Abstract

Pendahuluan: Tumor memerlukan proses pembentukan pembuluh darah baru untuk tumbuh dan bermetastasis yang dikenal sebagai proses angiogenesis. Angiogenesis dapat dinilai secara imunohistokimia dengan Microvessel Density (MVD). Proses angiogenesis dapat dipicu oleh berbagai faktor angiogenik. Beberapa penelitian menemukan ekspresi Cyclooxygenase-2 (COX-2) meningkat pada kanker. COX-2 dapat merangsang pengeluaran faktor-faktor angiogenik. Pada penelitian ini akan dilihat apakah angiogenesis, yang dinilai dengan MVD, pada karsinoma nasofaring memiliki korelasi dengan COX-2. Tujuan: Mengetahui korelasi ekspresi COX-2 dengan MVD pada penderita karsinoma nasofaring di RSUP H. Adam Malik Medan. Metode: Penelitian ini bersifat analitik dengan studi potonglintang. Dua puluh empat sampel diperiksa secara imunohistokimia untuk ekspresi COX-2 dan MVD. Kemudian hasil dianalisa dengan uji Spearman. Hasil: Ekspresi COX-2 positif ditemukan pada 17 (70,83%) subyek dan dijumpai negatif pada 7 (29,17%). Ditemukan 17 (70,83%) jaringan karsinoma nasofaring dengan MVD tinggi dan 7 (29,17%) dengan MVD rendah. Ditemukan adanya korelasi positif sedang antara ekspresi COX-2 dengan MVD (r=0,559; p=0,005). Kesimpulan: Ditemukan ada korelasi positif sedang antara ekspresi COX-2 dan MVD pada karsinoma nasofaring. Kata kunci : Karsinoma nasofaring, Cyclooxygenase-2, Microvessel density ABSTRACTIntroduction: Tumor needs new blood vessel formation to grow and metastasize. New blood vessel formation is known as angiogenesis. Angiogenesis is determined by Microvessel Density (MVD) using immunohistochemical examination. Angiogenesis was affected by various angiogenic factors. Recent studies found elevated COX-2 expression in cancers. COX-2 can stimulate angiogenic factors. This research assessed the correlation between COX-2 and angiogenesis, determined by MVD. Purpose: To find out the correlation between COX-2 and MVD in nasopharyngeal carcinoma in H. Adam Malik Hospital. Method: This is a cross sectional analytical research. Twenty four samples was observed by immunohistochemical examination for COX-2 expression and MVD. The results then were analyzed using Spearman correlation test. Result: Positive COX-2 expression was found in 17 (70.83%) subjects and negative in 7 (29,17%) subjects. High MVD was found in 17 (70.83%) subjects and low MVD was found in 7 (29.17%) subjects. There was moderate positive correlation between COX-2 expression and MVD (r=0,559; p=0,005). Conclusion: COX-2 is moderate positively correlated with MVD in nasopharyngeal carcinoma.Keywords :Nasopharyngeal carcinoma, Microvessel density, Cyclooxygenase-2.

Abstract

Intoduction: Cytochrome P450 2E1 (CYP2E1) is the enzyme as a part of CYP 450 enzyme families. It acts in phase I metabolism which result in the formation of electrophilic molecule and elevation of reactive oxygen species (ROS). Review: The last is responsible in development of cancer by damaging the protein and DNA, leading to mutation and increase cell proliferation. Nitrosamine as the most carcinogen for nasopharyngeal carcinoma (NPC) is the substrate of this enzyme. This chemical is contained in the salted fish, tobacco smoke, beer, and preserved food such as meat product. Nitrosamine needs CYP2E1 to activate the pro-carcinogen in order to act is carcinogenic effect. Polymorphism of CYP2E1 is associated with higher transcriptional activity and enzyme activity. Conclussion: It means the ability of this enzyme to activate pro-carcinogen of nitrosamine will increase, leading to higher chance to develop cancer, including NPC.

Abstract

Introduction: Epstein Barr Virus (EBV) infection is responsible for nasopharyngeal carcinoma (NPC). LMP1 as the product of express gene by EBV latent infection is responsible in inducing Matrix Metalloproteinase–9 (MMP-9) and Cyclooxygenase–2 (COX-2). The two factors are associate with the metastasis of cancer and were thought to be a prognostic factor in nasopharyngeal carcinoma patients.  Objective: The aim of this study was to analyze the overexpression of MMP-9 and COX-2 in NPC patients and the combination of those two with the clinical staging of NPC which lead to an evidence of those two in NPC. This study also needed to reveal the association of MMP-9 expression in response of COX-2 expression.  Methods: This study was a cross sectional design study with statistical analytic approach. Samples was paraffin blocks which taken from the patients diagnosed with NPC with non-probability consecutive sampling method and underwent immunohistochemistry staining. Fisher’s exact test and Chi-square test was conducted to analyze the data, significant if P<0.05. Results: From the total 98 samples, we found the highest overexpression of MMP-9, COX-2, and both of them in nasopharyngeal carcinoma were in WHO Type II (80.8%; 78.8%; 78.6%), T3 (35.9%; 40.0%; 38.6%), N3 (53.8%; 53.8%; 60.0%), M1 (69.2%; 67.5%; 75.7%), and stage IV NPC (69.2%; 67.5%; 75.7%), respectively. There were significant association of primary tumor size, lymph node enlargement, the presence of distant metastasis, and clinical staging in the overexpression of MMP-9, COX-2, and both of them, but not histopathological type. There was association between MMP-9 with COX-2 in nasopharyngeal carcinoma patients. Conclusion: Overexpression of MMP-9, COX-2, and both of them occurs correspondence with increase clinical staging. COX-2 is associate with MMP-9 and those two may be used as prognostic biomarker.

Abstract

Diseases of ear, nose, and throat (ENT) are common diseases in the world. The diseases are essential due to morbidities, which affect the physiological functions in the head and neck region. Cerumen impaction is the most common finding disease of ENT in children. The community service was done to find the distribution of ear, nose, and throat examination results of students in Letjen Jamin Ginting Junior High School, Berastagi, especially cerumen impaction which then were treated with cerumen removal. The community service also aimed to increase student's knowledge about ENT disease. The ENT examination was done to 188 students. The students with cerumen impaction were treated with cerumen removal. The students also educated about ENT disease to improve knowledge about ENT disease. Among 188 students, the number of male students was more than female with 98 students (52.1%), where 76 students (40.4%) were 14 years old. 150 students (79.8%) did not have any complaint about their ear, nose, and throat. Ear fullness was a common chief complaint from the students with 15 students (8.0%). Ear disease was a frequent disease with 44 students (47.3%) and cerumen impaction as the most common findings with 43 students (22.9%). In ENT disease education, the evaluation was done with the highest result was get by 8 students with 80% corrected answers. Ear disease had high prevalence with cerumen impaction as the most common findings of ENT examination.
