Abstract

Menurut WHO pada tahun 2014, lebih dari 1,9 milyar populasi dunia mengalami overweight dan 600 juta di antaranya dengan obesitas. Kondisi ini disebabkan oleh asupan kalori yang melebihi pengeluaran energi disertai berbagai faktor. Modalitas terapi yang ada saat ini terbatas pada diet, farmakoterapi, dan operasi. Tujuan dibuatnya literature review ini yaitu menemukan tanaman obat yang berpotensi sebagai antiobesitas. Studi ini menggunakan metode tinjauan pustaka dengan mengumpulkan sumber dari literatur ilmiah seperti WHO, PubMed, dan Google Scholar. Ekstrak Hibiscus sabdariffa ditemukan dapat menurunkan oxysterols dan meningkatkan ekskresi asam palmitat di feses yang penting dalam mengontrol glukosa dan lemak. Ekstrak ini menginhibisi SREBP-1c, PPARγ, lipase pankreas, serta ekspresi PPARγ dan C/EBPα. Ekstrak Gynura procumbens mampu menurunkan pembentukan LDL sehingga berefek terhadap kolesterol dan lemak lain serta memiliki efek kardioprotektif. Ekstrak ini dapat meningkatkan ekspresi gen PPAR-α yang meningkatkan oksidasi asam lemak dan menginduksi aktivitas lipoprotein lipase. Teh rosela dibuat baik dengan metode infusi, decoction, maupun maserasi. Teh daun sambung nyawa dikonstruksi melalui deaktivasi enzim primer dan sekunder, rolling, pembekuan, serta vacuum freeze-drying. Kombinasi keduanya akan dibuat menjadi bentuk teh, kemudian diadministrasikan secara peroral. Disimpulkan bahwa, kombinasi teh rosela dan daun sambung nyawa mengandung senyawa antiobesitas yang berpotensi sebagai terapi untuk penderita obesitas.

Abstract

Semen gigi digunakan sebagai bahan pengisi, lapisan pelindung rongga, bahan luting untuk mahkota, jembatan, inlay dan peralatan ortodontik. Semen gigi memiliki kekuatan mekanik yang rendah sehingga memerlukan bahan tambahan untuk meningkatkan kekuatan mekaniknya, seperti dengan menambahkan nanopartikel Seng oksida (ZnO). Tujuan penelitian ini untuk mengetahui pengaruh penambahan massa nanopartikel ZnO terhadap kekuatan tekan dan kekerasan semen kedokteran gigi. Penelitian ini menggunakan metode penelitian menggunakan telaah sistematis dengan memasukkan kata kunci pada database Google Scholar, Pubmed, ProQuest dan Science Direct dengan rentang waktu 2012-2022. Telaah sistematis ini dibuat berdasarkan guideline PRISMA-P dan dievaluasi dengan kriteria JBI Critical Appraisal tools.  Kriteria inklusi pada review ini adalah seluruh studi eksperimental yang sesuai dengan topik penelitian. Hasil pencarian berdasarkan kata kunci didapatkan 3.972 jurnal. Setelah dilakukan inklusi dan eksklusi didapatkan lima jurnal yang relevan untuk studi ini. Dari lima jurnal tersebut didapatkan bahwa kekuatan tekan dan kekerasan semen gigi terus meningkat seiring penambahan massa nanopartikel ZnO. Hal ini disebabkan karena partikel berukuran nano dapat mengisi ruang kosong antar partikel semen yang berukuran besar saat berikatan, juga meningkatkan ikatan interlock menjadi lebih kuat dan lebih homogen. Dapat disimpulkan bahwa terdapat pengaruh penambahan massa nanopartikel ZnO terhadap kekerasan dan kekuatan tekan semen kedokteran gigi.

Abstract

Tuberculosis infection among pregnant women often reveals unspecific clinical manifestations, and cutaneous tuberculosis is a rare form of extrapulmonary tuberculosis. The aim of this case report is to manage well Extra-Pulmonary tuberculosis manifestation, especially in pregnant women. A Pregnant woman with a gestational age of 6-7 weeks complained of red spots on cheeks. One year ago, the patient suffered cutaneous tuberculosis and was only treated with Rifampicin, Isoniazid, and Levofloxacin; pyrazinamide and ethambutol were not given due to a strong suspicion of allergy. Physical examination revealed plaque with scales, and PCR examination revealed Mycobacterium tuberculosis. She suffered relapse of cutaneous tuberculosis and was given rifampicin, isoniazid, and pyrazinamide. After 2 weeks the skin lesion improved significantly, and ethambutol was added to the regimen. The patient suffered from drug eruption, ethambutol was discontinued, and the skin lesion disappeared after four months of treatment. The regiment was continued for one year; showed better condition and delivered a healthy baby boy. Cutaneous tuberculosis is a rare form of extrapulmonary tuberculosis (1-1.5%) and is hard to diagnose and treat, especially in pregnant women. Allergic to one of its drugs can increase its risk of being resistant and increase its mortality and morbidity. As conclusion, prompt treatment is needed for tuberculosis patients, especially pregnant women. Early detection and examinations are required to diagnose it properly.

