Abstract

The type of study is quantitative descriptive with cross sectional study. The population of this study are neoplasm patients in September-Desember 2015 about 429 patients, with the sample are 81 documents. The sample collected by Simple Random Sampling technique. In this study, data collected by observation and noted with check list. Data analyzed by description and served using table. The result showed that the number of neoplasm cases at Sultan Agung Hospital of Semarang in February and March in 2016 are 414 patient. The accuracy of neoplasm code at Sultan Agung Hospital of Semarang is 0% accurate and 100 % inaccurate for morphological code. Then the neoplasm code is  48% accurate and 52 % inaccurate

Abstract

Based on the initial survey results indicate that the activity has not done well assembling proven medical records of 10 documents were taken at random showed as much as 34% incomplete and as much as 7% inconsistent. The research purposed to identify the completeness general consent, sign out sheet, initial assessment, and consistency of the general data of patients RSIA Kendangsari Merr Surabaya. This type of research is descriptive using retrospective study which collects data to look back on the causes or variables that influence a result. Conducted in April-July 2016. The sampling technique used purposive non-random sampling, on January until March 2016.The results showed the general consent highest data on incomplete answers is authentication patient / family / responsible (18%), on a sheet in and out of the highest incomplete is the date of authentication (74%), on a prior assessment of the highest data on the answer is incomplete is pain, and the last drug (100%). In the stuffing consistency shows name, date of birth, age, and gender shows the results as a whole (100%) is consistent. So the need to increase awareness and discipline officers assembling in equipping medical records, training, socialization, charging SPO manufacture of medical records

Abstract

Learning English cannot be separated from the four language skills namely Listening, speaking, reading and writing however grammar is also a main thing that must be mastered in English language mastery. Understanding the text content can also be started from the understanding of the language structure in the text. Therefore, this study aims to identify, classify, and analyze the types of clauses used on ICD -10 Volume 1. This study used a qualitative descriptive method which is divided into three stages: preparation, data collection and data analysis. The population in this study is ICD -10 Volume 1 and the sample of this study is ICD -10 Volume 1 chapter I, II, and III. The result  studyis according to the theories, on ICD- 10 Volume 1 there are two types of adjective clauses namely restrictive clauses and non-restrictive clauses but most of the adjective clause types is restrictive clause. The result of the study is expected to support all of ICD-10 users in order to understand English sentences on ICD-10 Volume 1 more.

Abstract

Archives have an important role in the process of presenting information for leaders to make decisions and satisfactory policy, therefore to be able to present complete information, quickly and correctly, there should be systems and procedures work well in the field to arsipan. Storage implementation in Islam Ibn Sina hospital pekanbaru ineffectiveness of the scanning process in the medical record file storage which is not destroyed so that when the file is needed requires a long time to look for it, it is constrained because no special power to do it. This study aims to investigate the implementation of medical record file storage which is not destroyed in Islam Ibn Sina Hospital Pekanbaru 2016. The research is descriptive qualitative research informants of 5 people, the technique used observation and interviews, data analysis using qualitative analysis, the triangulation method. The results showed that in the extermination medical record file shall be destroyed by burning exhausted all medical record file, there are 13 kinds of forms of medical records that are not destroyed and medical record file that is worth to be stored permanently in a way be scanned and stored on the hard disk in order to preserve the integrity medical record file of the damage and saves storage space outage

Abstract

Code is provision fixing code by using letters and numbers or combination of letters and numbers that represent data component. Based on the preliminary survey result from 10 medical record file on diagnosis of hypertension combination, heart disease and kidney disease founded 6 (six) of them are wrong code. The objective of this research is to determine of inaccuracy disease code for hypertension complication of heart disease and kidney disease at Islamic Hospital Ibnu Sina Pekanbaru. The method use in this research is mix method (quantitative & qualitative). The populations in this research are 52 of inpatient medical record file taken by total sampling. The result of this research obtained that inpatient medical record file which accurate in determining of combination code are 21 (40%) and inpatient medical record file which inaccurate in determining of combination code are 31 (60%). The factor that causes inaccuracy of hypertension code combination in heart disease and kidney disease is due to lack competence of medical recorder in coding.

