Abstract

Pre-conference and post-conference are part of implementing the Professional Nursing Care Model (MAKP) to improve the quality of nursing services in hospitals and minimize negligence or errors in nursing practice. Pre-conference and post-conference is the activity of the team leader and members of the nursing team in communicating related to nursing care given to patients during shift changes. In pre-conference and post-conference, the nurse in charge of care must plan daily activities for patients from admission to discharge from the hospital. However, implementation pre-conference and post-conference still need to be improved. This can affect the nursing care provided by nurses. This writing aims to describe pre-and post-conference activities during the COVID-19 pandemic, which students in the inpatient room carried out. This research uses the case study method. Implementation research results in pre-conference are in a suitable category, while post-conference results could be better. Therefore to optimize the implementation of pre-conference and post-conference, nurses need to improve their quality as Nurses in Charge of Caring (PPJA).

Abstract

Living with diabetes can lead to various psychosocial problems that affect health and the ability to manage diabetes. Diabetes is a disease that must be managed every day by patients, causing increased stress. This study aimed to analyze the relationship between self-esteem and diabetes distress in patients with type 2 diabetes mellitus. This study used an analytical observational design with a cross-sectional method. The sample in this study was 84 type 2 DM patients. Sampling was carried out in this study using a non-probability sampling technique using consecutive sampling. Self-esteem is measured using the Rosenberg Self-Esteem Scale (RSES), while diabetes distress is measured using the Diabetes Distress Scale (DDS). Data analysis in this study used the Spearman rank (rs) statistical test. Respondents in this study had low self-esteem, as many as 51 people (60.7%). The diabetes distress respondents were in the moderate distress category, as many as 43 people (51%). Based on the results of the study, it was found that there was a significant relationship between self-esteem and diabetes distress in type 2 DM patients with a correlation value of -0.548, which means that the direction of the correlation is negative with moderate strength. This shows that the higher the self-esteem of type 2 DM patients, the lower the diabetes distress experienced. Nurses are expected to think about psychological aspects by providing counseling interventions to patients to increase self-esteem to reduce diabetes distress experienced by patients.

Abstract

Flood disaster preparedness is necessary to provide information to reduce the impact of the disaster on human life and surroundings. This research aims to analyze the effectiveness of health education about flood disaster preparedness students at elementary school students. This research used pre-experimental designs with one group pretest-postest design type. The Sample in this study involved 20 students in 5th grade obtained with total sampling. The research instrument utilized a flood disaster preparedness questionnaire. Before being given health education, most students' knowledge was in ready and almost ready categories, with seven students for each level (35%). The result of the post-test showed that students' knowledge levels were at a very ready category of as many as five people (25%), ready level of as many as five people (25%), and almost ready as many as eight people (40%). This study concludes that there were differences in students' knowledge before and after being given health education (p: 0.032, α: 0.05). This show that health education on flood disaster preparedness through audiovisual media effectively increases the knowledge students of in elementary school. Therefore, audiovisual-based education can be used as an alternative medium to provide health education for students.

Abstract

Emergency care causes patients fear and anxiety in the face of treatment, so it requires the nurse's therapeutic communication to manage anxiety. This study aims to determine the effectiveness of therapeutic communication in emergency patients who experience anxiety. The research design uses a pre-experimental research design with a one-group pre-test post-test design approach, and a total of 32 respondents with a simple random sampling technique. The results showed that all respondents experienced a decrease in anxiety after therapeutic communication. Therapeutic communication techniques often used are listening, giving open-ended questions, repeating, indicating acceptance, and providing information. Therapeutic communication with patients experiencing emergency conditions is necessary to provide the support patients need. Nurses' understanding of therapeutic communication techniques according to the patient's condition can improve the achievement of nursing care goals and increase the satisfaction of professional nursing services.

Abstract

Congestive heart failure is a clinical problem that occurs in patients who have abnormalities, either hereditary or acquired in the structure and function of the heart, causing the development of a series of clinical symptoms of weakness and tightness as well as clinical signs of edema and crackles which result in hospitalization, poor quality of life, and shortened life expectancy. This study aims to determine the relationship between family support and re-hospitalization incidence in heart failure patients in the Kemuning room at Jombang Hospital. The research design used in this study is cross-sectional analytic. The population in this study were all heart failure patients in the Kemuning Room of the Jombang Hospital, with a total of 37 people. A sample of 33 respondents. The sampling technique used is purposive sampling. The research variable is family support and the dependent variable is the incidence of re-hospitalization in heart failure patients. which were analyzed using chi-square with α = 0.05. The results of this study indicated that the majority (60.6%) of respondents had good family support, numbering 20 people, and the majority (69.7%) of respondents who were re-hospitalized in patients with heart failure were up to six months, totaling 23 people. The analysis results obtained 0.000 <0.005, meaning there is a relationship between family support and the incidence of re-hospitalization in heart failure patients in the Kemuning room at Jombang Hospital. From this research, it is hoped that nurses can maximize the role of the family as one of the support systems in caring for congestive heart failure patients undergoing re-hospitalization.

