Abstract

A retrospective study on tuberculous meningitis had been carried out on 43 patients at the Department of Child Health, Dr. Pirngadi Hospital Medan during the year 1979. The peak incidence (55.81%) was found in the age group of less than 3 years of which 55.81% was in the second and 44,18% in the third stadium of the disease. Two of the cases were 4 months of age and 1 case was 14 years. None of the cases had got BCG immunization. Mortality rate was 37.21% of which 25% was in the second and 52.63% in the third stadium. Cerebrospinal fluid examination showed that 46.50% had cell counts between 101-500 cells/mm3. The symptoms were prolonged fever (62. 79%), convulsions (72.09%) and unconciousness (27.90%). Of these cases 83.72% had contact with tuberculous patients; 6.9% had positive tuberculin test and 48,83% showed pulmonary involvement on radiological examination. Nutritional status of these cases showed that 48.83% were undernourished and 34.82% were in poor nutritional status.

Abstract

A 4 years retrospective study was made on malnourished children attending the OPD of the Sub Department of Child Nutrition, Dr. Pirngadi Hospital during the period of 1976 - 1979. In this study 635 out of 720 malnourished children were under 3 years old. Gain in body weight was only seen in 23,7% of the children. Most of them (49, 7%} came only once. More effort such as child food demonstration, health and nutrition education is needed to motivate the parents to bring their malnourished children for consultation.

Abstract

The incidence and mortality of gastroenteritis associated with measles in infants and children had been assessed retrospectively in 1497 babies and children admitted to the Child Health Department/Dr. Pirngadi General Hospital Medan in the period of January to June 1982. Of these 1497 children, 698 suffered from gastroenteritis and 94 from measles. Of these 94 measles cases, 49 had diarrhoea and 13 had PEM (26.53%). The overall mortality of gastroenteritis was 14.61%, while the mortality of gastroenteritis associated with measles was 28.5 7%. The case fatality rate of gastroenteritis with measles and PEM was 35.5%. The highest age specific case fatality rate of gastroenteritis was in the age group of 0- 6 months. The feasibl1ity of active vaccination against measles at the early age of 6 months should be seriously considered.

Abstract

Since 1969-1979 trials with amoebicides had been done 9 times in OPD of the Child Health Department, Dr. Pirngadi Hospital, Medan. The children were treated ambulatorily. The tablets were administered under close supervision at the subdivision of Gastroenterology for 3 consecutive days. Stools examination had been done, with direct faecal film only, every day for the first 4 days and once a week for 3 weeks. Evaluation was done by assessing the parasitological and clinical cure rate. A clinical trial with metronidazole had been conducted in 3 groups of children. Each of the three groups consisted of 50 children, and were treated ambulatorily with a single daily dose for three days.The first group treated with a dose of 50 mg/kg bw/day had in a cure rate of 89% and 100%, the second group with a dose of 25 mg/kg bw/day 87% and 85%. The third group also with a dose of 25 mg/kg bw/day showed cure rate of 93% and 91%. These differences of results in group I, II and III proved to be not statistically significam. There were no side effects observed.Tinidazole and omidazole were indeed very effective for treating intestinal amoebiasis, the cure rate was 100%, and no side effects observed.

Abstract

A retrospective study of epilepsy cases had been done from January 1978 to December 1982. The aims of this study was to know the incidence, type and result of treatment of epilepsy in the Department of Child Health Dr. Pirngadi Hospital, Medan, Indonesia. 103 cases of epilepsy were investigated. This was 0,10% of total patients in OPD as well as inpatients during this period, consisting of 56,32% boys and 43,68% girls. According to the classification of the Intemational League Against Epilepsy (ILAE) all of the patients had generalized epilepsy, consisting of 68,93% primary generalized epilepsy and 31.07% secondary generalized epilepsy. Thus the incidence of epilepsy was low, with a preponderance of primary generalized epilepsy. Of these patients only 18.44% came regularly for control. They were treated with a single drug namely phenobarbital, with good results.

Abstract

A study of children’s immunization status and family size of doctor’s families in Dr. Pirngadi Hospital, Medan, lndonesia had been conducted on December 1982. The study was done by distributing questionaires to 105 doctors. From this study we found 247 children consisting of 19 children under 1 year of age and 228 over 1 year. Basic immunization had been done in 177 children (71.66%). Most of them had been vaccinated against Tuberculosis (95.95%). but only 2 children had been vaccinated against Measles (0.81%). The doctors who had 2 children were 30.47% and mean of total wanted children was 3.30. Awareness of immunization besides BCG and family planning should be increased among the doctors in Dr. Pinrgadi Hospital.

Abstract

A cross sectional study about the relationships between family size and marital age and the impact of educatimt, occupation and family planning programme was conducted in the region of South Tapanuli North Sumatera. The study was performed on 246 married couples by simple random sampling of households in the villages Pakantea, Tamiang, Muarasoro and Sumuran, during the period of September 25 up to October 3, 1982. The eariiest age of marriage for women was 14 years, ancl the latest 20 years. Most of the women (68%) married at 15-20 year. The main education of responders were primary school (67%). The occupational status was mostly (90%) farmer. The mean family size under 20 years old was 6.3 and over 20 years, 5.3. Fertility rate under 20 years was still high. Family Planning was not yet widely accepted in this area.

Abstract

A study on views of family planning among freshmen of University of North Sumatera, Medan was conducted for the third time in 1985. Questionnaires was distributed to 2245 freshmen of whom 1997 gave the response consisting of 1153 males and 844 females. The result showed that most of the freshmen (67,59%) planned to have only 2-3 children and get married at the ages of 25- 29 ye1rs· 31,30% wished to have 2 sons and 1 daughter,· 79,75% had the opinion that son or daughter is the same, nearly all of them (97,75%) were planning to have a small family, whereas 34,95% planned to choose IUD for their family-planning method. We can conclude that family planning is well accepted by the present generation.

Abstract

One hundred seventy-six cases with measles have been hospitalized at the Department of Child Health, Dr. Pirngadi General Hospital, Medan in 1973 - 1977. The case fatality rate was 26.1%. The most frequent complications found were Pneumonia, Encephalitis, Diarrhea and Dehydration. Age specific mortality, as expected, was highest in the 0 - 1 year age group (38.6%). A high proportion of the survivors (22.2%) showed recidual signs of brain damage and blind-ness. This study proved that active immunization against measles in Indonesia must have priority.

Abstract

Two surveys were conducted on children attending the Child Outpatient Department in the Dr. Pirngadi General Hospital Medan, to know about the spectrum and prevalence of the diseases of the teeth, and also the comparison of carious teeth formation between breastfed with bottlefed infant and children. The first survey was done in 1971 - 1973 on 18720 children, where 3000 (1 6.03%) of them had various dental diseases and abnormalities. We found that dental decay is prevailing at the age group of 3-8 years. The second survey was done in December 1977 on 100 children under five year of age, of whom 54 had caries while 46 of them had caries resembling bottlefeeding caries. No dental caries were found on children who got breastfeeding until the age of 41 months; bottlefeeding in infancy influences carious teeth formation.
