Abstract

Background: Various controversies arose in the study of PSA examination as an alternative of biopsy to detect prostate malignancy. Prostate Specific Antigen (PSA) is a specific protein secreted by the prostate gland and is affected by various conditions. PSA levels will certainly increase at the enlargement of the prostate gland. An accurate cutoff point that can detect prostate cancer is needed.Objective: To evaluate the PSA diagnostic test against prostate malignancy.Methods: This study used a diagnostic test method for 91 patients undergoing either prostate gland surgery or open prostatectomy at Bethesda Hospital Yogyakarta in January 2014-January 2016 period. Data were taken from medical records with inclusion criteria as follow: over 50 years of age, preoperative PSA, and PA results. This study was a descriptive analytic study. In the diagnostic test AuROC, cutoff points were determined. Chisquare test was performed to assess sensitivity, specificity, PPV, NPV, LHR -, and LHR +.Results: We studied 91 patients with a mean age of 70.24 ( 46-54 ) and mean PSA level of 27.2 ( 0.59-101 ). The results of PA prostate tissue examinations were adenocarcinoma in 15 patients ( 16.5 % ) and BPH in 76 patients ( 83.5 %) On all PSA levels, AuROC were 0.90. Specificity of PSA with 4 ng / mL cutoff, 10 ng / mL, 20 ng / mL, 50 ng / mL, and 100 ng / mL were 0.53, 35.53, 67.11, 96.05, 98.68. When the cutoffs were reduced to 4.01-10ng/mL, 10.01-20 ng/mL, 20.01-50ng/ mL,50.01-100 ng/mL, and > 100 ng/mL, the specifity were 76.32, 68.00, 69.74, 96.05, 100.00. Cutoff value of 50ng / mL had LHR + >10.00 ( 15:20 ).Conclusion: There is a strong correlation between PSA and prostate malignancy. PSA value> 50 ng / mL has high accuracy to detect prostate malignancy. A biopsy is needed to determine a definitive diagnostic because no cutoff value can be used as a benchmark.

Abstract

Background: Sausage is  ready to eat meals for children, adolescents, and older adults. The meat contained in a beef sausage might not be consistent to the one listed on the label. Fourier Transform Infrared (FTIR) spectroscopy combination with a chemometric method is one of the most used methods  to detect wild boar fat with rapid and consistent results. Results of analysis can classify fatty acid composition contained. Objective: The aim of this study is to determine the fat profile in sausage samples and differentiate the grouping of wild boar fat and beef fat in sausage preparation. Methods: The study was designed by making 7 different variations of wild boar reference sample concentrations, which were 100%, 75%, 65%, 50%, 35%, 25% and 100% beef. Six other samples were gathered from various street vendors. The results were analyzed using FTIR spectroscopy combined with chemometric with Partial Least Square (PLS) and Principal Component Analysis (PCA). Result: The results of analysis using Horizon MBTM apps showed optimal wave number within 1250-900 cm-1. The results of calibration with equation y= 0.994x + 0.334 and the value of (R2) determination was 0.998, and root mean square error of calibration (RMSEC) was 1.22%. The results of validation using parameter value of root mean square error of cross validation (RMSECV) 2.68%, and root mean square error of prediction (RMSEP) was 0.11%.Conclusion: PCA result showed that five from six samples possibly have same physical and chemical properties similar to fatty beef sausage.

Abstract

Background: Diabetes mellitus patients may have a different response tosimilar medication. Therefore, individualized treatment and monitoringof the therapy is needed to ensure the patients’ response. Alternativemedication or dose adjustment should be considered in order to achievethe therapeutic goal.Objective: The aim of this study was to find out patients’ response tomedication variation or dose adjustment.Methods: This study used descriptive design in PKU MuhammadiyahHospital and Panti Rapih hospital period October-December 2014.Participants are all patients who got pharmacological therapy for treatmentof diabetes mellitus type 2, including oral antidiabetics and/ insulin andmet the inclusion criteria. The response of therapy was checked 2 times,during check-in and check-out.Results: Analysis of response variation showed that individualizedresponse existed, in which positive response occurred in 10 patients andthe other had a negative response. The changes were dose adjustmentand medication alteration or medication addition. From 6 patients whoneeded medication adjusment, 5 patients had a positive response, andothers had a negative response. Whereas from 6 patients whose dosage ormedication was not changed, 5 of them had a positive response while onehad a negative response.Conclusion: Individualized treatment is needed to achieve a therapeuticgoal. Individualized treatment can be done through dose adjustment andmedication alteration, or medications addition.