Abstract

Models of care services for the elderly in terms of medical surgical care and mental health need to be improved. Elderly with disabilities are at high risk of adverse health outcomes, and the best model of care has yet to be determined. A good holistic care model to empower every individual in society to improve the quality of care needs to be recommended. For example, comprehensive health care programs such as the Program of All-Inclusive Care for the Elderly (PACE) for adults and the elderly. The research objective was to determine the effectiveness of PACE in improving medical-surgical care and mental health. The search strategy for English-language studies relevant to the topic was carried out using the ScienceDirect database, limited from 2011 to 2021. The keywords used were “PACE”, “Medical Surgical Treatment”, “Mental Health”, “elderly”. Article full text reviewed to select studies that fit the criteria. The inclusion criteria in this review are: the nursing service program model, namely PACE. The search used the keywords above with the ScienceDirect database that matched the inclusion criteria for only 7 articles. The PACE service model improves services in Medical Surgical and Mental Health care, and implement a collaborative care model using a comprehensive interdisciplinary team.

Abstract

Patients with paralytic ileus generally have complaints of abdominal pain of uncertain duration. The pain that appears can get worse with anxiety about something being experienced, such as a surgical action plan. Feelings of anxiety can be a stressor and cause nursing problems in the form of anxiety. Guided imagery therapy is known to be a non-pharmacological therapy in dealing with pain and feelings of anxiety in preoperative patients. This study aims to analyze the administration of guided imagery relaxation therapy to the problem of acute preoperative pain in patients with paralytic ileus. The research method used in this study was a descriptive case study. The research was conducted for three days on one patient with preoperative paralytic ileus in the Mawar Room at RSD dr. Soebandi Jember. Guided imagery therapy is carried out twice daily for 20-30 minutes. The results obtained from this study were that after pain management was carried out with guided imagery therapy, the patient's pain level decreased from the NRS 6 scale to the NRS 5 scale, respiratory frequency 20x/minute, SpO2 96%, blood pressure 130/100 mm Hg, pulse rate 101x/minute. Abstract

Pain is the main problem experienced by many postoperative patients. One of the therapies for patients with rectal cancer is surgery, so patients with rectal cancer also feel pain after getting surgery. > 80% of patients experience postoperative pain, but appropriate pain management only occurs in approximately 30-50%. Benson's relaxation therapy reduces pain intensity in postoperative patients without causing side effects. This study aims to analyze benson therapy to reduce pain in postoperative surgery for patients with rectum cancer. The method used in this research is a descriptive case study. The research was conducted for three days on one patient with postoperative surgery in the Seruni Room at RSD dr Soebandi Jember. Benson therapy is carried out twice a day for 10-15 minutes. This study found that Benson therapy can reduce the level of acute pain in postoperative patients from the NRS scale 6 to the NRS scale 3, respiration rate 20 x/minute, SpO2 98%, blood pressure 120/80mmHg, pulse 87x/ minute. The result of reducing the pain scale indicates that Benson therapy can be applied to treat acute pain in postoperative patients to make certain patients' conditions feel better.

Abstract

Chronic Kidney Disease (CKD) or chronic kidney failure is a disease due to decreased kidney function that causes the body to fail to maintain fluid and electrolyte balance metabolism, resulting in urea retention and other nitrogen waste accumulation in the body. This research aims to analyze given this position to lower oxygen consumption and increase maximum lung expansion, the ineffectiveness of the client's breath pattern is more optimal in CKD patients in the Adenium Room of DR. Soebandi Jember Hospital. This type of research is descriptive using the case study approach method. The subject in this case study was one patient affected by CKD with the ineffectiveness of breath patterns. Semifowler position intervention is performed where the position of the head and body is raised by 45°. Result: The application of a semi-fowler position (45° sitting position) for 3x24 hours according to the standard of procedure helps reduce shortness of breath and helps optimize RR in the client so that the problem of ineffectiveness of breath patterns can be resolved. Interventions in regulating the angle of sleep position can meaningfully produce good respiration, so it can be considered one of the interventions to optimize the ineffectiveness of breath patterns.

Abstract

Traumatic brain injury is a leading cause of death among motorized vehicle users and is caused by a failure to maintain road safety. Traumatic brain injury can result in physical, psychological, and even fatal changes. Patients with head injuries may experience impaired consciousness due to bleeding in the head, which causes an increase in intracranial pressure. Nursing interventions such as head positioning at 30° may be given to patients with traumatic brain injuries. To examine the implementation of giving a 30° head-up position to the level of consciousness in patients with traumatic brain injury. The results obtained from giving the patient a 30° head-up position are that the patient has an increased level of consciousness. The patient's level of consciousness at the initial assessment was GCS E2 VX M5, and nursing intervention was carried out by giving the head-up position 30° for three days, namely E4 VX M5. Giving a head-up position of 30° affects the level of consciousness in patients with traumatic brain injury.
