Abstract

Introduction: Although circumcision is the most common surgical surgery performed on newborn males, problems can still occur regardless of the technique utilized by the operator. Case description: This case report aims to highlight the case of a 13-year-old boy who presented with severe foreskin adhesion and meatal stenosis after circumcision and underwent surgical reconstruction. During surgery, the foreskin firmly bonds to the glans and cannot be scraped away. Even the glans penis is injured when released abruptly. The glans penis was then clearly visible and could be distinguished between the skin and glans penis after a circumferential incision was performed on the penis skin above the coronary sulcus. Adhesiolysis was then conducted from the proximal to the distal wound. After that, the meatoplasty was performed. Conclusion: Since issues related to complications of circumcision continue to emerge, it is important to have a proper understanding of the circumcision procedure and how to manage the complication. To our knowledge, this is the new insight into managing severe foreskin adhesion with a good outcome.

Abstract

Introduction: Indonesia first reported Covid-19 on March 2, 2020, which continued to increase and spread rapidly throughout Indonesia. WHO recommends molecular examinations for patients with suspected COVID-19 through molecular detection method or NAAT (Nucleic Acid Amplification Test), including Real Time-Polymerase Chain Reaction (RT-PCR) tests. This study aimed to describe the profile of RT-PCR test services on nasal and oropharyngeal swab specimens from patients treated at Dr. Pirngadi Hospital Medan and patients of primary health care (Puskesmas) or other hospitals referred for RT-PCR examination at the Laboratory of Dr. Pirngadi Hospital. Method: This was a retrospective, descriptive study using secondary data from medical records from August 2020 to July 2021. Using consecutive sampling, the study sample was all suspected and confirmed COVID-19 specimens collected at Dr. Pirngadi Hospital, Medan. All collected data were processed and analyzed using IBM SPSS version 20.0. Result: There were 7495 specimens of COVID-19 from Dr. Pirngadi Hospital from August 2020 to July 2021, of which 1264 (17%) were confirmed positive, and 6231 (83%) had negative results. Meanwhile, 12.274 specimens were obtained from primary health care or other hospitals, of which 2602 (2%) samples were confirmed positive, and 9672 (98%) had negative results. Based on sex, 884 men (44%) and 1108 women (56%) had confirmed COVID-19 cases. Conclusion: From August 2020 to July 2021, this study found 17%  and 2% of confirmed COVID-19 cases at Dr. Pirngadi Hospital and primary health care or other hospitals, respectively. COVID-19-confirmed issues were more common in women.

Abstract

Background: A rare form of tumor is a neuroendocrine tumor (NET). According to the 2015 National Comprehensive Cancer Network (NCCN) Clinical Practice Guidelines in Oncology, all endocrine tumors of the digestive system should be treated with neuroendocrine tumors of the gastrointestinal system. The incidence of NET is 2.5 cases per 100,000 people annually on average. Since NET has a variety of clinical symptoms, it can be divided into functionally active and inactive types. This study aimed to present a neuroendocrine tumor of the extrahepatic biliary tract. Case presentation: An 18-year-old female with a lump in the upper right abdomen with intermittent pain that had been felt in the last 8 months. There was evidence of previous weight loss and an easy appetite. Physical inspection revealed a mass with a fixed, smooth, flat edge and solid consistency. A vesica felea enlargement and a normal wall were discovered on the CT scan. Stone and sludge were not detected. The choleducus and cystic duct were normal-looking, and a 0.5 cm spherical stone was present. Bile fluid was found to move smoothly from the ductus cysticus to the distal and into the ampulla of Vater. Our patient was diagnosed early with an intra-abdominal tumor. An intraoperative cystic tumor measuring 9x7x8 cm and attached to the duodenum was discovered at the pancreatic cap. The pancreatic uncinate process underwent tumor excision. The patient was managed post-op in a stable condition, and the tumor's histology revealed a carcinoid tumor. Conclusion: NET is a rare type of tumor, but two third of it occurs in the gastrointestinal system. It is hard to detect because of its heterogeneous nature and various clinical presentation. Surgery is the first-line therapy in this disease, followed by chemotherapy, radiotherapy, and tumor debulking as palliative management.

Abstract

Introduction: Intraventricular hemorrhage refers to the presence of blood in the cerebroventricular system.  Intraventricular hemorrhage can lead to severe complications such as hydrocephalus. One of the therapeutic modalities for intraventricular hemorrhage is to perform CSF diversion using an external ventricular drain (EVD). To overcome complications related to the use of EVD, there have been several efforts to accelerate its excretion in ventricular blood, one of which is by intraventricular injection of fibrinolytic. The mixed results of studies on intraventricular fibrinolysis prompted investigators to conduct a review and meta-analysis of this treatment modality. Method: We conducted systematic reviews and meta-analyses on several literature databases and included articles in English. We were using the boolean operator AND to search for literature containing all keywords in the databases and analyze the data using SPSS and Revmann. Results: Unfavorable outcomes with the use of intraventricular fibrinolytic therapy in IVH occurred in a median of 51% of cases. The risk of an unfavorable outcome in IVH patients who received intraventricular fibrinolytic was 0.93 times compared to controls. The risk of an unfavorable outcome in IVH patients receiving rtPA and intraventricular urokinase was 0.95 and 0.73 times, respectively, compared to controls. The use of intraventricular fibrinolysis was able to reduce IVH volume by an average of 22.84 cc. The use of intraventricular fibrinolysis was able to improve the modified Graeb Score with an average change of 5.24 points Conclusion: Intraventricular fibrinolysis administration helps to reduce the risk of unfavorable outcomes in IVH cases.

Abstract

Introduction: The most crucial time for a child's physical and cognitive development is the first 1000 days of life. Their nutritional state will influence toddlers' future health, intelligence, and productivity in the first 1000 days of life. The study aimed to describe the first 1000 days of life in toddlers aged 0-2 years. Methods: This cross-sectional study used a household survey in 30 clusters. Five samples of children between 0-1 years old and five samples between the ages of 1-2 years old were selected by simple random sampling from each census block for 300 respondents. Nutrition adequacy was analyzed in Nutrisurvey 2007 program. All data were analyzed descriptively in the SPSS program. Results: The prevalence of stunting in children aged 0-2 in Tegallalang District was 12%. Among children 0-1 years old was 8%, while in children aged 1-2 years was 17%. 41% of pregnant women and toddlers were exposed to secondhand smoke. The rate of exclusive breastfeeding was 44%, vitamin A supplementation was 43.7%, deworming treatment was 52%, and only 20% of people had clean water. There were 115 toddlers aged 1-2 years w    deficits in carbohydrates, 140 toddlers with deficits deficiencies, and 80 toddlers with deficits in iron. Conclusion: The problem of stunting in Tegallalang District still exists. Community health workers should encourage women and their male partners regarding second-hand smoke, exclusive breastfeeding, vitamin A supplementation, and deworming treatment. Access to clean water is also urgently required. Further study is needed to determine the modifiable risk factors for stunting.