Abstract

Background: Inflammation is a protective physiological response to tissue injury that can be caused by harmful stimuli. If the inflammatory process is prolonged and cannot restore to homeostatic conditions, this may lead to pathological effects that can damage cells and cause various diseases. Elephantopus scaber is a plant that can easily be found in Indonesia. Elephantopus scaber is a type of plant that is often used as a traditional medicines. Several studies have shown that the compound bioactive content contained in plants has enormous potential as alternative medicine.Objective: This present study was to investigate the anti-inflammatory activity of ethanolic extract of Elephantopus scabe leaves.Methods: The Elephantopus scaber leaves were extracted using ethanol solvent into different concentration (50 mg/mL, 100 mg/mL, and 120 mg/mL). Diclofenac sodium was used as the standard. Anti-inflammatory assays were performed by the human red blood cell (HRBC) membrane stabilization method and heat-induced hemolysis method. Phytochemical screening that used in the present study was a conventional method.Results: Phytochemical screening showed the presence of flavonoids, tannins, and saponins. In the present study, ethanolic extract of Elephantopus scaber leaves has anti inflammatory activity by protecting the stability of red blood cell membrane. The highest protection capability possessed by the ethanolic extract of Elephantopus scaber leaves in both human red blood cell (HRBC) membrane stabilization method and heatinduced hemolysis method was at a concentration of 100 mg/mL.Conclusion: The ethanolic extract of Elephantophus scaber has antiinflammatory activities by in vitro assays.

Abstract

Backround: It is very important to know and detect various stroke symptoms because stroke is an emergency condition. Lack of knowledge about stroke symptoms leads to delay on stroke treatment.Objective: The aim of this study is to compare clinical symptoms and risk  factors between ischemic and hemorrhagic stroke.Methods: This study was a case control study. The subjects in this study were ischemic and hemorrhagic stroke patients. Subject data were recorded in electronic stroke registry at Bethesda Hospital, Yogyakarta.Results: The most common clinical symptoms in both groups were limb weakness (76.4% vs 71.4%), whereas the rarest was face drooping (2% vs 3.6%). Hypertension was the most common risk factor in both group (48% vs 71.4%), whereas atrial fibrillation was the rarest. This study did not find the specific correlation between any clinical symptoms to ischemic stroke incidence, however, decrease a level of consciousness was significant to hemorrhagic stroke incidence (OR: 2.738, 95% CI: 1.503-4.990, p: 0.001). Previous stroke (OR: 2.413, 95% CI: 1.314-4.433, p: 0.005) and dyslipidemia (OR: 4.862, 95% CI: 2.613-9.045, p: 0.000) were significant increasing risk of ischemic stroke. Hypertension was only the significant increasing risk of hemorrhagic stroke (OR: 3.680, 95% CI: 2.086-6.492, p: 0.000).Conclusion: Decrease level of consciousness has a significant correlation to hemorrhagic stroke incidence, but there is no specific symptom correlate to ischemic stroke incidence. Previous stroke and dyslipidemia are significant risk factors for ischemic stroke, whereas hypertension is the only significant risk factor for hemorrhagic stroke.

Abstract

Tuberculosis (TB) is still to be worlds major health concern. About 8,6 million cases occur in 2012 worldwide. TB doesnt mainly affect the lungs but it can affect extra pulmonary system. Central nervous system (CNS) is one complication of TB that should be considered as it is associated with high morbidity and mortality. Tuberculoma is one form of CNS TB that is rare and challenging in diagnostic. It will be presented a case of adult female with history of on-going TB treatment that presents with altered mental status, focal neurological signs, and increased ICP signs and symptoms. The head CT with contrast shows multiple ring enhancement with perilesional edema that consistent with brain tuberculoma. She was admitted to ICU and she got anti-TB drugs, and systemic steroid.

