Abstract

F-box and WD40 repeat domain-containing 7 (FBXW7) adalah protein F-box yang merupakan komponen sistem ubiquitin proteasome yang berhubungan dengan beberapa protein onkogenik penting seperti Notch, c-Myc, cyclin E and c-Jun dalam mengatur proses pertumbuhan dan perkembangan sel, metabolism sel, diferensiasi dan apoptosis. Beberapa studi terkini menunjukkan bahwa FBXW7 berperan dalam perkembangan beberapa stem cell, mencakup proses self-renewal, multipotensi, diferensiasi, dan survival. Cancer stem cell (CSC) diketahui merupakan salah satu penyebab resistensi tumor terhadap berbagai kemoterapi, kekambuhan dan metastasis tumor. FBXW7 berperan terhadap kondisi quiescence pada CSC dengan mengatur substrat downstream c-Myc. Kondisi quiescence pada CSC dapat diinhibisi dengan memblokade peran FBXW7 melalui pathway upstream and downstream FBXW7 sehingga diharapkan dapat mengatasi resistensi dan kekambuhan tumor. Artikel telaah pustaka ini akan memaparkan peran FBXW7 pada stem cell normal dan pada cancer stem cell hubungannya dengan pengembangan terapi.

Abstract

Kanker paru masih menjadi masalah kesehatan yang sulit diatasi bukan saja karena meningkatnya jumlah perokok di Indonesia tetapi juga disebabkan tingginya mortalitas dan morbiditas penderita. Beberapa panduan pengobatan merekomendasikan penanganan multimodalitas. Tatalaksana kanker paru cenderung tidak optimal pada usia lanjut padahal hampir separuh penderita kanker paru berusia di atas 65 tahun. Pasien kanker paru berusia lanjut sering memiliki penyakit komorbid, status tampilan buruk serta risiko toksisitas terapi tinggi. Tulisan ini merupakan tinjauan pustaka tatalaksana kanker paru pada usia lanjut.Lung cancer remains a health problem in Indonesia because of increase of young smokers and its high mortality and morbidity. There is a tendency of inadequate management of lung cancer among elderly patients despite fact that almost half of lung cancer patients aged over 65 years. Lung cancer management is still challenging and need comprehensive approach since older patients are more likely to have decreased functional reserve which limit their ability to undergo surgery or receive chemotherapy. Several guidelines recommended multimodality treatment. Ageing is also associated with increased co-morbid medical conditions that could exacerbate and influence outcome. This article will discuss treatment of lung cancer in elderlies.

Abstract

Introduction Obesity has become a significant public health problem in developing countries such as Indonesia. According to WHO, 13% of adults aged 18 years and over were obese in 2016. In Indonesia, 21.8% of adults were obese. In obesity, the body's resistance to insulin will develop. Some studies showed a probable link between dietary fiber and insulin resistance. This research aims to investigate the role of the dietary fiber on insulin resistance in obesity. Methods: This study is a literature study to determine the effect of dietary fiber on insulin resistance in obesity with sources from scientific publications 10 years back. The databases were PubMed and Google Scholar. The search term used was using the explode function for subgroup terms with operators (“and,” or) for “dietary fiber”, “obesity”, “insulin resistance”. Hand-searching was used to identify further potential eligible studies. There were no language restrictions, however only publications with full texts available were included. Total 138 publications titles and abstract were screened for their relevance to this literature review. Results: A total of 25 publications were finally included. There are cross-sectional studies, randomized clinical trial, cohort studies, and article review. Some studies showed that dietary fiber had an effect on improve insulin resistance, but other studies did not find this effect. Conclusion: The studies of dietary fiber effect on insulin resistance have inconsistent results. In the future, further studies are required for better understanding about the effect of dietary fiber on insulin resistance in obesity

Abstract

Background: Malnutrition and tuberculosis (TB) have a bidirectional relationship, which interacts between each other. In chronic infection, there is an imbalance between protein degradation and protein synthesis which marked with the loss of fat-free mass (FFM). Malnutrition can cause the atrophy of the thymus gland resulted in the reduction of lymphocyte production. Malnutrition in TB patients will reduce the quality of life. On the other hand, a good quality of life will increase the treatment success rate and decrease the risk of morbidity and mortality. Materials and Methods: A cross-sectional study was conducted in 12 primary health centers (PHC) chosen randomly from 23 PHC in Pekanbaru, Riau Province. The random selection was performed by using simple random sampling (random number generator). Data were collected from May until July 2019. Samples were selected using a consecutive sampling method and 72 subjects fulfilled all research criteria. The interview was used to collect basic characteristic data, dietary intake data, and quality of life score. Anthropometric measurement (body weight, body height, and FFM) and laboratory examination (total lymphocyte count) were done.Results: Research showed median age subjects was 33 years old (18-59 years old). Most of the subjects were male (56.9%), had a middle level of education, low income, were active smokers with underweight nutritional status. More than 50% of subjects had low protein intake, low fat-free mass, normal lymphocyte count and had a good quality of life.Conclusion: There was a statistically significant correlation between fat-free mass with PCS (r=0.239, p=0.044), but not for protein intake and total lymphocyte count.Keywords: fat-free mass, protein, quality of life, total lymphocyte count, tuberculosis

