Abstract

Primasatya Husada Citra (PHC) Hospital Surabaya is one of the private hospitals that is required to providequality services one of which is a medical record. There are several points about medical records services thathave not been implemented optimally such as 43% incomplete filling of medical record files includingincomplete informed consent, and late return of medical record document by exceeded the standard 1x24hours. The purpose of this research is to determine the policy strategy in the medical record unit of the PHCHospital Surabaya using the SWOT method. The research method was the SWOT method that identifiedstrengths, weaknesses, opportunities, and threats. The research instruments used questionnaires andobservations. The subject of this research was 6 medical records officers. The results show that the value ofthe strength factor is 3.69 and the weakness factor is 1.42 while the opportunity factor value is 3.6 and thethreats factor is 1. The position of the medical record unit, PHC Hospital Surabaya, is in quadrant I so thesuitable strategy is SO (Strength-Opportunity), the strategy includes the development of integrated informationsystems, and improve the management of medical records related to recording and documentation formaintaining the quality of medical record documents.

Abstract

SIMRS at RSIA Srikandi IBI Jember has been running since 2011. However, based on the preliminary studyfound several problems including, there are some data that is not stored properly, information generated bySIMRS is still incomplete or incomplete, and the discovery of several system users who are not satisfied withthe SIMRS. Therefore, it is necessary to further study how to evaluate the success of the implementation ofthe SIMRS by using the Update D&M IS Success Model evaluation method (Delone and Mclean 2003). Thepurpose of this study is to evaluate the performance of SIMRS in supporting service activities based onvariables of system quality, information quality, service quality, intensity of use, usage, user satisfaction andnet benefits. The analysis used is the analysis of the relationship with the Spearman rank test using the SPSS24.0 program. The results of this study indicate the significance value of all variables <0.05 and positive value,so that the quality of the system, the quality of information, the quality of service is related to the direction ofthe intensity of use and user satisfaction. Usage intensity and user satisfaction are also related to the directionof net benefits. A very strong influence occurs on service quality on the intensity of use with a significancevalue of 0,000 and a coefficient correlation value of 0.826. The influence that is going on on the quality ofinformation on user satisfaction with a significance value of 0,000 and a coefficient correlation value of 0.425.

Abstract

18 of 31 sub-districts in Jember Regency are areas prone to floods, landslides, tornadoes and tsunamis.The number of disasters resulted in the difficulty of officers to record the number of victims that exist and thedelivery of information related to disaster data, needs in the area of the disaster and so forth is difficult toconvey. This research was aimed to design a prototype application for recording disaster victims to help thelocal government of Jember specifically for supporting disaster mitigation. The method applied in this studywith 2 stages, namely the first stage to analyze the problems and system design needs, researchers relatedto the collection of necessary data such as retrieving data on disaster victims in Jember Regency andlegislation related to application users and the second stage doing system design such as making flow andmodeling of disaster victim registration applications that provide solutions to problems in the scope of usingavailable resources. Data collection techniques are interviews, observation and documentation withqualitative data analysis methods. The results of this study are prototype design of web-based disasterregistration applications that are designed according to user needs using Microsoft Visio applications. Theresults of the design of the prototype design are expected to be a reference for web-based disasterregistration application specifications in the Jember Regency government environment.

Abstract

Patrang Health Center has implemented medical records management, but there are problems in the implementation of medical record management. when duplicating medical record numbers at Patrang Health Center. Duplication in the Patrang Community Health Center results in a decrease in the quality of medical records. The purpose of this study was to analyze the causal factors and determine the priority causes of duplicate medical record numbers at Patrang Health Center. This type of research is qualitative. The technique of collecting data in this study is observation of data duplication of medical records and interviews with respondents based on 7M indicators. Data processing with CARL criteria. The results of this study were found in studies that found the main priority of less useful duplication of the medical record number on the medical record data map with a score of 144. Further problems were officers, filing space was less extensive, filing shelves were inadequate and patients did not carry KIB with a score 108. With the discovery of priorities leading to duplication, the study provides the main solution that discusses the medical record number written on the front and sides of the file as well as other solutions to overcome the problem and made a draft policy to reduce the incidence of duplicate medical record numbers that have been approved.

Abstract

The neonatal preliminary assessment form is one of the important forms, because it includes informationabout newborns. Baladhika Husada Hospital in Jember is one of the hospitals which still does not have apreliminary assessment form specifically for newborns or neonates, there still uses initial assessment formsfor general patients. The purpose of this study was to design a neonatal initial assessment form at BaladhikaHusada Hospital Level III Jember. This type of research is a qualitative study with research subjects includingSp.A doctors, midwives, nurses, chief medical records, and medical records officers. Data collectiontechniques in this study used interviews, observation, brainstorming. The results of this study were seen fromthe physical aspects of the neonatal initial assessment form using white paper with a size of 70 grams F4 (33cm and 21.5 cm) in portrait form. Viewed from the anatomical aspects of the neonatal heading assessmentform, the logo, the name of the agency, the address of the agency, and the title of the form, for theintroduction, are represented by the title. Viewed from the aspect of the contents of the neonatal preliminaryassessment form, the data added consisted of obstetric status, anthropometric vital signs, apgar scoreassessment, and physical examination starting from B1 - B7. Suggestions from researchers on the results ofthe neonatal initial assessment form design are expected to be implemented at Baladhika Husada HospitalLevel III Jember.

