Abstract

Chronic renal failure is a disorder in which the body’s ability renal function failed to maintain metabolism, fluid and electrolyte balance. Hemodialysis is a process used patients in a state of acute illness requiring long-term therapy. The patients with renal failure often experience fear because of the conditions they experienced pain this can be a physical stressor that can lead patients to levels of stress and anxiety. Research design using cross- sectional. The population includes hemodialysis patients of Grandmed Hospital Lubuk Pakam. Samples consisting of 43 respondents meeting inclusion criteria were selected using purposive sampling method.This study presented based on the frequency and characteristics of respondents with results of the appointment that anxiety levels can be categorized as mild anxiety (18,6%), moderate anxiety ( 41,9%), and severe anxiety (39,5%). For coping mechanisme of adaptive coping (39,5%) and maladaptive coping (60,5%) category. Analysis of this research data using chi-Square. The results showed that there was an association of anxiety level with coping mechanism in patients with chronic renal failure who underwent hemodialysis at Grandmed Hospital Lubuk Pakam with p = 0,031. The more adaptive the respondent’s clutch the lighter the level of anxiety experienced. The results of this study was expected to nurse or hemodialysis officers further improve the provision of counseling to patients hemodialysis so that patients are able use adaptive coping in the face of anxiety.

Abstract

Diabetes mellitus is a degenerative disease, characterized by an increase in glucose levels in the blood. This causes diabetes patients to experience sleep disturbances. One of the basic human needs that must be fulfilled is sleep. The research objective is to determine the factors that cause sleep disorders in diabetes mellitus patients. Descriptive research design that aims to identify sleep disturbance factors of diabetes mellitus patients at Grandmed Lubuk Pakam Hospital. The sampling technique used purposive sampling method with a total sample of 57 people. This study used a questionnaire of demographic data and sleep disorders. The results of the study of sleep disorders physical factors of BAK at night were 56 people (98%), pain in 43 people (75%), tingling and leg cramps in 39 people (68%), fatigue of 37 people (65%), thirst at night 35 people ( 61%), hungry at night 12 people (21%), snoring 34 people (60%). Sleep disturbance due to environmental factors in the form of environmental factors of respondents based on hot room temperature 48 people (84%), mosquito bites 36 people (63%), noise from home 34 people (60%), outside noise 24 people (42%) . The conclusion is that the factors that influence sleep disorders in diabetes patients are physical and environmental factors.

Abstract

Nursing care services certainly cannot be separated from the nursing process, starting from nursing assessments, nursing diagnoses, nursing interventions, nursing implementation, evaluation and documentation. There are several factors related to the implementation of nursing care at Rs GrandMed Lubuk Pakam in 2020. This study used an analytical survey research with a cross sectional study approach. The population in this study were all employees of 3 Depan. The sample in this study was used as many as 12 respondents according to the inclusion criteria. The results of this study indicate that, there is no relationship between nurse education and the implementation of nursing care (p = 1), there is no relationship between the work tenure of nurses and the implementation of nursing care (p = 0.293), there is a relationship between the workload of nurses and the implementation of nursing care (p = 0.293). p = 0.010), There is a relationship between incentives and the implementation of nursing care (p = 0.010), There is a relationship between work atmosphere and the implementation of nursing care (p = 0.045), There is a relationship between leadership style and the implementation of nursing care (p = 0.045). So it can be concluded that there is a relationship between workload, incentives, work atmosphere and leadership style on the implementation of nursing care.

