Abstract

Abstract

Anterior crossbite is the most common problem in Angle class III malocclusion and is aesthetically disturbing. As individuals get older, an anterior crossbite can worsen, impacting the esthetics of the face and the stomatognathic system’s function significantly and requiring immediate treatment. Due to genetics, habitual postures, ethnicity, and environmental variables, the class III malocclusion’s etiology is multifaceted. In terms of treatment, there are three options: growth modification, camouflage, and orthognathic surgical treatment. This case report aims to describe a non-surgical treatment with simple treatment mechanics in two patients who were still in their growth period. Two patients, a sibling aged 14 and 13 years, experienced an anterior crossbite with a deep overbite, diagnosed with Angle class III with skeletal class III relationships. Besides, the profile revealed a concave, with an overjet of -2mm and an overbite of 4mm. The treatments were then performed employing fixed orthodontic appliances with vertical U-loops for protraction of upper anterior teeth and class III intermaxillary elastics to correct molar relationships. The treatment was completed in two years, in which the anterior crossbite was corrected. The molar and canine relationships, which were originally class III, became class I. Loss of tooth 36 was closed, and tooth 38 has fully erupted. Treatment of anterior crossbite needed to be done as soon as possible to prevent more severe abnormalities. In conclusion, a treatment in the growth period with simple techniques resulted in significant improvements in function and aesthetics.

Abstract

Abstract

Introduction: Orthodontic treatment is one of treatment in the field of dentistry by moving the teeth to correct malocclusion and malposition of the teeth. After treatment is complete, there is still the possibility of relapse, which is the process of moving the teeth to their original position before orthodontic treatment. It is still a complex problem after the end of orthodontic treatment. Objective: This literature review aims to examine the process and mechanism of relapse so that it can be additional information in efforts to prevent relapse. Discussion: Relapse can occur due to various factors and relapse occurs rapidly on day 1 to day 24 after removal of the orthodontic appliance. Orthodontic tooth movement and relapse movement will show an increase in the number of osteoclasts in areas of stress and a decrease in the number of osteoclasts in areas of tension. This suggests that alveolar bone remodeling is an important element in the process of relapse. Conclusion: Efforts to deal with relapse need to be carried out with various approaches, including pharmacologically which can affect the alveolar bone remodeling process, especially in osteoclast cells which play a role in the process of tooth relapse movement. Keywords: orthodontic treatment, tooth movement, relapse

Abstract

Background: Orthodontic treatment is performed to correct the malocclusion, and after treatment is complete. The position of the teeth must be maintained because the teeth can move back to their original position, commonly known as relapse. Various attempts have been made, such as using retainers, and various studies have used pharmacological agents such as bisphosphonate risedronate to inhibit relapse, but these studies still used the injection method, which is inconvenient to apply and has a systemic effect. For this reason, changing the bisphosphonate preparation to an emulgel will facilitate its application and have a local effect, but it is necessary to test and determine its effectiveness in inhibiting the relapse. Method: An experimental laboratory study was conducted on 18 guinea pigs, which were divided into three groups: I (control), II (emulgel without risedronate), and III (emulgel with risedronate). The teeth were moved with open coil springs on both lower incisors for seven days, stabilized for seven days, and emulgel applied in groups II and III. The coil spring was removed, and the distance between the two incisors was measured on the third and seventh days. Result: Using One-Way ANOVA and Post Hoc LSD, the data analysis results showed significant differences between the control and the risedronate emulgel groups on the third and seventh days. The largest relapse occurred in the control group, while the smallest was in the emulgel group with risedronate. Conclusion: These results indicate that risedronate bisphosphonate in the form of emulgel influences inhibiting the movement of tooth relapse