Abstract

 Member of National Health Incurance (JKN) has the obligation to pay contributions themselves, therefore requires an understanding of JKN. That in mind people to participate in JKN. There are many contributing factors that include family heads themselves and their family participate in member of  JKN. These factors include the knowledge, attituade, information obtained, and family support. BPJS target JKN coverage in 2019 to be 100%.. The objective of this research to show the factors its relation with community participations in national health insurance at Tegalsari region of Ponorogo.  Thisresearch uses observatoinal study with case control approach. The populations for Tegalsari region head of family who had participated in JKN Mandiri are 149 participants. The chosen sample through proportinate and simple random sampling is 134 participants for casus and 134 no participant for control. Statistic test uses chi square. Base on the result, there is relation between knowledge and participation in JKN (p=0,000), relation between attitude and participation in JKN (P=0,002), relation between information obtained and partipation in JKN (p=0,026), relation between family support and participation in JKN (p=0,000, relation between income and participation in JKN (p=0,027)

Abstract

Indonesian Qualifications Framework (IQF) established in 2012 refers the Indonesian embodiment on quality and national identity associated with national education and training system. IQF is levelling of competences and qualifications framework to reconcile, equalize, and integrate the fields of education and job training and work experience in order to award the work in accordance with the recognition of the competence structure of employment in various sectors. There are nine (9) levels of IQF qualifications, starting with the first is lowest to ninth qualification is highest. The Bachelor of Science Health Information Management at the sixth level of IQF which currently did not exist a core curriculum standard in Indonesia, therefore it was needed to formulate a Baccalaureate Degree Curriculum on Health Information Management Model based on IQF. This was conducted on Bachelor of Science Health Information Management Esa Unggul University Jakarta in August-September 2016 by using the Guidelines of Curriculum Directorate of Higher Education and Student Learning, Ministry of Research Technology and Higher Education Higher Education Republic Indonesia. There are 5 (five) the Bachelor of Health Information Management Profiles successfully established include A Specialist Coding Clinical, A Data Analyst and Manager of Health Information, A Manager of Health Information Management Department, A Specialist Clinical Documentation Improvement (CDI), An Initiator on Designer and developer Electronic Health Records or Electronic Medical Records. Total credits 144 and eight semesters or four years.

Abstract

Medical records aimed at the orderly administration in an effort to improve health services in the hospital. One of the forms used in the inpatient medical record service is a medical resume . The medical resume form in hospitals Salatiga still did not agree with the theory, from the aspect of anatomy, physical and contents. This study aimed to analyze the design of medical resume form  in Salatiga City Hospital from the aspect of anatomy, physical and contents. And also provide design solutions design medical resume form  accordingly. This type of research is descriptive qualitative research. This research was conducted in Salatiga City Hospital on May 19 to May 26 in 2016, using the method of observation, interviews, documentary studies and FGD (Focus Group Discussion). Data processing techniques used in this study is reduction, display, conclusion drawing and verification. Observations based design medical resume form in Salatiga City Hospital that aspect has not been included heading anatomy complete form and lack of instruction. From the physical aspect is not well seen from the still opaque paper with a weight of 60 grams. While aspects of the content has been effective because it has provided to record the necessary data to the back of the form, but there has been no cause of death certificates.

Abstract

This code is flexible and accurate if it suitable with patient condition. In syafira hospital still found unaccuracy diagnosis code of patient hospitalization. This thing can be seen from the first time of survey (73,63) minutes that needed for 10 diagnoses only in 1 officer of coding. The purpose of this research was to know the relationship of coder workload and diagnosis code of accuracy patient hospital in pekanbaru 2016.The kind of this research is quantitative descriptive research. The sample was 114 persons which is random by using univariat and bivariat analysis with Chi Square.There is different of result in this research. Where can be seen from the accurate diagnosis; 84(73,7%),  while there was bad accuracy 30(26,3%). The high workload 26 (22,8%) and the low workload; 88(72,2%). There was relationship the workload and code accurate P Value 0,00<0,05.

Abstract

RSI Sultan Agung Semarang has 12 registration officer. The purpose of this research is to know the needs of registration officers on WISN method in RSI Sultan Agung Semarang in 2016. This research is descriptive research with cross sectional time approaches. Sampling techniques are used accidental sampling techniques. A method of the data used in this research are observation and interview. Based on observation we can conclude that registration for new patient that outpatient needs 7 minutes, while registration for old patient that outpatient needs 3 minutes, registration for patient that use insurance needs 3 minutes, registration patient by phone needs 3 minutes, and  admission patient needs 12 minutes. Based on the calculation with WISN formula, in order to make a better service we need to add one more registration officer