Abstract

Doxorubicin remains the most prescribed anticancer agent despite its unintended side effects on non-target organs. A limiting-dose strategy is used to lower incidence of cardiotoxicity. Andrographolide has therapeutic effects including antioxidant and anti-inflammatory. This study aimed to assess cardioprotective effects of andrographolide oral on lactate dehydrogenase (LDH), creatine kinase-myocardial band (CK-MB), and relative cardiac weight in doxorubicin-induced rats. Sixteen male rats Sprague Dawley randomized into four groups: receives saline i.p and vehicle orally (Normal), doxorubicin 16 mg/kgBW i.p and vehicle orally (Dox), doxorubicin 16 mg/kgBW i.p+andrographolide 30 mg/kgBW orally (Dox+And30), doxorubicin 16 mg/kgBW i.p+andrographolide 60 mg/kgBW orally (Dox+And60). Blood was collected via cardiac puncture and cardiac organs were weighed after four-weeks administration. Total LDH and CK-MB measured spectrophotometrically. LDH and CK-MB levels significantly elevated, and signs of acute toxicity in Dox group compared with Normal group. Co-treatment with andrographolide at 30 mg/kgBW and 60 mg/kgBW reduced signs of toxicity and significantly attenuated LDH and CK-MB levels compared with Dox group (P<0.05 and P<0.01). However, body weight and relative cardiac weight were not significantly different in all groups after co-treatment with andrographolide. In conclusion, andrographolide lowered LDH and CK-MB levels, therefore has a protective potency in alleviating toxic effects of doxorubicin.

Abstract

Type 2 Diabetes Mellitus (T2DM) demonstrates relative risk of cirrhosis towards livercancer. Although the relationship between of type 2 diabetic and cirrhotic toward HCC has beensuggested, the association between SREBP-1c and the number of prominent nucleoli hepatocyteshas not clearly explored. This study aims to prove the correlation between those variables byusing male Wistar rat model given fat diet continued with administration of Streptozotocininjections twice in low dosage and an injection of dietylnitrosamine once a week for 10 weeks. Atweek 16, they were sacrificed, the fresh liver tissue was processed for examining the expressionof SREBP-1c, while the fixative paraffin block was sliced and stained with Hematoxylin Eosin tocount the number of prominent nucleoli hepatocytes. The western blotting result demonstratedincreasing level of SREBP-1c in diabetic rat significantly. The liver sections showed nodulesconsisting of pleomorphic cells, inflammatory cells, and prominent nucleoli hepatocytes. Thecorrelation between level of SREBP-1c and total number of prominent nucleoli hepatocytes wasmeasured with non-parametric correlation method statistically. It demonstrated coefficientr=0,435 with p value=0,037. It is concluded that increasing number of prominent nucleolushepatocytes related to enhancing level of SREBP-1c in the process of T2DM towardsHepatocellular Carcinoma.

Abstract

Di masa pandemi Covid-19, foto rontgen menjadi umum digunakan untuk memeriksa pasien diduga Covid-19. Pada citra x-ray paru-paru yang terkena Covid-19 ditemukan adanya bercak putih atau flek. Namun, paru-paru yang memiliki flek ini tidak selalu disebabkan oleh Covid-19. Tujuan penelitian ini adalah untuk mengklasifikasikan beberapa jenis penyakit paru-paru dari citra x-ray, yaitu paru-paru dengan Covid-19, paru-paru dengan pneumonia, dan paru-paru yang memiliki flek dibandingkan dengan yang normal. Proses klasifikasi data pada penelitian ini dilakukan dengan membandingkan dua model yaitu CNN VGG-16 dan ResNet-50 dengan model yang telah dilatih sebelumnya. Metrik evaluasi yang digunakan dalam penelitian ini terdiri dari akurasi, presisi, sensitivitas, spesifisitas, skor F1, dan kecepatan waktu inferensi. Hasil menunjukkan bahwa VGG-16 lebih unggul dari ResNet-50 dalam hal kecepatan inferensi namun tidak dalam hal metrik evaluasi lainnya. Perubahan parameter juga menunjukkan hasil yang berbeda, epoch 200 adalah nilai optimal. Untuk mendapatkan hasil yang optimal diperlukan finetuning dengan menyesuaikan kondisi data yang digunakan. Sebagai simpulan, VGG-16 memiliki kemampuan klasifikasi yang lebih baik dibandingkan ResNet-50, namun perlu terus dikembangkan dengan memperbanyak data klinis yang aktual.

