Abstract

Standar prosedur operasional (SPO) merupakan suatu langkah-langkah/instruksi yang dibakukan untuk menyelesaikan suatu proses kerja rutin tertentu, dimana memberikan langkah-langkah yang benar dan terbaik berdasarkan konsensus bersama untuk melaksanakan berbagai kegiatan dan fungsi pelayanan. Salah satu upaya untuk menjaga keselamatan pasien adalah dengan menerapkan SPO dalam setiap tindakan keperawatan. Penelitian ini bertujuan untuk mengeksplorasi secara mendalam pengalaman perawat dalam mengupayakan pencegahan kesalahan dengan menerapkan standar prosedur operasional di rumah sakit umum X Medan. Desain penelitian yang digunakan adalah fenomenologi deskriptif. Partisipan dalam penelitian ini berjumlah 10 partisipan yang dipilih dengan teknik purposive sampling. Data yang dikumpulkan berupa rekaman hasil in-depth interview dilengkapi dengan field note kemudian dibuat dalam bentuk kata demi kata dan dianalisis dengan menggunakan pendekatan Colaizzi. Hasil penelitian ditemukan empat tema, yaitu: Menggunakan SPO setiap tindakan kepada pasien untuk mencegah kesalahan, 2) Menjadikan SPO sebagai instruksi kerja, 3) Mengikuti langkah-langkah tindakan sesuai SPO, dan 4) Menciptakan komunikasi efektif sebagai kunci keselamatan pasien. Hasil penelitian menunjukkan bahwa perawat menjadikan SPO sebagai aturan yang wajib diikuti setiap melakukan tindakan/asuhan kepada pasien yang diyakini dapat mencegah terjadinya kesalahan yang bertujuan menajalankan program keselamatan pasien sehingga dapat meningkatkan kualitas pelayanan keperawatan di rumah sakit.

Abstract

The success of the mechanism of coping in patients with cancer is breast does not only depend on the detection since the early , but also of understanding will be the mechanism causes . One of the conditions that can cause anxiety is the process of carrying out chemotherapy treatment . The purpose of this study was to determine the effect of the mechanism coping against the anxiety of cancer breast at home Sick Grandmed Lubuk Pakam . Type penel itian is using methods quasy Experiment with using the design study One-group pretest-posttest design. The determination of the samples is done by way of purposive sampling, with the number of samples seban yak 30 people . Analysis of the data is done by using a test Paired Sample T Test with a confidence interval of 95% and the value of α = 0.05. The results were obtained from studies have demonstrated the presence of changes in the level of anxiety of patients before and after a given application of the mechanism of coping with value p-value = 0.001 (α> 0.05). Conclusion , there is the influence of the mechanisms of coping against the anxiety of patients kankker breasts were conducted in accordance with the standard, For Institutions Health need for supervision and counseling to the public that the importance of maintaining health , especially in patients with cancer of breast in Rs. Grand Lubuk Pakam

Abstract

This study aims to determine the effect of Spiritual Emotional Freedom Technique on Pain Intensity in Post Sectio Caesarea Patients in the Hospital. Grandmed Lubuk Pakam. The benefit of Spiritual Emotional Freedom Technique Therapy is to ensure that the flow of psychological resistance, in the form of spontaneous negative thoughts or negative unconscious beliefs.This study included a qualitative study with One Group pre and Post test design conducted at the Grandmed Lubuk Pakam Type-B Hospital. The study was conducted from January to June 2020 with a population of 160 patients with sectio caesarea and the sample method used was using the method purposive sampling. The data collection technique used is using the Observation Test Statistic Sheet used in this study is Wilcoxon to see there is an independent influence with the Spiritual Emotional Freedom Technique technique in post Sectio Caesarea patients.Based on the results of research and analysis used it was found that the majority of respondents aged 23-26 years, Employment Employees with 23 respondents and the last educated junior high school with 14 respondents. There is an influence of Spiritual Emotional Freedom Technique therapy on pain intensity in Caesarean post section patients with a value of p = <.000 <0.05.

