Abstract

The touch and licking of the baby at the time of early initiation of breastfeeding will stimulate the release of the hormone oxytocin which will also affect the smooth muscle tissue of the uterus to contract, thereby strengthening uterine contractions in the fourth stage and helping to reduce bleeding after childbirth. The purpose of this study was to analyze the effect of early initiation of breastfeeding on uterine contractions in the fourth stage of labor. This study used a static group comparison design involving 10 respondents who were selected by simple random sampling technique. Respondents were divided into control group (5 people) and experimental group (5 people). Uterine contraction data were collected through observation. Data were analyzed by Fisher's exact test. Statistical test results show p value = 0.083; so it was concluded that early initiation of breastfeeding had an effect on uterine contractions in the fourth stage of labor. Abstract

The process of childbirth is identical to the onset of pain. The most dominant pain is felt especially in the active phase of the first stage of labor. The 1st stage of labor is the beginning of labor contractions characterized by progressive cervical changes ending with complete dilatation (10 cm). One of the problems that occur in first stage labor pain is caused by uterine contractions that cause cervical dilation and thinning. Through endorphine massage, which is a touch technique or light massage given to pregnant women before delivery that can normalize heart rate, blood pressure, and can increase the relaxed condition in the body of pregnant women by triggering a feeling of comfort through the skin surface so that by doing the massage for 20 minutes, then the pain in labor will be reduced. This study uses a quantitative research method with a static group comparison design. Measurements were made in the treatment group that was given an endorphine massage for 20 minutes in the active phase. The population in this study were all inpartu mothers who gave birth at Polindes Larasati. This research was conducted in Sekardadi Village, Jenu District, Tuban Regency. The sample in this study were some of the pregnant women who met the inclusion criteria as many as 14 respondents. McNemar statistical test results obtained p value = 0.031 (α < 0.05), thus there is an effect of endorphine massage on the adaptation of labor pain in the active phase of the first stage of inpartum mothers. From the results of the study, it was concluded that there was an effect of endorphine massage on pain adaptation because endorphine massage can stimulate the body to release endorphins which are pain relievers. It is hoped that health workers can provide health education about endorphine massage to the community so that later it is hoped that all people will be able to apply endorphine massage to overcome pain in labor. Abstract

Introduction.Every minute one Indonesian baby dies because they do not get breast milk in the first hour of birth, problems with breastfeeding can be due to milk production not coming out. One way to overcome the non-smooth milk production is by doing oxytocin massage through the back. This massage can relax the mother so that it stimulates the oxytocin reflex and smooth milk production. The purpose of this study was to determine the effect of oxytocin massage through the back on breast milk production in postpartum mothers. Method.This study uses a Quasy Experimental design involving 50 respondents who were taken by Systematic Random Sampling. Respondents were divided into 2 groups, namely the control group with 25 respondents and the experimental group with 25 respondents. Collecting data using a checklist observation. Data were analyzed using Paired simple T-test with a significant level of 0.05. Result&Analysis.This study showed that there is an effect of oxytocin massage through the back on breast milk production in postpartum mothers with p value = 0.000 (< 0.05). Discussion. Oxytocin massage through the back can stimulate the reflex hormone oxytocin. It is hoped that health workers can provide health education about oxytocin massage through the back to the community so that later it is hoped that all people will be able to apply oxytocin massage through the back to overcome the inability to produce breast milk.

Abstract

Anxiety is an individual mental or psychological state that brings up feelings like worry, fear, anxiety, confusion, and others. One of the treatments for those problems is five-finger relaxation. This study aims to determine the effect of five-finger relaxation on the anxiety level of primigravida mothers during the third trimester at the Tuban Public Health Center’s working area. The methods used Pre-experimental (static-group comparison design) and a cross-sectional study approach were used as the study’s method. The sampling technique used a simple random sampling with 36 primigravida mothers as the sample. SOP and questionnaires sheets were used as the instruments. Paired sample T-test was used to analyze the data. The study results showed a decrease in anxiety levels in third-trimester primigravida mothers in the experimental group after being given five-finger relaxation therapy with 1 treatment during the study. Five-finger relaxation is effective in non-pharmacological treatment. So it can be concluded that there is a difference in the level of anxiety of third-trimester primigravida mothers in the experimental group and the control group

Abstract

Lack of milk production in the first days after giving birth is caused by a lack of stimulation of the hormone prolactin and the hormone oxytocin. Decreased milk production is caused by decreased stimulation of the hormones prolactin and oxytocin. Efforts that can be made to stimulate an increase in the hormones prolactin and oxytocin include early initiation of breastfeeding, expressing breast milk and performing oxytocin massage, while breast care for postpartum mothers is less effective and efficient in increasing breast milk production. In Indonesia (Kemenkes, 2018) the coverage of infants who were exclusively breastfed in 2018 was 68.74%. In 2018, East Java got 77.51%, and especially in Tuban Regency (Tuban District Health Service data) in the same year it got 73.85%, then in 2019 it increased to 79.6%. However, in 2020 there was a decline to 76.93% of infants who were exclusively breastfed. Based on the data above, it shows that the success of exclusive breastfeeding is still not getting optimal results.Oxytocin massage training has never been carried out in Tegalagung Village, especially for breastfeeding companions such as Posyandu cadres, husbands or families. The method used in this community service activity is the method of lectures, discussions, simulations and demonstrations using leaflets and audio-visual media. The number of postpartum mothers who participated in the training was 20 people.The results of training activities for postpartum women assistants in the village of Tegalagung showed that the results of knowledge analysis showed that overall there was an increase in participants' knowledge after attending the training, meaning that there was an influence on increasing the knowledge of postpartum women assistants after participating in oxytocin massage training. So it was concluded that there was an increase in knowledge between before and after training about oxytocin massage.Recommended that health workers need to consistently and continuously provide training to postpartum mother companions (husband, family or cadres) in order to assist the process of behavioral change in postpartum mothers to exclusively breastfeed and provide care during the postpartum period.

