Case: A female patient, 70 years old was referred with a diagnosis of bilateral pneumonia by a pulmonologist. She complained of painful wounds all over her mouth. An intraoral clinical examination revealed white-thick lesion and multiple ulcerations, whilst an extraoral exam for exfoliative dermatitis has not been described. The working diagnosis was Covid tongue mixed with exfoliative cheilitis. Case management: The procedure was performed by teledentistry and direct visits to implement oral health care by asepsis, debridement and the application of Oxygene gel. Conclusion: The lesion was treated successfully due to the multidisciplinary approaches by an internist and pulmonologist by implementing integrated knowledge and was supported hugely by patient cooperation.

Abstract

Recurrent aphthous stomatitis (RAS)is a common oral mucosal disease in human. This disease affect 20-25% ofpopulation. The etiology remains unclear, but some of cytokine such as TNF-α is suspected to contribute in thepathogenesis of RAS. TNF-α is commonly measured by PBMC and lesional biopsy. These are invasive method andhigh skilled requirement. Another study used lesional swab to measure this cytokine without correlating the resultwith TNF-α measured by PBMC level. This study aimed to asses TNF-α level correlation between lesional swab andPBM of RAS patients. The study involved 10 subjects by inclusion criteria. Subjects have lesion swab and PBMCexamined. Samples were analyzed by ELISA method. There is a correlation between lesional swab and PBMC ofTNF-α level RAS patients but this correlation did not statistically significant.

Abstract

There are 80 types of known herpes virus, 8 of them can cause infection on humans. They are herpes simplex virus(HSV) 1 and 2, varicella zoster virus (VZV), cytomegalovirus, Epstein-Barr virus, human herpes virus (HHV6) Aand B, and paramyxovirus. HSV1, HSV2, and VZV are the virus known to cause oral mucosal diseases. This paperaims to review and discuss orofacial pain caused by herpes virus infection. Detail anamnesis about prodromal signand symptom with clinical features that vesicles, labial and intraoral lesions, and unilateral distribution of lesionsare characterized oral herpes virus infections. It can be concluded that detailed anamnesis and an understandingabout oral clinical sign and symptom may confirm diagnosis of herpes virus infections.

Abstract

Recurrent aphthous stomatitis (RAS) is a common inflammatory oral disease. The etiology remains unclear; howeversome studies and case reports showed any association between estradiol and progesteron level in this disease. The aimof this article is to evaluate estradiol and progesteron level in RAS subjects. Purposive sampling was used in this studywith 15 total subjects. Phase of luteal and ulcer reccurence characteristic are based on quisioner data. Estradiol andprogesterone level is examined by vena serum. Only one sample (6.7%)showed higher estradiol level than normal andthe rest (93.3%) are in normal level. Contrastly, 3 samples (20%)were in normal for progesterone level, while theothers, 12 samples (80%)were in low level. Estradiol level in RAS subjects tend to normal range, while progesteronelevels tend to be low

Unexpectedly, the levels of cobalamin in blood increased by 70% from the referral score. Patients urgently refer for hospitalization to recover her Hb level. After blood transfusion, patients discharged from the hospital but under hematologic observation for the exact etiology of her anemia, including the high cobalamin level. After two months observation, patients Hb gained normal level, however the vitamin B12 level remain. Patients reported decreased in number of ulcers. Conclusion:  It is worthy to always check on cobalamin level for patients with recurrent oral ulcers since lower level can directly causing it or the high level can lead to some life-threatening diseases which can also trigger the recalcitrant oral ulcers.
