Abstract

BACKGROUND Burns constitute a severe health problem in many countries. In Indonesia burns rank 4th of all trauma-related diseases and are a burden on the country’s health system. Adequate fluid resuscitation is the initial management of burns that determines the success of treatment. This study aimed to determine the relationship between adequate fluid resuscitation and incidence of acute renal failure in burn patients. METHODSA retrospective study of cross-sectional design was conducted on 30 burn patients who came to the Emergency Unit (ER) from January 2015-December 2017. Medical records were reviewed to examine the data on fluid resuscitation according to the Parkland formula and the laboratory data. Acute renal failure was defined as a creatinine level of more than 2.1 mg/dL after 7 days. Hypoalbuminemia was defined as an albumin level of less than 3.4 g/dL. Fisher’s exact test was used to analyze the data.RESULTSTwenty-two subjects received fluid resuscitation according to the Parkland formula and 8 did not. Twenty-five experienced complications such as acute renal failure (ARF) (13.3%), hypoalbuminemia (46.7%) and a combination of ARF and hypoalbuminemia (23.3%). One person died. Adequate fluid resuscitation was significantly associated with decrease incidence of ARF (p=0.015), but not significantly with hypoalbuminemia (p=0.214) and with mortality (p=0.267).CONCLUSIONAdequate fluid resuscitation decreased the incidence of ARF in burn patients. Consensus protocols for initial burn resuscitation and treatment are crucial to avoid the consequences of ARF after burn injury.

Abstract

Healthcare-associated infections (HAIs) is a great issue to concern in healthcare services because it accounts for prolonged hospital stay and may lead to morbidity or mortality. Proper hand hygiene behavior in the healthcare environment is indispensable in minimizing the risk of HAIs. This study aimed to identify the impact of hand hygiene education on healthcare workers' (HCWs) compliance, knowledge, and perception of hand hygiene at Atma Jaya Hospital, Jakarta, Indonesia. The compliance of hand hygiene was evaluated using the World Health Organization’s (WHO) five moments for hand hygiene as the reference. Data on hand hygiene knowledge and perception were collected using a translated version of the WHO’s hand hygiene knowledge questionnaire and hand hygiene perception questionnaire, respectively. Results showed a significant increase in compliance with hand hygiene after the online session’s intervention had been implemented, except hand hygiene compliance for the ‘after touching patient surroundings’ moment. There was also a significant increase in the HCWs' hand hygiene knowledge after the intervention. The HCWs' perceptions are also found to be positive even before the intervention. This study concludes that the HCWs' knowledge and compliance with hand hygiene at Atma Jaya Hospital increase significantly after an educational intervention had been conducted.

Abstract

BACKGROUND: MicroRNA (miRNA), short noncoding RNA, plays role in various physiological process such as aging through epigenetic regulation of gene expression. miRNA present intracellular as well as extracellular in body fluids. miRNA that present in blood circulatory system is often referred as circulatory miRNA (c-miRNA). A number of studies trying to identify c-miRNAs as biomarker for ageing have been reported, but majority did not yield results that corroborate one with another. This study reports the identification of a differentially expressed c-miRNAs between elderly and youth groups of individuals, the first step in tracking specific miRNAs that play role in physiologic ageing.METHODS: The miRNA expression profiles of grandfathers and grandsons from 2 Javanese families were compared to select 5 miRNA candidates with widest expression difference. The 5 candidates were subjected to validation using quantitative polymerase chain reaction (qPCR) in 11 elderly men and 9 young men of the same ethnicity to identify differentially expressed miRNA between elder and younger male groups in the represented population.RESULTS: Amongst 5 selected c-miRNA candidates, the hsa-miR-376c-3p was validated to be upregulated in the elderly group when compared to the young individuals. Bioinformatic analysis using miRTarBase 7.0, miRTargetLink Human and GeneCards® Human Gene Database suggest the involvement of hsa-miR-376c-3p in pathways relevant with cellular ageing.CONCLUSION: This study showed that hsa-miR-376c-3p in the circulating plasma to be significantly upregulated in a group of elderly Javanese males compared to their younger counterparts. The results of this study warrant further study to elucidate the specific role of hsa-miR-376c-3p in physiologic ageing mechanism.KEYWORDS: circulating, microRNA, miR-376c-3p, elderly, Javanese, male

Abstract

Maturation of female reproductive organs begins in adolescence. Education about vulvar hygiene is one of the efforts to improve the health of reproductive organs. The purpose of the study was to determine the level of knowledge and changes in attitudes and behavior of adolescent students towards vulva hygiene after receiving education about vulvar hygiene. The research method was pre-test and post-test using a questionnaire, a sample of 83 respondents through the inclusion criteria. Education about vulvar hygiene was given after the pre-test. The post-test was conducted immediately after the educational session and repeated one month later. The results of the study, the mean value of the pre-test regarding knowledge, attitudes, and behavior was 13.87; 16.57; and 4.78. The mean value of the post-test one month later regarding knowledge, attitudes, and behavior was 26.87; 44.90; and 12.53. Paired T-test on knowledge, attitudes, and behavior showed a p-value of 0.000; 0.000; and 0.000. Conclusion: It was found that the value of knowledge, attitude, and behavior was low before being given education about vulvar hygiene. There was a significant change in value after providing education about vulvar hygiene. This shows the importance of education about vulvar hygiene in adolescent students. Keywords: Vulvar Hygiene, Knowledge, Adolescent