Abstract

The intake of a low-protein diet during pregnancy can lead to alteration of fetal programming with long-term postnatal consequences exposing offspring with malnutrition to metabolic syndrome in adulthood. Accordingly, a more affordable alternative source of protein, such as soy, is used to improve nutrition. The objective of this study was to examine the advantages and disadvantages of soy as an alternative protein source to improve offspring nutrition with Intrauterine Growth Restriction (IUGR). The method used was a scoping review, and the design was selected to provide coverage on a certain topic with the concepts from available literature. Systematic searches were performed in six databases: PubMed, EBSCO, ScienceDirect, SCOPUS, Sage Journals, and Cochrane Library. Data collection included reports published from January 2013 to January 2023. Nine articles meeting the inclusion criteria were obtained and analyzed for review. Protein from a plant source is considered a good alternative in restoring nutrition to malnourished offspring in early life. Furthermore, the dietary deficiency of soy can be corrected with close monitoring. As a conclusion, the quality of life of offspring with IUGR is improved through a well-planned supplementation.

Abstract

Waktu pajanan layar adalah waktu yang dihabiskan secara pasif menatap hiburan ataupun permainan berbasis layar (TV, komputer, perangkat seluler). Kondisi ini banyak terjadi terutama pada masa pandemi Covid-19. Semakin tinggi waktu pajanan layar maka kemungkinan akan meningkatkan pula dampak negatif pada perkembangan anak yang akan memengaruhi kualitas sumber daya manusia dikemudian hari. Penelitian ini bertujuan untuk mengetahui hubungan waktu pajanan layar dengan keterlambatan perkembangan pada anak balita yang melibatkan 138 balita pada bulan Agustus sampai dengan November 2022. Penelitian ini didesain sebagai penelitian cross sectional dan dilakukan di Posyandu yang terdapat di wilayah Puskesmas Tanjung Duren Utara, Jakarta Barat, dengan menggunakan kuesioner waktu pajanan layar dan Kuesioner Pra skrining Perkembangan (KPSP) untuk menilai perkembangan balita. Data menunjukkan bahwa sebanyak 74 subjek (53,6%) memiliki waktu pajanan layar lebih dari 1 jam per hari (dikategorikan sebagai waktu pajanan layar tinggi) dan sisanya tergolong waktu pajanan layar rendah. Dari nilai KPSP yang diperoleh, 10 subjek (7,2%) diduga mengalami keterlambatan perkembangan, dan uji statistik menggunakan uji Mann-Whitney menunjukkan nilai p 0,000. Dengan demikian dapat disimpulkan bahwa terdapat hubungan yang bermakna antara lamanya waktu pajanan layar dengan kemungkinan keterlambatan perkembangan pada anak balita.

Abstract

Pure red cell aplasia (PRCA) is a rare disorder caused by failure or abnormalities in erythropoiesis only, among all cell lines of the bone marrow. Patients with PRCA usually present with no specific clinical presentation, thus its recognition depends on a clinician’s high index of suspicion, to facilitate early diagnosis and treatment to provide a better prognosis. Tuberculosis (TB) is the infectious disease with the highest casualty rate worldwide and is still a problem in many countries, including Indonesia. This study is a case report of a pulmonary TB patient with PRCA as a complication. PRCA was confirmed by the bone marrow puncture (BMP) examination as the gold standard. In this case, PRCA was inferred to occur due to the use of Isoniazid in the TB treatment regimen. The discontinuation of Isoniazid administration subsequently results in the normalization of Hb levels. In this paper we would highlight PRCA as a differential diagnosis that should be taken into consideration in symptomatic anemia in patients with tuberculosis under standard treatment.

Abstract

A long-term high-fat diet (HFD) was proven to induce metabolic dysfunction and causes various organ inflammation, including the retina. As the regulator of lipid metabolism, PPARa plays a role in retinal lipid metabolism and served as one of the targets for decreasing lipid deposition in the retina. Turmeric and mangosteen peel are Indonesian medicinal herbs with enormous health effects, including antiinflammation and hypolipidemic properties. This study aims to determine the effect of ethanol extract from turmeric and mangosteen peel on PPARa  gene expression in the retina of an HFD-induced rat model. Twenty male Wistar rats were divided into 5 groups: negative control, positive control, turmeric, mangosteen, and fibrate. At the end of the study, total cholesterol and triglyceride levels from the blood were measured. The retina was extracted to conduct Realtime PCR for PPARα gene expression. The result showed a significant difference in triglyceride levels between positive control and turmeric groups, and PPARa gene expression in the retina between the control negative, positive, and turmeric groups, but no significant difference was found in other groups. This study concludes that the extract of turmeric increases the expression of the PPARa gene expression in the retina in an HFD-induced rat model.
