Abstract

Ammonia and urea blood levels in premature infants during the first week of life were analyzed. Twenty seven blood samples from premature infants at the age of 1, 3 and 7 days were collected. The concentration of ammonia was determined using Amitest meter (TSUGAI), while the urea concentration was measured using regular techniques. Even though the ammonia levels did not exceed 180 pg/d1., the data showed that the critical situation occur in the first three days of life when the ammonia concentration was the highest and urea was also concentrated. Abstract

Previous studies showed that susceptibility to leprosy and antibody response toward M. leprae in Javanese population was under control of HLA-DR alleles. To investigate whether this susceptibility was also associated with HLA-DQ, the study had been continued with phenotyping of HLA-DQ alleles and antibody assay to the same population which consisted of 79 leprosy patients, 41 tuberculoid (TT/BT) and 38 lepromatous (LL/BL) type, and 50 healthy controls. The HLA-DQ typing had been performed by using a sequence specific oligonucleotyping (SSO). method, while the anti M. leprae antibody had been tested by ELISA and INHIBITION-ELISA.The results show that HLA-DQB501 is associated with leprosy, either tuberculoid or lepromatous type (OR 3.27; 95% Cl 1.42-7.60). When all HLA-DQ1 alleles are analyzed, a significant association is found only with lepromatous leprosy (OR 9.18; 95% Cl 1.89-86.30). IgG antibody anti 36 kD M. leprae is found to be associated with HLA-DQ102. The level of this antibody is higher in HLA-DQ102 positive individuals compared to those negative one (P). No correlation is found between HLA-DQ alleles and the seropositivity of either IgM or IgG.In conclusion, the susceptibility to leprosy in this population is also controlled by genes in HLA-DQ locus. This study also supports the previous findings that HLA-DQ1 is a universal marker for the susceptibility to lepromatous leprosy, while the infection with M. leprae per se is not controlled by HLA genetic factor.Key words : leprosy - M. /eprae - genetic factor - HLA-DQ - HLA-DQ1 - HLA-DQB501

Abstract

A diagnosis of thalassemias has advanced from clinical to molecular in concordance with the advances in molecular biology. Since the introduction of polymerase chain reaction procedure - a practical in vitro procedure of deoxyribonucleic acid amplification - various diagnostic methods have been developed, to detects either gene deletions or point mutations. In a population where the spectrum of mutations is not too heterogenous, direct methods such as dot blot or reverse dot blot hybridization, ligase chain reaction and amplification refractory mutation system may be applied with high effectivity dan efficiency. But, in a population where the spectrum of mutations is very heterogenous other methods such as mismatch analysis, denaturing gradient gel electrophoresis and single strand conformation analysis as the screening step followed by deoxyribonucleic acid sequencing are chosen. Each of the above methods has advantages and shortcomings, in relation to various problems among others the sensitivity, the specificity, the ease, the reproducibility and the cost.In this paper the molecular diagnostics, concerning the principle, the advantages and the shortcomings, espebially that have been used in the field are discussed.Key words: gene disorders - ligase chain reaction - dot blot and reverse dot blot hybridization - amplification refractory mutation system - denaturing gradient gel electrophoresis - single strand conformation analysis - chemical cleavage of mismatch

Abstract

Aging may affect functional and structural changes of the urinary tract which modify the response against infection. Sructural changes in elderly such as prostate hypertrophy, urinary tract stone, the use of catheter, and the presence of cystoceles should be treated properly. Attention should be paid on functional abnormalities in elderly including bladder atonia, neurologic defects, and prolonged bed rest. Inadequate immune response in elderly may also play a role in the changes. Those factors make the treatment of urinary tract infections in elderly have to be carried out more radically.Complicated urinary tract infections, infection in males with prostate hypertrophy, and pyelonephritis need prolonged treatment. Recurrent urinary tract infections in female elderly patients need early prophylactic treatment. Doing a culture of the causative microorganisms and a sensitivity test for the appropriate antibiotics should be taken into consideration in the selection of the antibiotics. Abstract

Total iodine contents in the urine samples were directly tested with and without destruction using Ion Selective Electrode (ISE) method. The dry destruction was carried out with volume variation of 0.5 M NaOH solution and wet destruction was done with 4 ml of concentrated H2504 and 1 ml of concentrated HNO3 mixture, followed by volume variation of 1% p-aminophenol solution and temperature to reduce 103- form to form. The Chloride ion contents were also determined using technique. It was found that the optimum of NaOH amount, the optimum of 1% p-aminophenol and temperature (wet digestion) was 2 ml, 12 ml, and 60°C respectively. The chloride ion contents in 6 urine samples were in the range of 2373-4495 ppm and didnt interfere on r determination. The total r contents in the six samples without destruction,using wet and dry destructions were in the range of 280-392 ppb, 291-505 ppb and 306-516 ppb respectively. The t-test showed that the total Iodine content in urine samples with and without destructions was significantly different.Key words: Iodine - urine - ISE method - wet and dry destruction - chloride

Abstract

The management of soft tissue sarcoma of the extremities is very complicated, particularly if the tumor is large and has infiltrated into the deep structures. Forequarter amputation is still one best choice for either curative or palliative purposes, although this operation is very mutilated.This report is aimed at presenting the benefit of forequarter amputation in patients with soft tissue sarcomas in the humeri region. Two patients with extrasceletal myxoid chondrosarcoma and pleomorphic rhabdomyosarcoma in the humeri region were managed with forequarter amputation and neoadjuvant chemotherapy. Abstract

Diclofenac sodium is a nonsteroidal antiinflammatory drug (NSAID), which is drug of choice for rheumatoid arthritis. It is commercially available as enteric coated tablet and emulgel for topical dosage form.Topical drug was prepared and developed for systemic purposes in the treatment of rheumatoid arthritis, to reduce the risk of adverse drug reaction given orally. As an oral antirheumatoid drug, it caused dizziness and irritation on gastric mucosa.The study was carried out to assess the effect of semisolid formulation variation on its percutaneous absorption. The experiment was performed by preparing 4 formulas of ointment i.e. hydrocarbon base, washable base, 0/W emulsion base, W/O emulsion base and commercial cream E as a control drug. All of the formulas and standard cream were tested in vitro for the rate of dissolution and its percutaneous absorption on rabbits.The Dissolution Efficiency (DE) of washable emulsion, 0/W emulsion base,- and the commercial cream was correlated with their Ka from the experiment of percutaneous absorption. Among the three formulas, the C ointment (0/W emulsion base) showed the best correlation. The order of the correlation declined as follows C, B and E. From the result it can be concluded that the change of formulation could affect the dissolution and in turn could affect the absorption of the active compounds.Key words : ointment - cream - diclofenac - in vitro - in vivo

Abstract

Livedo reticularis (LR) is a skin lesion which is characterized by a reddish blue mottling in a fishnet reticular pattern. Although many diseases have been considered as causative of LR, Wegeners Granulomatosis (WG) has never been reported to be associated with LR.A case of LR was reported in a 70-year-old woman suffering from WG. However, it is difficult to conclude whether LR is a part of WG or only a coincident case. The vasculitis features as shown by histopathological skin examination suggest that the skin lesion might be a part of WG syndrome.Key words : livedoreticularis - Wegener granulomatosis - saddle nose - skin biopsy - vasculitis granulomatosis
