Abstract

Dengue hemorrhagic fever (DHF) is an infectious disease that can result in death, caused by dengue virus and transmitted through Aedes aegypti mosquitoes. The incidence of dengue fever every year, requires more serious attention to increasing awareness of the disease. The purpose of this study was to determine the Maya Index (MI) and density of Aedes aegypti larvae in Labuh Baru Timur subdistrict, Payung Sekaki, Pekanbaru city. Method of the reseach was survey research using questionnaire and larva identification using single larva method method. A total of 131 houses that have been surveyed got 589 containers. MI status based on breeding risk index and hygiene risk index, found high as many as 23 houses (17.58%). Free number of larvae of 67.94, Container index (CI) of 8.32, House Index (HI) 32.06 and Breteau index of 37. HI and CI figures are still high that has not met the government target. The density of larvae is on the 3 - 5 scale, ie the medium category, meaning that the area has a medium transmission risk to the spread of DHF.

Abstract

Stroke is the first cause of death in women and the second leading cause of death in men in industrialized countries. In Asia, every year an estimated 500 thousand people experience a stroke. Therefore recognizing stroke risk factors and managing them appropriately is key to designing primary prevention strategies in non-stroke populations and secondary prevention to avoid recurrent strokes. This research was cross sectional study to describe profile stroke patients in Arifin Achmad hospital Riau Province. Research data was taken through questionnaire and patient medical records. In this study, stroke patients in Arifin Achmad Hospital were mostly <60 years old, 65.80% and male sex 64.50%. The proportion of hypertension is 90.80%. Diabetes mellitus 7.90%, dyslipidemia 63.20%, smoking 21.10%, obesity 32.90%, and sedentary life 73.70%.

Abstract

Successful treatment of acne vulgaris (AV) can impact patient’s quality of life. AV treatment outcome is associated with the patient adherence. The aim of this study was to know the association between acne vulgaris and adherence among medical students of Universitas Riau. The samples were students with AV according to inclusion criteria as 39 respondents. In this study, respondents got check the grade of the AV according to Lehmann classification, the adherence was measured using the translation and modification of Elaboration d’un outil d’evaluation de l’observance des traitements medicamenteux (ECOB) Questionnaire that has been tested. In this study, mild AV was found among 69.23% of the respondents as the most AV grade found among the respondents. Adherence rate was found to be poor as 53.85% of the respondents have poor adherence to medication. Statistic test showed there was a correlation between AV grade and adherence with the p-value of 0,006 (p<0,05).

Abstract

Stenotrophomonas maltophilia is an important multidrug-resistant nosocomial pathogen associated with high mortality. This infection did not elucide clearly in Riau. We reported two Cotrimoxaxole Resistant Stenotrophomonas maltophilia in patient of Arifin Achmad Hospital which refered from peripheral hospital in another region. They had similar clinical appereance such as long stay hospitalization, bad clinical appearance and had a history of meropenem exposure before. We performed urin culture to make appropriate diagnostic and treatment also. Stenotrophomonas maltophilia bacteria was found in urine isolate. The sensitivity test showed that this bacteria had Cotrimoxasole resistant that standard antibiotic of this bacteria. The management of these cases become complicated due to decrease of renal function in one patient. We had to make dose adjustment of parenteral levofloxacin, giving good nutrition,  counted fluid balance and repaired others clinical conditions.Beside of its high mortality and morbidity, the outcomes of both these patients were good.

Abstract

Taenia solium taeniasis (taeniasis) and cysticercosis in human are infectious diseases caused by pork tapeworm Taenia solium. They are  zoonotic diseases and still a health problem in the world, especially in developing countries such as Indonesia. The prevalence of taeniasis and cysticercosis in Indonesia is still quite high at 2% - 48% with the highest prevalence in Papua. Human taeniasis can manifest as a asymptomatic or intestinal minimally symptomatic. Cysticercosis can involve of neural tissue (neurocysticercosis) may manifest epilepsy seizure, headache, neurological symptom, cognitive disabilities, and death.  The habit of eating raw or undercooked  pork, lack of sanitation, and a lack of supervision the slaughterhouse are some of the factors causing taeniasis and cysticercosis in human. Avoiding eating raw pork, cooking meat over temperature 54oC, supervision at slaughterhouses, and maintaining hygiene is a way to prevent taeniasis and cysticercosis, therefore it is necesarry to do more intensive programs to prevent and control the diseases.