Abstract

Based on the conditions experienced by the second-semester students of the Nursing Undergraduate Study Program at IIK NU Tuban, shows that motivation and learning outcomes are still low and tend to be less than optimal. One of the main causes is boredom. It's not boring teaching material, it's just that the lecturers are still not able to apply the models, strategies, methods, and learning media or approaches used. Efforts made by researchers to increase learning motivation are by applying Group Investigation (GI) learning techniques. With the application of this method, it is hoped that lecturers, students, and researchers will be able to determine the effectiveness of the Group Investigation (GI) method in increasing learning motivation for second-semester students in Learning Basic Nursing Sciences at IIK NU Tuban. This study used a pre-experimental design, with a one-group pre-test and post-test design. The sampling method used total sampling with a total of 139 respondents. The instruments used are SOPs and questionnaire sheets. The observation sheet is used to determine whether the GI method can be carried out according to the SOP. Data analysis used the Wilcoxon statistical test with SPSS 26. The results of the statistical test obtained Asymp. Sig. (2-tailed) p = 0.000 where the value of p < (0.000 < 0.05) then H1 is accepted, which means that there is an effect of the application of the Group Investigation (GI) Method on the Learning Motivation of Semester II Students in IDK Learning at IIK NU Tuban. Based on these results, it can be concluded that the application of the Group Investigation (GI) method can increase the learning motivation of second-semester students in IDK learning at IIK NU Tuban.

Abstract

Stres kehamilan adalah salah satu fenomena yang dialami oleh setiap ibu khususnya ibu yang pertama kali mengalami kehamilan (primigravida) yang dipicu oleh adanya prasangka-prasangka buruk yang akan menimpa dirinya, ketika akan bersalin berdasarkan pengalaman yang selama ini diperoleh utamanya pengalaman yang dialami langsung selama proses kehamilan, salah satu penyebab terjadinya stres adalah diproduksinya adrenalin dan noradrenalin yang memberi dampak pada ibu utamanya pada bayi berupa abortus, dan bayi dengan BBLR, kelahiran bayi prematur sampai pada kematian janin.     Oleh karna itu Tim melakukan pengabdian kepada masyarakat menawarkan solusi antara lain edukasi dan pelatihan manajemen stres dan relaksasi bagi ibu hamil melalui posyandu di Desa Genaharjo. Metode yang dilakukan pada pengabdian masyarakat yaitu dengan ceramah interaktif, curah pendapat, tanya jawab dan praktik    relaksasi    mengatasi    stres  selama pada    masa    kehamilan.    Setelah    selesai pembelajaran dilakukan evaluasi pembelajaran, dan pencatatan pelaporan. Hasil deteksi dini masalah kesehatan mental (Stres) ibu hamil sebagian besar ibu hamil mengalami lelah dan nafsu makan menurun pelatihan ini di ikuti oleh 25 ibu hamil pertama dan 3 kader posyandu. Pelatihan ini dilakukan selama 7 hari dari mulai koordinasi kegiatan sampai evaluasi kegiatan. Dari hasil pelatihan manajeman stres dan relaksasi dari 21(84%) ibu hamil merasa lega 4(16%) ibu hamil merasa tidak ada perubahan hal ini dikarenakan ibu kurang fokus dalam melakukan latihan relaksasi.Kesan dan pesan dari peserta sangat senang mendapat penyuluhan dan berharap ada kegiatan lanjutan di Desa Genaharjo dalam mempertahankan kesehatan ibu hamil.

Abstract

Background: The phenomenon of the risk of falling that occurs in the elderly in recent years has increased. Many cases of falls in the elderly result in soft tissue injuries, thigh and wrist fractures, and even death. Falling is a physical problem that is often experienced by the elderly, as you get older, the higher the decline in physical, mental and bodily functions.Objective: To Analyze the effect of the otago home exercise programme on decreasing the level of falling risk in the elderly in Tegalrejo Village, Merakurak District.Methods:The research used pra-experimental: pra-pasca test (one group pra-post design) with a cohort approach. The Sampling technique used was purposive sampling with a total sample of 102 elderly.Results: The results of the research conducted after being analyzed using the wilcoxon test with a significance level of α=<0.05, with value p = 0,000<0,05 value results Asymp sig(2-tailed)=0,000 which shows the influence between the two variables into a strong category with a positive direction.Conclusion: Otago home exercise programme has an effect on decreasing the level of falling risk in the elderly in Tegalrejo Village, Merakurak District.

Abstract

Nursing care documentation is a crucial element in quality nursing practice. Dr. R. Koesma Tuban Regional General Hospital (RSUD), as a healthcare institution committed to quality care, introduces the source-oriented recording method to enhance the quality of nursing care documentation. This article presents an introduction to the importance of nursing care documentation, along with the background, objectives, and benefits of implementing this method at RSUD Dr. R. Koesma Tuban. The Literature review supports the decision to implement this method, highlighting its benefits in improving accuracy, compliance, care coordination, and healthcare service efficiency. The structured implementation method involves team training, document review, socialization, implementation, monitoring, evaluation, and advanced training. The implementation results indicate improvements in accuracy, compliance, care coordination, and healthcare service efficiency. In conclusion, the application of the source-oriented recording method at RSUD Dr. R. Koesma Tuban is a strategic step to enhance the quality of healthcare services and patient safety.