Abstract

Background: Coronavirus disease-19 (COVID-19) pandemic caused the government to implement social movement restrictions affecting the household economy and school closure that will lead to change in the home food environment, parental feeding practices, and the end will influence child’s eating behavior. Objectives: This study aimed to explore the parental feeding practices among young school-aged children during the COVID-19 pandemic. Methods: Eighteen informants consisting of mothers and a grandmother have participated in the online in-depth interview. Moreover, 4 fathers had also been interviewed to triangulate the data and to increase the meaningful insight. A question guideline was used to guide the in-depth interview process. Thematic analysis using an inductive approach was applied to analyze the transcripts. Results: Five themes were identified around changes in food restriction, pressure to eat, giving the explanation, child involvement, and modeling. Conclusions: During the COVID-19 pandemic, parents demand their children to have good eating behavior. However, they tend to give autonomy more often to their child to get the snacks. It would make the children lead to having poor and unhealthy eating behaviors.

Abstract

Infeksi SARS-CoV-2 dapat menyebabkan disregulasi respons imunitas tubuh yang menimbulkan sekresi sitokin proinflamasi yang berlebihan, menyebabkan kondisi yang disebut sebagai badai sitokin. Badai sitokin ini memiliki peran penting dalam progresi penyakit COVID-19 karena dapat menyebabkan disfungsi multi organ, gangguan koagulasi, dan kematian. Sitokin proinflamasi yang paling banyak mengalami peningkatan selama badai sitokin pada pasien COVID-19 adalah IL-6. Dari pengetahuan ini, peneliti mulai mencari tahu kemungkinan digunakannya terapi inhibisi IL-6 sebagai bagian dari tata laksana COVID-19. Tiga macam terapi inhibisi IL-6 yang sedang diteliti adalah inhibitor reseptor IL-6, antagonis IL-6, dan inhibitor JAK. Ketiga agen terapi inhibisi IL-6 ini terbukti dapat memperbaiki klinis dan mengurangi mortalitas pasien COVID-19 dengan gejala berat. Akan tetapi, pemberian terapi inhibisi IL-6 masih belum memberikan hasil yang konsisten pada pasien COVID-19 yang disertai dengan gejala kritis sehingga pemberian pada kelompok ini harus lebih hati-hati. Oleh karena itu, penelitian lebih lanjut mengenai efektivitas dan keamanan terapi ini harus tetap dilakukan.

Abstract

This study aims to determine the correlation between fiber intake and insulin resistance in normal and overweight employees in Jakarta. This study is a cross-sectional study of 58 employees of a pharmacy company aged 19-49 years with normal or overweight and normal blood sugar. BMI, waist circumference, physical activity, smoking status and food intake were assessed. The fiber intake was assessed by 3 days estimated food record and the insulin resistance was assessed by HOMA IR obtained from fasting blood sugar and insulin. Data analysis technique used the Spearman Correlation test with the Statistical Package for Social Science (SPSS). Result obtained fiber intake and HOMA IR has r= -0.390 and p = 0.002. Based on the result of the study, it can be concluded that there is a negative correlation between fiber intake with insulin resistance in normal and overweight employees in Jakarta.

Abstract

Indonesia is among the top three countries globally with the highest tuberculosis  burden. During the past decades, Indonesian health authorities have struggled to improve tuberculosis care quality in health care facilities by optimizing the regulation and strengthening the private sector contributions. The coronavirus disease 2019 (COVID-19) pandemic has hardly affected the Indonesian health care system, including the National Tuberculosis Control Program. While the end of the COVID-19 pandemic in Indonesia is uncertain, the measure to control tuberculosis must not be weakened. Early identification and measurement of the problem size are essential to decide the most appropriate approach to maintain the sustainability of National Tuberculosis Control Program, particularly in health care facilities during the COVID-19 pandemic. This article points out the possible threats to the sustainability of TB care in Indonesia during the COVID-19 pandemic, including some approaches to overcome those problems.
