Abstract

Tyrosine kinase inhibitors (TKIs) telah direkomendasikan oleh National Comprehensive Cancer Network (NCCN) sebagai terapi lini pertama pada pasien non-small cell lung cancer (NSCLC) dengan mutasi EGFR, yaitu gefitinib (generasi pertama) dan afatinib (generasi kedua). Beberapa penelitian telah dilakukan terkait efikasi golongan TKIs ini, namun masih sedikit yang melakukan tinjauan sistematis (systematic review) secara khusus pada afatinib dan gefitinib. Systematic review ini bertujuan untuk memberikan gambaran secara komprehensif terkait efikasi afatinib dan gefitinib sebagai terapi lini pertama. Database yang digunakan untuk mengidentifikasi studi yang relevan yaitu Science Direct, PubMed dan Google Scholar. Penelitian ini terbatas pada penelitian randomized control trial, penggunaan afatinib dan gefitinib sebagai terapi lini pertama, penggunaan afatinib dan gefitinib secara monoterapi serta artikel yang menggunakan bahasa Inggris. Penelusuran berbasis data elektronik menghasilkan 2089 artikel; 48 artikel yang sudah sesuai dengan judul dan abstrak, namun hanya 8 artikel yang sesuai dengan inklusi yang ingin ditinjau. Secara umum efikasi afatinib dan gefitinib lebih unggul dibandingkan kemoterapi berbasis platinum, dapat dilihat dari progression free survival dari kedua obat ini yang lebih lama dibandingkan kemoterapi berbasis platinum. Walaupun nilai overall survival-nya tidak begitu lebih baik, afatinib dapat memperlambat perkembangan tumor lebih baik dibandingkan gefitinib. Efek samping yang sering terjadi pada afatinib dan gefitinib yaitu ruam dan diare, sedangkan neutropenia, mual, anemia sering terjadi pada penggunaan kemoterapi berbasis platinum. Afatinib dan gefitinib memberikan efikasi yang lebih baik dibandingkan kemoterapi berbasis platinum, namun afatinib memiliki efikasi yang lebih unggul dibandingkan gefitinib dalam pengobatan lini pertama pada pasien NSCLC dengan mutasi EGFR.Kata kunci: Afatinib, efikasi, gefitinib, non-small cell lung cancer  Afatinib and Gefitinib Efficacy on Non-small Cell Lung Cancer EGFR Mutation Patients: A Systematic Review AbstractTyrosine kinase inhibitors (TKIs) have been recommended by the National Comprehensive Cancer Networks (NCCN) as first-line therapy in non-small cell lung cancer (NSCLC) with EGFR mutation, namely gefitinib (first generation) and afatinib (second generation). Several studies have been conducted related to the efficacy of these TKIs, but few have conducted systematic reviews specifically afatinib and gefitinib. This systematic review aimed to provide a comprehensive overview of the efficacy of gefitinib and afatinib as first-line therapy. To identify the relevant studies, it used several databases such as Science Direct, PubMed, and Google Scholar. The review was limited to the randomized control trial study, the use of afatinib and gefitinib as first-line therapy, the use of afatinib and gefitinib as monotherapy, and articles written in the English language. An electronic data-based search identified 2089 articles; 48 articles matched the title and abstract, yet only 8 articles met the inclusion to be reviewed. In general, the efficacy of afatinib and gefitinib is more effective than platinum-based chemotherapy as showed that the progression-free survival of these medications is longer than platinum-based chemotherapy. Although the overall survival value is not better than platinum-based chemotherapy, afatinib inhibits the tumor growth better compared to gefitinib. Common side-effects in afatinib and gefitinib include rashes and diarrhea, while neutropenia, nausea, and anemia commonly occur in platinum-based chemotherapy. Afatinib and gefitinib offer better efficacy compared to platinum-based chemotherapy. Specifically, the afatinib has greater efficacy compared to gefitinib in first-line therapy for patient with NSCLC EGFR mutation.Keyword: Afatinib, efficacy, gefitinib, non-small cell lung cancer

Abstract

Drug storage is something that must be considered because it can affect the quality of the drug. Storage conditions must adapt to the stability of each drug. This study aims to determine the level of knowledge about the storage of syrup drug preparations in patients in all public health centers in Pekanbaru. The research method is analytic observational with cross sectional design. The research sample was the people who visited all health centers in Pekanbaru city which were selected by purposive sampling technique according to the inclusion and exclusion criteria and obtained a sample of 160 respondents. The instrument is a knowledge questionnaire that has been tested for validation and reliability. The knowledge questionnaire contained 14 valid questions (r=0.818). Presentation of data with bar charts and narrative text. The results of the study revealed that there were 66 respondents (41.25%) in the good knowledge category and 94 respondents (58.75%) in the sufficient knowledge category and none of the respondents had less knowledge in the category. From these results, it can be concluded that most people do not know well how to store syrup drugs, so it is necessary for the role of pharmacists to provide education to the public, especially regarding how to store drugs correctly

Abstract

Vaccines are biological products that are very fragile and easily damaged, so they require special handling in their management. RSD Madani is responsible for vaccine storage. Vaccine storage is a way to maintain the condition of the vaccine not being damaged so that the vaccine remains in good condition, thereby ensuring the quality of the vaccine. The purpose of this study was to evaluate the storage of the COVID-19 vaccine at the Regional Madani Hospital in Pekanbaru City against the 2020 CDOB standard and the storage indicators for the covid-19 vaccine SK of the Director General of Disease Prevention and Control Number HK.02.02/4/1/2021. The sample in this study was the storage room for the COVID-19 vaccine and all storage data for COVID-19 vaccine preparations in the Pharmacy Installation Warehouse at the Madani Regional Hospital, Pekanbaru City. This study uses a descriptive method, where the data collection method is by observing and interviewing the person in charge of the COVID-19 vaccine storage warehouse. The results showed that the storage of the Covid-19 vaccine at the Madani regional hospital in Pekanbaru city as a whole was not in accordance with the appropriate indicator regulations, namely, 33.33% for officers and training, 75% for buildings, 50 for building facilities. %, 84.6% for storage operations, 30.76% for storage maintenance, and 33.33% for qualification, calibration and validation.