Abstract

Background: MALT (Mucosa Associated Lymphoid Tissue) Lymphoma is an uncommon condition. It is an involvement of extranodal site in hematologic malignancy, Non-Hodgkin Lymphoma.Case Presentation: Pondok Indah, Puri Indah Hospital Jakarta treated a 38-year-old female with acute abdomen due to jejunal MALT Lymphoma perforation, one meter from treitz ligament. Emergency laparotomy was performed, the patient had a resection and end to end anastomose. She was hospitalized in intensive care unit for 4 days after the surgery and died because of prolonged septic shock.Conclusions: MALT lymphoma is an uncommon condition which is often encountered in emergency conditions due to acute abdominal pain caused by intra-abdominal perforation of the hollow viscus. Late diagnosis and treatment may cause severe outcome such as perforation and hypovolemic shock. This case report is expected to provide insight and to increase the awareness of the incidence of jejunal MALT lymphoma in Indonesia.

Abstract

Jumlah populasi lanjut usia (lansia) di Indonesia terus meningkat dan menyebabkan kerentanan terhadap kondisi terkait penuaan. MicroRNA (miRNA) telah diketahui mengatur berbagai proses biologi di manusiatermasuk penuaan dan mungkin patogenesis penyakit terkait usia. Penelitian ini bertujuan untuk mengidentifikasidan menganalisis secara in silico hsa-miR-331-3p di plasma darah yang diduga berperan dalam penuaan danmungkin penyakit terkait usia tertentu. Penelitian ini bersifat deskriptif dengan metode potong-lintang dan analitikmenggunakan pendekatan bioinformatika. Penelitian dilakukan pada Januari 2016 sampai Desember 2018 diLaboratorium Biomedis FK Unika Atma Jaya dan Laboratorium mirXES di Singapura. Plasma darah berasaldari dua pasang kakek-cucu laki-laki dengan usia kakek = 65 tahun dan usia cucu 17-25 tahun. Setelah RNAplasma darah diekstraksi dan dilakukan profiling menggunakan metode RT-qPCR, diperoleh daftar 50 miRNAyang diekspresikan secara berbeda bermakna dalam dua kelompok umur (tua dan muda) dengan nilai FoldChange (FC) antara 1,4 – 10,7. Berdasarkan daftar tersebut, dipilih hsa-miR-331-3p, kemudian dilakukan analisisin silico untuk memprediksi gen target dan jalur persinyalan biologisnya. Hasil studi ini adalah hsa-miR-331-3pyang upregulated pada orang tua diduga mentarget kuat gen ERBB2 dan NRP2 yang mungkin berperan dalammekanisme penuaan kronologis. Masih diperlukan penelitian lebih lanjut untuk mengetahui apakah hsa-miR-3313pturut berperan dalam mekanisme penuaan maupun patogenesis penyakit kanker.

Abstract

Tujuan: Penelitian ini bertujuan untuk mengetahui tingkat pengetahuan orang tua mengenai sirkumsisi serta hubungan pengetahuan dan sikap orang tua terhadap penentuan waktu untuk dilakukan sirkumsisi. Penelitian dilakukan pada orang tua peserta bakti sosial sunatan massal yang diselenggarakan di Markas Kopassus Grup 1, Serang, Banten pada 24-27 Oktober 2018. Metode: Penelitian analitik dengan disain penelitian cross sectional. Peneliti menggunakan kuesioner. Sampel penelitian ini diambil dengan menggunakan purposive sampling sebanyak 224 responden. Hasil: Hasil penelitian menunjukkan sebanyak 68,8% responden memiliki pengetahuan cukup mengenai sirkumsisi dan 31,3% responden memiliki pengetahuan kurang. 37,1% responden memiliki sikap yang sesuai terhadap waktu untuk melakukan sirkumsisi dan 62,9% responden tidak sesuai. Kesimpulan: Tidak ada hubungan signifikan antara pengetahuan mengenai sirkumsisi dan sikap orang tua terhadap waktu untuk melakukan sirkumsisi dengan nilai p=0,75.

Abstract

Students’ perceptions regarding the learning environment can be used in improving the quality of the learning environment. This study aims to evaluate the differences between expected and actual learning environment perceptions of students after going through a clinical rotation. This pilot study is an observational descriptive study that was conducted in the Medical Faculty of Atma Jaya Catholic University of Indonesia on students that have completed pre-clinical education and have entered clinical rotation in January – March 2020. This study used the Dundee Ready Education Environment Measure (DREEM) questionnaire. The data was collected from 59 students and was analyzed using SPSS 23.0. The total actual DREEM score was better than the total expected DREEM score. In the Students’ Academic Self-Perceptions (SASP) domain, there was a decrease in the actual DREEM score. Meanwhile, there was an increase in the Students’ Percepstions of Teachers (SPT) domain which was statistically significant. The actual DREEM score in the Students’ Perceptions of Learning (SPL) domain also showed better results in males than females (P < 0.05). Quality improvement and continuous innovation are very important in medical education.