Abstract

Increased illness causes problems in the health field one misbehaving in the from of auditory hallucinations. This can be overcome with pharmacological and non-pharmacological therapies. Non pharmacological therapy which can be used in the form of classical music therapy. The purpose of this research is to find out the effectiveness of classical music therapy to decrease the level of hallucinations in patients with auditory hallucinations. Type of this research is quantitative research uses quasi-experimental design with design research pre and post test without control. Sampling techniques in this study using a sample with a population of 22 respondents at Mental Hospital Prof. Dr. M. Ildrem Medan in 2020. The results of statistical analysis using the Paired t test indicates p value of 0,000 means that there is effectiveness in the administration of classical music therapy to decrease the level of hallucinations in patients with auditory hallucinations. The result is expected of music therapy to be one nursing intervention to decrease level hallucination with auditory of hallucination. Keywords: Patients with hallucination, music therapy, level of auditory Hallucination.

Abstract

The role of educators is to help patients improve their health by providing knowledge about medical treatments and actions received so that patients or families can find out important knowledge for patients or their families. The research objective was to analyze the relationship between the role of the nurse educator in discharge planning with the level of compliance of inpatients for control at Grandmed Lubuk Pakam Hospital. The research method is this research using analytic observational research using a cross sectional study. In a study with a cross sectional study, measurement and data collection were carried out on the cause and effect variables for a moment at a time. The results of the research in this study The statistical test results showed the value of p = 0.001. Ha is accepted if Ho is rejected, where Ho is rejected if the p value is ≤ α, 0.001 ≤ 0.05. The results of statistical analysis showed that there was a significant relationship between the role of nurse educators in discharge planning and the level of inpatient compliance for control. Conclusion: The role of nurse educators in discharge planning was in the good category, namely 23 people, the level of patient adherence to control was 24 people (60%).

Abstract

The number of patients with surgery has increased significantly from year to year. Recorded in 2011 there were 140 million patients in all hospitals in the world, Anxiety is a common thing in patients who are going to undergo surgery. Anxiety that occurs with preoperative patients can be felt since the start of being scheduled for surgery until the time for surgery arrives and hand massage can be useful to reduce pain because it has a relaxing effect and reduces anxiety. Purpose: to determine the effect of hand massage on the reduction of preoperative client anxiety levels at Patar Asih Lubuk Pakam Hospital in 2021. Method: Pre-experimental design with one-group pre-post test design. The research subjects were preoperative inpatients. Population: preoperative patients in Galilee inpatient room. Sample: purposive sampling. Results: After the hand massage treatment, the level of anxiety in the treatment group decreased significantly, while the control group did not experience a significant decrease. In the control group the pre-test average was 19.63, while the post-test average was 19.06, this indicates a very small decrease so that the decrease was not significant. In the treatment group the average before treatment was 19.94 after treatment decreased to 16.19. Conclusion: The results of hypothesis testing (p <0.05) prove that hand massage significantly reduces anxiety levels before surgery.

Abstract

Nursing care services certainly cannot be separated from the nursing process, starting from nursing assessments, nursing diagnoses, nursing interventions, nursing implementation, evaluation and documentation. There are several factors related to the implementation of nursing care at Rs GrandMed Lubuk Pakam in 2020. This study used an analytical survey research with a cross sectional study approach. The population in this study were all employees of 3 Depan. The sample in this study was used as many as 12 respondents according to the inclusion criteria. The results of this study indicate that, there is no relationship between nurse education and the implementation of nursing care (p = 1), there is no relationship between the work tenure of nurses and the implementation of nursing care (p = 0.293), there is a relationship between the workload of nurses and the implementation of nursing care (p = 0.293). p = 0.010), There is a relationship between incentives and the implementation of nursing care (p = 0.010), There is a relationship between work atmosphere and the implementation of nursing care (p = 0.045), There is a relationship between leadership style and the implementation of nursing care (p = 0.045). So it can be concluded that there is a relationship between workload, incentives, work atmosphere and leadership style on the implementation of nursing care.