Abstract

Telepharmacy is an online application used to provide pharmaceutical services in the modern era. Therefore, this study aims to develop a questionnaire on a research model that integrates three well-known theories, namely the unified theory of acceptance and use of technology (UTAUT), the protection motivation theory (PMT), and the DeLone and McLean information success model, to determine factors that influence people’s behavior towards the adoption of telepharmacy services. A cross-sectional observational method was used with the purposive sampling technique. A total of 118 participants who were at least 18 years old received the questionnaire. They were selected using convenience sampling from a variety of population groupings. Data analysis was then performed with the exploratory factor analysis (EFA). Factor extraction was carried out using principal component analysis and varimax rotation. The results show 8 factors consisting of social influence and support 15.54%; understanding of proficiency, confidence and quality of information 11.06%; condition of facilities and skills 6.64%; severity and suffering 7.87%; time opportunity support and system quality 7.74%; intention to adopt telepharmacy 7.34%; perceived vulnerability 6.80%; and attitude 6.37%. Furthermore, the overall construct formed can explain 72.40% of the total variance. These results indicate that the telepharmacy service adoption behavior questionnaire is valid and reliable.

Abstract

Giving the Sinovac vaccine for Covid 19 can cause several side effects. The existence of side effects that occur, of course, affect their lives, including a decrease in activity, productivity and quality of life. This study aims to determine the HRQoL score in the community after the Sinovac vaccine in Tampan District and Marpoyan District, Pekanbaru City. The research method is analytic observational with a cross-sectional design. The research sample was people who received the first vaccine recorded at the Puskesmas in Tampan District and Marpoyan District, Pekanbaru City, which was selected by purposive sampling technique according to inclusion and exclusion criteria and obtained a sample of 100 respondents. The instrument is a questionnaire EQ5D5L and EQ-VAS. The study results showed that there was little problem with the dimensions of pain (37%) and usual activities (14%), with a score of 0,944 (EQ5D5L) and 0,975 (EQ-VAS). In conclusion, the HRQoL scores on EQ5D5L and EQ-VAS are close to 1, which means close to perfect. This research is expected to be a reference for stakeholders in selecting preventive therapy for Covid 19 and can be used as initial data for cost-utility analysis research.

Abstract

Human resource is one the success of indicator of an aeffective and efficient hospital. Human resource planning in pharmaceutical logistic need to bee considered because pharmaceutical logistic plays an important role in the management of pharmaceutical supplies which will affect quality hospital services. Workload Indicators Staffing Need (WISN) method is a method calculate workforce requirements based on a real workload undertaken by workfoce. This study aim to determine the need workforce based workload in the pharmaceutical logistic RSUD Arifin Achmad Provinsi Riau. This study is observational method that is descriptive using free guided interview technique. Sampling in this study was carried purposive sampling method. The sample in this study are head installation pharmacy and pharmacist in charge of logistic. Based on WISN analysis, human resource requirement in pharmaceutical logistic RSUD Arifin Achmad are 17. Pharmaceutical logistic had 14 staff, so that pharmaceutical logistic is still short of 3 staff with a WISN ratio 0,82 which means there was a lack of staff in pharmaceutical logistic.

Abstract

Inappropriate administration of antibiotics can cause resistance to bacteria. Staphylococcus aureus is one of the strong biofilm-forming bacteria that cause antibiotic resistance. Calincing (Oxalis corniculata L.) leaves have excellent antibacterial activity, but their antibiofilm activity against S. aureus has not been reported until now. Currently, the discovery of new antibiofilm against S. aureus biofilms is significant to prevent the impact of infections caused by biofilms. This study was intended to determine the effectiveness of the ethanol extract of O. corniculata leaves in inhibiting and eradicating S. aureus biofilm formation. Planktonic testing, inhibition, and biofilm eradication activity were carried out using the microtiter broth method. Antibiofilm activity of O. corniculata leaves against S. aureus biofilm was analyzed by calculating the minimum concentration of biofilm inhibitor (MBIC50) and minimum biofilm eradication concentration (MBEC50). Data were analyzed using the Statistical Package for the Social Sciences (SPSS) with a 95% confidence level. Oxalis corniculata leaves showed inhibitory activity on the formation of the tested S. aureus biofilm. The ethanol extract of 1% O. corniculata leaves gave 76.23±0.01% antibacterial activity of S. aureus and 71.32±0.01% of mid-phase antibiofilm activity, and 69.33±0.01% maturation phase. The results also prove that the ethanolic extract of O. corniculata leaves can eradicate S. aureus biofilm formation. Therefore, the ethanol extract of O. corniculata leaves can be developed as a new antibiofilm against S. aureus.
