Background: According to the Regulation of the Minister of Health of the Republic of Indonesia Number 24 of 2022 concerning Medical Records, they are documents that contain patient identity data, examinations, treatment, procedures and other services that have been provided to patients. Hospital Minimum Service Standards Completeness of medical record filling 24 hours after completion must reach 100%. Objective: To determine the completeness factor in writing the patient's name in the patient's medical record at the hospital. Method: This research used the literature review method which was carried out on 14 journals and found incompleteness in writing patient names in hospital medical records. Results: Of the 14 journals regarding the completeness of writing patient names, none of the research results found were close to the very good category (100%). The highest percentage of completeness in writing patient names was found in research journals with a percentage of 98%, while the lowest percentage was 56%. Conclusion: From the results of a review of the completeness of writing patient names in hospital medical records, 14 journals were found to not have 100% completeness results and not all hospitals have SOPs regarding procedures for writing patient names. Suggestion: Create an SPO for procedures for writing patient names that comply with standards so that officers can carry out these activities with guidelines.

Abstract

Hospitalization is a condition where a child is sick and undergoing treatment in a hospital. This condition causes stress for children and parents. Stress in parents can have a negative impact on children because it affects the healing process. This research was carried out to see the effect of PMR therapy on reducing parental stress facing hospitalization in the children's inpatient room at Raja Ahmad Tabib Tanjungpinang Hospital in 2023. The research design is a quasi-experimental pre-test post-test design involving 2 groups, intervention and control group. The independent variable is PMR therapy and the dependent variable is stress level. The population in this study was 36 respondents, namely parents whose children were being treated in the pediatric inpatient room. The results of the analysis in the intervention group using the Paired T Test obtained a significant value of .001 < 0.05, so it could be said that Ho was rejected and Ha was accepted. There is a significant effect of Progressive Muscle Relaxation (PMR) Therapy on reducing the stress of parents facing hospitalization in the children's inpatient room at Raja Ahmad Tabib Hospital, Tanjungpinang. Meanwhile, in the control group, there was an increase in stress in the post-test group.

Abstract

The prevalence of hypertension in Indonesia reaches 34.11%, with the highest incidence of hypertension found in those aged 65 years and over at 63.22%. The number of elderly patients suffering from hypertension who undertake non-pharmacological therapy needs to be encouraged. Non-pharmacological therapy is always an option because it is not expensive, one of which is progressive muscle relaxation. Research objective: to determine the effect of progressive muscle relaxation therapy on reducing blood pressure in the elderly at the Tanjung Unggat Community Health Center UPTD. Quasi Experiment with One Group Pre-Post Test Design approach. The sample consisted of 26 elderly respondents who suffered from hypertension in the UPTD area of the Tanjung Unggat Community Health Center using a purposive sampling technique. The mean systolic and diastolic blood pressure before progressive muscle relaxation therapy was 152.50 mmHg and 94.81 mmHg. After treatment with progressive muscle relaxation therapy, the mean systole was 138.85 mmHg and the mean diastole was 88.62 mmHg. The results of the Wilcoxon Sign Rank Test statistical test obtained p-value = 0.000 (p < 0.05), namely H0 was rejected, which means there was an effect of providing progressive muscle relaxation therapy on reducing blood pressure in the elderly at the UPTD Tanjung Unggat Health Center. It is hoped that the elderly can apply progressive muscle relaxation to lower blood pressure.

Abstract

The aim of this research is to determine the Lumbar MRI examination procedure in LBP cases at the Bogor City Regional Hospital, namely STIR and Myelo Radial, the aim of which is to clearly see abnor malities in the intervertebral discs and stenosis in the cerebral spinal cord. This type of research uses descriptive qualitative methods with a case study approach. Data collection methods are carried out through observation, interviews and documentation. Then data analysis was carried out using open coding charts, so that conclusions could be drawn. The results of this study show that the Lumbar MRI examination procedure in cases of Low Back Pain (LBP) at the Bogor City Regional Hospital does not require special preparation, the patient comes to radiology for screening (installation of a pacemaker). The patient removes clothing and metal objects. Before the examination, the patient is asked to urinate first, the patient's position is supine (feet first), iso center 5 cm superior to the ASIS. Then it is briefly explained that during the examination you are not allowed to move and the duration of the examination is 15 minutes and the role of the sagittal STIR sequence and Myelo Radial. The role of the sagittal STIR sequence and myelo radial to suppress fat in the cerebral spinal fluid, conus medullaris and myelum in the spinal cord and to see masses Lesions and stenosis caused by narrowing of the bulging in the intervertebral disks are clear enough to provide a Lumbar MRI image for cases of Low Back Pain (LBP).

Abstract

Introduction: Cutaneous larva migrans (CLM) is a parasitic infestation that commonly occurs in warm climates among people who have contact with contaminated soil. This is caused by the migration of hookworm larvae into human skin. Clinically, itchy, erythematous, linear serpiginous tracts appear within days to months after exposure to infected sand or soil. Diagnosis is based on clinical presentation. CLM can heal on its own and usually resolves within a few weeks to several months without treatment . Case report: This case report is about a 30-year-old female patient with complaints of itching on the little finger of the left foot since 4 days ago accompanied by multiple erythematous papules with serpiginous arrangement, the surface of the left dorsal leg. Conclusion: CLM associated with hookworm is diagnosed clinically based on the typical clinical appearance, as well as a history of travel to endemic areas and exposure to contaminated soil/sand.

Abstract

Hyperthyroidism is a clinical condition caused by an increase in the concentration of thyroid hormone in tissues due to increased hormone synthesis by the thyroid gland in the form of increased release of endogenous thyroid hormone or exogenous extrathyroidal sources. Hypermetabolic hyperthyroidism reflects the patient's state of thyroid storm such as tachycardia, increased GI motility, diaphoresis, anxiety, fever, and manifestations of multiple organ dysfunction. Data collection in this case report was obtained based on anamnesis and physical examination. A 41-year-old male patient came to the emergency room at RSU Cut Meutia, North Aceh with complaints of fever accompanied by nausea, vomiting, loose stools, icteric sclera, and swelling in both lower legs. There has been a lump in the neck for 3 years and has been growing since 1 month. The patient was diagnosed with hyperthyroidism and has been taking anti-thyroid medication for 1 month. Patients often feel their hands shaking, and chest pounding, cannot stand hot weather, and often sweat excessively. The patient's jaundice was accompanied by increased liver function. The cause of liver failure in hyperthyroid patients is due to several factors. One of them is autoimmune which attacks autoimmune hepatitis. This can occur in 10% of patients with hyperthyroidism. This patient was given pharmacological therapy in the thionamide group, namely Thyrozole 1x10 mg, and beta blocker, namely Propranolol 1x 10 mg. Non-pharmacological therapy provides oxygen, bed rest, and adequate nutrition.