Abstract

Diabetes mellitus is a chronic non communicable disease. Characterized by an increase in blood sugar levels, the result is that the pancreas cannot produce or the body cannot use insulin. Foot exercise in diabetes mellitus is a leg movement exercise that aims to improve blood circulation in the legs and detect the severity of vascularity in the legs. The research objective was to assess the effect of foot exercise on the value of the Ankle Brachial Index (ABI) in diabetes mellitus patients at Grandmed Lubuk Pakam Hospital. Quasi experimental research design pretest and posttest without control. The study was conducted from October to December 2020 at Grandmed Lubuk Pakam Hospital with 43 respondents, the samples were taken using consecutive sampling technique. The intervention was carried out twice a day. Consistent exercise can increase the supply of oxygen in the blood to the peripheral tissues in the legs. The results of the research from the univariate analysis found that male gender were 23 respondents (53.5%), aged > 55 years were 21 respondents (48.8%), the use of blood sugar drugs was 39 respondents (90.7). While the bivariate results using the Wilcoxon test, it was found that there was a significant effect between the average ABI value before the intervention 0.84 and 0.95 after intervention, p = 0.000. The results of the study concluded that there was an effect of foot exercise on the value of the ankle brachial index (ABI) in diabetes mellitus sufferers, where leg blood flow was getting better after foot exercise.

Abstract

Surgery is performed by taking certain body parts, with the aim of improving the body that has abnormalities such as laparotomy. This action can disable the sound of the prism with the state of the body that is anesthetized. Early mobilization is one of the effective ways to re-stimulate bowel function and produce normal peristaltic sounds and patients can be selected. The purpose of this study was to determine the effect of early mobilization on the recovery of intestinal peristalsis in post-laparatomy patients at Grandmed Lubuk Pakam Hospital. This study used a one-group pretest-posttest design, with pre-experimental pre-test (early stage) and post-test (final stage) with a sample of 27 respondents while the research instrument used an observation sheet. Data analysis used bivariate analysis, namely the Paired Sample T-Test. The results of the bivariate test with Paired Sample T-Test proved that there was an effect of early mobilization on increasing intestinal peristalsis before and after the procedure, as evidenced by the p-value = 0.00 < 0.05. As early as possible, early mobilization doing in post-laparatomy patients, the faster the intestinal peristaltis recovery process will be.

Abstract

Hypertension is one of the deadliest diseases in the world, hypertension is an increase in systolic blood pressure above the normal limit of more than 140 mmHg and diastolic blood pressure of more than 90 mmHg, factors that cause increased blood pressure are physical activity, emotion, age, gender , nutritional status, drinking alcohol, smoking, stress. Data from the Grandmed Lubuk Pakam Hospital in 2018 the number of hypertension patients there were 178 cases of hypertension, the number of men was 82 people and women 96 people. One of the non-pharmacological therapies offered to reduce hypertension is massage therapy. The massage technique can remove blockages in the blood vessels so that blood and energy flow in the body returns smoothly. This research was conducted using a quasi-experimental design with the type of design to be used, namely the static group comparison method, the sampling technique used was non-probability sampling with purposive sampling technique, where the sample of this study was 20 people. The results showed that the pre-test sample in the treatment group was known to have the highest value of 160/100 mmHg and the lowest value of 130/90 and post-test mmHg with an average pre-test blood pressure value of 142.00 on systolic and 93.00 on diastolic, and 110.00 post-test. in systole and 79.00 in diastole. Based on the results above, it can be concluded that there is a relationship between reflexology neck massage with a decrease in blood pressure in patients with hypertension where the Wilcoxon Test results obtained p = 0.004 (systolic blood pressure) and p = 0.005 (diastolic blood pressure).

Abstract

Diabetes mellitus is a disorder of the endocrine system characterized by increased levels of glucose in the blood. The purpose of this study was to assess the effect of Buerger's Allen exercise on the value of the ankle brachial index (ABI) in patients with diabetes mellitus. The research design was a quasi-experimental pretest and posttest without a control. The study was conducted from March to May 2021 at Grandmed Lubuk Pakam Hospital with 48 respondents, with consecutive sampling technique. Data were analyzed using the Wilcoxon test. The Buergers Allen exercise intervention was performed twice a day, for three weeks. The procedure is performed by lifting the lower extremities up at an angle of 45-90 degrees for 2-3 minutes, then the patient sits at the bedside with the legs hanging down, accompanied by flexion, extension, pronation and supination movements for 5-10 minutes, and the patient lies down. with both feet resting for 10 minutes. There was a significant effect between the mean ABI value before intervention 0.83 and after intervention 0.95, p=0.00. The difference in mean ABI p=0.000, p<0.05. The results of the study concluded that there was an effect of buerger allen exercise on the ABI value before and after the intervention where the ABI value was getting better. It is recommended for nurses to make Buergers Allen exercise one of the independent nursing interventions in providing nursing care to patients with diabetes mellitus.