Abstract

Pathological neonatal jaundice is a liver or liver disease which causes yellowing of the baby’s skin and newborn’s eyes. The yellow color that occurs is caused by hight bilirubin in the blood because that baby’s liver is not yet mature enough. Objective : this study generally aims to determine whether there is an effect of phototerapy on the reduction in pathological neonatal care unit (NICU) room at the Grandmed Lubuk Pakam Hospital in 2020. Method : The type of ths research is expermental quantitative with One Group pre Test and post-test,namely research using comparison ( control ) groups, measurements are carried out before and after treatment. Provision of phototerapy is beneficial iin reducing the pathological neonatal jaundice sign. The number of samples with male sex is 31 people ( 57.4%) and the sample of female gender is 23 people (42.6%). Results : The administration of phototeray for the reducton in pathological neonatal jaundice signs using statistical analysis paired t- test obtained a value of p 0,000 <c 0.05. Conclusion : There s significant effect on the administration of phototherapy on the decrease in pathological neonatal jaundice signs. Suggeston : The researcher recommends that the researcher recommends that the researcher recommends that the researcher recommends that the researcher further adds the number of samples and a longer time so that the results obtained are more optimal.

Abstract

Hypertension is a condition when the systolic blood pressure (BP) is greater than 140 mmHg and the diastolic blood pressure is greater or above 90 mmHg. According to the America Heart Association (AHA), the America population aged over 20 years who hypertension has reached 74.5 milion people, but almost 90-95% of cases have no known cause. Hypertension has the potential to cause fatal complications end opens up greater opportunities for patients to suffer from storke, coronary heart disease, have a high risk of kidney failure, heart failure, and eye damage. Physical activity that used to lower blood pressure is the Isometric Handgrip Exercise which done with static exercises on contracting muscles, without any changes in muscle length or hand joint movement. Objective : This study aims to determine the effect of isometric handgrip therapy on blood pressure in patients with hypertension in the working area Batang Kuis Health Center. Method : This study uses a quasi-experimental quantitative method using a pre-test post-test design with a sample 29 people. The measuring instrument used in this research is the observation sheet. Data analysis was carried out using the paired t-test sample. Result: It seen the paired t-test of blood pressure before and after giving of isometric handgrip therapy, the mean value is 4.034 with std. Devitation 1.267 with a 95% confidence level with a value of 0.05 from a significant value of 2 tiles 0.000 or 0.000 < 0.05. Thus, it concluded that there is an effect of giving isometric handgrip therapy on reducing blood pressuer in hypertension patients in the Batang Kuis Health Center.

Abstract

Congestif Heart Failure (CHF) atau Gagal Jantung Kongestif adalah suatu keadaan dimana jantung sebagai alat pemompa darah tidak mampu lagi memenuhi kebutuhan darah untuk keperluan metabolisme tubuh. Tujuan Penelitian: Untuk mengetahui Pengaruh home based exercise training (HBET) sebelum dan sesudah dilakukan untuk mengatasi masalah fatique pada pasien CHF. Metode penelitian: Penelitian ini menggunakan penelitian kuantitatif, menggunakan pendekatan quasy experimental yaitu suatu pendekatan one group pre test and post test. yang digunakan untuk mencari pengaruh antara variabel independent dan variabel dependent melalui pengumpulan data dalam satu periode waktu yang bersamaan. Populasi dari penelitian 28 responden pasien dan sampel 25 responden dengan tehnik sampling purposive sampling dengan masalah CHF. Hasil penelitian: Setelah dilakukan analisa uji statistik dengan metode uji Paired Sample T-Test artinya Ho ditolak sehingga ada Pengaruh Home Based Exercise (HBET) Terhadap Fatique Pada Pasien CHF di masa Pandemi Covid 19 di Rumah Sakit Grandmed Lubuk Pakam. Kesimpulan: pada penelitian ini diharapkan pasien CHF yang mengalami fatique setelah dilakukan tindakan kondisinya untuk fatique berkurang.