Abstract

The upfront payment policy in the hospital caused controversy and rejection from the society. To anticipate this, some hospitals apply no upfront payment policy. However the policy causes an increase in bad debts. Seeing this dilemma, the authors will conduct research to analyze the different of hospital financial performance before and after no upfront payment policy. The research method used is comparative descriptive with analyzed by paired sample t-test. The result was no upfront payment policy has a positive impact on hospital financial performance. The results of this study were expected to be an input for hospital management and government to revise and refine on the upfront payment policy.

Abstract

Provision large amounts of fluids in a short period is known can cause hypervolemia. Therefore, an examination is needed to find out that the fluid resuscitation being administered does not cause hypervolemia. The purpose of this study is to assess the effect of hypervolemic resuscitation on cardiac contractility. The study was conducted on 10 male Sus Scrofa aged 6-8 weeks, as shocked animal models. There are 3 types of resuscitation treatments : normovolemic, hypervolemic-1, and hypervolemic-2. Cardiac contractility was assessed using DPmax and troponin-i levels. There was an increase in troponin-i levels after hypervolemic fluid resuscitation (p = 0.05). There is a decrease in cardiac contractility after hypervolemic resuscitation. Decreased cardiac contractility is associated with increased troponin-i (r = 0.720; p = 0.020). Based on the results, we conclude hypervolemic resuscitation causes changes in troponin-i levels, which reflect changes in cardiac contractility.

Abstract

Endometriosis is endometrial tissue (glands and stroma) that grow outside of the uterus. The goal of this research was to see the profile of endometriosis cases in Arifin Achmad general hospital in Riau province. The research method used is descriptive with cross-sectional approach. Based on patient characteristics, highest education was SMA (47,9%), majority patients cames from Pekanbaru city (58,8 %), the patients were at reproductive age (92.6%), most patients were nullipara (54.8%). Anatomical location were 45.1% located in the ovarium. The most common clinical symptom was dysmenorrhea (36.2%), 49.5% were stage 4, and the most frequently performed was laparotomy (71.8%). The conclusionof this study were endometriosis found primarily in reproductive and nulliparous ages, the most common anatomic location was in the ovarium and classified in stage 4, the most clinical symptom was  dysmenorrhea, and the most common operative action was laparotomy.

Abstract

Accumulation of triglycerides associated with  insulin resistance in type 2 diabetes mellitus (T2DM). Alpha lipoic acid (ALA) has hypotriglyceridemic effects by lowering triglyceride levels in blood and liver. This study aims to analyze effect of ALA on triglyceride levels in  type 2 diabetic rats. Fifteen Wistar male rats (±8-12 weeks, 200-250 gram) divided into 3 groups (control, T2DM  and  T2DM–ALA given for 3 weeks). Diabetes was induced by giving streptozotocin (50 mg/kg.bw) and nicotinamide (110 mg/kg.bw) after 15 minutes intraperitoneally. Diagnosis of T2DM was determined if the rats have perifer blood glucose >250 mg/dL after 3 days. The results showed that triglyceride levels in  ALA group (p=0,003) lower than T2DM group, T2DM group (p=0,031)  had higher triglyceride level than control and no difference of triglyceride level between  T2DM-ALA given group with control (p=0,696). This study concluded that ALA can affect triglyceride levels in T2DM thus prevents occurrence of hypertriglyceridemia.

Abstract

Diagnosis of hepatic cirrhosis enforcement requires the anamnesis, physical examination, and examination of supporting. The purpose of this research is to know the description of examination of liver function. The results showed on the basis of age age category is the most 25-65 of the year a number of 188 men (89.1%), most of which is gender male category a number of 135 people (63.98%), bilirubin total showed an increase of more than 1.1 mg/dL number of 154 people (90.59%), albumin showed a decrease of less than 3.8 g/dL number of 181 people (92.34%), AST shows an increase of more than 45 u/L number of 162 people (77.14%) ALT, shows an increase of more than 35 u/L number of 146 people (69.52%), HbsAg is present on a number of 58 patients, and 1 patient is present on the HVC.