Abstract

Based on the results of a preliminary study conducted at the Surabaya PHC Hospital in 2020 in the filing section of the Surabaya PHC Hospital there was still a missfile incident in the inpatient medical record file. Missfile is an incident where the medical record file is not found because of an error in placing the medical record file on the filing rack, this can cause a decrease in the quality of services provided. The importance of the role of medical record documents in creating information that is sustainable, researchers interested in examining the problem of the occurrence of errors (missfile). This study aims to analyze the analysis of the factors that cause the medical file file missfile in Surabaya PHC Hospital using the PTA (Problem Tree Analysis) method. This type of research is qualitative with observation and interview data collection techniques. The results of this study are the main factors causing missfile is the behavior of officers, level 1 contributing factors are There is no responsible person in the inpatient medical record file storage, less maximum use of facilities and infrastructure and SOP (Standard Operating Procedure) storage of inpatient medical record files never been socialized. Factors causing level 2 are the need for officers to be responsible for storing inpatient medical record files, lack of knowledge about tracers and the existence of maps in inpatient medical record files, lack of application of SOP (Standard Operating Procedure) does not match the reality because it is only limited accreditation fulfillment requirements. So the implementation of inpatient medical record file storage in Surabaya PHC Hospital is still not good, so that there is still a missfile. The hospital should increase the number of workers in the medical records unit in the filing section, conduct socialization on the importance of using tracers, accelerate the procurement of inpatient medical record file folders and apply SOPs as guidelines for officers in carrying out work. So that the implementation of inpatient medical record file storage for the better.

Abstract

Regional general hospital of sleman (RSUD) is a hospital located in Sleman Regency with type B education. As a teaching education, Sleman District Hospital often borrows medical records for research (Education) related to the release of medical record information with third parties necessary. Because the character of medical record documents and their contents are confidential, the hospital must guarantee the confidentiality of the contents of the medical record file and legal protection for patients and hospitals. This research was aimed to find out how the release of medical record information based on legal aspects of the security and confidentiality of medical records for education in Sleman Hospital. This type of research is qualitative by collecting data using interviews, observation and documentation. The results of research at the Sleman Hospital in the application of releasing medical record information for educational purposes in the Sleman Hospital are still not by existing regulations related to the unavailability of expedition books or medical record file borrowing books where the book is very important in knowing the existence of medical record files that come out. And a special place for researchers when researching medical record files is not yet available due to limited space in the medical record unit of Sleman District Hospital.

Abstract

Codification is an activity to transform a diagnosis into a code consisting of letters and numbers.Inaccuracies of coding can slow the process of insurance claims and decrease the quality of medicalrecords. Preliminary study results showed a percentage of inaccuracy in the diagnostic code of 4documents of a total of 10 outpatient medical record documents at PHC Surabaya Hospital. This researchaims to evaluate the correctness of an outpatient medical record file in Surabaya's PHC Hospital. This typeof research used qualitative research. The samples were 44 documents of the outpatient medical record inJanuary 2020. Data collection techniques using observation and documentation. The results of the studyshow that the code inaccuracies in an outpatient medical record document of 13 (29.5%) improper codeand 21 (70.5%) of the right code. The cause of its absence was an error in the code-granting suspect of15.4%, a mistake in giving control code and therapy of 61.5%, and an error in giving the fourth character inthe case of diabetes mellitus by 23.1%. Factors that lead to inaccuracy in the coding of an outpatientmedical record were the less thorough officers in conducting medical record reviews as well as mistakes indetermining the old case and new cases. The suggestions provided by researchers are to conduct routinecoding audits, repair SOP, socialization about the correct encoding procedures, and make a note for thecode which is often to be used.

Abstract

Bhayangkara Hospital Lumajang is one of SNARS accredited health agencies and SISMADAK is used in the reporting process to KARS. The problems is still found in its application, which resulted in staff being less satisfied with SISMADAK. The aims of this study was to evaluate SISMADAK in terms of user satisfaction using the PIECES method. The study was quantitative descriptive study, which is described as a descriptive evaluation by comparing user satisfaction with information systems. The result of description evaluate of SISMADAK using PIECES method in Bhayangkara Hospital Lumajang about satisfaction percentage found 81% of performance variable was showed very good, 81% of information variable was stated very good, 69% of economic variable was good, 76% control variable was stated very good, 70% of efficiency variable was stated very good, and 80% of service variable was responded very good. For further advices were the planned of SISMADAK users need related information that is not yet available, beside the staff should increase the confidentiality of data on SISMADAK. The conclusion of this study is SISMADAK able to facilitate users in the hospital accreditation process of KARS.

Abstract

Lung carcinoma or commonly known as lung cancer is one of the main causes of death from disease in theworld that reach 7.6 million deaths or about 13% of all cancer diagnoses. Baladhika Husada Jember Hospitalis a third-level hospital in Jember Regency that has oncology and chemotherapy services including a cancerdiagnosis. Based on existing data of the morbidity and mortality rates of pulmonary carcinoma are alwaysincreasing from 2016 until 2018. This study aims to analyze the risk factors for lung carcinoma such as age,gender, smoking history, genetic history, and predisposition of other lung disease based on medical records ofinpatients with the pulmonary disorder at Baladhika Husada Hospital in Jember. The research method usedquantitative analysis with a cross-sectional research design. The number of samples was 98 respondents.Data analysis used the chi-square test for bivariate and the logistic regression test for multivariate. Thebivariate analysis result shows that each independent variable had an influence on lung carcinoma cases.Multivariate analysis shows that the variable had a simultaneous influence (p value=0,000). Partially, eachvariable has an influence, age (p value=0,003), smoking history (p value=0,003), genetic history (pvalue=0,002), predisposition of other lung disease (p value=0,000), except on genders (p value=0,857). Theconclusion is the risk factors can explain the effect on the incidence of pulmonary carcinoma by 84.8%. Thisresearch is expected to be used as information to cope with the increased morbidity and mortality rates ofpulmonary carcinoma.