Abstract

Introduction: Peripartum cardiomyopathy (PPCM) is a disease characterized by an enlargement of the heart muscle wall. The disease occurs in the last month of pregnancy up to 5 months postpartum, with no previous history of heart disease. The etiology of the disease is unclear. Early recognition of this case can lead to decrease in morbidiy and mortality.Case: A 20-year-old woman came to the Emergency Unit with a shortness of breath. There is a history of give birth about 4 months ago. Physical examination shows a regular heart rhythm accompanied by rhonchi sound in the basal aspect of lung. In the M-mode analysis of ECG showed left ventricular systolic ejection function is decrease (56.5%) while the right ventricle is normal. In the Hospital, patient get treated with diuretics, aldosterone antagonist, angiotensin II receptor blocker and fluid management. Patient gave good responsed to the therapy regiment.Discussion: The early clinical manifestations of PPCM are difficult to recognize. In the majority of patients, 78% of symptoms are found at 4 months after delivery, only 9% of patients show symptoms in the last month of pregnancy. Clinical symptoms include atypical chest pain, abdominal discomfort, signs of pulmonary edema, orthopnea and paroxysmal nocturnal dyspnea. In the management of patients with PPCM is almost similar to management in other congestive heart disease.

Abstract

Background: Smoking has a negative impact on health. It makes an imbalance of the autonomic nervous system (ANS). It influenced the standard deviation of N-N interval (SDNN). SDNN has a duty as indicator for ANS controlled, consequently, SDNN in smokers is lower than non smokers because of ANS imbalance. Therefore, smokers have high risk for non communicable disease (NCD) in the future.Objective: This research’s purpose was to know the comparison of SDNN level among smokers and non smokers adolescent.Methods: This research was non-experimental study with descriptive comparative design and cross sectional approach. The samples divided into 2 groups with each of group consists of 20 students. They were chose by purposive sampling techniques. SDNN level was measured by electrocardiogram (ECG) for 5 minutes with the provision that they should no tea, caffeine, alcohol consumption and no heavy exercise within 2 hours before data collected, feel free from desire to go to bathroom, feeling relax when data collection were performed. The data was analyzed by Mann Whithey test.Results: There was a significant difference of between smokers’ SDNN and non smokers’ with p= 0.038 (p<0.05). Smokers’ SDNN was lower than non smokers’.Conclussion: Smokers’ SDNN level was lower than non smokers’.

Abstract

Background: Circumcision is the most widely minor surgery in the world, whether done by doctors, paramedics, or by traditional. Circumcision that removes part or all of the penis prepuce with the aim of removing smegma (dirt). Society circumcised their children at the age of 5-12 years. Analgesics may be given to patients, for instance, paracetamol, nonsteroidal anti-inflammatory drug (NSAIDs) such as aspirin and opioid groups such as tramadol.Objective: The aim of this study was to compare the effectiveness of paracetamol analgesics and tramadol in circumcised patients given prior to the act of circumcision.Methods:This study was a quasi-experimental study to determine different analgesic effects of paracetamol and tramadol administration, prior to circumcision. The subjects of this study are children aged 5-12 years as many as 36 children, which is divided into 2 groups of paracetamol and tramadol group. One hour before circumcision, subjects were given group-appropriate analgesics, then the Visual Analog Scale (VAS) was measured 1 hour after circumcision. The data obtained were made intopercentage on the scale of VAS <5 and> 5.Results: In the paracetamol group, there were 5 children (27.8%) with VAS 1-3 (mild pain), 13 children (72.2%) with moderate pain (VAS value 4-7). In the tramadol group, there were 15 children (83.3%) with mild pain (VAS score 1-3), and the remaining 3 children (16.7%) had moderate pain. No side effects of both analgesic drugs were found.Conclusion: Patients with mild pain complaints were more likely found in the tramadol group than in paracetamol group. Tramadol gave stronger preemptive analgesic results in circumcision compared to paracetamol.
