, high, and normal percentage of risk rating. The percentages of the dangerous activities in silo area are 8,82% in extremely high rate, 14,71% in high rate, and 47,06% in normal rate. On the other hand, the percentage of the dangerous activities in raw material warehouse are 14% in extremely high rate, 35% in high rate and 39% in normal rate. Those dangerous activities must be solved immediately by making risk control. The risk control which is suggested are giving sanctions, making checklist, maintaining the tools, replacing the tools if they are broken, changing the method, and giving training. The potential percentage of the dangerous activities which is expected to decrease in silo area is disappearing the extremely high risk, decreasing the percentage of the high and normal risk to 8,82%, and 23,53%. On the other hand, the potential percentage of dangerous activities which is expected to decrease in raw material warehouse is disappearing the extremely high risk, decreasing the percentage of high and normal risk to15,79% and 21,05%.

Abstract

Abstract: PT Injaplast is a manufacturing company that produces a plastic sacks (plastic woven bags,) which is located in Jl. Margomulyo No. 35, Surabaya. PT Injaplast currently doesn’t have a Occupational Safety and Health, so that the work accident still quite high. It can be seen from the company’s work accident data, which in 2013 and 2014 there are 12 minor accidents and 3 major accidents that occurred. The high rate of work accident can be minimized through the preparation of hazard identification, risk assessment, and risk control document (HIRARC). HIRARC can support the successful implemetation of K3. The purpose of HIRARC is to detect hazard exist, risk assessment, and making control to minimize the potential risk. The results of HIRARC document preparation shows that there are still many activities that endagers worker. There are 43% activities are classified as moderate risk and 15% as high risk. That risk level must be solved by making a risk control. The suggestion on risk control are giving a personal protective equipment, give safety sign (administrative control), add equipment (engineering control), and replace a personal protective equipment and tools (substitution). The estimation of decreased risk rating possibility that expected later will be created based on risk controls that have been given. The estimation of decreased risk rating possibility when the controlling risk executed is the risk with moderate category which originally was 43% decrease to 11% while the high category are initially 15% decrease to 3%.

Abstract

 PT. XXX has applied Health Safety Enviroment system, but still experienced relatively high level of accidents. The accident number was up to 11 out of a total worker of 390 in 2014. PT XXX would like to decrease to zero accident so as the production process runs smoothly.The method used in this problem was Behavior Based Safety. BBS method provides training for the worker to behave safely. This method gave good results in that all processes had an average of 80% safety behavior. BBS changed many dangerous behavior to become safety behavior, but there is still a dangerous behavior that cannot be changed.

Abstract