Abstract

One of the sign and symptoms of hypertension is headache. Headache occur due to atherosclerosis which causes spasm of the blood vessels (arteries) and decreased oxygen in the brain. The pain can be treated with non pharmacological management, one of which is a warm gingger compress (zingiber officinale). The purpose of this research activity was to measure the effect of giving warm ginger compresses on reducing the headache scale in hypertensive patients at Grandmed Lubuk Pakam Hospital. The type of research used in this study is a quasi experimental approach with a one group pretest-posttest design approach, with a purposive sampling technique. Giving warm ginger compresses to 37 patients suffering from hypertension at Grandmed Lubuk Pakam Hospital. Data collection was carried out using observation sheets and providing an intervention with warm ginger compresses in hypertensive patients. Observations of headaches in hypertensive patients carried out twice, namely before being given a warm ginger compress and after being given a ginger warm compress then the results were recorded on the observation sheet. Teh results of statistical tests using the paired test showed the value of p=0,000, p<? (0,000<0,05). The results obtained showed that there was a decrease in the level of headache in hypertensive patients before and after being given warm ginger compresses. It is expected that nurses can minimize the use of analgesic drugs to reduce headaches and use warm compresses for non pharmacological management.

Abstract

The Covid-19 disease that was detected at the end of 2019 quickly caused a pandemic, and became a global health issue. Progressive Muscle Relaxation (PMR) and Deep Breathing (DB) are exercises that can reduce blood pressure, heart rate, stress, lactic acid production, anxiety and pain. The purpose of the study was to assess the effect of PMR and DB on anxiety (anxiety) and quality of life (quality of life) of COVID-19 patients. The research design used a quasi-experimental with pretest and posttest with the control group. The research was conducted at Grandmed Lubuk Pakam Hospital. The sampling technique used purposive sampling, with a sample of 28 people, with 14 people in each group. Data were analyzed using paired t test. The procedure was carried out with 30 minutes of treatment every day for 5 consecutive days. There was a significant effect between the mean anxiety score before 43.08 (SD ±4.22) and after treatment 45.67 (SD ±2.41) and Quality of Life before 24.43 (SD±4.58) and after treatment 26.07 (SD ±3.58). The results of the difference in mean obtained p value = 0.00 on anxiety, and p = 0.01 on quality of life, or p value < 0.05. The results of the study concluded that there was a significant difference in the average difference between the PMR and DB intervention groups in reducing anxiety and increasing quality of life in Covid-19 patients. It is recommended for nurses to make PMR and DB one of the independent nursing interventions in providing nursing care to Covid-19 patients.

Abstract

babies are malnourished, development is less than optimal and growth is also not in accordance with age, this study has a goal, namely to find out what are the factors for the failure of Eklsusif breastfeeding, this study used 36 samples of mothers in total sampling using analytical survey crossectional method. The instrument used was a questionnaire sheet while testing with the chi- square test. The results showed that the majority of respondents worked as many as 23 people (63.9%), good knowledge as many as 20 people (55.6%), for a positive attitude as many as 19 people (52.8%), the majority of husband's support supported as many as 20 people (55.6%) the majority of respondents did not provide exclusive breastfeeding as many as 22 people (61.1%). The conclusion of the study shows that there is a relationship between work (0.006), knowledge (0.004), attitude (0.014) husband's support (0.025) with exclusive breastfeeding at Pratama Aulia Medika Clinic, Tualang District, Siak. This study expects mothers who have babies and are working to increase insight and management of exclusive breastfeeding is very important for the growth and development of babies in order to reduce diarrhea, and infections.
