ANALISIS KUANTITATIF KELENGKAPAN PENGISIAN FORMULIR PERSETUJUAN TINDAKAN KEDOKTERAN KASUS BEDAH

Abstract

AbstrakBerdasarkan studi pendahuluan bahwa dari sepuluh formulir persertujuan tindakan kedokteran yang diambil secara acak seluruhnya atau sebesar 100% tidak terisi lengkap. Pada empat review yang dilakukan untuk menelaah formulir yaitu review identifikasi, laporan isi informasi, autentikasi, pencatatan tidak ada yang semuanya terisi lengkap. Tujuan dari penelitian ini yaitu melakukan analisis kuantitatif kelengkapan pengisian formulir persetujuan tindakan kedokteran di RSUD Ambarawa.Jenis penelitian yang digunakan adalah deskriptif. Sampel penelitian ini adalah formulir persetujuan tindakan kedokteran kasus bedah di RSUD Ambarawa pada triwulan 1 tahun 2018. Besar sampel dihitung menggunakan rumus Solvin dan didapat 96 sampel. Metode pengumpulan data yaitu observasi dan studi dokumentasi. Hasil Penelitian menunjukan bahwa review identifikasi pasien terisi dengan lengkap sebesar 99%. Review lsi informasi (laporan penting) terisi dengan lengkap sebesar 5%. Review autentikasi terisi dengan lengkap sebesar 46%. Review pencatatan tidak ditemukan kesalahan. Pengisian formulir persetujuan tindakan kedokteran kasus bedah di RSUD Ambarawa sebesar 4 %  Abstract[English Title : Quantitative Analysis Completeness Form Informed Consent Case Surgery In RSUD Ambarawa] Based on a preliminary study that out of ten forms of intentional medical action taken completely randomly or at 100% incomplete. In the four reviews conducted to review the form of identification reviews, information content report, authentication, recording none of which are completely filled. The purpose of this research is to conduct a quantitative analysis of completeness informed consent form at RSUD Ambarawa. The type of research used is descriptive. The sample of this research is the informed consent form of medical case at Ambarawa General Hospital in first quarter of 2018. The sample size is calculated using Solvin formula and obtained 96 samples. Methods of data collection are observation and documentation study. The results showed that the patient identification review was completely filled by 99%. Review information filled (important reports) with complete 5%, Authentication review complete with 46% complete. recording review there is no mistake. Completed the form of approval of the medical action of the surgical case at RSUD Ambarawa at 4%
